Occupational exposure to formaldehyde and histopathological changes in the nasal mucosa.

To study the cytotoxic effect of formaldehyde on the human nasal mucosa 75 men with occupational exposure to formaldehyde or to formaldehyde and wood dust, were examined, looking particularly at early signs of irritative effects and histopathological changes in the nasal mucosa. All men underwent a medical examination and a nasal biopsy specimen was examined by a pathologist and graded from 0-8 according to the morphological changes. A high frequency of nasal symptoms, mostly a running nose and crusting, was related to exposure to formaldehyde. Only three men had a normal mucosa; the remainder had loss of cilia and goblet cell hyperplasia (11%) and squamous metaplasia (78%); in six cases (8%) there was a mild dysplasia. The histological grading showed a significantly higher score when compared with unexposed contents (2.9 v 1.8). There was no dose response relation, no malignancies, and no difference in the histological score between those exposed to formaldehyde or to formaldehyde and wood dust.

Occupational formaldehyde exposure and the nasal mucosa.

This study was undertaken to evaluate the possible cytotoxic effect of formaldehyde on the nasal mucosa in man. 38 men with an average age of 38 years and average exposure time of 10.5 years were studied. They were exposed to formaldehyde when processing laminae. All men passed a medical examination and a nasal biopsy. The histological findings were scored, 0-8, according to a system proposed by Torjussen. The findings were compared with a non-exposed reference group of 25 men (mean age 35 years). 35% of the exposed were smokers compared to 48% of the nonexposed. The average histological score was 2.8 for the exposed versus 1.8 for the non-exposed (p less than 0.05, Wilcoxon). The more common findings in the exposed were loss of cilia, goblet cell hyperplasia and squamous metaplasia. In four cases (11%) there was a mild dysplasia. Smoking had a slight modifying effect on the histological picture. The results indicate that the suspected precancerous findings in animal studies after exposure to formaldehyde might be present in workers exposed to formaldehyde at relatively low levels.

Vestibulo-oculomotor disturbances caused by industrial solvents.

Animal experiments show that intoxication with hydrocarbon solvents influences the vestibulo-oculomotor reflex arc and indicate that the solvents block the inhibition of the vestibulo-oculomotor reflex presumably exerted by the cerebellum. The blood concentrations necessary to elicit oculomotor disturbances in animals were smaller than those that disturbed the vestibular function. Accordingly, in human experiments styrene and toluene exposure did not elicit any positional nystagmus but caused an increased saccade speed and a diminished visual suppression of vestibular nystagmus. Some cerebellar clinical pathologic process in patients who suffer from a psycho-organic syndrome caused by solvents leads us to believe that the cerebellum might be the most vulnerable part of the brain. Some patients had a positional nystagmus, but the most prominent pathologic signs were elicited by the visual suppression test. Our findings compare well with the increased saccade speed and diminished visual suppression in patients with cerebellar tumors and infarctions.

Benign paroxysmal vertigo in childhood.

Fifteen children with a syndrome called benign paroxysmal vertigo are presented. This syndrome is characterized by attacks of vertigo of sudden onset lasting a few minutes and in extreme cases several hours or even up to 2 days. Additional symptoms are pallor, sweating, vomiting and nystagmus. Consciousness is not impaired. The attacks usually occur in the first 4 years of life. They are recurrent, the appearance varying from several times a week to once a year, and they may cease spontaneously after only months but usually years. The most important differential diagnoses are epilepsy and posterior fossa tumour. The etiology is unknown, but there may be a vascular disturbance affecting the posterior cerebral circulation with secondary disturbances of the vestibular nuclei.

Bell's palsy - part of a polyneuropathy?

Some authors have suggested that Bell's palsy should be considered a part of a polyneuritis cranialis and others, reporting Bell's palsy in patients with diabetes mellitus, proposed that it is an overt sign of a subclinical polyneuropathy. To study whether nerves other than the facial nerve were affected, the clinical findings of 112 patients with Bell's palsy were correlated with a quantitative EMG done on an asymptomatic leg muscle. The electromyographic results obtained from these leg muscles showed changes similar to those occurring in peripheral nerve lesions, and the changes correlated well with the degree of facial palsy. The results indicate that the patients with symptomatic facial dysfunction have an acute subclinical polyneuropathy.

The immotile-cilia syndrome in a newborn infant.

A newborn infant was suspected to have the immotile-cilia syndrome. He had situs inversus, atelectasis and other pulmonary changes. A biopsy from the nasal mucosa at the age of 21 months confirmed the diagnosis and showed a lack of ciliary orientation and absent dynein arms. Chymotrypsin values, sweat test and most immunoglobulins were normal. The boy is now three years old. So far he has had rather mild respiratory symptoms. The importance of early diagnosis for management and prognosis is stressed.

