Natural infection of wild rodents by Schistosoma mansoni. Parasitological aspects.

The evaluation of the role of rodents as natural hosts of Schistosoma mansoni was studied at the Pamparrão Valley, Sumidouro, RJ, with monthly captures and examination of the animals. Twenty-three Nectomys squamipes and 9 Akodon arviculoides with a schistosomal infection rate of 56.5% and 22.2% respectively eliminated a great majority of viable eggs. With a strain isolated from one of the naturally infected N. squamipes, we infected 75% of simpatric Biomphalaria glabrata and in 100% of albino Mus musculus mice. The adult worms, isolated from N. squamipes after perfusion were located mainly in the liver (91.5%) and the mesenteric veins (8.5%). The male/female proportion was of 2:1. The eggs were distributed on small intestine segments (proximal, medial and distal portions) and the large intestine without any significant differences in egg concentration of these segments. In A. arviculoides, the few eggs eliminated by the stools were viable and there was little egg retention on intestinal segments. Considering the ease to complete S. mansoni biological cycle in the Nectomys/Biomphalaria/Nectomys system under laboratory conditions, probably the same is likely to occur in natural conditions. In support to this hypothesis there are also the facts that human mansonic schistosomiasis has a very low prevalence in Sumidouro and endemicity among the rodents has not changed even after repeated treatments of the local patients. Based on our experiments, we conclude that N. squamipes has become a natural host of S. mansoni and possibly may participate in keeping the cycle of schistosomiasis transmission at Pamparrão Valley.

Nectomys squamipes (Rodentia: Cricetidae) as an experimental model for Schistosomiasis mansoni.

Twenty specimens of Nectomys squamipes born in captivity, were infected with 500 cercariae by the transcutaneous route. Coprologic examinations were carried out from the 5th to 23rd week after infection. On the 7th, 8th, 12th, 16th, and 23rd weeks the animals were sacrificed and perfused. The oogram was performed in segments of the small intestine (proximal, medial and distal portions) and the large intestine. The average pre-patent period was of 42 days. The average number of eggs varied from 350 on the 6th week, to 800 on the 13th. From the 14th week on, the average number of eggs eliminated was lower than 50 per gram of feces. The recovery of worms kept steady on the 7th, 8th and 12th week (16.85%; 15.45% and 11.95%), decreasing to 7.70% on the 16th week and 8.45% on the 23rd week. The proportion of male/female worms was about the same on the first two weeks, but from the 12th week on, the proportion was: 1.4/1 on the 12th week; 2.5/1 on the 16th week and 1.8/1 on the 23rd week. These observations suggest that N. squamipes may be used as an experimental model for schistosomiasis mansoni, to which it develops resistance mechanism, useful for immunity studies.

Feeding behavior of different species of Chagas' disease vectors stimulated with blood sources at different temperatures.

1. The feeding behavior of six triatomid species toward latex condoms filled with blood at 26 +/- 1 and 36 +/- 1 degrees C was observed for 4 h. 2. The species studied were Triatoma infestans, Triatoma brasiliensis, Triatoma vitticeps, Triatoma pseudomaculata, Rhodnius prolixus and Panstrongylus megistus, and 34 to 348 insects were studied in each group (average, 190). All the life stages of each species were used. 3. Although most insects (80%, average for 6 species) preferred the warmer blood, 54% of the individuals of all species fed on blood kept at 26 degrees C. For Triatoma pseudomaculata, a higher proportion of insects fed on blood at 26 degrees C (92%) than on blood at 36 degrees C (65%). 4. These results suggest that bait containing insecticides which are effective through the digestive tract may be useful in the control of these vectors even at room temperature.

[Rupture of the interventricular septum in acute myocardial infarct: diagnosis by Doppler color code. Clinical case].

Doppler Color Flow Mapping represents the most recent step in the evolution of Doppler techniques that display blood flow signals superimposed on real time, two dimensional echocardiographic images. The following case illustrates the utility of this technique in the diagnosis of a ventricular septal rupture complicating the course of an inferior wall acute myocardial re-infarction. An abnormal blood flow pattern from the left to right ventricle through the ventricular septum was promptly visualized. Early operative intervention may improve the prognosis of acute infarct-related ventricular septal rupture; Doppler Color Flow Mapping and Pulsed Doppler Echocardiography are at present the most rapid and reliable noninvasive techniques in the detection of post-infarct ventricular septal rupture and may greatly expedite medical and surgical management.

Production of prostacyclin, 6-keto-PGF1 alpha and thromboxane B2 by human umbilical vessels increases from the placenta towards the fetus.

The aim of this study was to investigate the production of prostacyclin (PGI2) and thromboxane B2 (TXB2) by incubated samples of umbilical arteries and veins taken at different distances (2, 10, 20, 30 cm) from the placenta to provide additional information relevant to the haemodynamics of umbilical blood flow. The production of PGI2, and 6-keto-PGF1 alpha (the stable metabolite of PGI2), was higher in both veins and arteries as the distance from the placenta at which the vessels were sampled was increased. A similar correlation between production by venous rings and distance from the placenta was observed for TXB2, but there was no apparent gradient of TXB2 production by the samples of arterial rings. No statistically significant variations were discernible in the ratio of 6-keto-PGF1 alpha:TXB2 (approximately 50 in the veins and approximately 20 in the arteries) in relation to the sampling distance. The significance of these high ratios is discussed in relation to umbilical blood flow and fetal well-being and development.

Production of prostacyclin (PGI2) and thromboxane A2 (TXA2) by vessels draining and not-draining benign and malignant tumours of the breast.

