Bone marrow involvement in non-Hodgkin's lymphoma in Zimbabwe.

Lymph node and bone marrow trephine biopsies of seventy two consecutive cases of non-Hodgkin's lymphoma occurring among black Zimbabweans were reviewed to determine bone marrow involvement. The bone marrow was involved in 23.6% of patients but the proportion of bone marrow positive cases was highest in low grade lymphomas, though these were the least common type of lymphoma encountered. Cells infiltrating the marrow showed high degree of concordance with corresponding lymph node histology. Low grade lymphomas (CLL excluded) had a predominantly diffuse pattern of marrow involvement while intermediate and high grade lymphomas had nearly equal proportions with diffuse and focal patterns. Bone-marrow involvement did not make a significant difference to the staging of non-Hodgkin's lymphoma as most cases not involving the marrow also presented with advanced disease. Mean age of patients was negatively correlated with histological grade but was not related to bone marrow involvement. Central nervous system involvement occurred mostly in the high grade lymphomas and was proportionately distributed between bone marrow positive and negative cases. 40% of non-Hodgkin's lymphoma were associated with HIV infection but HIV-associated lymphoma surprisingly very rarely infiltrated the bone marrow.

Protection of mice against mycobacterial infection by lymphoid cell vaccination.

Intracellular parasites may thrive by inducing the host's immune system to suppress the effector immune response that otherwise limits multiplication. Hosts are traditionally immunized with the parasite antigens that induce effector immunity. Alternatively, one might vaccinate the host with the host lymphoid cells involved in suppression. Multiplication of Mycobacterium marinum was prevented by vaccinating mice with cells prepared from the popliteal lymph nodes of mice in which the organisms were multiplying logarithmically, that were inactivated by fixation with glutaraldehyde. Cells obtained later during infection, when the donor mice manifest immunity, did not protect against infection. Thus, it may be possible to influence the course of a microbial infection by immunizing the host not only with components of the organisms, but also with the host components that are exploited by the organism.

Transient exercise-induced water intoxication and rhabdomyolysis.

Water loading only rarely results in adverse effects due to the high efficiency of the kidney in excreting free water. However, when renal diluting ability is impaired, such as in inappropriate vasopressin secretion, water intoxication can occur in otherwise normal individuals. We report the case of a 19-year-old man with acute voluntary water intoxication following exercise, which resulted in a transient defect in renal diluting capability. Hyponatremia was further complicated by rhabdomyolysis. We review the literature regarding other cases of hyponatremia following excessive water intake, and discuss the possible association between hyponatremia and rhabdomyolysis. We conclude that monitoring of muscle enzymes is indicated in acute hyponatremia, to allow for timely intervention intended to prevent rhabdomyolysis-associated acute renal failure.

Presence and characterization of the somatostatin precursor in normal human pituitaries and in growth hormone secreting adenomas.

Normal and tumoral human pituitaries release in vitro SRIH and contain messenger RNAs encoding preproSRIH. In the present study, we document the presence and characterization of the SRIH precursor in both human normal pituitaries and in GH-secreting adenomas. Molecular sieve filtration of normal pituitary and adenoma acid extracts revealed the presence of three immunoreactive SRIH peaks, distinct from SRIH 1-28 and SRIH 1-14. Western blot analysis performed under both nonreducing and reducing conditions confirmed the presence of the three different immunoreactive forms with respective molecular masses estimated as 21, 17, and 12 kilodaltons. When analyzed in reverse phase high performance liquid chromatography, the three immunoreactive SRIH material contained in the three peaks coeluted with the proSRIH extracted from rat hypothalamus and from a neuroblastoma cell line (N2A) transfected with the human preproSRIH complementary DNA. None of these three forms could bind concanavalin A. Digestion of the three forms with a specific endopeptidase resulted in the generation of SRIH 1-14, indicating that they can be considered as SRIH precursor or related forms. Estimation of proSRIH amount after molecular sieve filtration indicated that normal human pituitaries (n = 4) contained proSRIH in variable amounts (from 26.5-303 pg/mg proteins), the 21-, 17-, and 12-kilodalton forms being always present, in addition to SRIH 1-28. In contrast, among the 21 adenomas tested, only 11 contained proSRIH material (from 9-4480 pg/mg proteins). Moreover, neither SRIH 1-28 nor SRIH 1-14 could be detected in these adenomas. These data demonstrate the presence of high molecular mass SRIH in normal pituitaries which represent unprocessed proSRIH material. This provides an additional argument for a local synthesis of SRIH. Additionally, the fact that only 50% of GH-secreting pituitary adenomas here studied contain exclusively proSRIH may reflect an alteration in messenger RNA translation or in proSRIH processing, or both, resulting in the absence of mature SRIH in these adenomas.

