[Injury of femoral artery complicated with infection from injection of heroine].

Drug addiction has been one of the serious social problems. The peripheral phlebitis caused by drug injection is common, but the occurrence of pseudoaneurysm with infection of femoral artery from injection injury was rarely reported in China. From January 1995 to March 1996, six cases of injury of femoral artery with infection from heroine injection were admitted. The characteristics of the injury were described. The therapeutic measures and details of attention to be needed were discussed. It was concluded that this type of injury was different from the injury caused in agricultural, industrial or traffic accidents. The treatment of choice depended upon the type of injury.

[Effects of supplement Zn on levels of Zn in serum, growth hormone and hydroxyproline].

OBJECTIVE: In order to research the effects of supplement Zn on wound healing, the changes of Zn in serum and skin, growth hormone and hydroxyproline of skin were observed in burn rats. METHOD: 80 SD rats were randomly divided into four groups: Control group of 8 rats, N group (diet containing Zn 40 micrograms/g), W group (application of Zn on burn skin), H group (diet containing Zn 80 micrograms/g). 8 rats of each group were killed on 1, 3, 7 days after burn. RESULTS: The levels of serum Zn declined on 1 day postburn in N and W groups, whereas the content of serum Zn in H group elevated and were 2.4, 2.0 times as much as that of N and W groups respectively (P < 0.01). Zn in burn skin increased significantly on 3, 7 days in W group and was 1.3, 1.5 times as much as that of N group. Serum growth hormone increased in all groups postburn. The highest level appeared in H group. Hydroxyproline in burn skin decreased in all groups, but it began to increase on 7 day postburn in W group. CONCLUSION: Supplementation of Zn by diet rapidly raises the levels of Zn in serum, burn skin and growth hormone. It is beneficial to apply Zn to the wound to improve wound healing. Hydroxyproline increased significantly with application of Zn to burn wound. It is better to supply Zn by diet and topical application simultaneously.

[Pathological and immunohistochemical study on estrogen and progesterone receptors in endometrium of polycystic ovarian syndrome].

OBJECTIVE: To investigate the changes of endometrium in infertility patients with polycystic ovarian syndrome (PCOS). METHODS: The endometrium from 39 patients with PCOS were studied by HE stain and immunohistochemical method using anti-estrogen receptor(ER) and anti-progesterone receptor(PR) monoclonal antibodies. RESULTS: 87.2% (34/39) of the endometrium showed anovulatory proliferative phases; 51.3% (20/39) had signs of hyperplasia; 35.9% (14/39) showed asynchronization of endometrial glands and 46.2% (18/39) poor response of stroma to sex hormone. The levels of ER and PR in endometrium during proliferative phase were significantly higher than those of normal control, and the PR in the stroma of hyperplasia endometrium had an uneven distribution and was significantly less than that in the control group (P < 0.05). CONCLUSIONS: The endometrial histopathological changes and local decrease or absence of ER and PR in PCOS might be one of the causal factors of infertility.

[The expression of glutathione S-transferase pi in human ovarian cancer as an indicator of resistance to chemotherapy].

OBJECTIVE: To study the relationship between the expression of glutathione S-transferase pi (GST-pi) in cancer tissue and the chemoresistance in patients with ovarian carcinoma. METHODS: The expression of GST-pi in 53 cases of ovarian cancer, 20 ovarian benign tumors and 17 normal controls was determined by using immunohistochemical SP method and the results were studied in correlation with some clinical and pathological data. RESULTS: (1) Positive expression with GST-pi was demonstrated in 60.4% of malignant and 10.0% of benign ovarian tumors while the expression was negative in normal controls. (2) No significant difference for the expressions was shown in relation to the histopathology, the clinical staging and the size of residual tumors after cytoreductive surgery. (3) The positive rate of the expression was 47.2% (17/36) for the initially treated patients while those with tumor recurrence had a positive rate of 88.2% (15/17). (4) GST-pi positive cases showed less response rate of 37.5% (12/32) to chemotherapy as compared with that of 76.2%(16/21) for GST-pi negative cases. (5) The survival period of the patients with GST-pi positive expression was shorter than that of those with GST-pi negative expression. CONCLUSION: The expression of GST-pi in patients with ovarian carcinoma in closely related to the chemosensitivities clinically. Determinations of the GST-pi are useful for predicting the chemosensitivities and the prognosis of the disease.

[Clinical study of fructose 1,6-diphosphate on myocardial ischemia/reperfusion injuries].

Twenty ventricular septal defect patients who underwent cardiopulmonary bypass (CPB) were divided into 1,6-diphosphate group and controlled group (10 cases each). Blood samples were taken at pre- and post-cardiopulmonary bypass 30 min, 6 h, and 24 h to determine lactate dehydrogenase enzyme(LDH), creatine kinase enzyme(CK) and its isoenzyme(CK-MB) levels. We found that 1,6-diphosphate decreased the increasing levels of LDH, CK, and CK-MB after cardiopulmonary bypass. It is suggested that 1,6-diphosphate may attenuate myocardial ischemia/reperfusion injuries during cardiopulmonary bypass.

