Enhancing effect of levamisole on the phagocytic activity of human neutrophil polymorphonuclear leucocytes in vitro.

The effect of levamisole on the phagocytic activity of normal human polymorphonuclear neutrophil leucocytes has been studied in vitro. In concentrations comparable with the serum levels obtained in treatment with levamisole perorally, the engulfment of heat-killed yeast cells is moderately but significantly increased. It is shown that levamisole can only stimulate phagocytosis of adequately opsonized particles. The action of the drug is still unknown but appears to be directly linked to the phagocytic cell.

The estimated daily loss of trace elements from normal skin by desquamation.

The content of 21 elements (As, Au, Br, Ca, Cd, Co, Cr, Cs, Cu, Fe, Hg, K, La, Mo, P, Rb, Sb, Sc, Se, W and Zn) in normal epidermis has been determined with the aid of neutron activation analysis. The dermal loss of these elements via desquamation has been calculated in view of its possible importance in balance studies. For the bulk elements Ca, K and P, the calculated daily loss is considered to be negligible. For some of the trace elements, however, the loss may be appreciable. The daily loss of Fe by desquamation may be as much as one fourth of the daily urinary iron excretion; the corresponding fraction for Co, Cu, and Zn is one tenth.

Allergic contact response to vitamin A acid.

Three patients treated daily with vitamin A acid cream experienced after one week, seven weeks and 14 weeks respectively sudden redness and itching at the treated sites. Patch testing with the cream (containing 0.05% vitamin A acid) and with vitamin A acid 0.05% in absolute alcohol produced strongly positive reactions in the patients but only slight erythema in some of the controls. Since patch testing with vitamin A acid 0.005% in absolute alcohol elicited no response in controls but positive reactions in the three patients, it is probable that these latter reactions were due to contact allergy. Histologically the test reaction showed intact epidermis and dense dermal infiltration by mononuclear cells, and thus resembled a response of the tuberculin type.

Clofazimine. A new agent for the treatment of pyoderma gangrenosum.

Eight patients had pyoderma gangrenosum. They were given a phendimetrazine tartrate derivative, clofazimine (Lamprene [Britain]), which is a chemotherapeutic agent used mainly in certain mycobacterial infections and which also has phagocytosis-enhancing properties. The effect of this drug was remarkably good, with rapid healing of the lesions commencing 3 to 14 days after treatment was started. The mechanism for the effect of clofazimine in pyoderma gangrenosum is not known.

Acetylation of procaine amide in man. A preliminary communication.

The metabolism of procaine amide was studied in 41 cardiac patients who had achieved steady state plasma concentrations of the drug. Acetylated procaine amide accounted for 31 +/- 12% (range 16-63%) of the overall urinary recovery of the drug and is therefore a main metabolite in man. Plasma levels of the metabolite were usually lower but sometimes exceeded those of the parent compound with variations between 1 and 15 mug/ml. The metabolite had a weaker effect than procaine amide on the maximal electrical driving velocity of isolated atrial strips from guinea pig.

Polymorphic acetylation of procaine amide in healthy subjects.

The acetylation of procaine amide has been studied by means of gas liquid chromatography in 33 healthy human volunteers. The acetylator phenotype was determined by measuring unchanged and acetylated sulphapyridine in urine. Slow acetylators of sulphapyridine excreted significantly less procaine amide in acetylated from in the urine than rapid acetylators (9 plus or minus 1% against 19 plus or minus 4%). Hence, it is suggested that the acetylation of procaine amide is subject to the same genetic polymorphism as that of isoniazid and some sulfonamides.

Toxic skin reactions caused by tributyltin oxide (tbto) in socks.

During some hot summer-days in 1973 about 70 cases of acute feet dermatitis were reported among soldiers from one regiment. They had used newly laundered socks from the same batch. The socks were disinfected with a solution containing TBTO and a quarternary ammonium compound, but by mistake a roughly 7 times too high concentration of the solution had been used. Patch tests with TBTO, quarternary ammonium compound and standard substances were negative. There was no suspicion of phototoxicity in these cases, but according to the literature TBTO may be photodynamic and several case reports relate specifically to sunny summerdays. We consider our cases to be due to the primary irritant properties of TBTO. As the span between optimal concentration for disinfection and the primary irritant concentration is narrow, we consider it inappropriate to use this substance for disinfection of textiles which are to be in direct contact with the skin until it has been established whether, for example, sweat may extract TBTO from treated fibres.

Clofazimine-enhanced phagocytosis in pustulosis palmaris et plantaris.

The phagocytic ability of neutrophil leucocytes was found to be impaired in patients suffering from pustulosis palmaris et plantaris (PPP). This ability was studied with the aid of the yeast particle method. In 78% of 27 patients whose PPP was in a static phase, the phagocytic function was enhanced, while the PPP abated concomitantly and the pustules disappeared. Impaired phagocytosis and the beneficial effect of clofazimine signify the pathogenic importance of defective neutrophils in PPP.