The production of PGI2 (determined by bioassay) and TXB2 (determined by radioimmunoassay) was studied in the supernatant solutions obtained after incubation of vessel rings prepared from veins draining and not draining benign and malignant tumours of the breast. A significant increase (p less than 0.01) was found in the production of PGI2 by vessels draining the malignant tumours as compared to those not draining such tumours or vessels draining benign tumours. The changes in PGI2, and the tendency for TXB2 to be higher in vessels draining malignant tumours, were in the same direction so that they balanced out when the ratio PGI2/TXB2 was calculated: in consequence no significant changes in this ratio were found in malignant tumours as compared to the benign. A significant difference (p less than 0.01) was observed in the ratios of vessels draining tumours:vessels not draining tumours between malignant and benign tumours in relation to the production of PGI2. The present data demonstrate that the production of PGI2 by vessels draining malignant tumours of the breast is different to that obtained with vessels from patients with benign tumours, although the mechanism responsible for this difference is not known.

Production of prostacyclin, 6-keto-PGF1 alpha and thromboxane B2 by incubated samples of umbilical vessels: a comparison of methods for expressing the results on dry weight, wet weight, protein or DNA bases.

The production of PGI2 (determined by bioassay), and of 6-keto-PGF1 alpha and TXB2 (determined by radioimmunoassay) by samples of human umbilical vessels have been measured. The results have been calculated on four bases: dry weight, wet weight, protein and DNA. There was a higher production of PGI2 and 6-keto-PGF1 alpha by umbilical veins than by umbilical arteries; no significant difference in TXB2 production was observed between umbilical veins and arteries. The ratio of 6-keto-PGF1 alpha: TXB2 production was about 100 for the samples of veins and about 40 for the samples of arteries. The best methods of expressing the results were on the bases of protein and DNA, the latter basis being marginally the best. The least satisfactory method for expressing the results was that based on dry weight. The physiological and practical implications of the results are discussed.

[Paracrystalline inclusions of nutritive cells of galls induced by Diplolepis rosae L. on Rosa canina L].

Numerous crystalline inclusions are found in the nutritive cells lining the larval cavities of the galls induced by Diplolepis rosae L. on Rosa canina L. Electron microscopic investigations show that these intracytoplasmic inclusions consist of staggered, closely parallel, and slightly wavy filaments. In each filament three fibrils can be distinguished inside a less electron-dense matrix. The different aspects that the filaments may present according to the section plans are studied with an electron microscope equipped with a goniometric stage. Enzymatic digestion of ultrathin sections show that the paracrystals are essentially composed of proteins. This conclusion is sustained by the results of an ultrastructural autoradiographic study using tritiated aminoacids. The physiological significance of these paracrystals is discussed: their presence is probably related to a larval action which stimulates proteosynthesis in the cells surrounding the consumed ones.

Man-made lakes and man-made diseases. Towards a policy resolution.

Throughout the tropical world, in Africa. Asia and Latin America, the construction of water impoundments, for irrigation and other purposes. in areas of endemic water-related diseases, has inexorably intensified community levels of infection, and also created new areas of transmission. The clearest 'indicator' disease is a schistosomiasis, but other are involved such as malaria and the filariases. An assessment for the future suggests a worsening situation because of population growth, the demand for food production, and the increased technological capacity of the "bulldozer revolution' to effect earth impoundments. The typical sectoral dichotomy in which a department of agriculture creates a disease outbreak through its development activity, leaving its counterpart department of public health to cope on he curative basis, where possible, is strongly deplored. A policy position is offered whereby disease prevention measures are integrated with development projects from the beginning, and infrastructural investment and operational costs for health maintenance are identified and incorporated in the total benefit-cost analysis. The need for the communications network on ecological and health effects is pointed out; and a typology for registration of dams is offered in support of policy implementation.

[Schistosomiasis in Tunisia. Results after 10 years of the endemics control].

Despite the progress already made in the field of epidemiology and the control of schistosomiasis, the practical application of an efficient and economically acceptable methodology still remains a difficult problem to solve. In the saharian and pre-saharian regions of Tunisia, Bulinus truncatus was successfully controlled while at the same time most of the infected people were treated, so that the transmission did not show up through the appearance of new cases at the outset of the 2nd semester 1976. A system of epidemiologic supervision and yearly evaluation of the results has been implemented in order to strengthen this positive fact and to ensure the cure of residual cases.

[Intravenously disopyramide phosphate administration for treatment of arrhythmias].

Disopyramide phosphate (DF) was administered intravenously to 50 patients during 55 episodes of arrhythmias. The mean dosage employed was 2 +/- 1.4 mg/kg over a period of 3 minutes to 60 minutes (mean 16 +/- 6'), followed by an infusion at a dose of 0.4 mg/kg/hr in 9 cases. Atrial premature beats were suppressed in 3 of 4 treated cases (75%). Conversion to sinus rhythm was achieved in 4 of 5 (80%) cases of paroxysmal supraventricular tachycardia, unresponsive to vagal maneuvers in 7 of 10 cases (70%) of atrial flutter of recent onset (less than 48 hours) and in 13 of 17 (76%) cases of atrial fibrillation (less than one week in duration). In 9 of 11 cases (81%) with frequent, multiform, repetitive ventricular premature beats and in 7 of 8 cases (87%) of ventricular tachycardia, DF completely suppressed the arrhythmia. The drug increased sinus rate, PR and QRS intervals, but the difference was not statistically significant: the QRS of patients with complete bundle branch block and the QTc interval were significantly prolonged. Severe hypotension was observed in 10 patients after DF i.v., in 5 of whom withdrawal of therapy was deemed to be necessary. Nine other patients had anticholinergic side effects. Our study shows that DF administered i.v. can be used successfully in 75% of supraventricular arrhythmias and in 82% of ventricular arrhythmias.
