"Baby oh baby"--advances in assisted reproductive technology.

It is estimated that one couple in six in the United States has to deal with issues of infertility. It is assumed that worldwide rates are comparable. In 35% of cases, the infertility is caused by female reproductive problems, in 35% by male reproductive problems, in 15% by multiple factors and in 15% the cause is unknown. Medical and scientific advances in Assisted Reproductive Technology (ART) have created 12 different pregnancy producing options for infertile couples. An ART infant could have as many as five parents (i.e. a donor father, a donor mother, a surrogate or gestational mother, and the couple actually rearing the child). These technical, medical, and moral complexities have resulted in a nightmare of accompanying legal complexities: anonymous donors versus those with identification disclosed, parental rights, grandparental rights, the rights of siblings and of the extended families; sperm, ovum and embryo "ownership", custody, visitation and inheritance rights and multiple other issues challenge a system of laws that evolves far slower than the technological realities to which it applies. This presentation will describe Assisted Reproductive Technology advances and the legal implications inherent in them. Case histories will be discussed.

Sentencing diversion programs for A&D--improving their imperiled future.

Self-proclaimed U.S. "Conservatives" have become much more forceful in attacking all government social support programs, including cost-reducing sentencing diversion programs, as failing to unambiguously reduce conveniently assigned behavioral parameters or other behavior assumed to be causally related. Public anxiety and continuing need to feel "something is being done" is thus manipulated to divert anger away from the elected officials' understandable inability to solve complex, multifactorial problems. Several cases will illustrate how court-mandated attendance in Alcohol and Drug programs might better identify and respond to the frequent concerns by this population, about sexual dysfunction and/or high or high risk behaviors, hopefully promoting client/patient involvement in the process of treatment and potential change.

Sexual dysfunction with antihypertensive drugs.

The relationship of antihypertensive drugs have a long history of association with sexual dysfunction; however, this relationship is poorly documented. There appears to be a higher rate of sexual dysfunction in untreated hypertensive men compared with normotensive men. Sexual dysfunction increases with age and is associated with physical and emotional symptoms. There are few studies assessing sexual dysfunction with female and African-American hypertensive patients. Sexual dysfunction is associated with impairment of quality of life and noncompliance. Since group data may hide individual drug effects, baseline data should be collected on all patients before initiating therapy with any antihypertensive agent. Although questionnaires may not provide objective information on sexual dysfunction, the response rate to direct questioning may be less than the response rate on a questionnaire and may be affected by the gender or race of the interviewer. Research protocols using a double-blind, placebo-controlled design should assess sexual dysfunction in men and women in a standardized fashion.

Patient-therapist sex: 'just say no' isn't enough.

This article focuses on the incidence of patient-therapist sex and the generally negative consequences for both parties; the characteristics of therapists against whom claims are laid; and the legal and ethical issues arising from such relationships. The authors conclude that prevention, in the form of more realistic and adaptive training programmes, is a far better approach to this problem than 'postvention' and the punishment of transgressing therapists.

Is there sex after the prison door slams shut?

Despite the growing frequency of treatment-resistant STDs, North American literature and practice continue to stress repression and to evidence denial of prison sexuality. The authors outline their observations concerning prison sex, including officer-inmate sex. (Since this article was submitted, officer-inmate sex has received widespread public attention in Georgia, USA.)
