[Traumatic diaphragmatic hernia in children].

We have treated three children with traumatic diaphragmatic hernia appearing after blunt abdominal trauma. The hernia was on the right side in two patients and was accompanied by liver and lung injuries. The most common site of diaphragmatic rupture was the postero-lateral area in both sides. Diagnosis was suspected in all of them with chest radiograph before operative management. The transabdominal approach has proven to be efficacious in repairing the defects and often require emergency care. The diaphragmatic repair was performed using non absorbable interrupted mattress sutures. No prosthetic materials were required.

[Colonic atresia].

Nine cases of colonic atresia, treated in the Departament of Pediatric Surgery of "La Paz" Pediatric Hospital, are reported. They account for 11% of all our intestinal atresias. We review the different etiologic and pathologic theories and classificate our group according to the LOUW and HAYS method. In two cases their digestive anomalies and in one renal anomalies were found to be associated mortality was 11%.

[Current status of the treatment of congenital diaphragmatic hernia].

50 cases of congenital diaphragmatic hernia were operated during a five year period (1978-82). Authors review surgical and postoperative treatment. Eight cases were treated with pulmonary vasodilators, including tolazoline and dopamine, with very good results. Other cases were treated with hyperventilation. Mortality rate was 30%.

[Tumors in the newborn infant: development and surgical treatment].

From 1970-1982, 70 newborns with different kinds of tumors were studied at "La Paz" Children's Hospital in Madrid. Our review was directed to study sex, site, kind and malignity. This type of neonatal work is uncommon in pediatric literature. We make special emphasis in the follow-up of malignant. tumors. 63% of these tumors were benign ant the remaining were malignant. The overall mortality was 28%.

[Partial splenic embolization in hypersplenism].

Partial splenic embolization is a new procedure in the treatment of hypersplenism in infancy. We have used embolization of the spleen as an alternative to splenectomy in a patient with critical hypersplenism. The patient was a 6 year old girl with portal hypertension. The white blood cell and platelet count returned to normal 24 hours after embolization and tre frequency of bleeding episodes was reduced. There were no splenic abscesses and no other significant complications of the treatment.

[Tolazoline in postoperative period of diaphragmatic hernia (author's transl)].

During the last ten year period, 78 newborns with congenital diaphragmatic hernia were seen at the Clinica Infantil "La Paz", Madrid. Twenty seven cases died immediately after delivery and 51 were operated. Necropsy findings showed pulmonary hypoplasia and patent ductus arteriosus. Mortality rate in operated cases was 29%. It seems that pulmonary hypoplasia increases pulmonary vascular resistance resulting in a state of fetal circulation with right-to-left shunting through ductus arteriosus. In these cases right to left shunt with umbilical and temporal artery catheters measuring pre and postductal pO2 must be demonstrated. Authors show their experience with two cases of newborns with Bockdalek hernia treated with tolazoline in postoperative period. Both survived.

[Neonatal laceration of liver and spleen (author's transl)].

A very unusual observation of survival after surgical treatment of neonatal maceration of liver and spleen is reported. The patient had a normal birthweight and the only possible etiologic factor was a oxitocin-induced rapid delivery. The literature on this topic is reviewed.

[Effects of parenteral nutrition solutions on the venous walls (an experimental study) (author's transl)].

Lesions (thrombosis, intimal damage, perivascular inflammation) of the marginal ear vein of domestic rabbits were studied 48 hours after infusions of parenteral feeding solutions administered through a butterfly-type 25 needles. Six-hour infusions with a rather diluted solution (12 g dextrose, 1.25 g. aminoacids, 1.5 mEq., Na, Cl, K and PO4 and 1.5 mEq. Ca and Mg per 100 ml.) with osmolality of 930 and pH of 6.9 were slightly harmful, while the lesions were considerable after 12-hours infusions. A more concentrated solution (15.83 g. dextrose, 2.08 g. aminoacids, 2.5 mEq. Na, Cl, K and PO4 0.2 MEq. Ca and Mg per 100 ml.) with osmolality of 1,600 and pH 6.7 produced severe lesions both after 6 and 12-hours infusions. Similar results were obtained with a mixture of this solution with intralipid at clinical doses (4 g. fat for 100 ml. of solution). This mixture had 1,280 mOsm/1. and pH of 6.7. Rather surprisingly the lesions were even more marked when intralipid alone was infused (osmolality 317, pH 7.5). These results lead to the conclusion that practically all fluids used for parenteral feeding purposes either centrally or peripherally, are considerably harmful for the venous wall after a few hours of infusion, probably due to their shifts from isoosmolality and chemical neutrality. Specially puzzling was the harmful local effect of the otherwise well tolerated intralipid.

