[Life quality control in patients with severe bronchial asthma].

The aim of the study was to evaluate life quality (LQ) of patients with severe bronchial asthma (BA) in connection with the degree of the disease control. Seventy two BA outpatients were divided into two groups: 50 patients with uncontrolled BA, and 22 patients who had achieved full BA control thanks to combined therapy with salmeterol plus fluticasone propionate in a dose of 1000 mg per day during 3 months. Clinical symptoms, respiratory parameters, and daily requirement of broncholytics were under study. General and specialized questionnaires (SF-36; AQLQ) were used to evaluate LQ. The study revealed that full BA control lead not only to improvement of clinical symptoms and positive changes in the respiratory function, but to significant increase of LQ as well. Thus, achievement of full BA control as a complex goal of treatment increases patients' LQ, especially in social sphere.

[The clinicofunctional characterization of the phenotypes of severe uncontrollable bronchial asthma].

The aim of the study was to compare the clinical characteristics and the inflammatory process activity in the airway of patients with severe therapy-sensitive and therapy-resistant bronchial asthma (TSBA; TRBA). The subjects were 171 patients with severe bronchial asthma (BA) aged 18 to 60; the diagnosis was established prior to the study according to GINA 2002 criteria. There were 50 TSBA patients, 121 TRBA, and 50 controls. According to a range of criteria, BA in TRBA was more severe than that in TSBA. TRBA patients displayed significantly higher indices of inflammatory process activity, such as absolute and relative number of eosinophiles and neutrophiles in induced sputum (IS), and IS level of IL-5. Evaluation of IS inflammatory markers in patients with severe BA allowed substantiation of the central role of persistent inflammation as a risk factor of TRBA development.

Role of respiratory epithelium in the development of hyperreactivity of bronchial smooth muscles.

In guinea pigs sensitized with ovalbumin the respiratory epithelium lost its ability to modulate the responses of airway smooth muscles to histaminergic stimuli. Incubation of bronchial segments with IL-5 potentiated the contractile responses of bronchial smooth muscles to histamine in both intact and sensitized animals. Incubation of bronchial segments with IL-5 receptors moderated contractile activity of segments from sensitized pigs, but not in the segments from intact controls.

[Monitoring and treatment of severe bronchial ashma in adults: results of national multicenter trial NABAT].

AIM: To study quality of monitoring, features of a course and therapy of severe bronchial asthma (BA) in Russia. MATERIALS AND METHODS: The trial recruited 378 patients with severe bronchial asthma (SBA) in 7 cities of Russia. Each center participating in the study filled in individual case sheets recording demographic, clinical, monitoring, pharmacotherapeutic data. A special focus was on factors influencing the disease course. The chi-square and Mann-Witney criteria were used. RESULTS: It was found that current recommendations of BA patients' management are not used in full. Most of the patients do not receive adequate therapy. Phenotypes of uncontrolled course of severe BA are prevalent. The key role of adequate management of severe BA patients in control of the disease is shown. CONCLUSION: Wider use of updated recommendations on therapy and monitoring of BA with special focus on patients with uncontrolled course of the disease is urgent. Fulfilment of all the components of management of a patient with bronchial asthma recommended by GINA including the patient education and control over the disease triggers contribute much to effective treatment and control over BA.

[Contractile properties of smooth muscles of bronchial tubes at formation of hyperreactance of airways].

Contractile properties of smooth muscles of bronchial tubes of porpoises in formation of hyperreactance airways, were studied. Sensitization by ovalbumin results in formation of hyperresponsiveness of the airways as shown in increase in amplitude of contractile responses to histaminergic influences and oppression of adrenergic relaxations of segments. Respiratory epithelium oppresses contractile responses of airways in intact animals to histaminergic influences. Influence of epithelium on adrenergic responses depends on precontractile factor: they increase in prereduction by histamine. In formation of hyperresponse, the epithelium loses ability to modulate responses of smooth muscles of the airways. Incubation segments of smooth muscles of the airways in interleukin 5 increases contractile responses to histaminergic influences. Receptor of interleukin 5 (the soluble fraction) neutralizes the effect of exogenous interleukin 5, and incubation of receptor of sensitized animals oppresses histaminergic contractile responses.

Comparative study of eosinophils in the blood and induced sputum during bronchial asthma.

Eosinophilia characterized by accumulation of low-density eosinophils and high functional activity of normal-density eosinophils was detected in the blood from patients with bronchial asthma. Low-density eosinophils are characterized by low content of granules. In patients with bronchial asthma morphological and functional characteristics of bronchial eosinophils were similar to those of blood low-density eosinophils.

[Comparative assessment of various methods of determination of interleukin-5 production and their clinical significance in bronchial asthma].

Interleukin-5 (IL-5) is an important marker of inflammation in bronchial asthma (BA). The level of IL-5 was investigated by immune-enzyme assay (IEA); the expression degree of IL-5 mRNA was studied, before and after the conducted therapy, by the inhibition reaction-IEA (IR-IEA) in sputum and blood serum of patients. No differences between contents of IL-5 were found in blood plasma of patients with various disease degrees or of patients with different BA etiologies. The IL-5 contents in sputum were reliably different in different groups and depended on a disease severity, exacerbation and remission. An evaluation of an expression degree of the IL-5 RNAm in eosinophiles, derived from patients' blood, provided for clarifying the differences between acute asthma and other disease forms and for defining the therapy influence on the parameter in question. The IL-5 RNAm expression in sputum was reliably different in patients with moderate forms and with acute forms of the disease; it was decreasing due to treatment. Finally, the results of the evaluation of the IL-5 level and the study of mRNA expression of the cytokine mutually supplement one another and make it possible to evaluate the disease degree and therapy efficiency.

