[Rupture of hemorrhagic ovarian cysts. Value of celioscopic surgery].

The rupture of haemorrhagic cysts of the ovary occur frequently and are often missed. Review of the literature and an analysis of our series of 20 patients makes it possible to summarise the typical clinical picture, which is of a young woman who has sudden severe pelvic pain in the second half of her menstrual cycle, during intercourse, or when she has pelvic trauma. Ultrasound in 90% of cases shows up signs suggesting the diagnosis and these are: a mass beside the uterus, or an intra-abdominal effusion. Endoscopic surgery is indicated particularly in women of reproductive age who suffer this pathology which is always benign. The indications to treat the patients should always take in to account the histology; 100% of these are luteal cysts. There is no reported case of cancer of the ovary or of an organic haemorrhagic cyst. On the other hand occasionally a primary ovarian pregnancy may occur and that can be confused with a haemorrhagic cyst. Finally haemorrhagic cysts can occur with an intra-uterine pregnancy and this means that the corpus luteum must be preserved. We propose the following treatment for those cases with symptoms: simple peritoneal washout when pregnancy has been ruled out on the laparoscopy exploration and when the bleeding is widespread: systematic taking of a biopsy for histology if there is the slightest doubt that there is a pregnancy in the ovary and an intraperitoneal removal of the cyst when haemorrhage persists during the operation.

[Rupture of a healthy uterus in a prostaglandin-induced abortion in the second trimester].

The authors report one case of uterine rupture in a non scarred uterus when an analogue of prostaglandin E2 was being transfused. It was Sulprostone used to terminate a pregnancy because of fetal death in utero after 27 weeks of amenorrhea. This case history and an analysis of the literature makes it possible to point out the need to reach the diagnosis before signs become too severe and to show that pharmacological knowledge of the drug has to be improved as well as the ways of administering prostaglandin analogue. This is to be conducted together with improving the ways of terminating pregnancies in the second trimester. It shows that mechanical accidents can occur even where there are no obvious risk factors. In this case, pain continued from the time of the rupture under epidural anaesthesia. The physiopathology is reviewed. Finally, conservative treatment of the uterus should be carried out whenever possible in order to allow a new pregnancy to occur and to lessen the morbidity of the operation.

Pigmented cell lines of mouse albino melanocytes containing a tyrosinase cDNA with an inducible promoter.

Melanocyte cell lines, with characteristic dendritic morphology and melanosomes, were established from young mice of wild-type (C57BL/6) and of two albino (C57BL/6-c2J/c2J and BALB/c) inbred strains. The albino cells were cotransfected with two plasmids: pMTtyr1, containing the full-length tyr1 cDNA for tyrosinase encoded by the c locus, under the control of the inducible mouse metallothionein-I (MT-I) promoter; and pSVneo beta, allowing selection of transformants by G418 resistance. The intrinsic albino defect was corrected by the tyr1 cDNA in transfected cells, thereby validating the coding capability of tyr1 for tyrosinase. Black melanin was formed in the genetically black (B/B) C57BL/6-c2J/c2J cells and brown melanin in the genetically brown (b/b) BALB/c cells. Pigment was produced even without adding heavy metal (for induction of the MT-I promoter), thus obviating the need for adding it, but was formed more rapidly upon addition of ZnSO4 up to 100 microM. Stable transfected albino melanocyte lines with active tyrosinase and melanization were obtained. Addition of ZnSO4 at 200 microM was lethal to the cells. However, this toxicity--attributable at least in part to melanin precursors--was prevented if the cells sojourned at 100 microM ZnSO4 for two weeks before being exposed to the 200 microM level. Adaptation was lost when the cells were removed from 200 microM ZnSO4 for one week and then returned to it. Avoidance of toxicity under these conditions is thus the result of physiological detoxification mechanisms rather than selection for a genetic change.

[Changes in the cesarean section percentage].

More and more cesarean sections (C/S) are done, rate multiplied by 3 or 4 in occidental countries in these post two decennials, but are really too many done (?), as it is regularly reproched to the obstetricians, even though perineonatal morbidity and mortality rates have improved. By help of files studied in the gynecological and obstetrical Ward of Saint Antoine's Hospital Paris over 60 years, the authors have tried to answer to a few basic questions: The reason to be of the first C/S before labor and during labor, and the reason to be of iterative C/S in the same conditions. What results is: The rate of C/S has barely changed fur the following causes: Important dystocia known before or during labor, or Chronic foetal distress. On the other hand it increased because of: Acute foetal distress revealed during labor by alteration of the foetal heart beat (FHB). Stagnation of dilatation often accompanied by alteration of the FHB. An increase of iterative C/S due themselves to the increased number of scarred uteri. To reduce the progression of the number of C/S, we should therefore try to: Reduce the amount of scarred uteri, therefore have better knowledge of the indication of the first C/S. But is it feasible without compromising the benefits acquired by modern obstetrics, in means of perineonatal mortality and morbidity? Accept more often birth by vaginal route on the scarred uteri, the actual means of surveillance of labor appear to be able to permit it.

[Per-celioscopic surgery of the distal fallopian tube in infertility].

