Identification and partial characterization of an exercise-induced neutrophil chemotactic factor in bronchial asthma.

A heat-stable neutrophil chemotactic factor (NCF) has been identified in the serum of 13 atopic asthmatic subjects after treadmill exercise. Peak activity was detected at 10 min and returned to prechallenge values by 1 h. No NCF activity was detected in the sera of three nonasthmatic atopic or four normal nonatopic individuals performing the same task. NCF produced by exercise (NCFEX) had a similar time-course of release to NCF provoked by specific antigen (NCFAG). The appearance of circulating NCFEX and NCFAG closely paralleled the fall in peak expiratory flow rate/forced expiratory volume in 1 s (PEFR/FEV1). Histamine challenge in atopic asthmatics at concentrations giving a comparable change in PEFR/FEV1 to that evoked by exercise or inhaled antigen was not associated with the appearance of circulating NCF. In seven subjects NCFEX release was inhibited by prior administration of disodium cromoglycate. NCFEX and NCFAG eluted as single peaks of activity when applied separately to columns of Sephadex G-200, and both were an estimated 750,000 daltons. NCFEX and NCFAG also eluted as single peaks of activity, at between 0.15 and 0.30 M NaCl, following anion exchange chromatography on DEAE-Sephacel (pH 7.8). The isoelectric points of NCFEX and NCFAG were virtually identical (between pH 6.0 and 6.5) as determined by chromatofocusing on Polybuffer Exchanger 94. The activities of NCFEX and NCFAG were substantially reduced, in both a time- and dose-dependent fashion, after incubation with trypsin and chymotrypsin. Partially purified NCFEX and NCFAG promoted both stimulated random migration (chemokinesis) as well as directional migration (chemotaxis). These experiments indicate that NCFEX and NCFAG might be identical substances and raise the possibility that mediators by hypersensitivity are released during exercise-induced asthma in atopic subjects.

Complement receptor enhancement and chemotaxis of human neutrophils and eosinophils by leukotrienes and other lipoxygenase products.

The lipoxygenase products of arachidonic acid, 5-HPETE, 5-HETE, LTB4, LTC4 and LTD4, were examined for their capacity to enhance the expression of complement (C3b) receptors and to evoke chemotaxis of human neutrophils and eosinophils. With the exception of LTD4 all gave enhancement of C3b receptors. LTB4 and LTC4 enhanced over the concentration range 10(-7) to 10(-11) moles/l, and 5-HETE and 5-HPETE from 5 X 10(-6) to 5 X 10(-10) moles/l. The rank order of activity, as assessed by the magnitude of enhancement, was LTB4 (concentration for maximal effect = 10(-7) moles/l) greater than 5-HETE (5 X 10(-7)) greater than 5-HPETE (5 X 10(-6)) greater than LTC4 (10(-9)). High dose inhibition was observed with LTC4 and 5-HETE. Chemotaxis experiments performed in parallel over the same concentration ranges indicated that neither neutrophils nor eosinophils migrated towards LTC4 or LTD4. However, LTB4 evoked chemotaxis with a linear dose response from 10(-9) to 10(-7) moles/l and 5-HPETE and 5-HETE from 5 X 10(-8) to 5 X 10(-6) moles/l. At 10(-7) moles/l LTB4 was approximately 6 and 8 X more potent in chemotaxis than 5-HPETE and 5-HETE respectively. In general, complement receptor enhancement and chemotaxis of eosinophils were similar to that observed with neutrophils and did not vary with the patient source.

Effects of drugs on the production of stress ulcer in the rat.

The authors studied the effect of drugs with different mechanisms of action on the prevention of stress ulcer production in the rat. Stress ulcer was induced by a method developed by the authors: intact, starved rats were swum in water at 23 degrees C for 5 hours. Atropine (0.1-0.5 and 1.0 mg/kg i.m.), cimetidine (1.0-5.0 and 25 mg/kg i.p.), prostacyclin (PGI2) (5.0-25.0 and 100 micrograms/kg i.p.) and phentolamine (0.35-1.75-3.5 and 7.0 mg/kg i.m.) were shown to decrease the production of stress ulcers significantly, in a dose-dependent fashion. Propranolol (0.35-1.75-3.5 and 7.0 mg/kg i.m.) did not influence the production of stress ulcers. The finding that drugs with different actions could considerably reduce or prevent the production of stress ulcer appears to indicate the complexity of the neural, hormonal and biochemical processes involved in the pathogenesis. On the basis of the present results the authors suggest the use of a preventive therapeutic regimen in clinical practice with an appropriate combination of drugs.

Stimulation by oestradiol of soluble-protein synthesis in rat hypothalamus.

