[Value and dangers of indomethacin used in pregnancy. Apropos of 304 treated patients].

When Indomethacin in a dosage of 175 mg/day was used to treat threatened premature labour more than 70% of cases carried on until 37 weeks. This treatment therefore is an interesting alternative treatment to beta-mimetics whose the maternal side effects can be serious. As far as the cardiovascular systems of the fetus and neonate are concerned the effects in the dosage used are negligible. On the other hand there may be renal failure which is transitory at birth when delivery has taken place while treatment was going on. The treatment should not be prolonged for more than three weeks because Indomethacin causes oligohydramnios and this can cause in its place malpositions and maldevelopment of the lungs. Finally, it is not certain whether this treatment has any effect on fetal growth causing diminished growth, possibly.

[Treatment of urogenital prolapse with exertion-induced urinary incontinence using the Bologna technic. Apropos of 90 cases].

The authors describe this operation, which is carried out as a single procedure which they have been doing since 1982. Then the results in 90 patients are studied. The post-operative controls carried out on the clinical state of the patient and on the urodynamic tests show that this operation is very successful, both in curing stress incontinence and in giving a good anatomical result for correcting prolapse of the anterior wall of the vagina. In over one-third of the cases the post-operative follow-up has been carried out for 2 years or more. This follow-up has shown that the relapse rate over a period of time, both for the stress incontinence and the prolapse, is nil. The principal snags that still remain are: post-operative infection in about a third of cases; the rare but possible development of an enterocele and of dyspareunia (2%).

[Placental localization and scarred uteri. Apropos of 695 scarred uteri].

The authors studied a series of 649 uteri that had been scarred in order to try and find out whether the fact that the uteri had been operated on had made any difference to the site of the placenta as determined by ultrasound from the 32nd week of amenorrhoea onwards. The long-term effect of having a hysterotomy scar in the uterus on the position of the placenta in a subsequent pregnancy was studied. The results do not agree necessarily with those of other published series. The frequency of low-lying placentae found on ultrasound in the 3rd trimester does not seem to be higher when the uterus has a scar in it. On the other hand, there were more anterior situated placentae and more haemorrhages. Finally, it does seem that the quality of the old scar does affect the position of the placenta.

[Normal pregnancy after a Budd-Chiari syndrome treated by shunting: apropos of a case, review of the literature].

A patient who had a meso-atrial by-pass for a Budd-Chiari syndrome a year before conception carried her pregnancy to term. The authors have reviewed the literature. When they had studied the literature the authors took into account cirrhoses of the liver and portal hypertension in association with pregnancy. They concluded that a patient who had had a Budd-Chiari syndrome, particularly when she had also had a porto-caval shunt, was not in principle unable to have a pregnancy.

[Changes in indications for cesarean section from 1977 to 1983. Apropos of 18,605 deliveries].

Between 1977 and 1983 there were 18,605 women delivered in the maternity unit of St-Antoine Hospital. Of these 2,382 had caesarean operations, which is a percentage of 12.8. This percentage rose in a statistically significant way between 1977, when it was 8.8% and 1984 when it was 14.9%. Three groups of indications for caesarean operations were classified. These are: absolutely essential caesareans (feto-pelvic disproportion, placenta praevia, transverse lies). Prophylactic caesareans (uterine scars, breech presentations and fetal distress) and caesarean operations judged eventually to be necessary (dystocia in labour, high blood pressure and maternal pathology). The rise in the caesarean operations in the period of years studied came mainly from the increase in prophylactic caesarean operations, which reflected a progressive change in the attitude of obstetricians in the department. A study of the literature shows that this change coincided with a parallel change in many countries without there having been any concerted action between the countries, and also because there was a general desire to control the great rise in the number of caesareans and to find the optimum number that should be performed.

[Acute fatty liver of pregnancy].

A case is reported of acute fatty liver of pregnancy with maternal and foetal survival inspite of jaundice, encephalopathy, renal failure, gastro-intestinal haemorrhage and serious coagulation problems. The acute fatty liver is a rare disease of unknown aetiology which occurs during the third trimester of pregnancy. Early diagnosis based on liver biopsy obtained by the transjugular route, immediate delivery of the foetus and intensive care improve an otherwise fatal prognosis.

[Borderline ovarian tumors: an epidemiologic, diagnostic, prognostic and therapeutic study. Apropos of 20 cases reported at the Saint-Antoine Maternity Hospital].

Twenty cases of borderline ovarian tumors were seen in the Department of Gynecology and Obstetrics at the St. Antoine Hospital over 5 years. The epidemiological study of these 20 tumors, relative to 47 invasive ovarian carcinomas seen over the same period, shows that hypofertility is more prevalent and that patients are younger. Diagnosis rests mainly on the histological examination of the operative specimen. The overall prognosis is good but recurrences, which may be delayed, are possible. These patients, who are often young, may be managed conservatively, at the cost of prolonged follow-up, including cytologic and coelioscopic examinations.

[Comparative study of 15 borderline tumors and 31 cancers of the ovaries. Epidemiologic, diagnostic and therapeutic aspects].

46 epithelial tumours of the ovary were seen over a period of 5 years. These tumours could be subdivided into 15 borderline tumours and 31 invasive cancers. An epidemiological study did not make it possible to find special factors in relationship to these two types. Studying the circumstances under which diagnoses were made demonstrated the value of considering: pain, ultrasound pictures and cytology from the Pouch of Douglas. It is essential to carry out laparotomy in every case of an organic tumour of the ovary. It is only by histological examination that it is possible to tell whether a tumour is borderline of invasive. A protocol for carrying out treatment is suggested. The results confirm the excellent prognosis for borderline tumours and the seriousness of other epithelial cancers of the ovary.

[Chronic inflammatory bowel disease and pregnancy].

The authors have looked out the obstetric history of 13 young women who were suffering from ulcerative colitis or Crohn's disease between the years 1978 and 1982 and who had 25 pregnancies. This study made it possible to analyse the influence of these pathological conditions on the evolution of pregnancy as well as the effect of pregnancy on the evolution of the disease. The different therapeutic treatments that have to be given for these diseases, such as medical treatment using Sulphasalazine, enemata, corticosteroids and Azathioprin, or surgical treatments have been examined in their relationship with pregnancy. The results that have been obtained confirm those to be found in the literature in the sense that the two conditions are independent of one another and therapy for the pregnancy as well as for the bowel disease can be continued without too much consideration for the effect of the one condition on the other.

[A case of post partum gonococcal arthritis].

The authors report a case of gonorrhoea occurring immediately after delivery, characterized by acute inflammatory polyarthritis. The diagnosis was made from swabs taken from the lochia that were cultured on enriched media. The outcome of the disease treated with penicillin was good after 48 hours. The new-born's state of health after swabs take from the nostrils were positive was satisfactory all the time. A review of the literature shows how rare these conditions are, how serious they can sometimes be, and the management that should be undertaken in order to prevent complications occurring.

Obstetrical advantages and perinatal risks of indomethacin: a report of 818 cases.

In a series of 818 patients treated with indomethacin during pregnancy, the rate of perinatal complications was 1.8%. The risk was increased to 13% if indomethacin was being taken at the time of delivery, and when treatment was prolonged. The serious nature of certain complications (persistence of fetal circulation and oligo/anuria) means that indomethacin should only be used in cases in which betamimetic drugs are formally contraindicated or if there is a risk of unacceptable prematurity. Even in such cases, indomethacin should only be given for short period (< 1 week).
