Multifocal extraovarian serous carcinoma. A histochemical and immunohistochemical study.

Three cases of multifocal extraovarian serous carcinoma are presented. This rare, interesting tumor is characterized by a peritoneal carcinosis with ascites and by a histological pattern similar to one of the ovarian serous carcinoma without primary involvement of the ovaries. The pathological criteria for a differential diagnosis between this condition and a localization of ovarian carcinoma are discussed. Histochemical studies show the presence of mucins and the absence of ialuronic acid in the neoplastic cells. The positivity to the low molecular weight cytokeratins is strong in the normal mesothelial cells, and weak in the hyperplastic and neoplastic cells. The meaning of these findings in the tumoral histogenesis is discussed.

Hyperplasia and metaplasia of ovarian surface epithelium in women with endometrial carcinoma. Suggestion for a hormonal influence in ovarian carcinogenesis.

Surface ovarian epithelium and that of related inclusion cysts were comparatively studied in two groups of patients: one group of 50 non-pregnant women without myometrial, endometrial, or cervical hyperplasia and/or neoplasia; a second group of 50 women surgically treated for endometrial cancer and without evidence of ovarian pathology. Papillomatosis, hyperplasia and tubaric, squamoid, endometrioid and mucinous metaplasia were more frequently present in ovarian surface epithelium or in related inclusion cysts in patients with endometrial carcinoma. These findings may be correlated with a hormonal oncogenic stimulus involved in the genesis of endometrial cancer and suggests a new "hormonal" hypothesis in the pathogenesis of common epithelial tumors of the ovary.

Presence of cells with argyrophil granules in normal, hyperplastic, and neoplastic endometrium.

Cells with argyrophil granules were searched for in 131 consecutive specimens obtained from endometrial curettage. Particular care was taken with the fixation methods to avoid a false positivity to the silver impregnation according to Grimelius. We did not consider the grossly argyrophilic positivity on the cellular apex or of the whole cytoplasm, which was partially reduced by diastase digestion and probably due to the presence of glycogen and secretory mucoproteins. Cells with thin argyrophilic granules similar to those observed in endocrine APUD cells were present among glandular cells and/or among stromal cells in 15 of 131 examined specimens. They concerned 5 cases of proliferative endometrium, 3 of secretive endometrium, 5 of hyperplasia, and 2 of carcinoma. In 8 of the 15 cases with argyrophilic cells, immunohistochemical studies with the PAP method showed cells with the presence of FSH, S-100 protein, somatostatin, vasoactive intestinal peptide (VIP), gastrin, and neuron-specific enolase (NSE). The significance and origin of these cells in normal and neoplastic endometrium are discussed.

Otomastoid cholesteatoma in children: histopathological findings.

Clinical experience has shown that the otomastoid cholesteatoma is more aggressive and has a less favourable prognosis in the child than in the adult. In an attempt to verify whether, in children, the clinical behaviour of the cholesteatoma depends on the histomorphological characteristics of the perimatrix, we carried out a study of samples taken from 30 subjects under 16 years of age and from 30 adults (control group) during surgery for otomastoid cholesteatoma. The study took into account the number of plasmacytes, lymphocytes, macrophages, granulocytes and giant cells per high potency field (X 630). The results showed that in young subjects the perimatrix of the cholesteatoma is richer in mononuclear inflammatory elements with evidence of enzyme-collagenase activity. On the basis of this behaviour the authors conclude that the histomorphological characteristics of the surrounding matrix can play an important role in the pathogenesis of cholesteatoma and may explain the clinical differences that can be seen between cholesteatoma in children and in adults.

Computer-aided morphometric analysis of lymph nodes draining laryngeal carcinomas.

A new method of morphometric evaluation of nodal patterns in patients with laryngeal cancer was developed. In 8 selected cases with median tumor and unilateral metastasis, the morphometric data about the perimetastatic nodal tissue and contralateral node in the same patient were compared. The results showed the paracortical area to be consistently larger in lymph nodes without metastasis than in those containing metastases. The mean number of blasts, mitoses and postcapillary venules was also increased in the paracortical area in the non-metastasis-containing nodes. These data support the idea that delayed immunity may inhibit development of metastases. The need for objective morphometric methods in lymph node examination is emphasized.

Prognostic value of the prelaryngeal node in laryngeal and hypopharyngeal carcinoma.