[Choledochal cyst (author's transl)].

This paper reports six pediatric patients with congenital choledochal cyst. From the long-term follow-up results of the six patients authors conclude that primary excision of the choledochal cyst and Roux hepatic jejunostomy are the first choice treatment for this lesion. However, it may not be possible in all cases, and choledochocystoenterostomy would then be useful as a second choice.

Arterial endothelial changes after ischemia and perfusion.

The effects of six hours of ischemia and six hours of perfusion with 10 per cent dextrose and water solution, Sacks' solution and Intralipid on the endothelium of common femoral arteries in dogs were examined by light and by scanning electron microscopy and compared with normal arteries. Arteries that were ischemic or perfused with 10 per cent dextrose and water solution or Sacks' solution showed a flattening of the normal linear convolutions and extensive crater formation, with fragmentation and even complete loss of endothelial cells in many areas. The more severe changes occurred in perfused vessels. Fibrin and platelets covered the luminal surface in many areas in which there was extensive injury to the endothelium. Light microscopy revealed thickening of the internal elastic membrane and intimal fibrosis. Results of biopsies performed two weeks after perfusion showed slight, although incomplete, improvement in the structure of the endothelium. Arteries perfused with Intralipid had thickening of the intima with a proliferation of plump cells orientd linearly over the surface but with an absence of craters. Results of present studies confirm the observations of other investigators that injury to endothelial cells may be produced by ischemia and that perfusion with various solutions may produce an even more severe alteration in structure of the endothelial cell which does not return to normal within two weeks. The origin of new endothelial cells is not demonstrated by this study.

[Cervical recurrence in the pathologically negative neck (pNO): finding a needle in a haystack].

Recurrence in the pathologically negative neck (pN0) is one of the most striking events occurring in head and neck tumors. It is not even mentioned in many neck dissection studies. The presence of micrometastases has been suggested as an explanation for this event. In accordance to a recent study performed in our institution, we bring up this controversial issue. A definitive explanation for recurrence in the pN0 has not yet been proposed.

[Speech discrimination through telephone in patients implanted with a Combi 40+].

OBJECTIVE: To assess speech discrimination through line and mobile telephone in cochlear implanted patients. MATERIAL AND METHODS: Eighteen patients implanted with a Combi 40+ went through different speech discrimination tests with a line telephone and three mobile telephones, in quiet and noisy environment. RESULTS: Mean scores for telephonic speech discrimination were 84-92% using CID sentences, 27-58% using bisyllabic words in quiet environment and 15-41% using bisyllabic words in noisy environment. Among the mobile telephones tested, the Siemens M55 reached the best scores. CONCLUSIONS: Telephonic speech discrimination is achieved by a significant number of cochlear implanted patients. Certain mobile telephone models appear to be more advisable for these patients.

[Detection of p53 protein in advanced head and neck cancer].

P53 protein expression, synonymous to p53 gene mutation, is a common event in head and neck squamous cell carcinoma (HNSCC). However, the correlation between p53 expression and clinico-pathologic parameters remains unclear. Primary tumors for analysis were obtained from 99 patients with HNSCC. The expression of p53 was analyzed with monoclonal D07 antibody (Dako Corp., Glostrup, Denmark). Seventy five percent of 79 valid cases expressed the protein. A correlation was found between alcohol intake and p53 expression. None of the other clinico-pathologic factors were associated to p53 expression. Our results suggest that p53 may be involved in the development of HNSCC. However its clinical significance is still not completely understood.

[Acute suppurative thyroiditis secondary to fistula of the pyriform sinus].

Acute suppurative thyroiditis is very rare and often related to congenital abnormalities of the branchial pouches or immunodeficiency. A case of acute thyroiditis due to fistula of the left pyriform sinus in a young man with no relevant history is reported. We review the embryology, diagnosis, and treatment of this rare entity.
