Vesicorectostomy revisited.

Between 1948 and 1959 vesicorectostomy, a permanent anastomosis between the bladder and rectum, was constructed in 7 male patients. Scattered reports of this procedure exist but none since 1959 and long-term followup has never been reported. we present long-term followup, including renal function, excretory urography and voiding cystorectography, and review the quality of life in the 7 patients. The most frequent complication was stenosis of the anastomosis. None of the patients in this series with normal preoperative upper tracts showed radiographic or chemical deterioration postoperatively. Since this procedure maintains urinary continence, does not disturb the normal physiology of the ureterovesical junction and minimizes urinary reabsorption its use should be considered in certain patients as an alternative method of urinary diversion.

Gelatin implants in scleral buckling procedures.

In 375 scleral buckling procedures, an absorbable gelatin implant (Retina Gelfilm) was used. This series included 192 trapdoor and scleral pouch procedures, and 183 operations in which a gelatin implant was used beneath a silicone rubber implant. Two thicknesses of gelatin implants were used, 0.5 mm and 0.75 mm. Absorption time varied from 2 to 24 months, depending on the thickness of the implant and whether it was used alone or beneath a silicone rubber implant. Gelatin is an excellent absorbable implant when used alone or under a silicone rubber implant in selected patients with retinal detachment to produce a temporary elevation or posterior extension of the buckle, or both.

Primary leiomyosarcoma of the epididymas.

The fourth case of leiomyosarcoma of the epididymis is reported. It is suggested that radical orchiectomy, followed by frequent postoperative examinations, is an adequate method of treatment. Retroperitoneal lymph node dissection does not seem to be indicated.

Hepatotumorigenicity and peroxisome proliferation induced by the hypolipidemic CI-924 in a two-year study in male and female B6C3F1 mice.

The hepatic tumorigenicity of CI-924 (5,5'-(1,1'-biphenyl)-2,5-diylbis(oxy)(2,2-dimethylpentanoic acid)), a hypolipidemic agent, was evaluated in 50 B6C3F1 mice/sex/dose given drug in the diet at 0, 5, 25, and 75 mg/kg/day for 2 yr. Peroxisomal and drugmetabolizing enzyme determinations, as well as ultrastructural evaluations, were conducted in subsets of these same groups, because drugs of this class cause peroxisome proliferation and hepatic tumors in rodents. CI-924 elicited dose-dependent increases in the incidence of hepatocellular adenomas and carcinomas in both sexes that were statistically significant at 75 mg/kg. Stereologic evaluation revealed significant increases in hepatocellular peroxisome volume ratio, due to increased numbers of peroxisomes, in females at all doses and males at 75 mg/kg. Peroxisomal enzyme activity measurements revealed no change in catalase, but dose-dependent increases in carnitine acetyltransferase and cyanide-insensitive beta-oxidation in both sexes. Peroxisome proliferation, determined biochemically or ultrastructurally, was twice as great in females compared to males. Total cytochrome P-450 was increased in both sexes given 75 mg/kg. There were dose-dependent decreases in glutathione S-transferase in males and increased glutathione peroxidase in both sexes at 25 and 75 mg/kg. In conclusion, this study demonstrated that while CI-924 induced hepatic tumors in male and female B6C3F1 mice the associated peroxisome proliferation, while moderate in females, was only weak in the males after 2 yr of exposure.

Coronary arteriopathy in monkeys following administration of CI-1020, an endothelin A receptor antagonist.