Coincidence of dystrophia myotonica and pleomorphic adenoma of the parotid gland.

Two patients with dystrophia myotonica and pleomorphic adenoma of the parotid gland are described. The probability that this coincidence could be due to chance is less than 1 to 1,000 (P less than or equal to 0.001). There are several reports in the literature of the association of myotonic dystrophy with neoplasia. The reason for their association is not understood.

Acute "idiopathic" peripheral facial palsy: clinical, serological, and cerebrospinal fluid findings and effects of corticosteroids.

INTRODUCTION: The causes for peripheral facial palsy remain obscure in many patients. Evidence exists suggesting viruses, especially those belonging to the herpesvirus group, may be causative. This study was developed to evaluate this theory. METHODS: One hundred forty-seven patients with acute peripheral facial palsy of primarily unknown origin were studied. All were examined within 1 week of onset. Subsequent follow-up was undertaken until the palsy had recovered or become static. Paried cerebral spinal fluid and serum samples were obtained for serological evaluation to detect herpes simplex, varicella zoster, cytomegalovirus, measles, mumps, rubella, tick-borne encephalitis, adenovirus, Epstein-Barr virus, and human immunodeficiency virus, as well as the antibodies to Borrelia burgdorferi. RESULTS: Elevated antibiotic titers to Borrelia burgdorferi were observed in 11% of patients, whereas 9% of patients demonstrated elevated viral titers. Antibody pattern consistent with Epstein-Barr virus reactivation was present in 13%. A total of 67% were classified as idiopathic. CONCLUSION: Patients with reactivated Epstein-Barr virus were characterized by having a higher incidence of auricular pain and displayed diabetes mellitus in a higher frequency than in other groups. In the Borrelia group, neck/back pain was more common. Healing was less favorable in the Borrelia group despite an equal rate of palsy at onset and adequate antibiotic treatment. Corticosteroid treatment used in 44% of the patients did not significantly improve the functional outcome.

Patients' and cohabitants' reports on snoring and daytime sleepiness, 1-8 years after surgical treatment of snoring.

A follow-up study was performed with the aim to evaluate all individuals surgically treated by the uvulopalatopharyngoplasty (UPPP) or laser uvulopalatoplasty (LUPP) method from a whole county in Sweden, 1-8 years after surgery, by assessing snoring occurrence and daytime sleepiness, as experienced by the patients and their cohabitants. Questionnaires were mailed to all operated people (n = 457) and were returned by 91% (346 men, 69 women) preoperatively diagnosed as habitual snorers (n = 255), cases of obstructive sleep apnea syndrome (n = 110) and unspecified snorers (n = 48). In addition 345 bedpartners participated. For outcome of surgery, no significant differences in diagnoses or sex were found. Improvement in snoring occurrence was reported by 89.6% of the patients, confirmed by 92% of the cohabitants (r = 0.84, p < 0.01). The remaining snoring occurrence was significantly dependent on the surgical method and the time after operation. Excessive daytime sleepiness (EDS) was experienced by 73.3% of the patients compared to 67% reported by the cohabitants. Of 415 patients operated on, 18% were free from snoring and 25% were free from EDS. Individuals with a shorter time since surgery reported less snoring. The UPPP method gave a significantly better result than LUPP for the symptom 'snoring'.

Side effects after surgical treatment of snoring.

In a questionnaire study detailed side effects after snoring surgery were examined in 415 individuals 2-8 years after surgery. Three years later a new questionnaire was answered by those with side effects affecting taste, smell and voice (n = 74). At the first follow-up 255 had side effects of globus, regurgitation, taste, smell and voice. The globus was the most common (40%). In all spheres except the globus, a significant improvement was seen 3 years later. However, pharyngeal dryness and phlegm had a reported frequency of nearly 60%. No significant differences were seen between the uvulopalatopharyngoplasty and laser uvulopalatoplasty methods. Taste disturbances might be due to surgical damage to the nerves or oral dryness. The olfactory impairment present in 7 patients still needs to be explained.

Electronystagmographic findings in rats exposed to styrene or toluene.

A previously described experimental model for studying the effect of industrial solvents in the vestibular system of rabbits has been applied to rats. To achieve a constant concentration, the solvent was infused intravenously, dissolved in a lipid emulsion. Arterial blood levels were estimated by gas chromatography. The vestibulo-oculomotor behaviour of rats during repeated rotatory acceleration was investigated by electronystagmography. The effect of two solvents-toluene and styrene-on the rotatory induced nystagmus was examined. Both solvents caused an exaggerated reaction at arterial blood levels above 75 ppm. The investigation indicated that the rat will be suitable species for further electronystagmographic investigation of the influence of organic solvents on the vestibular system, for example in screening studies of the toxicity of these solvents.