The effect of oestradiol treatment on protein synthesis in the cytosol fraction (10(5) g supernatant) from ovariectomized mature and developing female rat hypothalami were studied. Results obtained on adults by double-label technique and SDS polyacrylamide gel or cellogel electrophoresis show that, as in the uterus, oestradiol-induced protein synthesis (IP) occurred in the cytosol fraction of the hypothalamus. The molecular weight of IP was about 40 000 dalton. During postnatal development, IP was also observed in cytosol fractions from the hypothalami of female rats 14, 21 and 28 days old. No clear-cut effect of oestradiol was found in the 7-day-old animals.

Biochemistry of chronic jejunal ulcer in patients.

Separation and measurement of adenosine triphosphate (ATP), adenosine diphosphate (ADP), adenosine monophosphate (AMP), adenine-adenosine, lipid phosphates (lipid P), ribonucleic acid (RNA), deoxyribonucleic acid (DNA), further of Mg2+-dependent, total (Mg2+-dependent plus Na+-K+-dependent) and Na+-K+-dependent ATPases was carried out in control (non-ulcerated) and ulcerated (mucosa up to 2.0 cm around the ulcer) jejunal mucosa of 11 patients with chronic jejunal ulcer, who had undergone a Billroth II gastric resection for duodenal ulcer. The levels of ATP, ADP, AMP, sum of ATP + ADP + AMP, lipid-P, RNA (related to 1.0 mg DNA) and the activity of Mg2+-dependent, total (Mg2+-dependent plus Na+-K+-dependent) and Na+-K+-dependent ATPase (related to 1.0 mg membrane protein) were significantly higher in the lacerated jejunal mucosa than in the control one. The results indicate that (1) no circulatory damage can be found in the ulcerated jejunal mucosa; (2) a cellular hyperactivity (increased ATP breakdown and increased ATP resynthesis, increased RNA and lipid synthesis) occurs in the ulcerated jejunal mucosa; (3) the biochemical structure of cells differs significantly in the control and the ulcerated jejunal mucosa.

The effect of disodium-chromoglykate on human serum neutrophil-chemotactic activity in antigen induced bronchospasm.

The prior administration of disodium-chromoglykate significantly inhibited the increase of serum neutrophil-chemotactic activity in house-dust induced bronchospasm. The serum neutrophil-chemotactic activity was probably mast-cell derived, because of the protective effect of disodium-chromoglykate and it may be related to Neutrophil-chemotactic Factor isolated in human pulmonary tissue. The moderately increased chemotactic activity after prior administration of disodium-chromoglykate in house-dust induced bronchospasm may derive from circulating basophils. The cellular chemotaxis (eosinophil, neutrophil) may has main importance in the pathophysiology of asthma bronchiale. The stimulation of eosinophil-associated effects may provide a new therapeutic line.

Serum neutrophil chemotactic activity and leukocyte count after house dust induced bronchospasm.

Neutrophil chemotactic activity of the serum, leukocyte count and total eosinophil count were determined before and after bronchial provocation with inhalation of house dust or acetylcholine. Airway parameters were determined by whole-body plethysmography. The airway conductance started to return to normal between 20 and 60 min after provocation as did the chemotactic activity. The largest increase of serum neutrophil chemotactic activity was measured 20 min after challenge with house dust, and the activity did not return to the initial value within 24 h, unlike specific airway conductance which was normalized at 24 h. No alteration of the leukocyte count was found. A significant decrease of the total eosinophil count was observed at 60 min, then a 1.5-fold increase was seen at 24 h. Positive correlation was found between the eosinophil count and the lung function. A putative pathophysiological role of the neutrophil chemotactic factor of mast cell origin in bronchial asthma is suggested.

A critical analysis of the gastric secretory response of patients with duodenal ulcer in dependence of their age and duration of complaints.