A selective non-peptide endothelin A (ETA) receptor antagonist, CI-1020, was administered to cynomolgus monkeys intravenously (i.v.) for 2 or 4 wk and orally for 4 wk. Groups consisting of 3 animals of each sex received CI-1020 at 1, 5, and 10 mg/kg/hr (i.v.) or orally at 250, 500, and 750 mg/kg body weight for 4 wk. Control animals received the vehicle only. In a separate experiment, 1 male was infused with 10 mg/kg/hr for 2 wk, and Monastral blue dye was administered i.v. to facilitate localization of lesions to the vascular walls. One female was administered saline and the dye and served as a control. One female at 1 mg/kg/hr was found dead at week 2, and 1 female at 5 mg/kg/hr was euthanatized during week 4 as a result of severe thigh swelling at the catheter site. Macroscopically, extramural coronary arteries appeared thickened and nodular in the 4-wk i.v. study in the female found dead at 1 mg/kg/hr, in 1 male and 1 female at 5 mg/kg/hr, and in 2 females at 10 mg/kg/hr. Histologically, Monastral blue pigment trapped in the walls of coronary arteries with arteriopathy was observed in the male treated with CI-1020 at 10 mg/kg/hr for 2 wk. Extramural coronary arteriopathy occurred at all doses in the 4-wk i.v. study, with higher incidence occurring in females than in males (7 of 9 treated females compared with 3 of 9 treated males). In the oral study, 1 female at 500 mg/kg/day and 1 male and 2 females at 750 mg/kg/day had coronary arteriopathy. Histological changes after 2 wk of treatment were characterized by intimal thickening, fragmentation of the internal elastic lamina, necrosis and edema of the media, and mixed inflammatory-cell infiltrates in the intima, media, and adventitia. After 4 wk of i.v. administration, arteriopathy was characterized by segmental disruption of the elastic lamina and intimal and medial fibrosis with complete replacement of smooth muscle with fibrous tissue. The adventitia was thickened as a result of fibrosis and mixed or mononuclear inflammatory-cell infiltrates. CI-1020 concentrations were higher in males (1.57 to 29 micrograms/ml) than in females (0.974 to 24.4 micrograms/ml) in the i.v. study. Transient systemic exposure with high maximum plasma concentration (Cmax) (120-352 micrograms/ml) in the oral study was insufficient to provoke arterial changes of the same magnitude as those noted with continuous i.v. administration. The regeneration of the media by fibrous tissue and the disruption of the elastic lamina may weaken the arterial wall and increase the susceptibility of the artery to the development of aneurysm.

Correlation of CASA velocity and linearity parameters with sperm mobility phenotype in turkeys.

Since all domestic turkeys are produced through artificial insemination, a measurable sperm characteristic that would be predictive of fertility would allow for the culling of poor males, resulting in improved reproductive efficiency. The sperm mobility test (SMT), which quantifies sperm penetration into an Accudenz solution, has been shown to correlate highly with fertilization potential of individual turkeys. Since this sire-selection test is based on the differences in sperm mobility between whole ejaculates from individual males, the objective of this study was to determine whether specific sperm velocity parameters would correlate with the SMT and to determine whether these characteristics could account for phenotypic differences in sperm mobility observed between males. The SMT was used to rank males within a flock (n = 110) in triplicate and to classify them into high, average, and low sperm mobility phenotypes on the basis of the sperm mobility index. Several sperm velocity parameters were evaluated for each male by a computer-aided sperm analysis (CASA) system, the Hobson Sperm Tracker. The types of measurements taken of 200 sperm tracks/ejaculate included the following: curvilinear velocity (VCL), average path velocity (VAP), straight-line velocity (VSL), linearity (LIN), beat-cross frequency (BCF), and mean angular displacement (MAD). Significant positive correlations were found between VSL, LIN, BCF, and sperm mobility, and a significant negative correlation was seen between MAD and sperm mobility. Subpopulations of sperm that had penetrated the Accudenz solution were isolated from each mobility phenotype and were analyzed by CASA, and significant correlations were again observed between VSL, LIN, BCF, and sperm mobility. We conclude that sperm velocity and linearity contribute to overall sperm mobility phenotype and are important characteristics of turkey sperm function. Key words: Motility, computer, spermatozoa.

Differential zinc transport into testis and brain of cadmium-sensitive and -resistant murine strains.

Recently, we showed that murine strain differences to the testicular toxicity of cadmium (Cd) are the result of variable transport of Cd across the blood-testis barrier. Because Cd is a nonessential trace element, it must be using the transporter for an endogenous substance. The objectives for this study were to determine the natural ligand for the transport system used by Cd to enter testis and brain, and to determine whether the transport of that natural ligand also differs among Cd-sensitive and -resistant murine strains. Because zinc (Zn) and Cd are cations of similar size and charge, and because Cd has been shown to inhibit Zn uptake in a variety of systems, we hypothesized that Cd was using Zn transporters to enter tissues. In this study we characterized Zn transport into the testis and brain of Cd-sensitive and -resistant murine strains. We found that the transport of 65Zn into testis and brain of Cd-resistant A/J mice was significantly reduced compared with that in Cd-sensitive 129/J mice. In 129/J mice, unlabeled CdCl2 significantly reduced 65Zn transport by 56% in testes and by 47% in brain. Pretreatment with Zn had no significant effect on 109Cd transport rates into testes or brain of 129/J or A/J mice, but did reduce the percentage of the injected 109Cd dose in testes of 129/J mice by 44% within 60 minutes. From these results we can conclude that Cd is using transport systems that normally function to regulate Zn levels in testes and brain. Murine strain resistance to the testicular effects of Cd is associated with a concomitant attenuation of the Zn transport system in testis.
