Polymyositis after ciguatera toxin exposure.

Biopsy-proved polymyositis subsequently developed in two patients who were severely poisoned by ciguatera fish toxin. Ciguatera toxin may have several mechanisms of action and may represent more than one toxin. The patients' clinical courses and the unlikelihood of coincidence of contracting both diseases suggested to us a causal relationship. Although we cannot prove this relationship, we suggest a mechanism by which the toxin predisposed the muscle to inflammation.

Treatment of age-associated memory impairment with guanfacine.

Alpha2 adrenergic agonists have been shown to improve memory test performance in amnesic humans and aged nonhuman primates. In a group of drugs in this class that were tested for their effects on age-related memory impairments in aged monkeys, guanfacine was the most effective for improving mnemonic function at doses that were without significant side effects. These data prompted studies of guanfacine for its effect on learning and memory in persons with age-associated memory impairment (AAMI), the results of which are now reported. The data suggest that guanfacine may have modest mood-improving effects but had no significant effects on learning and memory in the subjects tested.

Neurologic signs in senescence.

We examined 2,029 volunteers 50 to 93 years of age in a cross-sectional study of nine bedside neurologic tests to determine the frequency of "abnormal" responses in uncomplicated aging (senescence). Rates of abnormal responses remained constant until age 70 years, after which they increased significantly. The number of abnormal signs per subject also increased, especially over 70 years of age. These results provide normative data against which these signs may be compared when applied as a clinical screening battery for diffuse cerebral dysfunction.

Pursuit gaze and the occipitoparietal region. A case report.

A patient lost smooth pursuit gaze to the side opposite a right occipitoparietal hemorrhage. Saccadic gaze was preserved. Pursuit gaze broken by saccades was present toward the side of the lesion. Optokinetic nystagmus (OKN) was defective when a tape was moved toward the patient from the hemianopic fields. Within two weeks, the contralateral pursuit gaze defect had cleared; however, the visual field and OKN defects and the ipsilateral pursuit gaze abnormality persisted.

On the notion of doll's eyes.

The descriptor doll's eyes is used (and sometimes abused) in reporting clinical signs at the bedside. Originally coined by Bielschowsky, its meaning is often compromised by prior experience with dolls that exhibit human pathology as part of their normal condition. Radiographic examination of antique dolls provides insight into their mechanism of horizontal conjugate gaze, a function rarely encountered in contemporary dolls.

Dandy-Walker malformation in identical twins.

The mother of a stillborn anencephalic child subsequently had twins with macrocephaly due to a cystic malformation of posterior brain structures, compatible with the Dandy-Walker syndrome.

Remission of the Lambert-Eaton syndrome and small cell anaplastic carcinoma of the lung induced by chemotherapy and radiotherapy.

A patient presented with the Lambert-Eaton syndrome and small cell anaplastic carcinoma of the lung was subsequently diagnosed. Treatment with cytotoxic chemotherapy and radiotherapy resulted in remission of both the cancer and the neuromuscular disorder. This case represents the first documented remission of the Lambert-Eaton syndrome following nonoperative treatment of the primary malignancy and emphasizes the therapeutic value of early diagnosis of this disorder.
