Prevalence and timing of sexual assaults in a sample of male and female U.S. Army soldiers.

OBJECTIVE: This study was designed to assess the prevalence and timing of sexual assault experiences in a sample of U.S. Army soldiers. METHODS: Self-administered surveys were completed by 555 male and 573 female soldiers in combat service and combat service support units. RESULTS: One-fifth of the women reported a completed rape (22.6%), and 50.9% of women and 6.7% of men reported any sexual assault. The majority of sexual assaults occurred before the soldiers entered the military, and 25% of women and 1% of men reported an attempted or completed rape during childhood. Sexual assault history also varied by sociodemographic characteristics. CONCLUSION: Results suggest that a history of childhood sexual abuse may be more widespread among female soldiers than among civilian women, and that ascribed and achieved status characteristics may differentially expose soldiers to sexual assaults both before and after they enter the military. Health care assessments should include details of a soldier's sexual assault history.

Oral SIV, SHIV, and HIV type 1 infection.

Several strains of simian immunodeficiency virus (SIV), including uncloned and molecularly cloned SIV strains, can cross intact mucosal surfaces after oral exposure in both adult and neonatal rhesus macaques, resulting in viremia and disease. Cell-free SIV strains as well as infected whole blood have resulted in systemic infection after oral inoculation. Neonatal macaques, exposed orally to the chimeric SHIV-vpu+, a derivative of SIVmac239 that encodes the env gene of the T cell-tropic HIV-IIIB, have also become persistently infected. These data indicate that oral exposure to various virus strains, including T cell-tropic variants, leads to infection. After nontraumatic inoculation, the oral route was more efficient than the rectal route in permitting SIV entry in adult macaques. Infection and AIDS resulting from oral exposure of adult macaques have implications for the transmission of the human immunodeficiency virus type 1 (HIV-1) during oral-genital contact.

A code of ethics for the medical dosimetrist--the American Association of Medical Dosimetrists experience.

The Committee on Protocol and Ethics of the American Association of Medical Dosimetrists (AAMD) has developed a Code of Ethics for a radiation oncology society of medical dosimetrists. The purpose of the code of ethics is fourfold: (1) Establish an ideal of professional conduct specific to the medical dosimetry profession; (2) Develop a statement of the moral values and commitment of the AAMD; (3) Recognize professional relationships and obligations; and (4) Define goals to which the medical dosimetrist should aspire. The Code of Ethics was adopted as AAMD policy in October 1995.

Metanephric adenoma of the kidney. A clinicopathologic, immunohistochemical and electron microscopic study of two cases.

We report two cases of metanephric adenoma in 40 and 48 year-old women. These rare kidney tumors were composed of cuboidal epithelial cells forming tubules, glomeruloid structures and sheets. Ultrastructural and immunohistochemical studies revealed that the tumor cells are similar to epithelial cells of developing nephrons. These features differentiate the metanephric adenoma from tubulo-papillary renal carcinoma, nephroblastoma, and cortical adenoma. According to its invariably benign course, the metanephric adenoma treatment could be restricted to a simple tumorectomy.

A code of ethics for the medical dosimetrist--the American Association of Medical Dosimetrists experience.

The Committee on Protocol and Ethics of the American Association of Medical Dosimetrists (AAMD) has developed a Code of Ethics for a radiation oncology society of medical dosimetrists. The purpose of the code of ethics is fourfold: (1) Establish an ideal of professional conduct specific to the medical dosimetry profession; (2) Develop a statement of the moral values and commitment of the AAMD; (3) Recognize professional relationships and obligations; and (4) Define goals to which the medical dosimetrist should aspire. The Code of Ethics was adopted as AAMD policy in October 1995.

[Infiltrating lipoma].

BACKGROUND: Deep or infiltrating lipoma is an often misdiagnosed clinical entity. We report two typical cases. CASE REPORTS: Two women, aged 92 and 62 years, were seen for a tumefaction on an upper limb with progressively increasing volume. The clinical presentation suggested deep lipoma, confirmed by magnetic resonance imaging and histology which eliminated liposarcoma, the main differential diagnosis. Surgical excision was successful. There has been no recurrence. DISCUSSION: These two observations recall the clinical, diagnostic and therapeutic features of infiltrating lipoma.

Improvement of the Cockcroft-Gault equation for predicting glomerular filtration in cancer patients.

