Hepatic and splenic scintigraphy in idiopathic systemic amyloidosis.

Technetium-99m sulfur colloid (SC) and Technetium-99m pyrophosphate (PYP) scintigrams in a patient with idiopathic systemic amyloidosis were abnormal. Hepatic SC distribution was nonhomogeneous. Spleen showed reduced SC uptake and a large focal defect at its upper pole. Splenic localization of PYP was seen on a bone scan. On histologic examination the abnormal scintigraphic sites showed heavy amyloid deposition. Splenic localization of skeletal radiotracer due to amyloidosis has not been previously reported.

Influence of respiratory maneuvers on jugular venous reflux.

The cause of jugular venous reflux during a radionuclide cerebral angiogram is uncertain. The authors describe a patient in whom physiologic respiratory maneuvers had a profound influence on the jugular venous reflux during cerebral radionuclide angiography. The massive jugular reflux was abolished by intravenous injection of a radiotracer bolus during inspiration. When brain blow data are difficult to interpret because of reflux, the authors suggest that radiotracer be administered intravenously during inspiration.

Dissociation of reticuloendothelial cell and hepatocyte functions in alcoholic liver disease: a clinical study with a new Tc-99m-labeled hepatobiliary agent.

Tc-99m-sulfur colloid scintigrams were abnormal in four patients with hepatic dysfunction due to chronic alcohol abuse. Minimal uptake of radiocolloid in the liver suggested local reticuloendothelial (RE) cell failure. Imaging with a new hepatobiliary agent, Tc-99m-PIPIDA, revealed rapid hepatic accumulation and excretion of radiotracer with adequate visualization of the organ. Scintigraphic findings in these patients indicated a dissociation of hepatocyte and RE cell functions. Demonstration of adequate hepatocyte function with severe RE failure in alcoholic liver disease using a Tc-99m-labeled hepatobiliary agent has not been previously reported.

Adrenal scan in 17-alpha-hydroxylase deficiency: false indication of adrenal adenoma.

A patient who was thought to have testicular feminization syndrome and primary aldosteronism had an adrenal scan that suggested an adrenal adenoma. After later diagnosis of 17-alpha-hydroxylase deficiency, she was treated with glucocorticoids rather than surgery. Her clinical course and a repeat adrenal scan confirmed she did not have a tumor.

Cystic lymphangiomatosis of the spleen: a radiologic-pathologic correlation.

Cystic lymphangiomatosis of the spleen is an extremely rare pathologic entity. We report the radiologic, sonographic, and scintigraphic findings in two cases of splenic cystic lymphangiomatosis. In both patients echograms demonstrated multiloculated cystic lesions in the intra-abdominal masses, which were identified on radiocolloid scans as abnormal spleens. The gross and microscopic findings of the excised spleens were characteristic of cystic lymphangiomatosis. The findings on echograms and scintigrams correlated well with the abnormal splenic morphology. The clinical presentation, the findings on diagnostic imaging, and the gross and microscopic appearances of the pathologic spleens in our patients were remarkably similar. When a massively enlarged spleen with a multicystic appearance is encountered during a diagnostic workup, awareness of the scintiscan and sonographic findings might aid in the noninvasive preoperative recognition of cystic lymphangiomatosis.

Radiolabeling of mammalian spermatozoa and their use to monitor sperm transport in females.

Radiolabeling of mammalian spermatozoa with 131I, 67Ga, 111In, and 99mTc was investigated. Spermatozoa were labeled with 99mTc for in vivo studies because of a high labeling yield (70 to 90%) combined with the lack of impairment of sperm motility. Ovariectomized sheep were brought into estrus by sequential administration of progesterone and estradiol cyprionate. Sheep were necropsied up to 6 hours after insemination with 99mTc-labeled ram sperm and their reproductive tracts were resected and examined with a rectilinear scanner. Radioactivity was clearly observed in the fallopian tubes, with larger amounts in the vaginas, cervices, and uteri. In contrast, when 99mTc-spermatozoa were replaced with 99mTcO4-, much less radioactivity remained in the reproductive tract at resection and this was evenly distributed. Some label left the 99mTc-spermatozoa in vivo, but radioactivity remained on the cells long enough to consider attempting to monitor sperm transport in vivo in a suitable species.
