Estimation of time as a measure of pain magnitude.

A new method of measuring pain is presented where the patient's estimation of time is used as a pain-related parameter. Pain patients experience a certain time period as increased when compared with pain free control subjects. The increase in time estimation correlates roughly with the clinical estimate of pain severity in different diagnostic groups. Successfully treated patients change their time estimation towards normal. It is considered that this method could be used to evaluate a certain patient's pain and the effectiveness of treatment in relation to the mean time estimate of other patients with the same diagnosis.

Surgical decision in the treatment of extradural haematoma.

Extradural haematomas (EH) do not always present with the classical picture of skull trauma followed by a lucid interval, deepening coma, unilateral enlarging pupil and hemiplegia. Symptoms and signs can be puzzling and complex in many cases of EH and may thus lead to dangerous therapeutic delay. Uncertainties also arise concerning the length of the postoperative intensive care in unsuccessful cases. In this study the variables which from clinical experience seemed to be of main prognostic importance were related to the outcome in 44 patients with EH. A surgical decision-making table was compiled from these relationships in order to facilitate the decision process related to the diagnosis and therapy of EH.

Is the vasospasm following subarachnoidal hemorrhage an immunoreactive disease?

Subarachnoidal hemorrhage (SAH) from an arterial aneurysm is often followed by vasospasm which may lead to severe or even fatal ischemic brain lesions. The cause of the vasospasm is still unknown. In the present study it is shown that patients with SAH and roentgenological and/or clinical vasospasm have a significantly higher frequency (52%) of circulating immune complexes in the blood than patients with SAH without spasm (9%). This finding indicates that the vasospasm following SAH may be elicited via an immunoreaction.

Surgical decision analysis. Methodological aspects of statistical evaluations in therapeutic trials.

This report aims at discussing some of the sources of error and pitfalls involved in evaluating the efficiency of alternative modes of therapy in clinical materials. One of the main obstacles in such analysis is that the therapeutic groups are basically different due to the original selection of patients. In a previous paper (Pellettieri, 1980) a special method was worked out to circumvent this problem. The method requires that the variables of main prognostic importance can be identified. Patients in the different therapeutic groups who have identical patterns of combination of variables (risk profiles) can then be compared and the efficiency of different treatment programs can be evaluated. We believe that this method can be used in other fields of medicine.

Complications of skull caliper traction.

During a period of 20 years 88 patients with fracture dislocation of the cervical spine were treated by attaching Crutchfield tongs to the skull. Sixty-seven per cent of all patients had some complications. The different types of complications are reported and discussed in order of frequency.

Liposarcoma of the meninges. A case report.

A report on a liposarcoma in the meninges of a 70-year-old woman is presented. Pre-operative clinical and angiographic findings indicated that the tumour might be a meningeoma. Pathological examination revealed a predominantly lipoma-like, well-differentiated liposarcoma with round-cell and pleomorphic areas. The angiographic and pathological differential diagnosis are discussed.
