Hand-assisted laparoscopic nephrectomy versus transperitoneal anterior approach in living donors for renal transplantation.

The objective of this study was to compare two surgical approaches for living donor nephrectomy: transperitoneal anterior approach and the hand-assisted laparoscopic nephrectomy. Between January 2001 and October 2003 we performed 63 kidney transplantations from living donors. The transperitoneal anterior approach was used in 36 cases and the hand-assisted laparoscopic nephrectomy in 27. Outcomes were compared in terms of hospital stay, postoperative analgesia, and graft quality. Mean hospital stay was 4.7 days in the transperitoneal anterior approach group and 3.7 days in the hand-assisted laparoscopic group (P < .005). Postoperative analgesia dosage was significantly lower in the hand-assisted laparoscopic group (P < .001). Surgical complications and graft quality were similar. We concluded that hand-assisted laparoscopic nephrectomy patients had shorter hospital stays and less pain in the postoperative period, with better cosmetic results and equivalent graft quality compared to transperitoneal anterior approach patients.

A model of suture-induced urolithiasis with urographic control in the bladder of the rat.

The characteristics of a model of urolithiasis in the bladder of male Wistar rats were evaluated. Two techniques were compared: one knot of suture material through the bladder wall, or 15-throw knots inside the bladder. Rats were randomly assigned to one of six groups, according to the technique and suture material used (polyester, silk, or chromic catgut). An excretory urogram was performed at days 30, 60, and 90. At the moment of positive radiographic diagnosis, animals were sacrificed and stones were obtained. All rats were sacrificed at day 90 regardless of the x-ray results. Variables evaluated were average days to a positive urogram, average weight of all uroliths, and percentage of animals developing urolithiasis throughout the study. There were no statistical differences between groups for the average weight of stones, which ranged from 0.008 to 1.502 g for individual cases. All three materials were more lithogenic with the 15-throw knot inside technique. No significant differences were detected for lithogenesis criteria between suture materials. X-ray dispersion spectroscopy evidenced calcium phosphate composition of two stones from chromic catgut, and magnesium phosphate composition of two stones from chromic catgut, two from silk, and two from polyester. Neo-epithelization over the suture was confirmed microscopically in bladders with the one-throw technique. We conclude that (1) this is an effective model of urolithiasis, with predictability in time, composition, and amount of stone formation; (2) the presence of more suture material inside the bladder is more lithogenic; and (3) excretory urograms are highly sensitive and specific for the noninvasive diagnosis of bladder urolithiasis.

Congenital brachymetatarsia: three cases.

We report on three cases with congenital brachymetatarsia, one with bilateral affectation, together with the results of a lengthening of the short metatarsal bones by progressive axial distraction using an external minifixator. The method proved to be easy to implement, permitting immediate functionality of the ankle and early load bearing. The immediate results (cosmetic) and later findings (functional) were excellent in all three cases.

[Colpocytology in a preventive gynecological ambulatory service].

BACKGROUND: evaluate the results of Pap smear in women attended at a gynecology preventive ambulatory (University of São Paulo Medical School Clinical Hospital). METHODS: 6821 women were submitted to a medical interview, clinical and gynecologic exam. Cervical and vaginal cytology (Pap smear) were analyzed according to the Papanicolaou method and classification. All women were grouped according to their age into three categories:under 40 years old, between 41 and 60 years and over 60 years. RESULTS: most of them, and mainly the younger ones, had been submitted to a previous Pap smear in a period shorter than 1 year (44.2%). The majority of women recognized the usefulness of the exam and knew its interval; the group that best knew its importance and interval was the one with ages between 41 and 60 years, while most women over 60 years did not know both its importance and interval. Cytological material was considered insufficient for analysis in 15,1% and inadequate in 1.1%, and for those with adequate material results were classes I (21.7%), II (59.9%), III (2.0%), IV (0.1%) and V (0.1%). Distribution of cervical intraepithelial neoplasia (CIN) were similar in the three groups. The main microbiologic findings was Gardnerella sp. (8,6%) and Human papillomavirus (HPV) incidence was significantly lower among women over 60 years. CONCLUSION: Cytological diagnoses of neoplastic modifications were performed in 2,2% and Gardnerella sp. was the most prevalent microbiologic agent. Distribution of HPV showed a decrease with age. Older women had lower knowledge on the importance of regular Pap smear examination.