[Influence of isoflurane and sevoflurane on metabolism of oxygen free radicals in cardiac valve replacement].

To investigate effects of isoflurane and sevoflurane on the levels of lipid peroxides (LPO) in plasma and superoxide dismutase (SOD) in red blood cells during myocardial ischemia/reperfusion, and to study myocardial protection of these two volatile anesthetcs, 30 patients undergoing cardiac valve replacement were randomly divided into isoflurane (Group I), sevoflurane (Group S), and controlled group (Group C). Patients of Group I were anesthetized with isoflurane mainly, Group S with sevoflurane, and Group C with fentanyl. LPO and SOD levels were measured at five points before and during the operation. The results were as follows: SOD levels in all groups did not change significantly. LPO levels in Group C increased during the operation, while in other two groups didn't. Rates of automatical rebeating of hearts in Group C were lower than that of hearts in other two groups. It is suggested that isoflurane and sevoflurane can inhibit increasing of LPO levels to attenuate myocardial ischemia/reperfusion injuries mediated by oxygen free radicals during open-heart surgery.

[The clinical features and management of borderline ovarian tumors].

OBJECTIVE: To study the clinical features and management of borderline ovarian tumor (BOT). METHODS: Retrospectively study 70 cases of BOT. RESULTS: Age 16-76 years old, average 45.6 year old; histological type: mucinous 46(65.7%), serous 21(30%); stage: I 46 (65.7%), II 1(1.4%), III 11(15.7%), IV 0%, relapse 12(17.1%). Tumor marker: elevated CA125 11/21, carcinoembryonic antigen (CEA) 6/10. TREATMENT: all patients were operated, 45 were given adjuvant therapy. OUTCOME: No evidence of disease (NED) 45, with evidence of disease (WED) 4, died of disease (DOD) 11, died of intercurrent disease (DID) 10. 11 patients who were given conservative surgery were NED, there were 2 long term CR among 16 patients with residual disease who were given adjuvant treatment. The 5, 10, 20 and 30-year survival is 90.0%, 87.1%, 87.1% and 85.6% respectively. COX hazard regression model analysis showed that histological type (r = 24.825), stage (r = 2.236) and residual disease (r = 2.083) were the main factors which impact on the prognosis of BOT. CONCLUSIONS: (1) The proportion of mucinous tumor is higher than that of serous tumor in this group; (2) Surgery is the proved only effective treatment of BOT till now, without any residual disease is expected; (3) Adjuvant therapy can't improve the prognosis of BOT; (4) CA125 and CEA can be two effective tumor markers in monitoring of BOT.

[The effect of 25 micrograms misoprostol on induction of labor in late pregnancy].

OBJECTIVE: To determine the efficacy and safety of 25 micrograms misoprostol for labor induction in the third trimester of pregnancy. METHODS: 48 term pregnant women with single pregnant vertex present intact membrane were selected in study group. All case in study group indications for labor induction, without contra-indication of labor induction and misoprostol. The 48 women were randomly assigned A(25 micrograms) and B(50 micrograms) misoprostol groups. Misoprostol was placed in the posterior vaginal fornix every 4 to 6 hours. The maximum dose in 24 hours is 200 micrograms. Misoprostol was not given after either spontaneous rupture of membrane or beginning of active labor. RESULT: The sucesess rate of induction labor was groups 77.8%, 81.0% in group A and B, respectively. The average duration from administration of misoprostol to start of labor in two groups was 796.9 +/- 359.9 minutes and 807.4 +/- 405.2 minutes, respectively; the average duration from start of misoprostol use to vaginal delivery was 978.6 +/- 464.4 minutes, 977.5 +/- 421.4 minutes, respectively. The incidence of vaginal delivery had no statistics significant difference between two groups (P > 0.05). The incidence of hypertonic uterine contraction or hypertonic contractionchysystole with abnormal pattern of fetal monitoring in group A was lower than that of group B, but there was no significant difference (P > 0.05). CONCLUSION: 25 micrograms misoprostol is an efficient and safe dosage for labor induction in term pregnancy.

[Normal-sized ovary carcinoma syndrome--clinical and pathological analysis of 11 cases].

OBJECTIVE: To evaluate clinical-pathologic features, diagnostic criteria, effective treatment and prognosis of a clinically rare "normal-sized ovary carcinoma syndrome". METHODS: Histologic slides of 11 cases from 1984 to 1993 of normal-sized ovary carcinoma syndrome were retrospectively reviewed. Seven of the 11 cases were extraovarian peritoneal serous papillary carcinoma (EPSPC). The remaining 4 cases were serous adenocarcinoma of the ovary (2 cases), malignant mesothelioma (1 case) and metastatic poorly differentiated adenocarcinoma of unknown origin (1 case). All patients received relatively thorough surgical resection combined with multiple courses of adjuvant chemotherapy. RESULTS: The patient with malignant mesothelioma survived for 90 months, one case with EPSPC survived for 87 months up to Dec 1997. The average survival time of 9 patients who died of their diseases was 17.8 months, being longer than that reported in the literature. CONCLUSION: Following accurate diagnosis of normal-sized ovary carcinoma syndrome, radical tumor resection is the first treatment of choice. For residual tumor, postoperative chemotherapy or radiotherapy is essential to obtain better prognosis.