The role in tumor spread and the prognostic value of metastasis in a prelaryngeal node were investigated in 124 cases of laryngeal and hypopharyngeal carcinoma selected from about 900 total laryngectomies and pharyngolaryngectomies. Metastasis in a prelaryngeal node was noted in 26 cases: 22 without capsular rupture (N+) and 4 with capsular rupture (N+R+). Vascular embolism (E+) was never noted. Patients with a metastatic prelaryngeal node had a tumor of the piriform sinus in 13 cases (50%), a transglottic tumor in 6, glotto-subglottic in 3, glottic in 2 and supraglottic in 2. They also more frequently had involvement of the cervical nodes (81%) and of the thyroid gland. The 5-year survival in these patients is very poor: only 3 patients (11.5%) were alive and free of disease. These data demonstrate the role of prelaryngeal nodes in tumors that affect the hypopharynx and the antero-inferior portion of the larynx and that directly reach this node through the anterior lymphatic peduncle.

Follow-Up Studies↗

Local morphologic changes related to laryngeal carcinoma.

In a series of 178 patients who underwent partial or total laryngectomy for laryngeal carcinoma, the morphologic changes at a distance from the neoplasm were investigated with whole organ serial sections. We found 60 cases with extensive squamous metaplasia of the epithelium outside of the vocal cords, 24 case with hyperplastic epithelium of the vocal cords, 29 cases with mild or moderate dysplasia, 5 cases with foci of in situ carcinoma, 3 cases with a second carcinoma with minimal stromal invasion, and 4 cases with a double synchronous primary carcinoma. In 55 cases we observed the contemporary presence of a carcinoma and a mono-or bilateral laryngocele. In the excretory ducts of the salivary glands present in the laryngeal mucosa we found a reserve cell hyperplasia in 100% of the cases, a mature squamous metaplasia in 25%, and an oncocytic metaplasia in 14%. These observations led us to consider the laryngeal cancer as an organ disease, in which the oncogenic stimulus operates on all the laryngeal structures and determines lesions of different severity up to neoplasm.

Morphometric analytical evaluators of lymphoid populations in nonneoplastic lymph nodes.

A morphometric analysis using an Apple II microcomputer and shape analytical morphometry (SAM) software was performed of the nuclear shape in the lymphoid population of normal (nonneoplastic) lymph nodes to provide baseline parameters useful for distinguishing between different subtypes of lymphomas. Fourier harmonic analysis and two indices, a contour asymmetric evaluator (CAE) and a shape asymmetric evaluator (SAE), are proposed as non-dimensional shape parameters. Results obtained with the CAE and SAE indices indicate that they represent sensitive shape descriptors able to reveal even subtle irregularities of nuclear outline. With these parameters, centrocyte nuclei were easily differentiated from those of all other lymphoid cells while an interesting close relationship was found between lymphocytes and centroblasts. Lymphocyte nuclear shape was shown to be less regular than generally reported, suggesting the existence of subtle differences between stimulated and unstimulated nuclei. Immunoblast nuclei were shown to have the most regular shapes.

The histologic typing of tonsillar carcinoma.

In a retrospective review of 170 cases of malignancies of the tonsil the authors report on 81 malignant epithelial tumors. In 56 cases (69.1%), the tumor (squamous cell carcinoma) was composed of solid epithelial nests with different degrees of keratinization. In 24 cases (29.6%) the tumor (undifferentiated carcinoma of nasopharyngeal type) had histologic characteristics similar to the undifferentiated carcinomas observed in the nasopharynx. Only one case was unclassifiable. Squamous cell carcinomas occurred more frequently in males (6:1), in older age, with more frequent extratonsillar and nodal extension and a poorer prognosis. Undifferentiated carcinoma had a similar incidence in both sexes, occurred also at an early age, and more often seemed to be limited to the tonsil. The 10-year disease-free survival rate was 30% for undifferentiated carcinoma; all the patients affected by squamous cell carcinoma had a recurrence within 6 years of the diagnosis.

The s.c. 'hairy polyp' or 'dermoid' of the nasopharynx. (An unusual observation in older age).

A case of 'hairy polyp' or 'dermoid' of the nasopharynx in a 71-year-old man is described. This tumor-like condition is usually discovered at birth or during the first years of life. The oldest patient previously recorded in the literature was a 66-year-old man. The occurrence of such masses in older patients suggests that the condition is a malformation and not a neoplasm.

Ovarian dysgerminoma. A clinico-pathologic study.

