[Different flow augmentations in coronary, carotid and renal arteries with ECP in experimental canines].

Different flow augmentations were found in canine's coronary, carotid and renal arteries with ECP because of their different anatomical locations in aorta. We used the Electromagnetic Blood Flowmeters to measure the blood flow pre-ECP and during ECP. The results showed ECP increased the diastolic flow of coronary, carotid and renal arteries by 24.48% +/- 1.29%, 27.18% +/- 0.94% and 20.68% +/- 0.80% respectively, decreased their systolic flow by 4.77% +/- 0.48%, 6.45% +/- 0.99% and 9.60% +/- 0.70%, but augmented their mean flow by 18.24% +/- 2.01%, 19.38% +/- 1.47% and 8.42% +/- 0.94% respectively in experimental dogs. These data demonstrate that ECP can greatly increase the mean flow of coronary and carotid arteries but at the same time patently decrease the systolic flow of renal artery.

Telomerase activity and expression of the telomerase catalytic subunit gene in non-small cell lung cancer: correlation with decreased apoptosis and clinical prognosis.

OBJECTIVE: To investigate the level of telomerase activation (TA) and telomerase catalytic subunit (hEST2) gene mRNA in patients with non-small cell lung cancer, and determine whether they are associated with tumor cell apoptosis, stage, and clinical outcome. METHODS: Primary tumor specimens from 58 patients untreated with chemotherapy and 10 cases of histologically benign and adjacent lung tissue were analyzed. TA and hEST2 were measured by means of a modified telomerase repeat amplification protocol (TRAP) assay and in situ hybridization (ISH), respectively. Terminal deoxynucleotidyl transferase (TdT)-mediated biotin dUTP nick end-labeling (TUNEL) was used to evaluate apoptotic cells. Reverse transcription polymerase chain reaction (RT-PCR) was used to detect bcl-2 mRNA expression. RESULTS: TA and hEST2 were detected in 45 (77.6%) and 43 (74.1%) of 58 tumor specimens, respectively, and not detected in specimens of adjacent and benign lung tissue. One case expressed hEST2 as a weak positive. Statistically significant positive association was found between the level of TA and hEST2 (r = 0.85, P = 0.001). TA and hEST2 were associated with tumor stage, but not associated with tumor grade, gender and patient age. Positive rate of bcl-2 mRNA was 38 (65.5%) of 58 tumor specimens. The mean apoptotic index in the bcl-2 positive group (9.5 +/- 1.3) was lower than that in the bcl-2 negative one (19.8 +/- 2.1, P < 0.05), suggesting that apoptotic index may be inversely associated with bcl-2 expression (r = -0.48, P = 0.041). Bcl-2 expression in the TA and hEST2 positive group (92.1% and 89.4%) was higher than that in the negative one (50.0% and 45.0%, P = 0.043 and P = 0.032, respectively). The apoptotic index was lower in the TA or hEST2 positive group (8.2 +/- 1.4, 10.7 +/- 1.1) than in the negative one (20.5 +/- 1.6, 24.2 +/- 2.1, P < 0.05). A statistically significant inverse association was found between TA or hEST2 and apoptotic index (r = -0.45, P = 0.02 and r = -0.51, P = 0.001, respectively). Positive correlation was also detected between TA or hEST2 and bcl-2 expression (r = 0.86, P = 0.01 and r = 0.73, P = 0.024, respectively). The level of hEST2 mRNA and apoptotic index were associated with clinical outcome in a multivariate cox regression analysis. CONCLUSIONS: High TA and hEST2 were frequently detected in primary non-small cell lung cancer untreated with chemotherapy, having high bcl-2 expression and a low tumor cell apoptotic rate. This suggests that both TA and hEST2 are correlated with the deregulation of apoptosis. hEST2 and apoptotic index have prognostic significance in patients with non-small cell lung cancer.

[Cloning and identifying of renal cell carcinoma differentially expressed genes].