Desflurane versus propofol anesthesia: a comparative analysis in outpatients.

This study compares the induction, hemodynamic, and recovery characteristics of a general anesthetic with desflurane to one with propofol. Sixty outpatients presenting for orthopedic surgery received either a propofol induction of anesthesia followed by desflurane and nitrous oxide (Group 1), a propofol induction followed by propofol infusion and nitrous oxide (Group 2), a desflurane and nitrous oxide induction and maintenance (Group 3), or a desflurane induction and maintenance (Group 4). The quality of induction was inferior in Groups 3 and 4 with more breath-holding and excitation than in Groups 1 and 2. However, there was a more rapid emergence in Group 4 patients than any of the other groups. Group 4 patients were able to say their names (5.6 +/- 2.0 min vs 10.3 +/- 3.3 min, 8.6 +/- 3.1 min, and 9.3 +/- 1.5 min for Groups 1, 2, and 3, respectively) sooner after the discontinuation of anesthesia. Nonetheless, intermediate recovery was similar in Groups 2 and 4 being numerically but not statistically more rapid than in Groups 1 and 3. This pattern of intermediate recovery was also demonstrated by psychomotor function test results. Although there was no difference between the groups in postoperative narcotic requirement, more patients in Group 3 vomited (50%) than in either Group 2 (0%) or Group 4 (12.5%). Hemodynamically, the anesthetics were very similar. Although desflurane was a difficult drug to use for induction of anesthesia, this study demonstrates that desflurane is a suitable maintenance anesthetic for ambulatory surgery because it provides a rapid awakening and an intermediate recovery similar to propofol.

Modest restriction of dietary protein during pregnancy in the rat: fetal and placental growth.

Pregnant rats were fed to appetite on diets containing 6, 9, 12 or 18% protein and the fetuses were delivered at 20 days. Compared with the 18% protein diet (7.4 MJ), there was a significant increase in food and energy intake in the rats on the 9% (8.7 MJ) and 12% (8.4 MJ) diets, but not on the 6% (7.4 MJ) diet. The efficiency with which the dietary energy was used for weight gain was reduced on the 6, 9 and 12% diets compared with the 18% diet. The trend towards a decrease in the number of viable fetuses and an increase in the number of resorptions on the 9% and 6% diets did not reach statistical significance. The placental weight was not different to the 18% group (520 mg) on the 12% diet (540 mg), but was significantly increased on the 9% diet (590 mg) and significantly decreased on the 6% diet (510 mg). Fetal weight was greatest on the 12% diet (1.81 g) and significantly decreased on the 9% diet (1.71 g). The fetal:placental ratio, around 3.3 for the 18 and 6% diets, was significantly increased on the 12% diet (3.7) and significantly decreased to 2.9 on the 9% diet. After adjusting for the effect of the fetal position in the uterine horn, the number of pups per litter and an estimate of the number of failed implantations the effect of diet on placental weight was no longer statistically significant, but the effect of dietary protein on fetal weight was more significant statistically, with little change overall on the fetal:placental ratio.(ABSTRACT TRUNCATED AT 250 WORDS)

An improved technique for needle localized biopsies of the breast.

A technique using basic geometric principles is described that allows accurate determination of the location of the nonpalpable lesion in relation to the localizing wire tip and the skin. This method allows the surgeon to make the skin incision directly over the lesion, thus avoiding the creation of tissue flaps and minimizing the amount of tissue removed. While this technique is most helpful for lesions that are remote from the wire tip or buried deep within the breast, these principles are important in all biopsies of the breast. As the number of mammograms performed continues to increase, so will the number of needle localized biopsies, and surgeons must be familiar with techniques to simplify and improve these procedures. Breast conserving treatment for early stage carcinoma of the breast is an accepted form of treatment, and meticulous attention to the technical details of the biopsy will allow it to serve as the definitive surgical procedure. Avoidance of a second operation on the breast is preferable for the patient and will help to reduce health care costs.

