Recurrence and heterogeneity in painless thyrotoxic lymphocytic thyroiditis. Report of five cases.

Five patients had recurrent episodes of spontaneously resolving thyrotoxicosis (SRT). Three displayed only the classic low-intake variety of this disorder, while two had alternating episodes of high-uptake and low-uptake thyrotoxicosis. Two of the three with low uptake only who had thyrotropin displacement activity (TDA) assays had negative results on this test, while one of the patients with alternating high- and low-uptake thyrotoxicosis had a positive TDA assay result, suggesting that the pathogenesis in the two types of SRT may differ.

Pituitary enlargement and primary hypothyroidism: a report of two cases with sharply contrasting outcomes.

Primary hypothyroidism may be associated with pituitary enlargement. Two cases of pituitary enlargement with suprasellar extension associated with primary hypothyroidism are presented. In one patient, prompt regression of the pituitary enlargement occurred with replacement doses of L-thyroxine. In the second patient, there was progressive enlargement of the pituitary gland while the patient was on L-thyroxine therapy with documented normalization of serum levels of thyroid-stimulating hormone. We recommend repeat evaluation of all patients with pituitary enlargement that is thought to be secondary to primary hypothyroidism to document regression of pituitary size after L-thyroxine therapy.

Cushing's syndrome associated with functional black adenoma of the adrenal cortex.

A 35-year-old woman who had bone pain in the extremities was found to have Cushing's syndrome, for which she had adrenalectomy. Pathologic examination revealed a solitary black adenoma. Bone scan and roentgenograms showed regional osteoporosis of the extremities and avascular necrosis of the hip. Her cushingoid manifestations and bone pain resolved after adrenalectomy and vitamin D therapy.
