[Intrauterine growth and its variations in different populations].

Intrauterine growth of newborns in the population of Novi Sad shows markedly low values in the median and the tenth and ninetieth percentiles between the 35th and the 37th gestation week. These data suggest a higher pathology of gravidity in the third trimester and inadequate antenatal care. After the 38th gestation week the intrauterine growth curve shows an extremely positive trend. In the tenth percentile the curve for male newborns exceeds the value of the tenth percentile for newborns in Prague, Pécs, Denver, and Aberdeen.

Pretreatment determination of isohaemagglutinin titter values in patients with malignant lymphomas and metastatic solid tumors.

Isohaemagglutinin synthesis starts 2-4 months after birth, growing progressively and reaching adult values at the age of 5-10 years. Isohaemagglutinin concentration decreases with age. Isohaemagglutinins are mostly immunoglobulins belonging to the class IgM, but also IgA and IgG. Agglutination titter shows correlation with the total concentration of those three immunoglobulin isotypes. For the time being there are few data on the isohaemagglutinin titter level in various diseases. Purpose of this work is to determine whether there are any isohaemagglutinin titter alterations in patients with neoplasia. Isohaemagglutinin titter was investigated in 177 patients treated at the Institute of Oncology and Radiology and 340 blood donors. Out of 177 patients, 31 had Hodgkin's lymphoma (HL), 89 had non-Hodgkin Lymphoma (NHL) and 57 had metastatic solid tumors (MST). Statistical evaluation included Kruskal-Wallis and Mann-Whitney tests. In all groups of patients isohaemagglutinin titters were considerably lower as compared with the healthy population (p < 1 x 10e-5). There was a significant difference in titter values (p = 0.003) between O blood group patients with NHL where anti-A1 titter was significantly lower (Med = 8; range: 1-256) compared with anti-A1 titter in patients with O blood group suffering from MST (Med = 16; range: 2-64). Anti-B titter in the same groups of patients also showed lower values (p = 0.042); in NHL anti-B titter values was Med = 4, range: 1-32 vs Med = 8, range: 1-64 in MST. In the group of patients with HL, A blood group was far more frequent (17/31) compared with the group with MST (22/57) (p = 0.02). Pretherapy determination of isohaemagglutinin titter in patients with malignant diseases shows that it is significantly lower than the titter in healthy population. Abnormally low isohaemagglutinin titter value irrespective of the type and site of the malignant tumor, points to insufficiency of the IgM-related humoral immune response, to malignancy as a systemic disease, and places isohaemagglutinins among biological markers.

Incidence of cerebral dysfunction as a parameter for decision making in the management of preterm labor.

The study presents a retrospective analysis of the data on pregnancy, delivery and cerebral dysfunction in 296 premature infants of singleton pregnancies, body weight > 1000 g, delivered at the Department of Obstetrics and Gynaecology in Novi Sad during 1997. Preterm newborns are exposed to a high risk of perinatal pathology, with 66.55% newborns from pathological course of pregnancy. Vaginal delivery--vertex presentation was registered in 69.93%, breech presentation in 6.41% and cesarean section in 23.65%. The preterm children have lower Apgar Scores and umbilical artery Ph values than term children. Cerebral dysfunction was diagnosed in 32.09%, whereas the incidence is inversely proportionate to gestational age and body weight at birth. The mode of delivery does not significantly affect cerebral dysfunction (p < 0.05) which means that intrapartal trauma is not a frequent cause of cerebral distress of preterm newborns. The labor occurring before the 34th gestational week can be performed vaginally if the fetus is either in vertex or complete breech presentation, if the course of labor is regular and cardiotocography findings within physiologic values.

[A case report of collision in twin pregnancy].

A parturient who had never been examined during pregnancy was admitted to the maternity ward at the end of the first phase of labour. Owing to a stoppage in the expulsion of the regularly rotated head, the forceps was applied. The extraction of the head was followed by the collision of the shoulders. Behind the fetus's neck, in the uterine canal, there was the head of the other fetus. The attempt of the reposition of the head of the other twin in general anesthesia was a failure. Since auscultation revealed no heart tones in any of the twins, the perforation of the head of the other fetus was performed, and in this way the extraction of the first and then of the second fetus was possible. The authors emphasize the importance of the echographic detection of the position and site of the fetuses in twin pregnancy.

[Vacuum extraction in modern obstetrics].

From 1970 to 1979 the authors analysed 1721 vacuum extractions. The method was introduced into the Department in 1965 and performed in 0.96% of cases. From 1970 to 1980 the use of VE increased from 1.97% to 4.93%. The most frequent indication for its use was the prolonged expulsion phase (49.78%). While in premature children the method was applied rarely (2.32%), in children weighing over 4000 g it was indicated in 16.44% of cases. The newborns delivered by VE proved to have increased traumatic morbidity rates (anoxia perinatalis 25.56%, haemorrhagia intracranialis 20.39%, cephalhaematoma 33.58%). The mortality of newborns delivered by VE amounted to 6.39%. There was 1.10% of unsuccessful vacuum extractions (19 parturients: in 16 forceps and in 3 cesarean section were applied). In the authors' opinion, vacuum extraction is the method of choice in the vaginal operative completion of labour: VE does not diminish the pelvic cavity or fix the position of the head of the fetus but allows the flexion of the head by natural forces. After each unsuccessful vacuum extraction it is necessary to examine its causes. The choice of the method for completing labour depends on the obstetric situation. Intracranial hemorrhages amounting to 20.39% warn the authors to reexamine the methods of their work.

[Delivery with vacuum extraction--indications, technique and complications].

This study is a retrospective analysis of data on deliveries which ended up in vacuum extraction at the Clinic of Gynecology and Obstetrics in Novi Sad during two periods: 1983-85, and 1993-95. The aim of this study was to analyze indications, technique and complications of deliveries which ended up in vacuum extraction. Out of the total number of deliveries-17,110 in the period 1983-85, 4.86% ended up in vacuum extraction. The perinatal morbidity was 10.82%, the perinatal mortality 15.13/1000. In the period 1993-95, there were 18,599 deliveries, whereas 2.65% ended up in vacuum extraction. The perinatal morbidity was 8.93%, while perinatal mortality 11.12/1000. The risk of morbidity occurring in surgical procedures can be decreased if prompt vacuum extraction is performed.