[Transcatheter arterial chemo-embolization combined with CT-guided percutaneous intratumoral injection of lipiodol-ethanol for the treatment of primary hepatocellular carcinoma].

OBJECTIVE: To study the therapeutic effect of transcatheter arterial chemoembolization (TACE) combined with CT-guided percutaneous intratumoral injection of lipiodol-ethanol(ITLE) for primory hepatocellular carcinoma. METHODS: One hundred and two patients with primary hepatocellular carcinoma (PHC) were divided into two groups. In group A, 52 patients treated with TACE, and in group B, 50 patients were treated with TACE and ITLE. RESULTS: In group A 1-, 2-, and 3-year survival rate of the treated patients was 68.3%, 41.2%, and 21.4%, respectively. Histologic examination of cancer specimens obtained from hepatectomy and biopsy showed that TACE caused complete necrosis in only 26.1% of the treated cases. In contrast, in group B patients treated with TACE combined with ITLE, the 1-, 2-, and 3-year survival rate was 100%, 85.2% and 68.4%, respectively. Combination therapy resulted in complete histologic necroses in 86.4% of the treated cases. CONCLUSION: The therapeutic effect of TACE combined with CT-guided ITLE is much better than that of TACE alone.

[Analysis of clincal poliomyelitis in Shandong Province from 1993-1996].

Wild poliovirus has not been isolated in Shandong province since Apr. 1991. However, 71 clinically polio cases were diagnosed by an expert group, including 50 potential Vaccine Associated Paralytic Poliomyelitis (VAPP) including 31 having the sabin strain in the stools, from 1993 to 1996. The overall frequency of VAPP was one case per 1.25 million doses distributed. The relative frequencies of first dose in the OPV series was one case per 218,800 doses vs one case per 1.01 million for second dose. Based on data from the 31 cases, we estimated that the rates had been one case per 438,000 first dose and one case per 6.42 million second dose and one case per 10.44 million among the contacts of the cases. This results seemed to be higher than from other reports. Clinically confirmed poliomyelitis cases in Shandong were diagnosed by the experts group. The classification of clinical polio cases is important in the polio eradication program.

[Alteration of fatty acid components of erythrocyte membrane and insulin resistance in diabetic patients].

OBJECTIVE: To study the effect of alteration of erythrocyte membrane fatty acid components and fluidity on insulin resistance (ISR) in NIDDM patients. METHODS: The fatty acid(FA) components and fluidity of erythrocyte membrane in 52 NIDDM patients with and without hyper-insulinemia were examined by a high-performance liquid chromatographic method and flourescence polarization technique. RESULTS: The content and composition erythrocyte membrane arachidonate (AA) were significantly lower in NIDDM patients than in controls, and also markedly lower in patients with hyperinsulinemia than those without hyperinsulinemia. The diabetics had increased erythrocyte membrane microviscosities as compared with controls, and membrane microviscosities were higher in diabetics with hyperinsulinemia than those without hyperinsulinemia. AA contents of erythrocyte membrane were negatively correlated with membrane membrane microviscosities and positively with ISI in NIDDM patients. CONCLUSION: In NIDDM patients, hyperinsulinemia and ISI bring about AA metabolic abnormalities and decreased AA contents in cell membrane. Alteration of cell membrane FA components results in the changes of membrane fluidity, and the latter can increase IRS.

[PCR/SSCP used as a complementary method for HLA oligotyping].

Seventy samples randomly selected from Hunan Han nationality normal individuals were typed for HLA DPA1 polymorphism by PCR/SSO with the primers and SSOs. Silver staining PCR/SSCP was also performed on the polymorphic second exon of all samples, which demonstrated the accurate gene frequencies of DPA1*01,02 to be 0.2632 and 0.7327, respectively. Using PCR/SSCP technique, we found that PCR/SSO mistyped a DPA1*01/02 heterozygote as single DPA1*01 gene because of weak hybridization signals. PCR/SSCP also revealed DPA1*02 in Hunan Han population to be composed of a single subtype. The results indicate that PCR/SSCP can be used to clarify the samples with atypical or irregular PCR/SSO hybridization patterns and to analyze the possible polymorphisms outside SSOs sequences. Thus this method may guarantee the accuracy of HLA oligotyping.

Pathological characteristic of Alzheimer's disease produced by lesion in nucleus basalis of Meynert in rats.

OBJECTIVE: To find out if the lesion in nucleus basalis of Meynert (nbM) can induce some morphological changes characteristic of Alzheimer's disease. METHODS: Kainic acid was injected into nbM of the rats, and the behavioral deficiency and the morphological changes in the cortex and hippocampus were observed by methenamine silver staining and electron microscopical examination. RESULTS: After 9-15 months of breeding following nbM-lesion, we observed many pathological changes in this animal model, which were characteristic of Alzheimer's disease in human, and especially we could find for the first time the formation of senile plagues after 15-month breeding. CONCLUSION: It is proposed that the degeneration of nbM neurons might be primary and responsible for the pathological changes in other brain tissues in sporadic Alzheimer's disease.