The different parameters of the basal and maximal gastric secretory responses were measured in 120 patients with duodenal ulcer. The correlations between the different gastric secretory parameters [volume, H+ concentration, basal acid output (BAO), maximal acid output (MAO), maximal acid output-basal acid output, basal acid output . maximal acid output X 100] were studied in these patients, in dependence of their age and duration of complaints. A positive and mathematically significant correlation was found between the following parameters: 1. age of patients and H+ concentration of gastric basal secretion (r = 0.326; n = 120; P less than 0.001); 2. age of patients and basal acid output (r = 0.200; P less than 0.05); 3. duration of complaints and volume of gastric basal secretion (r = 0.186; P less than 0.05); 4. duration of complaints and basal acid output (r = 0.237; P less than 0.05); 5. age of patients and BAO . MAO-1 X 100 (r = 0.216; P less than 0.05); 6. duration of complaints and BAO. MAO-1 X 100 (4 = 0.197; P less than 0.05); 7. volume of gastric maximal and basal secretory response (r = 0.398; P less than 0.001); 8. H+ concentration of gastric maximal and basal secretory response (r = 0.480; P less than 0.001); 9. MAO and BAO values (r = 0.521; P less than 0.001); 10. H+ output and H+ concentration of gastric basal secretory response (r = 0.620; P less than 0.001); 11. H+ output and volume of gastric basal secretory response (r = 0.779; P less than 0.001); 12. H+ concentration and volume of gastric maximal secretory response (r = 0.298; P less than 0.01); 13. H+ output and H+ concentration of gastric maximal secretory response (r = 0.720; P less than 0.001); 14. H+ output and volume of gastric maximal secretory response (r = 0.802; P less than 0.001). A negative and mathematically significant correlation was observed between the values of BAO . MAO-1 X 100 and of MAO - BAO (r = 0.435; P less than 0.001). It has been concluded 1. duodenal ulcer disease protects the human gastric fundic mucosa against "senile decay"; 2. the changes of gastric basal and maximal secretory responses, induced by duodenal ulcer, do not supply a pathological base for gastric surgery in patients, on dependence on their ages and duration of complaints.

[Internal fixation for fractures of the lateral malleolus with titanium "crown"-plates (author's transl)].

This team of authors constructed a so called "crown"-plate from titanium, suitable for fixation of lateral malleolar fractures. The plate seems to be ideal for Weber-B type fractures, which are level with the joint space, and it can be applied in the more distal (Weber-A) and proximal (Weber-C) fractures too. The titanium plate was used in 166 fractures with good results. On the basis of the follow up assessment of 151 healed cases the authors feel encouraged to suggest the use of the titanium crown plate for internal fixation of lateral malleolar fractures.

Biochemistry of antral ulcer in patients.

Separation and measurement of adenosine triphosphate (ATP), adenosine diphosphate (ADP), adenosine monophosphate (AMP), adenine-adenosine, lipid phosphates (lipid-P), RNA and DNA, furthermore, separation of Mg2+-dependent, total (Mg2+-dependent plus Na+--K+-dependent or Mg2+--Na+--K+-dependent) and Na+--K+-dependent ATPases and measured of their activities were carried out in tissue samples obtained from "ulcerated antral mucosa", from "non-ulcerated antral mucosa" and the muscular layer beneath the examined mucosa specimens, of patients with antral ulcer. It has been found that (1) the tissue levels of ATP, ADP and AMP, of the sum ATP + ADP + AMP, adenine-adenosine, lipid-P, RNA all calculated for 1.0 mg DNA content, further, the activities of Mg2+-dependent, total (Mg2+-dependent plus Na+--K+-dependent) and Na+--K+-dependent ATPase were significantly higher in the ulcerated antral mucosa than those in the control antral mucosa; (2) no significant biochemical alterations were found in the muscles under the ulcerated and non-ulcerated antral mucosa of patients with antral ulcer; (3) the extents of increases in the substrate levels of ulcerated antral mucosa depended on their original levels in the control antral mucosa. The results indicate a cellular hyperfunction in the ulcerated antral mucosa, in comparison with the control one. No circulatory injury could be demonstrated in the ulcerated antral mucosa.

Experimental evidence for cytoprotective effect of atropine on the rat gastric mucosa.

The inhibitory effect of atropine on the gastric mucosal damage produced by topical aspirin has been studied in the rat, in the presence and in the absence of intragastrically applied 160 mmole/l HCl. The gastric mucosal damage was produced by aspirin given intragastrically in doses of 192 mg/kg. The topical application was done alone or in the presence of 160 mmole/l HCl. The ulcer incidence, the number and size of gastric mucosal damage were studied during 4 h. The aspirin solutions were administered with or without atropine (in doses of 4 and 10 mg/kg). The atropine was added immediately after the administration of aspirin. The results were as follows 1. A dose-dependent inhibition of ulcer incidence, number and size of gastric mucosal damage was produced by atropine in both groups of animals, when the aspirin was given alone or together with 160 mmole/l HCl; 2. The inhibitory effect of atropine was significantly weaker in the group treated with aspirin plus 160 mmole/l HCl than in th aspirin-treated group. It has been concluded the ulcer-preventing effect of atropine is mediated partly through the inhibition of gastric H+ secretion, and partly some "cytoprotective effect"; the extent of the "cytoprotective effect" of atropine was about half that on gastric H+ secretion.
