Amidinosemicarbazido derivatives of pyrazole having various substituents on pyrazole ring were prepared and their effect on platelet function and blood coagulation was determined in vitro. Platelet aggregation and serotonin release induced by ADP, collagen, arachidonic acid and thrombin were markedly inhibited by pyrazole derivatives at mM concentrations. Compounds with a hydrophobic nucleus at position 1 of the pyrazole ring showed the most potent antiplatelet activity. On the contrary, their effect on blood coagulation was faintly inhibitory.

In a patient with typical features of Bartter's syndrome, intrarenal reabsorption of sodium and water at different sites of the nephron, during maximal water diuresis, was studied twice in the course of the disease. During the first study, chloride reabsorption at the loop of Henle and renal diluting ability were not impaired. Chloride reabsorption at the loop of Henle was 0.72 (normal values 0.66-0.90), minimal urine osmolality 71 mosm/kg (normal values less than 75 mosm/kg) and chloride fractional clearance 0.44 (normal values 0.3-6.8). An impaired chloride reabsorption at the loop of Henle was found 6 months later. Chloride reabsorption at the loop of Henle was 0.28, minimal urine osmolality 197 mosm/kg and chloride fractional clearance 8.23. These data suggest that the defect in chloride reabsorption at the loop of Henle is not a pathophysiological abnormality of the syndrome because it appeared at a later time than the other symptoms.
To assess the sensitivity, specificity and clinical value of prealbumin as liver test, prealbumin plasma levels were measured in 100 patients with liver disease and in 65 patients without clinical evidence of liver impairment. The sensitivity of prealbuminemia was higher than that of albumin, pseudocholinesterase, apolipoprotein and prothrombin activity. Its specificity was higher than that of pseudocholinesterase and comparable with the specificity of other liver tests. Prealbumin plasma levels were progressively decreasing in patients with liver cirrhosis graded as Child's A, B and C, respectively. In these patients prealbuminemia was correlated with galactose elimination capacity, assumed to be an index of maximal liver functional capacity.

To evaluate the reliability of urinary enzymes as markers of renal tubular damage in obstructive jaundice, research was carried out on 26 Sprague-Dawley rats submitted to bile duct ligation and on 16 sham-operated rats. The fractional clearances of lysozyme (CfrLYS) and of malto-dehydrogenase (CfrMDH)-indices of tubular function-and the fractional excretions of gamma-glutamyltransferase (UfrGGT) and of alpha-glucosidase (UfrAGL)-indices of tubular anatomic damage - were measured 5, 10, 20 and 30 days after operation. Creatinine clearance, urinary sodium excretion, urinary potassium excretion, proteinuria, plasma bilirubin and bile acids were also measured. Kidneys were taken for histology. All rats submitted to common bile duct ligation had high levels of bilirubin and bile acids; proximal tubules were damaged and the extent of the lesions increased with time. However, creatinine clearance, urinary sodium excretion, proteinuria, CfrMDH and UfrAGL gave no indication of renal lesions, whereas CfrLYS and UfrGGT were significantly higher 20 and 30 days after bile duct ligation, respectively. These findings show that CfrLYS and UfrGGT could be useful tests for renal tubular lesions in jaundice.
In a 15 day double-blind clinical trial 39 patients affected with rheumatic disease have been enrolled to evaluate the therapeutic effect of rectal administration of Piroxicam, in comparison with Indomethacin. At the end of the study, 20 patients had been treated with Piroxicam and 19 with Indomethacin. Nine patients in the Indomethacin group and one in the Piroxicam group dropped-out. Both drugs safety resulted good in the patients who completed the study, whereas 5 out of 10 dropped-out patients stopped the trial in consequence of severe side-effects of Indomethacin. Piroxicam induced a very good improvement in 76% of the patients, moderate in 19% and no improvement in 5%; Indomethacin induced a very good improvement in 75% of the patients, moderate in 15% and no improvement in 10%. No significative modifications resulted from the control of the laboratory blood tests. Piroxicam (30 mg/die) showed a therapeutic activity similar to Indomethacin (100 mg/die). The rectal administration of Piroxicam can be then considered a very good alternative to the oral one, particularly in the patients in which oral use of NSAID is counter-indicated.
Twelve cirrhotic patients with portal hypertension and esophageal varices were treated with nadolol for 1 month, and the effects on renal and hepatic hemodynamics and function were studied. A significant decrease in cardiac output, portohepatic gradient, and effective hepatic blood flow was found, whereas mean arterial pressure, liver function, effective renal blood flow, glomerular filtration rate, and urinary sodium and potassium excretions were not affected. In seven patients esophageal varices were also reduced. A significant correlation was found between the decrease in portohepatic gradient and that in cardiac output. The percentage of cardiac output distributed to the kidneys was significantly increased after nadolol treatment. In conclusion, nadolol seems to have properties useful for the treatment of portal hypertension in patients with liver cirrhosis.