Upper respiratory tract infections and general anaesthesia in children. Peri-operative complications and oxygen saturation.

Conflicting reports regarding the hazards of anaesthesia in children presenting for surgery with an upper respiratory tract infection have appeared in the literature. In the present study 130 children undergoing general anaesthesia with face mask for myringotomy and grommet insertion were graded as having either an acute or recent upper respiratory tract infection or were asymptomatic according to predetermined clinical symptoms and signs. The severity of respiratory and related complications were scored during induction, emergence and recovery. The peripheral oxygen saturation was recorded during induction, emergence, transfer to the recovery ward and in the recovery ward itself. There were no significant differences (p greater than 0.05) in the complication scores between the three groups of children. However, the incidence of hypoxaemia (oxygen saturation less than or equal to 93%) was significantly greater during transfer in the acute infection group (p = 0.001) and the recent infection group (p = 0.02), as well as during recovery in the acute group (p = 0.03) compared with asymptomatic children.

The DM20 protein of myelin: intracellular and surface expression patterns in transfectants.

DM20 is an abundant CNS myelin-specific protein whose role in myelinogenesis is unknown. We have cloned the DM20 cDNA from adult mouse brain total RNA using the polymerase chain reaction and expressed it in HeLa cells. DM20, detected by immunofluorescence in stable transfectants, is present in some cells in large, intensely fluorescent intracellular clumps that probably represent elements of the rough endoplasmic reticulum and Golgi apparatus. Frequently, intense DM20 fluorescence could be detected at the plasma membrane. These findings are consistent with previous studies demonstrating that an intracellular "pool" of DM20 and its larger isoform, proteolipid protein, exists and that a substantial lag occurs between synthesis and insertion of these proteins into the expanding myelin membrane. Permanent DM20 expressors in contact with one another do not display any ultrastructural rearrangements at regions of cell-cell contact, in contrast to what we have previously reported for P0, a PNS-specific protein shown to mediate adhesion of the extracellular faces of the Schwann cell during PNS myelinogenesis. We believe that these results indicate that if DM20 is indeed an adhesion molecule, this property is likely to be significantly more subtle than P0-mediated adhesion.

Autocrine and paracrine effects of peptides on human pituitary cells.

In contrast to normal human pituitaries, GH-secreting adenomas cannot process in vivo ProSRIH whereas they do it in vitro. The existence of an endogenous factor able to inhibit ProSRIH processing in vivo was postulated and such a role was analyzed for GHRH. Results showed that when GH adenomas are incubated in vitro with GHRH 10(-8) M, their ProSRIH contents are decreased, percent inhibition being negatively correlated to the amount of endogenously released GHRH. When incubation is performed in the presence of GHRH antibody in order to block the effect of endogenous GHRH, Pro-SRIH content is increased. The same effects are observed on SRIH release: inhibition by GHRH, stimulation by GHRH antibody. Normal rabbit serum had no effect. It may therefore be concluded that the absence of ProSRIH maturation observed in adenomas in vivo may be the consequence of the GHRH release that is known to be higher from GH adenomas than from normal pituitaries.

A three-year prospective study of 137 cases of acute leukaemia in Zimbabwe.