Post-chemonucleolysis computed tomographic findings.

The CT findings observed 6 months and 2 years after chemonucleolysis in 50 and 34 patients are reported. Emphasis is placed on the persistence of the hernia image in nearly 70% of the cases, together with an increase in degenerative intradiscal signs and degeneration of the facet joints; however, this did not lead to poor results 2 years after chemonucleolysis.

[Comparison of different methods for species identification of genus Candida].

Different species of genus Candida can cause a wide range of pathologies, since mucocutaneous trivial infections to disseminated serious forms. Traditionally, taxonomy of yeast has been performed taking into account morphologic and physiologic studies, but they depend on the culture conditions of strains, what cause certain difficulties. Thus, recently, molecular biology methods have been tried. The aim of this work is to correlate taxonomic studies of most important pathogenic species--C. albicans, C. krusei, C. parapsilosis, C. tropicalis and C. glabrata--all of them performed by phenotypic traditional methods, commercial ones, and by a molecular method, PCR fingerprinting. Comparing useful methods for C. albicans identification, corn flour agar and germinative tube formation, no statistical differences between them are observed (chi2 = 0.5, p = 0.4795). By comparison between methods to discriminate different Candida species, physiological tests, CHROMagar, API 20C and PCF fingerprinting we observed no significative differences in proportion of accurate results, in test that can identify any Candida species, such as physiological assays, API 20C and PCR fingerprinting. The proportion of unequivocal results is greater than the obtained performing the CHROMagar culture method (p < 0.001).

[The use of calcofluor white for studying Malassezia species by direct microscopy].

Fungi of Malassezia genus are known as normal flora in human beings. However, different pathologies due to Malassezia, have been described. Traditionally, early diagnosis was delayed because of the difficulties in culture isolation of these organisms. The aim of this work, is to evaluate the technique of observation microscopy with calcofluor, for identification of Malassezia in both, clinical samples and isolates. In comparison to traditional method of direct examination with lactophenol-blue, calcofluor method offers an advantage because it turns easier the observation of fungal elements and its budding pattern. This technique contributes then, to identify species of Malassezia. The analysis of clinical specimens with calcofluor followed by observation under fluorescence microscopy is a simple and rapid method for the identification, and contribute therefore to the early diagnosis.

Lymphoproliferative diseases of fowl: characterization of transplantable G-B1 Marek's disease tumor cells in culture.

A transplantable Marek's disease tumor, derived and maintained through in vivo passage in syngeneic G-B1 chickens, was established in cell culture and characterized. The cells (GBT cells) grew in suspension and appeared morphologically similar to other Marek's lymphoblastoid lines except for prominent nucleoli in GBT cells. The number of chromosomes increased during in vitro cultivation from near triploid to subtetraploid. Chromosome no. 3 was abnormally short. The GBT cells retained surface B1 histocompatibility antigen of the original tumor as well as Marek's disease tumor-specific surface antigen (MATSA); however, infectious Marek's disease virus was not detected by immunofluorescence, electron microscopy, inoculation into susceptible birds, or assay on susceptible cells in culture. Inoculation of as few as 100 cultured cells into syngeneic G-B1 chickens results in tumor formation at the inoculation site, with eventual death of the recipients.

[Radiological contributions in perennial rhinitis].

We considered over 500 cases of Perennial Rhinitis (PR). A radiologic study has been performed to determine mucosa alterations. The percentage of allergic PR is found to be of forty-eight percent. Radiologic alteration are found in 43% of non allergic PR. We considered also addition factors such as sex, age, occurrence of asthma and levels of IgE.

[Evaluation of social functioning in patients at a health center].

The Social Functioning Scale (SFS) is described, together with its rationale and development. The instrument consists of 35 items, and was administered to a sample of 320 adult patients of a health center. Social functioning is assessed in a semistructured, 30 minute interview. The patient reports the level of satisfaction related to his/her role performance in five major areas: occupational, social relationships, economic, marital and familiar. Reliability and validity have been established for the SFS. The reliability coefficients for all areas were over 0.80 (p = 0.01). A varimax factor analysis was applied and five factors emerged for the construct validity. This accounted for 52 percent of the total variance. The use of the SFS is recommended since its validity and reliability have been demonstrated. The instrument could be useful for Mexico, and also for other Spanish speaking countries.