OBJECTIVE: To clone and identify RCC specially expressed genes different with normal kidney tissue. METHODS: A technique called suppression subtractive hybridization was used to construct the library which contains the differently expressing cDNAs between RCC and normal kidney. Then the RCC specially expressed genes were cloned from it. RESULTS: Human RCC subtractive library with high subtractive efficiency was set up successfully. The amplified library contains 350 positive clones. Sequence analysis was performed in 5 clones. All of the sequences were unknown before and the cDNA inserting GYLZ-RCC18 had three copies. Northern blot analysis showed that GYLZ-RCC18 cDNA expressed highly in RCC, but there was no any signal could be detected in normal kidney. Using SMART RACE technique, we obtained the full length of novel gene of GYLZ-RCC18. We also identified that GYLZ-RCC18 family contains 3 subtype genes. CONCLUSIONS: The highly efficient cDNA subtractive library lays a solid foundation for screening and cloning new and specific oncogenes or tumor suppressor genes of RCC. The novel differentially expressed genes provide an important clue to studying the mechanism of the occurrence and development of RCC.

[The recognition of the epidemic area of tsutsugamushi disease on Nan Peng Lie Island in China and the strategy of prevention].

OBJECTIVE: To investigate the region for the epidemic area of tsutsugamushi disease. METHODS: Epidemiological studies, Orientia tsutsugamushi isolation, and preventive measures were used. RESULTS: The region belonged to epidemic area of south subtropical zone. The main host was found Rattus norvegicu. The main biological vector was Leptotrombidium deliens. The seasonal trends of the quantity of Rattus norvegicu and Leptotrombidium deliens were consistent with the incidence change of the disease in the region. Orientia tsutsugamushi has been isolated from Rattus norvegicu and Leptotrombidium deliens. Data showed that Orientia tsutsugamushi isolated strains mainly belongs to Karp. Sero-epidemiological data showed that antibodies of Orientia tsutsugamushi are 100.0% positive in the residents of the region and 4.0% in army men. After taking comprehensive preventive measures, the incidence was descending. CONCLUSION: Nan Peng Lie island was proved an epidemic area of tsutsugamushi disease.

[Effect of yinchen dandao decoction on plasma fibronectin, biliary thromboxane A2 and prostaglandin I2 in human and animal suffering from acute cholangitis].

OBJECTIVE: To study the therapeutic mechanism of Yinchen Dandao decoction (YCDDD) in treating acute cholangitis. METHODS: Twenty-four patients with acute cholangitis and 36 cholangiolithiasis patients were randomly classified into the treated group and the control group, and all of these patients had undergone surgical operation. The treated group were given orally YCDDD 3 days after operation for 1 week, and plasma fibronectin (FN), biliary thromboxane A2(TXA2) and prostaglandin I2(PGI2) were measured and compared before and after oral administration of the drug. The models of cholangitis were established in rabbits, which were also randomly grouped into the treated group and the control group, and the same parameters (FN, TXA2, PGI2) were observed as in rabbits. RESULTS: The level of FN was significantly lower in cholangitis patients than that in the cholangiolithiasis patients before operation (P < 0.01), and compared to the control group, it obviously raised after YCDDD treatment (P < 0.01), the same happened in the cholangiolithiasis patients. The value of TXA2 and PGI2 in the bile was obviously higher (P < 0.01, P < 0.05) in cholangitis patients than those in chlangiolithiasis, and obviously reduced after YCDDD administration (P < 0.01). The level of FN in the treated group and the level of biliary TXA2 and PGI2 in rabbits with cholangitis were significantly higher than those in the control group (P < 0.01). CONCLUSION: YCDDD caused the increase of FN and decrease of biliary TXA2 and PGI2, which might be one of the mechanisms of the drug carried out in cholangitis patients.

[Effect of cytokines on repair of tendon injury].

