[Hospital emergencies in drug addiction. Analysis and interpretation of data recorded at 3 DEAs of the Turin area].

The present research forms part of a wider research organised as part of an NRC sub-project on the psychotherapy of drug addiction. It aims to examine drug addiction services in order to assess the interrelations between these services, their ways of operating and the use drug addicts make of them. An attempt is also made to identify the response models offered by these structures and their relation to the specific demand by the drug addicted users, the aim being to show that the initial response has in itself a largely unexploited capacity to direct the requirements of the drug addicted patients.

Diagnostic and therapeutic problems in the psychopharmacological treatment of the elderly patient.

Ageing is certainly one of the most important current problems; in fact, according to statistics, the old population of the world is continuously increasing. Depression represents the most frequent psychiatric problem in the aged, if it is considered that almost 25% of the old population are affected by it. An early diagnosis and a proper treatment are crucial problems. Because of the numerous side effects of tricyclic antidepressants, today we are able to use some third-generation antidepressants which show unquestionable advantages in the therapy of depression in the elderly. Trazodone is one of these antidepressive drugs and represents a real leader among the antidepressants. Our care is to work so that these recent and efficient drugs may be correctly and properly utilized by physicians prepared to face this often totally new task.

Maprotiline: an antidepressant with an unusual pharmacological profile.

Maprotiline, which differs from other typical tricyclic antidepressants for its tetracyclic structure, is a highly selective inhibitor of norepinephrine reuptake. Despite its in vitro and in vivo inhibitory activity on NE uptake, 21 repeated daily injections of maprotiline (20 mg/kg i.p.) neither attenuated the norepinephrine-stimulated cAMP accumulation nor reduced the number of beta adrenergic recognition sites in rat frontal cortex. Also, the number of brain serotonin2 recognition sites labeled by [3H]ketanserin remained virtually unchanged in rats receiving 21 daily injections of maprotiline. [3H]Desmethylimipramine and [3H]mianserin specific binding sites were also unmodified by repeated maprotiline injections. However, after 3 weeks of daily administrations, maprotiline elicited a significant decrease in the number of [3H]flunitrazepam binding sites and a decrease in the apparent affinity of [3H]beta-carboline ethylester binding to crude synaptic membranes prepared from hippocampal and hypothalamic homogenates. These changes appear to be unrelated to modifications in the concentration of endogenous gamma-aminobutyric acid present in the membrane preparation, since the addition of 100 microM bicuculline to the incubation mixture decreases [3H]flunitrazepam binding to the same extent in saline- and maprotiline-treated rats. It is suggested that repeated maprotiline injections may elicit an increase in the hypothalamic and hippocampal tissue levels of an endogenous substance(s) which binds to the benzodiazepine/beta-carboline recognition sites.

[Services for prevention, cure and rehabilitation of drug addiction in the Vercelli and Novara provinces].

The drug addiction prevention and treatment services in the provinces of Vercelli and Novara are examined. The aim of the present work was to study an area in a transitional stage, where agricultural and industrial work and society is changing as are cultural values but where there is sufficient prosperity both to encourage the spread of drug addiction to permit the establishment of appropriate care. The local services for the prevention and cure of drug addiction and the rehabilitation of addicts were set up in 1978. The present work aims to assess the current situation and function of those services.

[Psychoneurosis and drug abuse].

In a short communication addressed particularly to medical doctors, the Authors make a statement about the concepts of psychoneurosis and psychosomatic disease, taking into consideration the hypothesis of a psychopharmacological treatment. The Authors consider the drug as a complementary tool in the relationship between the doctor and the patient, useful in making easier the overcoming of the conflicts and as a coadjuvant in a strategy of both psychopharmacology and psychotherapy.

[Clinical and biochemical contributions on the action of maprotiline and clomipramine in depressive states (preliminary observation)].