The authors present the clinical, pathologic and therapeutic report on 5 cases of ovarian dysgerminoma in patients between 15 and 30 years of age, classified as Stage Iai in 3 cases and Stage III in 2 cases. Treatment was surgical in 4 cases; in 1 case the surgery was followed by radiation and chemotherapy. All the patients survive free of disease from 4 to 11 years. One of these neoplasms showed a particular histologic pattern in some way similar to the "spermatocytic seminoma" of the testis. This should be the first time that such a type of ovarian dysgerminoma has been reported in the literature.

Morphologic and classificatory considerations about 140 cases of carcinoma of the nasopharynx.

The morphologic aspects are reported for 140 cases of nasopharyngeal carcinoma collected from a retrospective review of malignant tumors of this region. Of 97 cases of undifferentiated carcinomas of nasopharyngeal type, 63 were composed of solid cords of epithelial cells (Régaud type), 22 showed isolated cells in a lymphoid stroma (Schmincke type), and in 12 the neoplastic cells were aligned with a fibrosarcoma-like aspect (spindle-cell type). The 36 cases of squamous cell carcinoma showed various degrees of differentiation. Of the remaining 7 cases, 3 were transitional cell carcinomas, 1 was an adenoid cystic carcinoma, and 3 were unclassifiable carcinomas. The undifferentiated carcinoma was present in all ages, with a male : female rate of 1.27 : 1. The squamous cell carcinomas were prevalent in males (3.5 : 1) and older patients. The 10-year survival rate was 30% in the group of patients with undifferentiated carcinoma, whereas patients with squamous cell carcinoma died within 4 years of the diagnosis.

APUD cells in normal and hyperplastic prostates.

Argyrophilia and argentaffinity, as basic properties of APUD cells, were investigated in 50 normal and hyperplastic prostates, which included both autopsy and surgical specimens from patients of various ages. Normal prostates (including glands from 3 foetuses) had 62% of argyrophil-positive granules in the glandular epithelia, while only 44% of the hyperplastic glands were positive. Argentaffin-positive cells were found in 12% of the surgical hyperplastic cases. Both argyrophil and argentaffin cells were distributed in zones, often in lobule-like shapes, lying along the basal membrane. On the basis of these findings, there is a discussion on the possible roles played by the so-called APUD cells in hyperplastic and neoplastic growths of the prostate, such as carcinoid tumours (apudomas) or endocrine-associated syndromes in the course of prostatic cancer.

Distant metastases in ovarian carcinoma.

Distant metastases are unusual at presentation and during the course of ovarian carcinoma. The aim of the present study was to determine the incidence and prognostic factors of distant metastases consistent with stage IV disease in ovarian cancer patients. A retrospective chart review was conducted on 162 patients with epithelial ovarian carcinoma treated at our Unit between 1991 and 2000. Pertinent clinical information, pathologic data, treatment, and prognostic factors for survival following documentation of distant metastatic disease were collected. The significance of the association between metastatic status and various clinical variables was assessed using the standard chi-square test. Survival time was calculated from the time of diagnosis of ovarian cancer and from the time of diagnosis of the distant metastases. A logistic regression procedure was used to estimate the odds of metastatic status given the presence of certain clinical variables. A total of 67 metastatic sites were diagnosed in 50 patients. Thirteen patients (8%) had distant metastatic disease at the time of diagnosis, 37 patients (22%) had distant metastases at the time of recurrent of progressive disease. Site of metastases were: liver, 21; pleura, 11; lung, 8; central nervous system and skin, 7 each; extra-abdominal lymph nodes and spleen, 5 each; bone, 2; and breast, 1. Significant risk factors for the development of distant metastases were stage, grade, and lymph node involvement. Median interval time between diagnosis of ovarian cancer and documentation of metastatic disease was 44 months (range 3-105), and at the time of diagnosis of distant disease, 36 of 50 patients (72%) had other sites of disease (intra-abdominal or extra-abdominal). Median survival from diagnosis of distant disease was 12 months (range 1-58). In univariate analysis performance status (P = 0.03), the presence of other sites of disease (P = 0.04) and interval time between diagnosis of ovarian cancer and documentation of distant metases (P = 0.03) were the only factors significantly associated with survival. Long interval time remained significant for prognosis in multivariate analysis also (P = 0.04). Distant metastasis consistent with stage IV disease is a late complication that occurs in about one third of ovarian cancer patients. Prognosis after documentation of distant metastases is poor. We conclude the most important prognostic factor associated with survival is the interval time between diagnosis of ovarian cancer and documentation of distant metastases.
