Circadian variation in the onset of acute myocardial infarction: lack of an effect due to age and sex.

The circadian patterns in the onset of symptoms of acute myocardial infarction were assessed in a prospective study involving 450 patients. Analysis of the data in the 424 patients in whom the time of onset was known did not show any statistically significant circadian rhythms, either for the group as a whole or for subgroups divided according to sex, age group (< 60, 60-69, 70-79 and > or = 80 years old), or the presence or absence of diabetes mellitus. The frequency of onset of symptoms was highest in the morning (06.01-12.00 h) for the group as a whole (29.2%). Patients aged < 70 and > or = 80 years old showed a peak in the morning, whereas patients aged 70-79 years old showed a peak in the afternoon (12.01-18.00 h).

[Injuries in childhood: a study on emergency department admissions of pediatric population in an Emilia-Romagna USL].

A study of childhood injuries of 0-14 year-olds children based on emergency room records of the five hospitals was conducted in USL 39 of Emilia-Romagna during 1988. The incidence of visits was 164, 14/1000 child-years with 95% confidence intervals (CI = 163, 28-165). The rate was 198, 93/1000 child-years (CI = 195, 36-202, 51) among the 0-4 year-olds, 140, 65/1000 child-years (CI = 139, 17-142, 13) in the 5-9 age group and 170, 86/1000 child-years (CI = 169, 77-171, 95) among 10-14 year-olds. The male to female ratio was 1.4 for the 0-4 year-olds, 1.7 for the 5-9 year-olds and for the 10-14 year-olds. The most frequent causes of injuries were falls (49.52%), being road accidents (22.1%) and bites-stings (17.4%); 4.8% of the injured children were admitted to hospital and 0.044% dead.

[Holter electrocardiography. Analysis of 700 cases].

Seven hundred patients, 337 male (48.2%), 363 female (51.8%) requiring Holter Electrocardiogram were studied. The predominant age group ranged from 50 to 59. Symptoms were more frequent in women. Lown's classification was used to determine the frequency of arrhythmias; 47% were supraventricular arrhythmias and 56% were of ventricular origin. Most of the latter (30.9%) were included in Group I of the classification. The remaining cases appeared in a decreasing distribution in the other groups. This suggests that in our population, severe arrhythmias are infrequent. With the hypothesis that distribution of symptoms is independent from arrhythmia onset, a multiple contingency table was designed. The X2 value was 82 with freedom of 316 degrees. It confirms that 1) severe arrhythmias (ventricular tachycardia, frequent ventricular premature beats or couplets) may go unnoticed, and on the other hand 2) Symptoms are not related with occurrence or severity of arrhythmias.

[Use of new forms for the evaluation of the clinical apprenticeship of nursing students].

The development of forms oriented to the evaluation of the practical training of nursing students is described. The activities of the nursing students were listed and the relevant activities for each practical training period were identified. The level of difficulty of the activity, the skills needed and the performance level to be reached by the end of the practical training period was agreed on and clearly stated for each activity. Criteria for evaluating the socio-affective and relational skills were also identified. The 28 forms developed were used for the evaluation of the practical training of 254 students. Some critical reflections and suggestions for improvement are presented.