This study has been conducted on 17 patients suffering from neurotic and psychotic depressions, who had not received a pharmacological antidepressive therapy since at least 1 month. 6 patients suffering from psychotic depression were treated with 100 mg clomipramine i.v. daily for 20 days; 11 patients, 7 of whom suffered from psychotic depression and 4 from neurotic depression, were treated with 150 mg maprotiline i.v. daily for 20 days. In both groups the therapy has been continued orally. In 17 patients a remarkable improvement and even in some cases complete recovery has been obtained. The two psychotropic antidepressive drugs are, furthermore, both active in the same symptoms. The determination of serotonin in human platelets confirms the strong inhibition of its re-uptake by clomipramine and the absence of re-uptake of 5-HT with maprotiline. A constant and progressive increase of 5-HIAA was noted in patients treated with clomipramine. This increase may be considered as a consequence of the increased metabolism of the larger quantity of 5-HT used by the central synapses. The interpretation of the increased levels of 5-HIAA in patients treated with maprotiline is more difficult. Further trials, for instance on the plasmatic tryptophan level, will perhaps solve the problem.

Peripheral-type benzodiazepine receptors and diazepam binding inhibitor-like immunoreactivity distribution in human peripheral blood mononuclear cells.

Peripheral-type benzodiazepine receptors (pBZr) in human lymphocytes have been detected only in mixtures of peripheral blood mononuclear cells (PBMC). The present investigation was designed to describe precisely the location of pBZr in the various sets and subsets of PBMC, purified using monoclonal antibodies to specific PBMC surface markers. Site densities and affinities of pBZr were measured in the intact cells by conventional binding, using 3H-PK 11195 as a ligand. Moreover, we used a specific radioimmunoassay to identify in these cells the presence of the polypeptide diazepam binding inhibitor (DBI), a putative endogenous ligand for various benzodiazepine receptors including the peripheral type. Two major findings are derived from these studies: first, the coexistence of pBZr and DBI, or closely related immunoreactive material, in all major lymphocyte sets and subsets, as well as in monocytes. And second, the significant correlation (r = 0.87, p < 0.001) observed between the density of pBZr in a given cell type and its abundance of DBI like-immunoreactivity (DBI-LI). For both pBZr and DBI-LI content the cell distribution was monocytes > B cells and large granular lymphocytes > T cells (CD3+ set or CD4+ and CD8+ subsets) (ANOVA: pBZr: F = 114.11, p < 0.001; DBI-LI: F = 20.79, p < 0.001). The results are discussed in terms of the possibility that DBI and pBZr might share a relevant interaction in immunocompetent elements, thereby contributing to a new route of connection between the immune and the nervous systems.

Bulimia nervosa with and without obsessive-compulsive syndromes.

The present study was performed in a group of bulimic (BN) females (1) to assess prevalence rates of comorbid obsessive-compulsive phenomena; (2) to investigate whether BN patients display a characteristic cluster of obsessive-compulsive symptoms; and (3) to determine whether obsessive-compulsive symptoms influence the clinical picture of BN. Thirty-eight DSM-IV BN females were interviewed by means of the Structured Clinical Interview for DSM-III-R (SCID) to assess the prevalence rate of obsessive compulsive disorder (OCD); the Yale-Brown Obsessive-Compulsive Symptom Scale (Y-BOCS) Symptom Check-List was also used to evaluate the presence of obsessive-compulsive symptoms. The phenomenology of BN females with obsessive-compulsive syndromes (OCS) as detected by the Y-BOCS was compared to that shown by a "control" group of nonbulimic OCD females. Finally, the eating-related psychopathology of BN women with and without OCS was compared. The current prevalence rates of OCD and of subthreshold obsessive-compulsive syndrome (sOCS) in our sample were 10.5% and 15.8%, respectively. Thus, a total of 26.3% of BN females had a current OCS that comprised both clinical disorders and subthreshold syndromes. No differences were detected between obsessive-compulsive symptoms of these females and those of the control group of nonbulimic OCD females. BN females with OCS had higher ratings on the Eating Disorder Inventory (EDI) total score and on the "drive for thinness" and the "bulimia" items of the scale, as compared to BN females without OCS. In conclusion, it appears that a considerable proportion of BN females display OCS, which sometimes are not severe enough to fulfill diagnostic criteria for OCD. Moreover, in these patients, obsessive-compulsive symptoms are undistinguishable from those of OCD females, and exert a negative influence on the clinical picture of the bulimic disorder.

[Schizophrenia and multiple sclerosis].