OBJECTIVE: The review the effect of cytokines on repair of tendon injury and the relevant mechanism. METHODS: By broadly consulting recent issues about cytokines involved in tendon repair, a variety of cytokines with effects in repairing injured tendon was made and the possible mechanisms were summarized, with unsolved problems discussed. RESULTS: There were many cytokines participated in the procedure of tendon repair, among which insulin-like growth factor (IGF-1), transforming growth-beta 1 (TGF-beta 1) played significant roles. Most of the relevant researches were limited in experimental study in vitro. CONCLUSION: Cytokines possibly can accelerate tendon repair and show great potentials in future clinical application.

[Clinical observation on the effect of 4% pilocarpine gel used one dose per day in the treatment of glaucoma].

PURPOSE: To compare the efficacy and safety of 4% pilocarpine gel(PG) and 1% pilocarpine solution(PS). METHODS: Twenty cases(40 eyes) with ocular hypertension or primary open angle glaucoma(POAG) in early stage were treated by 4% PG(once per night) and PS(4 times per day). We compared the side-effects and the changes in the intraocular pressure (IOP), pupil size and diopter of 2 groups after medication. Self-control was chosen. RESULTS: The effect of 4% PG on IOP can last 20 hours. The IOP decreasing range of the PG group was from 9.3% to 13.8%, while the PS group was from 12.4% to 16.2%. The decreasing range at 10 AM for four weeks of the PG group was from 11.3% to 17.6% while the PS group was from 17.4% to 19.7%. There is no statistically significant difference between two groups. There are more obvious changes in the pupil size and diopter in the PS group and less complain of blurred vision in the PG group. CONCLUSION: The IOP decreasing effect of 4% PG is comparable with that of 1% PS, but the 4% PG is better than 1% PS as concerning the patient compliance and the side effects. So the 4% PG can be used as a substitute for the 1% PS in the clinic.

Mitotic recombination produces the majority of recessive fibroblast variants in heterozygous mice.

Mice heterozygous at Aprt (adenine phosphoribosyltransferase) were used as a model to study in vivo loss of heterozygosity (LOH) in normal fibroblasts. Somatic cell variants that exhibited functional loss of the wild-type Aprt in vivo were recovered as APRT-deficient cell colonies after culturing in selection medium containing 2, 6-diaminopurine (DAP), an adenine analog that is toxic only to cells with APRT enzyme activity. DAP-resistant (DAP(r)) fibroblast variants were recovered at a median frequency of 12 x 10(-5) from individual ears from progeny of crosses between mouse strains 129/Sv and C3H/HeJ. The frequency of DAP(r) variants varied greatly among individual ears, suggesting that they preexisted in vivo and arose at various times during development. Polymorphic molecular markers and a cytological marker on the centromere of chromosome 8 made it possible to discriminate between each of six possible mechanistic pathways of LOH. The majority (about 80%) of the DAP(r) variants were a consequence of mitotic recombination. The prevalence of mitotic recombination in regions proximal to Aprt did not correlate with meiotic map distances. In particular, there was a higher than expected frequency of crossovers within the interval 59 cM to 67 cM. The high spontaneous frequency of Aprt LOH, mediated primarily by mitotic recombination, is fully consistent with our previous results with human peripheral T cells from individuals known to be heterozygous at APRT. Thus, this Aprt heterozygote mouse is a valid model for studying somatic mutagenesis and mitotic recombination in vivo.

Phosphorylation of CD45 by casein kinase 2. Modulation of activity and mutational analysis.

CD45 is a receptor-type protein-tyrosine phosphatase (PTP) that is required for antigen-specific stimulation and proliferation in lymphocytes. This study was designed to determine the nature of specific kinases in lymphocytes that phosphorylate CD45 and to determine the effect of phosphorylation on CD45 PTP activity. A major cytoplasmic lymphocyte kinase that phosphorylated CD45 was identified as casein kinase 2 (CK2) by use of an in-gel kinase assay in combination with immunoprecipitation, immunodepletion, and specific inhibition. Mutational analysis of CK2 consensus sites showed that the target for CK2 was in an acidic insert of 19 amino acids in the D2 domain, and Ser to Ala mutations at amino acids 965, 968, 969, and 973 abrogated CK2 phosphorylation of CD45. CK2 phosphorylation increased CD45 activity 3-fold toward phosphorylated myelin basic protein, and this increase was reversible by PP2A treatment. Mutation of Ser to Glu at the CK2 sites had the same effect as phosphorylation and also tripled the Vmax of CD45. CD45 isolated in vivo was highly phosphorylated and could not be phosphorylated by CK2 without prior dephosphorylation with phosphatase PP2A. We conclude that CK2 is a major lymphocyte kinase that is responsible for in vivo phosphorylation of CD45, and phosphorylation at specific CK2 sites regulates CD45 PTP activity.