In order to determine the morphological and biological covariables which better predict the glomerular filtration rate in cancer patients, we performed the present study in a population of 123 patients (78 men, 45 women) with various tumor types; 55 of these patients had previously received cisplatin, and 12 had undergone unilateral nephrectomy. The 51Cr-EDTA plasma concentration versus time data of 80 patients were analysed according to a population pharmacokinetic approach by using the Nonlinear Mixed Effects Model (Nonmem) program. The best fit for 51Cr-EDTA clearance estimation was given by the following formula: [formula: see text] (with ABW for actual body weight in kg, age in years, sex = 0 if male and sex = 1 if female, and Scr for serum creatinine in mumol/l). Actual body weight was the most predictive morphologic parameter, and the adjustment was not improved by taking into account the ideal body weight. The GFR of patients previously treated with cisplatin was 18% lower than that of untreated patients age for age. However, this covariable was not present in the final model because it was redundant with other covariables, likely Scr. The formula was then prospectively evaluated with the data of 43 other patients. The mean (+/- SD) ratio between GFR predicted according the Nonmem formula and the observed GFR was 0.95 +/- 0.23 which did not differ significantly from unity. Conversely, the mean ratio between creatinine clearance calculated according to the Cockcroft-Gault equation and the observed GFR (0.86 +/- 0.21) differed significantly from unity. This study shows that in cancer patients the formula to calculate GFR drawn from Nonmem analysis is more accurate than the Cockcroft-Gault equation. However, an accurate determination of GFR requires specific techniques as 51Cr-EDTA clearance investigation.

[Bilateral scrotal panniculitis in a prepubescent child].

Scrotal panniculitis or scrotal fat necrosis is an uncommon acute scrotal pathology. We report a case of scrotal fat necrosis in a 9 1/2 year-old over-weight boy with bilateral tender scrotal masses, located beneath the tests. Pathologic findings were those of subcutaneous fatty indurated masses with lipogranulomatous foci. The etiology of scrotal fat necrosis is unknown. A similar condition has been described in young children exposed to cold. In obese prepubescent boys, a greater sensitivity to cold and a higher saturated fatty acid concentration of the scrotal adipose tissue would induce fat necrosis. Our study of fatty acid composition by gas-liquid chromatography showed an elevation of stearic acid. The spontaneous resolution of scrotal fat necrosis is always the rule and allows symptomatic treatment without surgical investigation.

[Antigenicity of fresh, cryopreserved, or liquid-medium-preserved human parathyroid adenomas].

We have studied the ability of the cryopreservation and culture techniques to reduce the antigenicity of human parathyroid tissue by suppressing HLA DR bearing cells. Antigenicity was studied with an immunoperoxidase technique applied on frozen sections. Antibody against HLA DR, CD1a, CD3, CD22, CD45RA, CD68 and H et Y antigens were used. In fresh parathyroid tissue, endothelial cells, histiocytes and interstitial dendritic cells expressed HLA DR antigens. Antigenicity of cryopreserved tissue were not altered. In cultured tissue, interstitial HLA DR bearing cells have disappeared but antigenicity of endothelial cells were not modified.

Childhood maltreatment history as a risk factor for sexual harassment among U.S. Army soldiers.

Four different types of childhood maltreatment were examined as predictors of unwanted sexual experiences and acknowledged sexual harassment among male and female active duty soldiers in the United States Army. Predictor variables included childhood sexual abuse, physical-emotional abuse, physical neglect, and emotional neglect. Three types of unwanted sexual experiences in the workplace were examined as outcome variables: gender harassment, unwanted sexual attention, and coercion. Both sexual and physical-emotional abuse during childhood were found to be predictors of unwanted sexual experiences and of acknowledged sexual harassment in the workplace. Among female soldiers, the most severe type of unwanted experience-coercion-was predicted only by childhood physical-emotional abuse. Among male soldiers childhood sexual abuse was the strongest predictor of coercion. A greater variety of types of childhood maltreatment predicted sexual harassment outcomes for male soldiers. Childhood maltreatment and adult sexual harassment were predictors of psychological well-being for soldiers of both genders.

[Hepatic mesenchymal hamartoma in children. Immunohistochemical, ultrastructural and flow cytometric case study].

Mesenchymal hamartoma is a rare liver lesion. This lesion was found in a 7-month-old girl with high serum alphafaetoprotein serum levels and was composed of loose connective tissue containing a certain number of epithelial cells of biliary or hepatic origin. Immunohistochemical studies showed that cytokeratins 7 and 19 were localized in bile duct epithelium. The ultrastructural study showed that the hamartoma was composed of well differentiated ductal structures surrounded by a myxoid mesenchyma with cysts formed either from degenerative mesenchymal areas or from dilated ducts. Flow cytometric analysis of nuclei from frozen tissue revealed that the lesion was DNA aneuploid, with a DNA index of 1.28.

Fracture-dislocation of the lunotriquetral coalition.

Bony fusions involving the carpus have a much higher prevalence in blacks relative to whites. This article describes a case of lunotriquetral coalition fracture-dislocation in an African American. This lesion is best treated through open reduction and pin fixation.

Tryptophan ingestion by gestant mothers alters prolactin and luteinizing hormone release in the adult male offspring.

The effect of tryptophan administration to pregnant rats on the development of serotonergic systems and serotonin-related hormones in the offspring was studied. The male offspring of rats treated with tryptophan (200 mg/kg/day) during the second half of gestation showed a 4- to 7-fold increase in serum prolactin 40 and 70 days after birth and a 2-fold increase in serum luteinizing hormone 70 days after birth. The forebrain of adult offspring of tryptophan-treated rats showed an increase in serotonin and 5-hydroxyindoleacetic acid levels. Present data suggest that tryptophan regulates serotonergic differentiation during early development.