On the basis of published data on the frequent incidence of psychiatric disorders in demyelinating diseases, the paper report the case of a 22 year old patient who was brought to the Authors' attention with a schizophrenia-type disorder, with prevalently negative symptoms, and suffering from the advanced stages of a demyelinating pathology. Anamnestic and clinical characteristics reported by other Authors were also evident in this patient: the absence of familiarity for schizophrenic disorders, the absence of earlier psychiatric pathologies, the constant progress of psychosis and limited response to pharmacological therapy. The main results of clinical and instrumental tests are discussed.

[Mental retardation and psychiatric pathology. Statistical investigation and follow-up].

The Authors describe the organization and aims of the rehabilitation institute "G. Ferrero" of Alba, Cuneo district, Italy. Afterwards they present, analyse and compare 34 items regarding patients over 14 years of age that were present on the 30 November 1978 and 1988. The most important data are: 1) a large rise in average age and stay at the institute; 2) a similar rise in the mental retardation level; 3) the lack of link between handicap level and possibility for the subject to go back to the original family; 4) a considerable increase in the subjects with psychosis or cerebropathia; 5) the poor prognosis for psychotic subjects, that are affected 10 years after and almost never succeed in going back to their original family or in finding a job; 6) big changes that changing types of patients produced in the institute's aims and in treatment, with partial renunciation to rehabilitation and development of nursing and ergotherapy.

[Major depression and "deterioration" of intellectual function].

Forty five subjects with major depression or bipolar disorder were studied during the symptom-free period using the Wechsler and Rorschach tests. The tests revealed a reduced capacity to utilise data obtained by analysis and to integrate them by the organisation of complex thought processes as well as frequent loss of contact with reality. This finding seems to demonstrate that the apparently symptom-free periods of depression often display alterations in the capacity for thought.

[Laughter and smiling. The gesture between social philosophy and psychobiology].

The human individual experiences his gestuality unaware, making it even more revelatory of his current state of mind. Certain actions represent innate behaviour, while others are discovered or assimilated during learning. The actions that signify the sending of any kind of message to another person, become "gestures". Laughter is an example of expressive behaviour and, as such, plays an important part in social interaction, insofar as it supplies signals that influence the response of other individuals. After a review of the philosophical and sociological interpretations inherent in laughing and smiling, the correlations between weeping, smiling and laughing are examined specifically from as ethological standpoint. The neurophysiological aspects whereby the crying, smiling and laughing mechanisms depend on a multifactorial control system, that implies the activation of limbic and frontal circuits, a motor cortex response and an activation of the autonomic response of the vegetative nervous system, correlated to the cortico-hypothalamic ways, are then examined. The pathological observations carried out in Man are identified with the study of psychomotor epilepsy in the context of "gelastic crises", cataleptic-gelolegic crises "and the pseudo-bulbar syndrome" with crying and spastic laughing. Finally, the possible therapeutic implications correlated with the laughing mechanism are discussed. In fact, the facial contractions, associated with the emotional experience connected with them, would appear to function as a standard brain signal. Interesting are the findings of changes in neuromediator and endorphin modifications implied in the neurophysiological smiling and laughing mechanism. A further basic element is the involvement of the autonomous system which can be correlated with the concept of the trophotropic response according to Hesse, with an increase in parasympathetic tone and reduction in sympathetic tone.

[Use of first and second generation antidepressants. A clinical study].

The study assesses the quantitative percentage use of second generation antidepressants (non-tricyclics or modified tricyclics) in clinical psychiatric practice in comparison with first generation products. The study was conducted on patients presenting in the surgery and psychiatric ward of Novara and in Turin Psychiatric Clinic in an-June 1987 and covers all patients receiving antidepressant drugs. For each individual given thymoanaleptic drugs, the type used and the reasons for its selection were examined as well as the combined use of antidepressants with other psychotherapeutic drugs. The data show that second generation antidepressants have come to play a major therapeutic role, though further research is needed to define their specific use and as far as possible to eliminate their use as a "vicarious" substitute for traditional drugs.

[Epidemiologic study of functional disorders in the elderly].

Numerous observations have evidenced the high prevalence of psychosomatic disturbances in the third age, so much so that this type of pathology is second in order of frequency in the elderly. In the light of these data a study has been carried out on functional disturbances in the third age related to certain psychological and sociobiological variables. The purpose of the research was to assess the effective influence of these "extrinsic" variables in the psychosomatic disturbance observed through the psychophysiological test carried out in the different groups examined. The results of the study are discussed and some keys for interpreting the data observed are put forward.