Usefulness of diffusion-weighted MRI with echo-planar technique in the evaluation of cellularity in gliomas.

The purpose of this study was to evaluate the utility of diffusion-weighted magnetic resonance imaging (MRI) with echo-planar imaging (EPI) technique in depicting the tumor cellularity and grading of gliomas. Twenty consecutive patients (13 men and 7 women, ranging in age from 13 to 69 years) with histologically proven gliomas were examined using a 1.5 T superconducting imager. Tumor cellularity, analyzed with National Institutes of Health Image 1.60 software on a Macintosh computer, was compared with the minimum apparent diffusion coefficient (ADC) and the signal intensity on the T2-weighted images. The relationship of the minimum ADC to the tumor grade was also evaluated. Tumor cellularity correlated well with the minimum ADC value of the gliomas (P = 0.007), but not with the signal intensity on the T2-weighted images. The minimum ADC of the high-grade gliomas was significantly higher than that of the low-grade gliomas. Diffusion-weighted MRI with EPI is a useful technique for assessing the tumor cellularity and grading of gliomas. This information is not obtained with conventional MRI and is useful for the diagnosis and characterization of gliomas.

3D TOF turbo MR angiography for intracranial arteries: phantom and clinical studies.

This work investigated whether turbo magnetic resonance angiography (MRA) can replace conventional MRA in screening examination of intracranial arteries. A phantom was used to evaluate the effect of the zero-filling interpolation (ZFI) technique on spatial resolution and partial volume effect. Thirty-one consecutive patients underwent both turbo MRA with a slice thickness of 0.7 mm (data were measured as 1.33 mm sections) and conventional MRA with 1.0 mm sections. In the phantom studies, ZFI did not improve the spatial resolution, but the partial volume effect was somewhat reduced. In the clinical evaluation, turbo MRA showed better signal-to-noise and contrast-to-noise ratios of the intracranial major vessels than conventional MRA. The lesions included cerebral aneurysms less than 3 mm in diameter, occlusive vascular disease, arteriovenous malformations, and arteriovenous fistulas. These were all depicted on both turbo MRA and conventional MRA. Turbo MRA is a useful screening procedure because of its capability of delineating lesions in approximately half the usual imaging time. J. Magn. Reson. Imaging 1999;10:939-944.

Value of dynamic susceptibility contrast magnetic resonance imaging in the evaluation of intracranial tumors.

The degree of tumor malignancy generally correlates to tumor grade, and the direct measurement of tumor vasculature is desired. Dynamic susceptibility contrast magnetic resonance imaging can provide relative cerebral blood volume and, therefore, is one of the most reliable methods to evaluate tumor vasculature in vivo. Tumor vessel size is extremely variable due to complex tumor angiogenesis, and the gradient-echo echo-planar imaging (GE-EPI) technique, which is sensitive to the total vascular bed, is well suited for this purpose. As many studies have shown, dynamic susceptibility contrast magnetic resonance imaging is more useful for grading glioma than conventional magnetic resonance imaging. We found that this technique can also provide supplementary information to differentiate between malignant lymphoma and glioma because the absence of tumor neovascularization of malignant lymphoma leads to low rCBV, which is in contrast to those of malignant gliomas. Indeed, this technique can be used for the differentiation of extra-axial tumors such as between meningioma and neurinoma. Recently, this technique has been focused toward determining the stereotactic biopsy site, monitoring the embolization of effect in meningioma, or evaluation of treatment effects after radiation therapy. However, the value of tumor rCBV is affected by many conditions such as the T1 relaxivity effects of gadolinium in the extravascular space. To establish the usefulness of this technique, further examination will be needed.

