A Mössbauer study of ferri- and ferrocytochrome c.

The Mössbauer spectra of horse heart ferri- and ferrocytochrome c were obtained at room temperature using lyophilized powders. The Mössbauer data indicate that the iron in both lyophilized samples is in a low-spin state. The high quadrupole splittings suggest that the iron atom is in an asymmetric ligand field. Upon reduction the asymmetry increases, suggesting a change in the bonding between the protein moieties and the iron atom.

First do no harm: routine use of aminoglycosides in the operating room.

For decades aminoglycosides have been used routinely in intraocular surgery to prevent and treat endophthalmitis. However, as more and more studies are conducted, the reality is that these drugs are causing more harm than they are preventing. Summarized herein are the most recent studies concluding that even low-dose aminoglycosides can cause toxic retinal damage. The conclusion, with pertinent recommendations from the Johns Hopkins Hospital, Wilmer Eye Institute Infection Control Committee, is that the questionable benefit of use of aminoglycosides intraoperatively is not justified by the risk of injury.

Shoulder pain in wheelchair athletes. The role of muscle imbalance.

Shoulder rotator cuff impingement syndrome is a common and disabling problem for the wheelchair athlete. In this study we investigated the role of shoulder strength imbalance as a factor for the development of this syndrome. Nineteen paraplegic male athletes underwent clinical and isokinetic examination of both shoulders with peak torque values measured in abduction, adduction, and internal and external rotation. Twenty athletic, able-bodied men without shoulder problems were tested as controls. Ten (26%) of the paraplegic athletes had rotator cuff impingement syndrome. The results of the isokinetic testing demonstrated that 1) the paraplegics' shoulders were stronger than the controls in all directions (P < 0.05); 2) the strength ratio of abduction: adduction was higher for paraplegic athletes (P < 0.05); 3) paraplegics' shoulders with rotator cuff impingement syndrome were weaker in adduction and external and internal rotation than the paraplegic athletes without impingement syndrome (P < 0.05); and 4) paraplegics' shoulders with rotator cuff impingement syndrome had higher abduction:adduction and abduction:internal rotation strength ratios than the shoulders of paraplegics without impingement syndrome (P < 0.05). We concluded that shoulder muscle imbalance, with comparative weakness of the humeral head depressors (rotators and adductors), may be a factor in the development and perpetuation of rotator cuff impingement syndrome in wheelchair athletes.

Studies on the undecapeptide of ferricytochrome c using ESR, Mössbauer and visible spectroscopies.

The ESR, Mössbauer, and visible spectra of the undecapeptide of ferricytochrome c (HPp) were measured for both the solid peptide and solutions of the peptide at pH 1.5, 7, and 10. At the low pH, the iron exists in the HPp in the high-spin species. At neutral and alkaline pHs, the low-spin ferric species predominate. The results of these measurements support the previous suggestions concerning the ligands in the fifth and sixth position of HPp at the neutral and alkaline pHs. The spectra of lyophilized preparations of HPp show the presence of both high and low-spin ferric species. The study of the Mössbauer spectra of the lyohilized preparations as function of temperature shows that there exists a spin-spin equilibrium between the two spin states. The relative amounts of the two components vary with the preparation. In these preparations there exist two components, one of which exists in the equilibrium mixture and the second in the high-spin form.

Reducing pain and anxiety in children.

Children faced with clinical procedures may become distressed in anticipation of the experience of pain. This article describes psychological interventions designed to minimise the trauma children associate with potentially distressing situations. Although time-consuming, the author suggests they are valid tools in the control of pain.

Institutions and the transformation of personal values. Are the traditional values of caring and service in jeopardy?

Groups bring about changes in the values of their members in two main ways--through their organizational structures and through the identifications that individuals make with other group members. These two forces help to transform a member's personal values to the values of the group. In health care, it is widely believed that such a transformation is necessary to make health-care professionals more efficient in their jobs. However, the author suggests that this value transformation may be propelling health-care professionals away from the traditional mainstays of the profession--caring and service. To explore this idea, the author first discusses the relationship between individual values and group values. He then examines how value changes occur within a group by using two different examples of groups--the corporation (a good example of how organizational structure shapes the values of its members) and the mob (a good example of the role social identification plays in shaping the values of group members). The article concludes with an exploration of how values are transformed in a health-care setting and how this transformation may be negatively affecting the attitudes of health-care practitioners toward the goals of caring and compassion.

[Plantar pyoderma due to Stenotrophomonas maltophilia].

INTRODUCTION: Stenotrophomonas maltophilia is a Gram negative, opportunistic bacteria. We describe a case of community-acquired superficial pyoderma due to this bacteria in a immunocompetent host. CASE-REPORT: A 59-year-old man presented with an ulceration of the sole for two months. Several topical and systemic treatments had failed to cure the lesion, including oral ofloxacin for ten days. Microbiological study of the lesion showed numerous Gram negative bacilli with growth of S. maltophilia. This bacterium initially responded to ofloxacin, but became resistant after the oral use of this antibiotic. Finally, a topical treatment with polymyxin/tetracycline ointment resulted in complete healing within two weeks. DISCUSSION: S. maltophilia may be responsible for superficial pyoderma and serious opportunistic infections in immunocompetent persons. Topical antimicrobial therapy is efficient when oral quinolone therapy has failed to cure the lesion.
