The short-term use of benzodiazepines.

The extent of benzodiazepine use in the western world is discussed. The expanding use of these drugs appears to correlate with the development of an increasing number of benzodiazepine derivatives. A description of the various categories of anxiety is followed by a discussion of the common signs and symptoms of the anxiety states. After a discussion of the differential diagnosis of the anxiety states, a review of the indications for benzodiazepines is given. Pharmacokinetic factors in the clinical choice of specific benzodiazepines are reviewed. The use of benzodiazepines in other conditions such as alcohol abstinence syndrome, acute toxic delirium, and insomnia is considered. A review of common side effects and treatment complications is offered.

Lorazepam, a new benzodiazepine derivative, in the treatment of anxiety: a double-blind clinical evaluation.

Ninety-five adult outpatients suffering from moderate to severe anxiety were randomly assigned to treatment with lorazepam or placebo in a double-blind four-week study. Patients were evaluated at pretreatment and after two and four weeks of treatment using three rating scales. Lorazepam, at an average daily dose of 3.2 mg twice daily, was highly effective in relieving anxiety as documented by clinically and statistically significant differences over placebo in most of the items on all rating scales at all rating periods. No serious adverse effects were reported, and there were no adverse interactions with other medications, including digitalis and aspirin.

Studies with oral lorazepam in anxiety neurosis associated with depressive symptomatology.

Twelve studies were undertaken under a common four-week protocol to investigate the efficacy of lorazepam compared with placebo in reducing the severity of moderate to severe anxiety in 264 patients also having significant depressive symptomatology. Analysis of the results indicated that lorazepam gave generally better anxiety relief than placebo; in the majority of comparisons the differences were substantial enough to be both statistically and clinically significant. No data were obtained evidencing significant toxicity in either treatment group.

Comparison of the psychiatry learning system and traditional teaching of psychiatry.

Pressures of time, changing curricula, and increased enrollment in medical schools require the development of more efficient teaching methods to replace or supplement the traditional lecture format. The faculty of the Department of Psychiatry and Behavioral Sciences at the Medical University of South Carolina has developed an individualized course in basic psychiatry which can replace approximately 50 hours of lectures ordinarily delivered to students during the rotation on clinical psychiatry. The individualized course, referred to as the Psychiatry Learning System (PLS), was received by half the class during the year 1973-74, and the lecture course was delivered to the other half. These two groups were compared on several learning outcomes, including attainment of theoretical knowledge about psychiatry, observational skills, interviewing ability, and problem-solving ability. Overall performance ratings by clinical supervisors were also obtained. The PLS group scored no worse on any of the measures but statistically better on half of them than did the lecture group.