We studied 137 cases of acute leukaemia seen between December 1985 and November 1988, using traditional staining techniques together with cytochemistry and in cases of probable acute lymphoblastic leukaemia (Sudan Black negative) by immunophenotyping. Not all tests were carried out in every case (some cases of ALL could only be classified as T or non-T). Paediatric group (age less than or equal to 14 yrs): 75 cases--acute lymphoblastic leukaemia 52, acute myeloid leukaemia 18, acute undifferentiated leukaemia 5. Peak incidence in 5-9 year group. Male:Female ratio = 1.7:1. acute myeloid leukaemia was associated with chloromas in 2 cases (11 pc). Adult group: 62 cases--acute lymphoblastic leukaemia 23, acute myeloid leukaemia 36 and acute undifferentiated leukaemia 3. Peak incidence in 50-54 age group. Male:Female ratio = 1:1.2. Acute lymphoblastic leukaemia subtypes (all ages) T 16, Common 20, Null 12, 'non-T' 16, B cell 0, untyped 11. 69 pc were of L2 morphology. In T-ALL, 11 had thymomas and Male:Female ratio = 15:1. Male:Female ratio for 'non-T' = 1.5:1. Acute myeloid leukaemia subtypes (all ages) M1 3, M2 8, M3 14, M4 19, M5 8, M6 2, M7 1. Overall incidence of acute leukaemia appears increased at 0.91 per 100,000 per annum from previous studies in Zimbabwe. Common ALL (mean age = 13 years) is an emerging problem and now outnumbers T-ALL (mean age = 10 years). This may be related to a general improvement in living standards and health in Zimbabwe.

Effects of isoproterenol on T-wave amplitude and heart rate: a dose-response study.

This report examines the hypothesis that electrocardiographic T-wave amplitude is sensitive to graded increases in beta-sympathetic stimulation of the heart. Beta-adrenergic activity was manipulated pharmacologically in 9 healthy men by bolus infusion of isoproterenol in each of six doses: 0.1, 0.25, 0.5, 1.0, 2.0, and 4.0 micrograms. Results indicated that elevations in heart rate above placebo values increased as a linear function of isoproterenol dose. In contrast, the dose-response curve for T-wave amplitude was best described by a quadratic function: an initial reduction in T-wave amplitude at low levels of isoproterenol infusion was followed by a significant reversal of this effect at higher doses. Comparison of the heart rate and T-wave amplitude data points to limitations in the use of the latter as an index of beta-adrenergic activity. One of several possible explanations for the T-wave results would entail a mechanism that preserves ventricular function at high levels of beta-sympathetic stimulation.

[Presence and characterization of somatostatin precursor in human growth hormone secreting pituitary gland tumor].

Previous studies have demonstrated that normal and tumoral pituitaries release in vitro somatostatin (SRIH) and contain messenger RNAs encoding the preproSRIH. In the present study, we document the presence and characterization of the SRIH precursor in a growth hormone (GH)-secreting human pituitary adenoma, using two biochemical procedures: Sephadex G50 filtration and high performance liquid chromatography (HPLC). Sephadex G50 filtration of a 2M acetic acid extract demonstrated the presence of three SRIH-like immunoreactive (SLI) peaks, distinct from SRIH 1-28 and SRIH 1-14 used as standard. Molecular mass of the SLI material present in each peak was estimated as 21, 17 and 12 kilodaltons (kDa). SRIH 1-28 and/or 1-14 were not detected in this extract. Reverse phase HPLC was performed on a C18 column; all the three forms of SLI material coeluted with the rat hypothalamic proSRIH indicating a high homology between the human pituitary proSRIH and that of the rat hypothalamus. These results demonstrate for the first time the presence of high molecular mass proSRIH forms in a GH-secreting pituitary adenoma, and thus enable us to conclude for a local pituitary production of SRIH.

Detection of interleukin-3 in the serum of mice infected with Mycobacterium lepraemurium.

Infection of mice by Mycobacterium lepraemurium is accompanied by ablation of erythropoiesis in the bone marrow and gross enlargement of the spleen. This, together with increased monocytopoiesis and the earlier demonstration of macrophage colony-stimulating factor in the serum of infected mice, suggested the activity of additional cytokines. Eight weeks after infection of mice by M. lepraemurium, interleukin-3 (IL-3) activity was demonstrated in the serum (titer, 1:3200). The serum titer of IL-3 activity was maximal after 13 weeks (greater than 1:6400) and was slightly reduced after 18 weeks (1:6400). That the IL-3 activity detected in the serum of the M. lepraemurium-infected mice reflected the presence of IL-3 itself was confirmed by a neutralization assay using anti-murine IL-3 antibodies; IL-3 activity in the serum of mice 13 weeks after infection was completely abolished by the anti-IL-3 antibodies. Finally, a 1-kb signal of IL-3 RNA was detected in the spleens of M. lepraemurium-infected mice 13 weeks after infection.
