Temperature, metabolic adaptation and crying in healthy full-term newborns cared for skin-to-skin or in a cot.

The aim of the present study was to compare temperatures, metabolic adaptation and crying behavior in 50 healthy, full-term, newborn infants who were randomized to be kept either skin-to-skin with the mother or next to the mother in a cot "separated". The babies were studied during the first 90 min after birth. Axillary and skin temperatures were significantly higher in the skin-to-skin group; at 90 min after birth blood glucose was also significantly higher and the return towards zero of the negative base-excess was more rapid as compared to the "separated" group. Babies kept in cots cried significantly more than those kept skin-to-skin with the mother. Keeping the baby skin-to-skin with the mother preserves energy and accelerates metabolic adaptation and may increase the well-being of the newborn.

Frequency in allergy to proteins of latex in health care workers.

OBJECTIVE: The objective of this study is to evaluate allergy prevalence to latex proteins in health care workers at the Laboratory and Surgery Room of Hospital CIMA Chihuahua. METHODOLOGY AND RESULTS: A thorough clinical chart was recorded for all health care workers studied: hematic biometry, total IgE by ELISA method, specific IgE to latex antigen by ELISA (pharmacy CAP system), cutaneous tests with latex antigen (Aphi de México, Hevea Brasiliensis Biopal Inc. Spoken WA), through scarification, together with histamine and Evans (positive-negative control). The number included is 99 individuals. Specific IgE to latex in 4 cases was positive (4%) and in the cutaneous tests to latex, 24 cases (24%) resulted positive. CONCLUSIONS: The study reports a prevalence of (4%) when performing the specific IgE to latex and (24%) to the cutaneous test with antigen to total latex. This data allows us to continue evaluating the personnel at risk at the hospital, with better results in the administration of Labor Medicine at this medical institution.

Covering the uninsured through TennCare: does it make a difference?

Tennessee created TennCare in 1994 to address the needs of "poor and uninsured citizens ... excluded from the health care system." Under TennCare, Tennessee implemented managed care in its Medicaid program and used savings anticipated from the switch to expand insurance coverage to uninsured and uninsurable adults and children. Our analysis of the expansion suggests that it improved access to care, reduced unmet need, and encouraged use of preventive services, particularly for children. These changes coincided with higher levels of satisfaction with care among TennCare beneficiaries.

High-dose mitomycin C in isolated hyperthermic liver perfusion for unresectable liver metastases.

In order to reduce systemic side effects and increase intrahepatic mitomycin C (MMC) concentrations, isolated hyperthermic liver perfusion (IHLP) has been performed using MMC. This article describes the pharmacokinetics of MMC in IHLP and presents our clinical experience with its use in six patients suffering from unresectable liver metastases. Primary tumors consisted of colorectal carcinomas in three cases, breast cancer in two, and a choroidal melanoma in one. Dosages of MMC varied between 0.5 and 1.0 mg MMC/kg body weight. MMC was added as a bolus directly into the extracorporeal circuit. Intrahepatic temperature was elevated to 40.0-41.0 degrees C by hyperthermic perfusion. MMC concentrations were measured in peripheral blood (preperfusion, then at 5, 30, and 55 min during perfusion, and finally at 5 and 60 min and 6 and 24 h after perfusion) and in recirculating perfusate (5, 30, and 55 min). While markedly elevated MMC concentrations (maximum 6290 ng/mL) were found in the liver perfusate, systemic concentrations remained low (maximum 45 ng/mL), indicating no considerable leakage. MMC concentrations in the perfusate constantly decreased during perfusion. After rinsing with 1500 mL saline, a mean concentration of 52.5+/-33 ng MMC/mL was measured in the washout from 5 patients. In 1 patient with a colorectal carcinoma, MMC concentrations in the perfusion medium were 10-fold and in the plasma 2-fold higher than in the other patients. This high MMC concentration caused severe intrahepatic vascular damage and finally led to the patient's death. In conclusion, IHLP and intrahepatic perfusion with MMC resulted in a high response of hepatic tumors. Systemic exposure of MMC can be reduced effectively by isolated perfusion. However, hepatic toxicity of MMC must be considered.

A porcine model for investigation of hyperthermic isolated liver perfusion.

Our study was aimed at developing a reliable method of hyperthermic isolated liver perfusion (HILP) in pigs and at assessing its local and systemic side effects. HILP was performed via the hepatic artery and portal vein in 15 animals. The perfusate consisted of blood (200 ml), oxypolygelatine (500 ml), Ringer's solution (1000 ml), and trapped intrahepatic blood. HILP was carried out for 45 min at a mean perfusate inflow temperature of 41.2 degrees C. The mean portal flow and pressure were adjusted to 500 ml/min and 20-25 mm Hg; the mean arterial flow and pressure were 130 ml/min and 40-60 mm Hg, respectively. After 20 min of perfusion the mean temperature in the right and the left liver lobe were 40.8 degrees C and 40.3 degrees C and remained almost constant over the whole perfusion period. Liver enzymes (alanine aminotransferase and aspartate aminotransferase) and serum lactate levels showed slight increases after perfusion but normalized within 1 week. Histology of liver parenchyma showed only mild pathological changes, which were also reversible within 7 days. The presented method of HILP is a safe and reproducible technique for isolated hyperthermic liver perfusion. Based on this animal model, experimental HILP with different chemotherapeutic agents can be investigated in order to assess hepatic and systemic toxicity of this therapy.