A new theory for the understanding of the therapeutic results of two channels of Chinese Acupuncture: Urinary Bladder (U.B.) and Tou Mo. It is possible to underline that these two channels represent in the adult the trace of anatomical structures present in the human embryo. The series of medial posterior branches at their skin output are the Yu points where the myotomic slides split in their epiaxial and hypoaxial parts (statural and kinetic muscles). Tou Mo channel is the adult trace of the embryonic primitive line.
27 cases of sportsmen suffering from single or multiple myosis in different muscular groups (tendomyosis trapetii, splenii capitis, sacrospinalis, longissimus dorsi, glutaei medii) treated by combined soft-laser-therapy are presented. Pathogenesis of the disease and therapeutic results on the basis of recent clinical and experimental knowledge are discussed.
The authors, describe a specimen of subjects from Majano, Friuli, one of the communes that were largely struck by earthquake in 1976 and therefore by its social and cultural consequences which still play an important role in the life of those people. The specimen refers to those patients who spontaneously reported about the setting-up of the symptomatology with reference to the earthquake even when the disease had set up subsequently. Acupunctural points--mostly psychical--have been used. 70% of the subjects have recovered. 30% have markedly improved and recovered after a treatment on local points. A good deal of subjects had been treated with drugs and even acupuncture without successful results. The results are here considered following a psychosomatic view and the dermatoneuromeric theory.
In 18 cirrhotics with impaired renal perfusion intravenous injection of aminophylline 3 mg/kg b.w. increased mean renal blood flow (133-Xenon washout) from 1.77 +/- 0.71 to 1.99 +/- 0.44 ml/g/min (p less than 0.01). No significant change in cardiac output was found. In a further 10 cirrhotic patients the administration of aminophylline 3 mg/kg decreased the mean tubular reabsorption of sodium in the proximal section of the nephron (distal delivery) from 10.5 +/- 6.2 to 16.3 +/- 16.5 (p less than 0.01). Free-water production increased as a consequence of the increased supply of sodium to the diluting segment. Thus, aminophylline acts favourably on the functional renal impairment that occurs in hepatic cirrhosis.
The plasma levels of factor VIII related antigen (F VIIIR:Ag; determined by rocket electrophoresis) and reticulo-endothelial function (measured by means of the removal kinetics of human albumin millimicrospheres) were studied in a group of patients with chronic renal failure. F VIIIR:Ag plasma levels were increased, median 304% (interquartile ranges 211-443%) compared to the controls, median 112% (interquartile ranges 100-144%); P less than 0.01. Reticulo-endothelial function was decreased, median 8.3 micrograms/min/kg body wt (interquartile ranges 6.2-10.1 micrograms/min/kg body wt) compared to the controls, median 15.0 micrograms/min/kg body wt (interquartile ranges 11.9-19.0 micrograms/min/kg body wt); P less than 0.02. A significant inverse relationship was found between plasma levels of F VIIIR:Ag and the reticulo-endothelial function rs = 0.075; P less than 0.01). As the main site of catabolism of F VIIIR:Ag is the reticulo-endothelial system, the impairment of its function appears to be a possible explanation for the elevated plasma levels of the F VIIIR:Ag found in patients with end stage renal disease.
The Author presents clinical cases suffering from facial pains of vascular origin treated with Helium-Neon 5 m.w. soft-laser. Local points belonging to the maxillary branch of the V pair, distant points increasing ACTH and beta endorphin levels, nose and ear points have been stimulated. 80% of our subjects previously treated with pharmacological therapy with none or little results, presented excellent and good results with segmental and hormonal neuroreflexotherapy. Results are discussed in the light of dermatoneuromeric theory.
