Color Doppler sonography in the assessment of varicocele testis.

Color Doppler sonography is a new reliable and noninvasive diagnostic method in the evaluation of varicocele testes. Thirty-two persons (14 with varicoceles and 18 controls) participated in the investigation. We found two different graphs (spectral analysis) in men with varicoceles testis during the Valsalva's manoeuvre. Semen analysis from these males with varicocele was compared with an age-matched control group without varicocele. The analysis revealed significant difference in qualitative motility and qualitative motility after 24 hours but no significant difference in other parameters of semen quality. There were significant difference in the normal semen morphology but no difference in abnormal forms among the two groups. We did not find a spectral picture that could predict a compromised sperm quality in the varicocele group. These data show that the color doppler is an important tool in the detection of a varicocele. It is important for the patient to understand the Valsalva's manoeuvre and we suggest that all males being evaluated in an infertility clinic undergo a color doppler scan with and without the Valsalva's manoeuvre and that a spectral analysis be performed to locate even small varicoceles.

[Colonic ileus. Treatment of colonic ileus, a 10-year patient material].

Morbidity and mortality have been evaluated in 238 patients with acute large bowel obstruction admitted over a ten year period. In two thirds, the cause of obstruction was a primary colonic carcinoma. Most of these patients (61%) had advanced disease and were over 70 years of age. Volvulus and cancer recurrences each accounted for 10%, and diverticulitis for eight percent. An overall mortality of 24% was found. Of 115 patients who had the obstructing lesion--primary carcinoma or diverticulitis--resected, a mortality of 8.7 was seen, higher for primary (14.9%) than for staged (4.4%) resections, in contrast the latter had a significantly longer hospital stay, 30 days as against 17 days when a primary resection was performed. Six non-lethal anastomotic leaks occurred. It is concluded, that patients with large bowel obstruction, most often caused by a colorectal carcinoma, are a high-risk group. In this retrospective study staged procedure compared with primary resection had lowest morbidity and mortality.

Asymptomatic varicocele testis.

The incidence of asymptomatic varicocele among 542 unselected military conscripts in Denmark was 9%. Semen from 39 of the 49 men with varicocele was compared with findings in age-matched, varicocele-free controls. The overall quality of the semen was reduced in the varicocele group (p < 0.001), with significantly lower parameters of sperm concentration, total sperm count, qualitative motility and sperm motility after 24 hours. There was also significant difference in morphology, with more amorphous spermatozoa and more with tail defects (p < 0.05) in the varicocele group. The study indicated an association between asymptomatic varicocele and compromised sperm quality. The advisability of surgical treatment was not clarified, but the two groups of men will be further observed with special regard to this question.

[Testicular varicocele. Reason for surgery and recurrence frequency].

In the period 1979-1989, 63 patients were operated on for testicular varicocele. The study was a retrospective follow-up from case records and from a questionnaire. Thirty-eight had local complaints, 19 were operated on because of infertility and six had an asymptomatic varicocele. 10% had minor, reversible complications. 92% answered the questionnaire and 38% had a personal contact. 21% had recurrence of the varicocele although to a minor degree. In the infertility group, 42% succeeded in conception after operation.

Treatment of hydrocele testis with aspiration and injection of polidocanol.

Hydroceles of the testis were treated by aspiration and injection of the sclerosant solution polidocanol in 20 patients. Three patients had recurrences but otherwise the complication rate was nil. Injection of polidocanol is a promising treatment that should be investigated further for primary therapy of hydrocele of the testis. In contrast to some other agents this drug does not cause pain during or after the injection.

Surgical treatment of acute malignant large bowel obstruction.

OBJECTIVE: To evaluate the morbidity and mortality in all patients operated on urgently for acute large bowel obstruction caused by carcinoma of the colon or rectum during a 10 year period. DESIGN: Retrospective study. SETTING: Aalborg Hospital, Denmark. SUBJECTS: 156 consecutive patients operated on for obstructing primary colorectal cancers. MAIN OUTCOME MEASURES: Operations done, morbidity and mortality. RESULTS: 95 patients (61%) had advanced disease (Dukes' stage C or "D") and their median age was 73 years (range 38-93). 97 had the obstructing lesion resected with a 30 day mortality of 5%. 43 patients underwent primary resection and 4 died (9%), and 54 underwent staged resection with one death (2%). Complications were common, particularly after staged resections, median hospital stay being 19 days after primary, compared with 30 days after staged resection. 59 patients (38%) had palliative operations with 29 deaths (49%); in 39 the tumor was completely unresectable. CONCLUSION: Patients with obstructing primary colorectal cancers are a high risk group who are characterised by advanced disease and old age. Only prospective trials comparing different operations can assess whether it is possible to achieve a reduction in mortality.

[The role of oat bran in the treatment of hypercholesterolemia].

Review of published studies suggests that high dietary content of soluble fibre can reduce serum cholesterol by up to 17%. The reduction may be proportional to the degree of hypercholesterolemia before the change in diet. The mechanism is not fully elucidated. It may be analogous to the effect of anion-binding-resins on the metabolism of bile acids and cholesterol but it may also be due to partial substitution of soluble fibre for dietary saturated fat which causes elevation in serum cholesterol.

[Soft tissue tumors, particularly intramuscular lipomas].

The pathology, clinical features and treatment of intramuscular lipomata are reviewed on the basis of two case reports. The intramuscular lipoma is a benign tumour which feels hard on contraction of the muscle involved in comparison to ordinary lipomata.

[Anterior gastropexy using the Boerma method in gastroesophageal reflux].

The results obtained in 38 patients with gastro-oesophageal reflux who had been submitted to operation by Boerema's method are presented. Twenty-nine (76%) became entirely symptomfree and eight (21%) experienced reduction of symptoms. Postoperative oesophageal stenosis was demonstrated in five patients.

The occurrence of malignant disease in adult nephrotic syndrome.

The aim of the present study was to determine the incidence of malignant disease (MD) in adult patients with nephrotic syndrome (NS). Eighty-eight consecutive patients (38 females; 50 males; mean age 47 years; range 15-86) seen in a renal clinic in a 13-year period with NS were studied. Eight of the patients had or developed MD. One patient had MD (medullary thyroid carcinoma) and NS diagnosed concomitantly and two patients had MD (both malignant lymphoma) diagnosed 19 and 21 months after the diagnosis of NS, respectively. In conclusion, only few patients in a renal clinic at debut of NS do have a simultaneous MD. Furthermore, their risk of developing such a disease is at worst only slightly raised.

Serum creatine kinase-B activity in patients with chronic lower-limb ischemia and after leg amputation.

Serum creatine kinase-B (CK-B) activity was measured and electrocardiograms (ECG) recorded before and after operation in two groups of ten orthopedic patients. Group I underwent lower-limb amputation because of severe, chronic ischemia of the leg and group II (controls) had knee prosthesis implantation or knee ligament surgery. In group I the number of patients with CK-B activities exceeding the discrimination value (0.25 muka-tal/l) for acute myocardial infarction preoperatively and 2, 24, 48, 72 hours, and at 7 days postoperatively were, respectively, three, three, five, three, two and nil. In Group II the serum CK-B activity remained below the myocardial infarction discrimination value in all patients at all times. No ECG abnormalities indicating myocardial infarction appeared in any patient of either group. The study indicates that severe, chronic lower-limb ischemia and amputation of the leg may cause elevation of non-cardiac CK-B activity in serum that can interfere with enzymatic recognition of acute myocardial infarction.