Brain lesions: when should fluid-attenuated inversion-recovery sequences be used in MR evaluation?

PURPOSE: To compare qualitatively and quantitatively the contrast of brain lesions detected with fluid-attenuated inversion-recovery (FLAIR) and intermediate-weighted sequences at magnetic resonance (MR) imaging. MATERIALS AND METHODS: In this prospective study, 47 patients suspected of having a brain lesion underwent MR imaging with FLAIR, intermediate-weighted, and T2-weighted sequences. Qualitative assessment was performed of lesion conspicuity, detection, overall image artifact, and additional clinical information. Contrast and contrast-to-noise ratio (CNR) were calculated between lesions and the normal brain or cerebrospinal fluid (CSF). RESULTS: FLAIR images were equal to intermediate-weighted images for overall lesion conspicuity and detection but were associated more often with image artifacts. Lesion-to-background contrast was significantly higher on FLAIR than on intermediate-weighted images. FLAIR images failed to demonstrate multiple sclerosis (MS) plaques located in the basal ganglia and brain stem. CONCLUSION: Although FLAIR images provided additional information in some cases, they did not have distinct advantages over intermediate-weighted images. When cases of MS are evaluated, intermediate-weighted images are preferable to FLAIR images. Except in cases of MS, either FLAIR or intermediate-weighted sequences should be added to T2-weighted sequences at MR imaging.

Insulin receptor substrate-1 enhances growth hormone-induced proliferation.

GH exerts a variety of metabolic and growth-promoting effects. GH induces activation of the GH receptor (GHR)-associated cytoplasmic tyrosine kinase, JAK2, resulting in tyrosine phosphorylation of the GHR and activation of STAT (signal transducer and activator of transcription), Ras-mitogen-activated protein kinase, and phosphoinositol 3-kinase signaling pathways, among others. GH-stimulated tyrosine phosphorylation of insulin receptor substrate (IRS) proteins has been demonstrated in vitro and in vivo. IRS-1 is a multiply phosphorylated cytoplasmic docking protein involved in metabolic and proliferative signaling by insulin, IL-4, and other cytokines, but the physiological role of IRS-1 in GH signaling is unknown. In this study, as noted by others, we detected in murine 3T3-F442A pre-adipocytes GH-dependent tyrosine phosphorylation of IRS-1 and specific GH-induced coimmunoprecipitation with JAK2 of a tyrosine phosphoprotein consistent with IRS-1. We further examined this interaction by in vitro affinity precipitation experiments with glutathione-S-transferase fusion proteins incorporating regions of rat IRS-1 and, as a source of JAK2, extracts of 3T3-F442A cells. Fusion proteins containing amino-terminal regions of IRS-1 that include the pleckstrin homology, phosphotyrosine-binding, and Shc and IRS-1 NPXY-binding domains, but not those containing other IRS-1 regions or glutathione-S-transferase alone, bound JAK2 from cell extracts. Tyrosine-phosphorylated JAK2 resulting from GH stimulation was included in the amino-terminal IRS-1 fusion precipitates; however, neither tyrosine phosphorylation of JAK2 nor treatment of cells with GH before extraction was necessary for the specific JAK2-IRS-1 interaction to be detected. In contrast, in this assay, specific insulin receptor association with the IRS-1 phosphotyrosine-binding, and Shc and IRS-1 NPXY-binding domains was insulin and phosphotyrosine dependent, as previously shown. To test for significance of IRS-1 with regard to GH signaling, IRS- and GHR-deficient 32D cells were stably reconstituted with the rabbit (r) GHR, either alone (32D-rGHR) or with IRS-1 (32D-rGHR-IRS-1). As assayed by three independent methods, GH induced proliferation in 32D-rGHR cells, even in the absence of transfected IRS-1. Notably, however, GH-induced proliferation was markedly enhanced in cells expressing IRS-1. Similarly, GH-induced mitogen-activated protein kinase activation was significantly augmented in IRS-1-expressing cells relative to that in cells harboring no IRS-1. These results indicate that IRS-1 enhances GH-induced proliferative signaling.