[A comparison of complete and truncated birth histories to measure fertility and child mortality].

"During the latter part of 1986, national probability sample surveys of women of reproductive ages were carried out in... Peru and the Dominican Republic. These surveys were made as part of the Demographic Health Surveys project (DHS). In each country, one survey was conducted with the standard core questionnaire developed for DHS; the other survey was based on an experimental questionnaire. The major difference between the two questionnaires is the inclusion in the experimental one of a monthly calendar, which records pregnancies, contraceptive use, reasons for contraceptive discontinuation, breastfeeding, post-partum amenorrhea, post-partum abstinence, women's employment and place of residence for the period 1981-1986. This paper presents results from the first stage of the analysis of the Peruvian data: a comparison of basic characteristics of the two samples and an assessment of the completeness of reporting of recent births and infant and child deaths, i.e., a comparison of information in the truncated and full birth histories." [Feeding and assessment of nutritional status of spanish adolescents (AVENA study). Evaluation of risks and interventional proposal. I.Methodology].

BACKGROUND: Adolescence is a decisive period in human life due to the multiple physiological and psychological changes that take place. These changes will condition both nutritional requirements and eating/physical activity behavior. It has been demonstrated that these "adolescence" factors are of significant influence in health status during adult life. Due to its importance and adequate development the project has been granted by the Fondo de Investigación Sanitaria of the Institute of Health Carlos III. OBJECTIVE: To develop a methodology to evaluate the health and nutritional status of a representative population of Spanish adolescents. Specific attention is paid to three specific health problems: obesity, anorexia nervosa/bulimia, dislipidemia. METHODOLOGY: The following magnitudes will be studied: 1) dietary intake, food habits and nutrition knowledge; 2) daily physical activity and personal approach; 3) physical condition; 4) anthropometry and body composition; 5) hematobiochemical study: plasma lipid phenotypic and metabolic profile, blood cell counts; 6) genotipic profile of cardiovascular risk lipid factors; 7) immune function profile related to nutritional status; 8) psychological profile. CONCLUSION: This project includes the co-ordinate activity of five Spanish centers of five different cities (Granada, Madrid, Murcia, Santander, Zaragoza). Each center is specialized in a specific area and will be responsible for the corresponding part of the study. From the data obtained, we will elaborate a specific intervention program in order to improve nutrition and neutralize the risk for nutritional related problems in adolescence. By this, we will contribute to improve the health status of the Spanish population in the new millennium.

[Psychocognitive stimulation in dementias].

Alzheimer's disease is a neurodegenerative disorder that provokes a progressive loss of memory and of other cognitive functions, with additional symptoms in the behavioural and psychological sphere, loss of autonomy and an important overburdening for the family and the social milieu. Dealing with this disorder and other related dementias requires a multidimensional strategy that is able to face the different needs raised by the patient and his family. Given the lack of a curative pharmacological treatment, psychosocial interventions have emerged over the years that are directed at optimising the function of the patient and supporting the family in caring for him, with very different techniques in each of the spheres of intervention (cognitive, behavioural, environmental, family). In this review, we make a description of the interventions that have been most studied in the field of cognitive stimulation, from the most global approaches (reality orientation therapy, reminiscence therapy, psychostimulation programs) to the most specific ones in the field of memory rehabilitation (training of the explicit memory, technique of delayed remembrance, vagueness, error-free learning, procedure training of the memory and the use of external help), and the results are shown of some studies carried out using these techniques of intervention.

[Differences in the synthesis of IgA antibodies against T-dependent antigens in males and females].

Rats immunized with T-dependent antigens revealed a greater specific IgA and IgM antibody response in females. There was no significant difference in the IgG antibody response. Quantitative differences in the IgA and IgM antibody response between males and females may be explained by genetics and hormonals factors. Greater IgA response in females may be another factor explaining the majority of female survival.

Collection of survey data on contraception: an evaluation of an experiment in Peru.

The objective of this analysis is to compare two different approaches to the collection of information on contraceptive use. The data for this comparison are derived from the 1986 Demographic and Health Surveys (DHS) in Peru. Approximately 7,500 women were interviewed with the standard DHS questionnaire, whereas about 5,000 women received an "experimental questionnaire." The major difference between the questionnaires is the inclusion of a six-year monthly calendar in the experimental questionnaire that records pregnancies, contraceptive use, and postpartum information, in contrast to the more common tabular format of the standard questionnaire. The analysis demonstrates that although reports of contraceptive knowledge, ever-use, and current use are relatively robust to the variations in questionnaire design, estimates of past use are dependent on the survey instrument. Several different comparisons indicate that reporting of information on contraceptive histories in the experimental questionnaire is superior to that in the standard one.