[Contractile responses of the airways smooth muscles in experimental bronchial asthma; the role of interleukin 5 in formation of hyper-responsiveness of bronchial tubes].

By mehanographic method, contraktile responses of the airways smooth muscles in experimental bronchial asthma in porpoises intact and incubated with interleykin-5, were studied. Sensitization of ovalbumin animals results in development of hypersensitivity to inhalation of the fiber with external attributes of infringement of bronchial passableness such as cough and a shortness of breath. Morphologically the process is accompanied by destruction of epithelium of the bronchial tubes, development of an immune inflammation in a bronchial wall, and a hypertrophy of the muscular layer. In the sensitized animals, development of hyper-responsiveness of the airways smooth muscles to histamine was obvious as shown in a significant decrease of the threshold concentration and an increase in the maximal amplitude of reduction. Interleukin-5 was shown to strengthen this process. Responses to holinergic and R2-adrenerdic influences practically did not change. The data obtained corroborate the hypothesis of development of the interleukin-5-depending bronchial hyper-responsiveness in absence of eosinophile damage in the mucous membrane of the bronchial tubes.

[Azithromycin levels in the chorion tissue within the first three months of the pregnancy term].

The levels of azithromycin in the chorion tissue within the first 3 months of the pregnancy term (42 to 56 days post coitus) were determined. Three days after the use of azithromycin (Sumamed) the antibiotic concentration in the villi tissue was 0.065 to 0.26 mcg/mg (the average of 0.129 mcg/mg). The azithromycin concentration above 0.125 mcg/mg was detected in 38.5% of the women. The results confirmed the fact of significant azithromycin accumulation in the chorion tissue. The percentage of the azithromycin transplacental transfer within the above mentioned pregnancy term varied from 2.3 to 9.3 (the average of 4.6).

IL-5 expression in the sputum of patients with bronchial asthma.

Expression of IL-5 mRNA and the content of IL-5 in the sputum of patients with asthma of different severity were studied before and after treatment. The expression of IL-5 mRNA in mild asthma differed from that in severe and moderate asthma before and after treatment. The level of IL-5 before therapy was different in patients with mild and severe disease. In patients with severe asthma the level of IL-5 differed before and after treatment.

[Polymorphism of interleukins and interleukin receptor genes: population distribution and association with atopic bronchial asthma].

Population distribution and pathogenetic significance for bronchial asthma (BA) of the eight polymorphic variants of six interleukin--(IL) and interleukin receptor genes, C-589T, G/C 3'-UTR IL4, C-703T IL5, T113M IL9, Q551R, 150V IL4RA, and G1972A IL5RB, was examined. In the population samples of Russians, Tajiks, Buryats, and Tuvinians racial and ethnic specificity of these polymorphisms was established. These specific features were manifested as population-specific "enetic portraits" in respect of polymorphic allele frequencies. Analysis of the BA patients and their relatives from Tomsk by use of transmission/disequilibrium test (TDT) revealed the presence of a statistically significant association between the C-703 IL5 allele and the disease (P = 0.005). This is the first evidence of an association between the IL5 gene polymorphism and BA.

[Analysis of the association between the T113M polymorphism of the human il-9 gene and bronchial asthma].

The T113M polymorphism resulting from the missense mutation in exon 5 of the human interleukin 9 (IL9) gene was tested for association with bronchial asthma (BA). The genotype frequency analysis did not reveal a deviation from the Hardy-Weinberg equilibrium. A comparison of the genotype frequency distributions in a control group of healthy individuals and in patients with BA suggested an association between T113M and the clinical phenotype. However, this association was not confirmed by the affected family-based association control (AFBAC) or the transmission/disequilibrium test (TDT).

[Genomic medicine in the solution of pulmonological problems].

Main points of the new direction genomic medicine and its applications to solution of pulmonological problems are outlined. Information on the molecular epidemiology of cystic fibrosis and bronchial asthma is presented. The frequency of major cystic fibrosis gene mutations and bronchial asthma candidate gene polymorphisms was defined in Siberian populations. The familial case histories of patients with bronchial asthma were used to assess the contribution of interleukine (IL4, IL5, IL9) and nitric oxide synthase (NOS3) genes to susceptibility to bronchial asthma.

[The sauna in the treatment of children with atopic dermatitis].

Complex sauna therapy for children with atopic dermatitis gives positive clinical results and improves the condition of their autonomic nervous system. Marked clinical response but unsatisfied autonomic nervous system regulation necessitate long-term sauna treatment to cure this disease.

[Polymorphism of the theta1 and mu1 glutathione s-transferase genes (GSTT1, GSTM1) in patients with atopic bronchial asthma from the West Siberian region].

Polymorphism for the GSTT1 and GSTM1 null alleles was analyzed in 69 patients with atopic bronchial asthma (BA) and in 57 healthy individuals from Tomsk. The two samples did not differ in frequencies of genotypes 0/0 and + of either gene or in frequencies of genotype combinations. No association was observed for GST and BA severity. Thus, the GST null alleles proved to be unimportant for BA.
