Classification of the severity of acoustic trauma based on pure tone threshold audiometry.

A simple-to-use classification system is presented for expressing the hearing loss resulting from acoustic trauma. They system is based upon the implications for social hearing as seen the pure tone air conduction hearing test, which is classified according to specifically segmented areas of the audiogram. A study of the audiometric results of 102 ex-army men with AcT and 25 controls with normal hearing included a detailed questionnaire to relate the patients' functional complaints to their audiograms, and the clinical picture was then completed by the administration of speech audiometry tests both in quiet backgrounds of various signal-to-noise ratios. Three main severity categories of hearing impairment were established. These are described and illustrated.

Prevalence of specific IgA and IgM anti-HBc antibodies compared with HBV DNA in the sera of HBsAg chronic carriers.

OBJECTIVE: We evaluated the significance of IgA antibodies directed against the hepatitis B virus core antigen (IgA anti-HBc) as a marker for viral replication. STUDY DESIGN/METHODS: Serum samples of 143 hepatitis B surface antigen (HBsAg) carriers and 189 HBsAg-negative subjects were studied. Hepatitis B virus (HBV) DNA was detected by polymerase chain reaction. IgA anti-HBc was determined by a capture enzyme-linked immunosorbent assay developed in our laboratory. The results were compared with those for IgM anti-HBc, which were determined by a commercially available method. RESULTS: IgA anti-HBc was detected in 57 (40%) and HBV DNA in 38 (27%) of the HBsAg carriers. Among the HBsAg-negative subjects, IgA anti-HBc and HBV DNA were detected simultaneously in four samples. All 42 HBV DNA-positive samples were IgA anti-HBc positive. IgM anti-HBc was detected in 27 (64%) of them. CONCLUSIONS: IgA anti-HBc is a sensitive marker for HBV replication, and its absence may exclude HBV replication. The role of IgA anti-HBc in monitoring response to therapy and predicting clinical course is being evaluated.

Epidemiologic methodology in evaluating health technology.

Epidemiologic methods provide a basis for evaluating new medical technologies of varying complexity and sophistication. Use is made of the concepts of efficacy, effectiveness and efficiency, and cost-effectiveness. The available methods, observational and experimental, are reviewed in the context of evaluation of the intensive coronary care unit (ICCU). Most early studies that showed a benefit in low in-hospital case fatality rates for patients with myocardial infarction in the ICCU had serious flaws in design and analysis. Better studies failed to show any benefit, and some showed an increase in the case fatality rates in the ICCU when compared with the medical wards or even with patients treated at home (for noncomplicated infarctions). Most recent studies have attempted to predict which patients should and which should not be admitted to the ICCU, in order to improve efficiency.

Validation study on medical recording practices in primary care clinics.

The validity of records of visits to physicians was studied in three primary care clinics in Beersheba, Israel. A wide variation was found between clinics in the completeness of recording and in the proportion of records that could not be found. The results suggest that data on clinic utilization obtained by an analysis of records may be an underestimate. They should be used only if completeness of recording has been tested and found satisfactory.