Effects of trichloroethylene on the human vestibulo-oculomotor system.

Ten healthy volunteers were subjected to a vestibulo-oculomotor test battery before, during and 1 hour after trichloroethylene exposure. The concentration of trichloroethylene in inspiratory air was 32-78 ppm (176-429 mg/m3). The concentration of trichloroethylene in venous blood was followed throughout the experiment. The mean pulmonary uptake was estimated. Each test person was also subjected to a control experiment, breathing air free of trichloroethylene. A decreased ability to visually suppress the vestibulo-oculomotor reflex during sinusoidal stimulation was noticed during trichloroethylene exposure. One hour after exposure the test subjects showed a decreased maximum velocity of the voluntary saccade and a decreased ability to follow a sinusoidally moving target.

The effect of stimulus predictability and age on human tracking eye movements.

Human tracking eye movements were recorded in healthy volunteers of varying age. The target motion was either predictable (pure sinusoids or sinusoidal frequency sweep) or unpredictable (pseudorandomized). Gain and phase values for the tracking and the smooth pursuit eye movement signal were calculated as a function of target frequency from 0.2 to 2 Hz. Max. target velocity was 20 or 40 deg/sec. In the low frequency area the smooth pursuit gain for predictable stimulation was higher than the gain elicited by the pseudorandomized stimulation. In the high-frequency area, gain values did not differ significantly. For the predictable stimulation, the phase of the smooth component was always a lag, increasing with increasing frequency. At low frequencies of the pseudorandomized signal a phase lead was observed. At higher frequencies the lead turned into a lag that was greater than for the predictable stimulation. The young and old groups showed reduced smooth pursuit gain values, compared with the main group specially when the target waveform was pseudorandomized. The reduced smooth pursuit ability was mostly compensated for by an increased amount of saccades.

The plasticity of compensatory eye movements in rotatory tests. I. The effect of alertness and eye closure.

Fifteen voluntary subjects with a mean age of 26 years (17-39), participated in two rotatory experiments. The rotatory test used was sinusoidal harmonic acceleration (SHA) test at frequencies of 0.01-0.32 Hz. Compensatory eye movements were measured by means of EOG. Experiment A included rotation in darkness during alerting tasks (day 1), rotation in darkness with no tasks (day 2) and rotation in darkness during alerting tasks (day 3). In the alertness tests, the gain varied between 0.5 and 0.77. Phase values decreased with increasing frequency. The alertness results between days 1 and 3 did not differ in gain or phase. The gain of day 2 (no tasks) displayed significantly reduced gain at all frequencies, and a more pronounced decrease in phase than in the alertness tests. Experiment B comprised testing with eyes open (day 1) and eyes closed (day 2) during alerting tasks. These tests displayed significantly decreased gain at all frequencies with eyes closed compared with eyes open and with more negative phase at 0.08-0.32 Hz with eyes closed. The results indicate a central effect of the alertness level on compensatory eye movements, and the effect of eye closure might be explained by bio-mechanical and possibly even central effects on the eye movements. In low-frequency testing, as short a test time as possible should be used, and due to different alertness levels, the results should be evaluated with great caution.

Treatment of sinus empyema in adults. A coordinated Nordic multicenter trial of cefixime vs. cefaclor.

In sinus empyema, H. influenzae is the most prevalent pathogen in some subpopulations and in case of therapeutic failure. Cefixime, the first oral cephalosporin of the 3rd generation, is highly potent in vitro against H. influenzae. To study the efficacy and safety of cefixime in adults with acute sinusitis, a coordinated, double-blind multicenter trial was designed for purulent cases, as confirmed by antral aspiration. A total of 364 patients were enrolled in the study with 125 cases randomized to the reference group, assigned to treatment with cefaclor. Evaluation was based on clinical outcome and on antral reaspiration (86% of the cases). No significant differences between the treatment groups were found, as regards short-term or long-term clinical outcome. However, the clinical examination overestimated the therapeutic results. Only 4% of the patients were considered as failures, but the re-aspiration demonstrated remaining suppuration in 14% of all cases (p less than 0.001). Based on re-aspiration, the failure rate among patients with initial growth of pathogens was lower for cefixime (8%) than for cefaclor (20%) (p less than 0.05). Such a difference was not found among patients with growth of H. influenzae. No serious adverse reactions were recorded, but loose stools and diarrhoea were significantly more frequent in the cefixime treatment group. Five patients (2%) in the cefixime treatment group discontinued their treatment due to adverse events.