Detection of intracranial hemorrhage with susceptibility-weighted MR sequences.

BACKGROUND AND PURPOSE: Detection of hemorrhage is important in the diagnosis and management of a variety of intracranial diseases. We evaluated the sensitivity of the following sequences for depicting chronic hemorrhagic foci associated with susceptibility dephasing: gradient-recalled echo (GRE) imaging, GRE-type single-shot echo-planar imaging (GRE-EPI), spin-echo-type single-shot echo-planar imaging (SE-EPI), turbo spin-echo (TSE) imaging, half-Fourier single-shot turbo spin-echo (HASTE) imaging, and segmented HASTE (s-HASTE) imaging. To our knowledge, no previous comparison has been made with these techniques in the same patient. METHODS: Fifty patients with suspected chronic hemorrhage were examined prospectively with the above six sequences. Contrast-to-noise ratio (CNR), sensitivity to detection of lesions, conspicuity of internal architecture, and sensitivity to small hemorrhagic foci were evaluated. RESULTS: Hemorrhagic foci were found in 35 patients. The CNR of the GRE, GRE-EPI, SE-EPI, TSE, s-HASTE, and HASTE sequences was 30.9, 23.7, 3.6, 6.1, -29.3, and -13.1, respectively; the number of small hemorrhagic foci detected was 85, 96, 44, 22, two, and one, respectively, for the supratentorial white matter; 70, 40, 19, four, zero, and zero, respectively, for the supratentorial cortical/subcortical region; and 73, 50, 26, 37, zero, and zero, respectively, for the infratentorial/skull-base region. CONCLUSION: The GRE sequence was best for detecting susceptibility dephasing associated with chronic intracranial hemorrhage. GRE-EPI, while comparable to GRE in the supratentorial compartment, was reduced in its sensitivity near the skull base, and may be used as an alternative to GRE in uncooperative, unsedated, pediatric, or claustrophobic patients. SE-EPI should not be used in screening for intracranial hemorrhage.

Contrast enhancement of intracranial lesions: conventional T1-weighted spin-echo versus fast spin-echo MR imaging techniques.

BACKGROUND AND PURPOSE: The T1-weighted fast spin-echo (T1-FSE) MR imaging sequence is not used routinely, since the speed advantage is not as dramatic as it is in T2-weighted imaging. We evaluated the T1-FSE sequence to determine whether this technique can replace the conventional T1-weighted spin-echo (T1-SE) sequence for routine contrast-enhanced imaging. METHODS: Sixty-nine patients with intracranial enhancing lesions underwent both T1-SE and T1-FSE sequences in a random order after administration of contrast agent. Acquisition time was 55 seconds for the T1-FSE sequence and 2 minutes 38 seconds for the SE sequence. The conspicuity of enhancing lesions, peritumoral edema, and gray-to-white matter contrast as well as motion and flow artifacts were analyzed. Signal-to-noise ratios of enhancing lesions, gray matter, and white matter as well as contrast-to-noise ratios (CNRs) of enhancing lesions, with gray matter with white matter as the standard, were calculated. RESULTS: The conspicuity of enhancing lesions was better on T1-FSE sequences than on T1-SE sequences, although the difference in the CNRs of enhancing lesions did not reach significance. Images obtained with the T1-FSE sequence showed less flow and motion artifacts than did those obtained with the T1-SE sequence. The conspicuity of peritumoral edema and gray-to-white matter contrast was lower on the T1-FSE images than on the T1-SE images. CONCLUSION: The T1-FSE sequence reduces imaging time and has the potential to replace the conventional T1-SE sequence for the evaluation of enhancing lesions in the brain when time is a consideration.

