[Jejuno-ileal short-circuit operations in the treatment of obesity. 16 personal cases (author's transl)].

The authors report a personal experience of 16 cases and in the light of data in the world literature, the authors discuss the controversial method of treatment of very severe obesity when medical treatment has failed. The onset of various complications linked mainly to intestinal malabsorption, should make one careful in the indications for surgery which may, however, be applicable in certain cases.

[Electric study of myotomy of the small intestine (author's transl)].

The electric activity of the small intestine above and below an oblique myotomy was studied in the rabbit. In most cases, there was noted a definite increase in the amplitude of the electric recording above the functional obstruction showing increased peristalsis. This phenomenon seems to become attenuated with time, perhaps owing to the technical conditions of this study. Myotomy of the small intestine seems, in the light of these recordings, to be very efficacious. The authors compare it with other procedures which may be used to slow intestinal transit and suggest carrying out long or multiple myotomies, perhaps associated with one of the other techniques to increase the efficacy.

[Zollinger-Ellison syndrome and parathyroid adenoma. Limits of Wermer's syndrome].

The authors report the case of a patient with pancreatic and parathyroid adenomatosis and discuss whether it belongs to Wermer's syndrome. After a study of the various possible endocrine involvements, the complexity of hormonal inter-relations is studied, the prognosis and classification of multiple endocrine tumours are then discussed.

[Intracystic hemorrhages of pancreatic pseudocysts].

The authors report ten personal cases and review the literature on 99 other cases, and study primary intra-cystic hemorrhage or post-operative hemorrhage, occurring during the course of pancreatic pseudo-cysts. These hemorrhagic pseudo-cysts are very often associated with chronic pancreatitis; they may rupture into a hollow viscus, the peritoneal cavity or into Wirsung's duct. Arteriography and duodenoscopy with retrograde pancreatography, should permit one to reduce the frequency of these latent, undiagnosed forms until surgical operation has been carried out. Even more dangerous, post-operative bleeding often represents a recurrence of primary intra-cystic hemorrhage which has been undiagnosed or neglected. Surgical treatment should give a greater place to operations of excision which simultaneously treat false cyst, the responsible vascular lesion and the chronic casual pancreatitis.

[Afferent projections to the locus coeruleus nucleus in the cat. Study by the horseradish peroxidase technic].

Using a retrograde tracer technique with horseradish peroxidase, we have revealed some afferent projections to the locus coeruleus complex from the contralateral pontine tegmentum, raphe nuclei, substantia nigra, nucleus of the solitory tract, dorsal motor nucleus of the vagus and other regions of the ponto-bulbar reticular formation as well as from hypothalamic and preoptic areas.

[Wirsung's duct and chronic pancreatitis. Changes in the morphology of the pancreatic ducts after pancreatic duct shunts. 23 cases followed-up].

The authors study in chronic pancreatitis the morphology of Wirsung's duct in 31 patients who had undergone repeated operations, 23 of them were submitted in a first stage to an anastomosis between the pancreatic duct and the digestive tract. The main causes of failure were obstructions of the anastomosis, biliary complications and continuation of the pancreatic disease. The difference in prognosis between pancreatitis with a dilated pancreatic duct, and those with a filiform duct, is perhaps due to lesions of different histological appearance and course. The best results were obtained in patients able to give up alcohol and in whom it was possible to carry out a broader anastomosis on a dilated and unobstructed pancreatic duct.

[Chronic painless pancreatitis. Apropos of 9 cases].

The authors report 9 cases of painless chronic pancreatitis and review the literature on this apparently rare condition. They emphasise the contrast between absent or minimal pain and the severe pathological lesions. In the light of these cases and pathological specimens, they attempt to interpret the pain during chronic pancreatitis. Pain seems to occur when there is a downhill course.