Portal hemodynamics in cirrhotics with portal hypertension using color Doppler velocity profile.

OBJECTIVE: To investigate portal hemodynamics and its correlation with esophageal variceal bleeding (EVB) in cirrhotics with portal hypertension by using a newly-developed technique, color Doppler velocity profile (CDVP). METHODS: Hemodynamics of portal trunk (PT), right anterior branch (RAB) and splenic vein (SV) were evaluated in 48 cirrhotics with portal hypertension and 35 normal volunteers by CDVP. The parameters included maximum cross-sectional velocity (CS-Vmax), flow volume, congestion index (CI), profile parameter N and pattern of flow curve. Stepwise logistic regression model was employed to determine EVB-relating factors in cirrhotics. RESULTS: CS-Vmax in PT and RAB was significantly lower in cirrhotic group than that of normal group, being 14.91 +/- 3.08 cm/s, 9.44 +/- 2.70 cm/s vs 30.52 +/- 6.75 cm/s, 12.82 +/- 2.69 cm/s, respectively (P < 0.01 for both). Flow volume of PT and SV was significantly higher in cirrhotic group compared with that of normal group, being 25.16 +/- 10.48 ml.min-1.kg-1, 15.83 +/- 9.18 ml.min-1.kg-1 vs 20.43 +/- 5.57 ml.min-1.kg-1, 5.81 +/- 2.04 ml.min-1.kg-1, respectively (P < 0.01 for both). CI of PT, RAB and SV was significantly higher in cirrhotic group than in normal group, being 0.142 +/- 0.0654, 0.105 +/- 0.0496, 0.0884 +/- 0.0431 vs 0.0326 +/- 0.0142, 0.0757 +/- 0.0342, 0.0483 +/- 0.0230, respectively (P < 0.01 for all). In dynamic variation of flow volume over time, RAB and SV in cirrhotic group increasingly presented flat pattern and periodically changed pattern, respectively (P < 0.01 for both). Between cirrhotic subgroups without and with EVB history, there were significant differences in flow volume, CI and N value of SV, splenic size, degree of esophageal varices (EV) and portal hypertensive gastropathy (PHG), and stepwise logistic regression revealed that N value of SV, splenic size, degree of EV and PHG were four independent factors in relation to EVB. EVB scores calculated from the regression equation had a close correlation with EVB. In patients with EVB score > 0, 88.9% of them had EVB, and in those with EVB < 0, 76.9% of them had no history of EVB. CONCLUSION: In cirrhotics with portal hypertension, portal venous system has the features of elevated vascular resistance and hyperdynamics, and the latter mainly results from increased blood flow in SV. EVB score may become a valuable parameter in predicting occurrence of EVB.

[Evaluation of therapeutic effects of portoenterostomy: reported of 426 cases].

OBJECTIVES: To investigate the selection of the types of anastomosis and to evaluate the therapeutic effects of portoenterostomy. METHODS: From 1993 to 1998, 426 patients with obstructive jaundice underwent portoenterostomy. The patients included 236 cases (56%) of hilar strictures in hepatolithiasis, 98 cases (23%) of end-stage periampullary tumors, 64 cases (15%) of proximal cholangiocarcinoma, and 28 cases (6%) of congenital choledochus cyst. Reconstruction of biliary tract was performed after resection of affected duct or palliative bypass for end-stage carcinoma. The types of anastomosis included extra or hilar hepatic bilioenterostomy (160 cases, 37%), and intra-hepatic duct anastomosis with different type of hepatectomy (266 cases, 63%), in which anastomosis was performed by round ligament approach (34 cases, 8%), and anastomosis through gallbladder fossa (15 cases, 4%). RESULTS: Short- and 1-6 year follow-up showed that jaundice of patients was completely relieved by suitable type of bilioenterostomy. CONCLUSIONS: The good therapeutic effects of bilioenterostomy are based on the correct selection of anastomosis methods.