Twenty five patients who had distal tubal blocks were operated on laparoscopically between the months of November 1986 and April 1988. Eleven fimbrioplasties and 20 salpingostomies were carried out, 16 patients also had division of adhesions. Both tubes were operated on in 6 patients and only one in 19 patients. After 12 months the cumulative pregnancy rate was 28.4% overall composed of 40% for fimbrioplasties and 20% for salpingostomies. No pregnancy occurred more than 10 months after the operations. There was no inter or post operative complication. The results of this study and an analysis of the literature shows that laparoscopic operations are a worthwhile alternative to laparotomy in distal obstructions of the tubes that are not very severe. In severe stages however, it would seem better to carry out an assisted reproduction technique.

[The diagnostic value of hysteroscopy in extra-uterine pregnancy].

Hysteroscopy was carried out in 30 in-patients who were admitted with suspicion of ectopic pregnancy between January and November 1989. The presence of a continuing intra-uterine pregnancy was excluded in each case. Twenty one ectopic pregnancies and nine non-viable intra-uterine pregnancies were assessed in this way. Carrying out plasma Beta HCG level assessments and transvaginal ultrasound had not made it possible to diagnose the condition before operation in 16 (53%) of the patients. In one case where it was difficult to interpret the hysteroscopic findings laparoscopy was carried out but this was not helpful; in all the other cases hysteroscopy gave the exact diagnosis. This examination appears to us as a result of this study to be useful in differentiating an extra-uterine pregnancy from a non progressive intra-uterine pregnancy.

[The in vivo effect of the local administration of progesterone on the mitotic activity of human ductal breast tissue. Results of a pilot study].

Breast tissue samples were taken during surgery in premenopausal women with various benign breast diseases. Surgery was scheduled between day 11 to 13 of their menstrual cycle, before presumed ovulation and endogenous production of progesterone. Each patient was treated 11 to 13 days before surgery by daily percutaneous topical application on breast of either a placebo gel, a gel containing progesterone or a gel containing estradiol. Treatments were assigned at random and the study conducted double-blind. The mean estradiol concentration in breast tissue was significantly higher (3,409 pg/g) in the estrogen-treated group than in the placebo (365 pg/g) and the progesterone (523 pg/g) treated groups. The mean progesterone concentration in breast tissue was significantly higher (69.1 ng/g) in the progesterone treated group than in the placebo (1.95 ng/g) and the estradiol (3 ng/g) treated groups. Mitotic activity was calculated by counting with light microscopy mitoses in epithelial cells of normal lobular area. Mean mitotic activity was significantly lower in progesterone treated group (0.04/1,000 cells) than in placebo (0.10/1,000 cells) or in estradiol (0.22/1,000 cells) treated groups. High concentration of progesterone sustained in human breast tissue in vivo during 11 to 13 days does not increase, but actually decreases mitotic activity in normal lobular epithelial cells.

A new series of ellipticine derivatives (1-(alkylamino)-9-methoxyellipticine). Synthesis, DNA binding, and biological properties.

A new series of ellipticine derivatives, 1-(alkylamino)-5,11-dimethyl-9-methoxy-6H-pyrido[4,3-b]carbazoles, were synthesized as potential DNA intercalating antitumor drugs. The structure of these compounds were confirmed by 1H NMR spectroscopy and mass spectrometry. These compounds are able to bind to DNA with an affinity of about 10(6) M-1, and their intercalating characteristics (lengthening and unwinding of DNA) depend upon the length of the chain in position 1. The cytotoxicities of these compounds on L1210 and NIH-3T3 cells are quite similar, and fluorescence techniques showed that the compounds are localized mainly in the cytoplasmic granules of the cells. One of these compounds appears to show a very high antitumor activity (equivalent to the more active known ellipticine analogues: 10-[[gamma-(diethylamino)propyl]amino]-6-methyl-5H- pyrido[3',4':4,5]pyrollo[2,3-g]isoquinoleine (BD40), 1-[[gamma-(diethylamino)propyl]amino]-9-methoxyellipticine (BD84) and 2-[beta-(diethylamino)ethyl]-9-hydroxyellipticinium chloride (DEAE).

Interaction of an intercalating antitumoral agent: 9-hydroxy-2-methyl ellipticinium (NMHE) with chromatin.

In this work we study the effects of an intercalating antitumoral agent: 9-hydroxy-2-methyl ellipticinium (NMHE) on the structure of chromatin, using micrococcal nuclease and DNase 1 as structural probes. The binding of the drug to chromatin, either in vitro or in the nuclei, induces two structural changes of chromatin: (a) an unfolding of the overall structure which results in an activation of the rate of degradation of chromatin by micrococcal nuclease and (b) a disorganisation of the core particle structure leading to the unwrapping of the DNA from the histone core. Moreover, by studying the interaction of MMHE with nuclei labeled in the active regions of the genome through a nick-translation reaction, it appears that the drug is overconcentrated in these regions and does not induce any new structural changes. The interaction of NMHE with DNase 1-sensitive regions of chromatin indicates that these regions are already "open" or relaxed and represent a preferential target for the drug.

Effects of the 6-alkyl substitution of ellipticine on the uptake by NIH-3T3 cells and on the cytotoxicity.

New ellipticine derivatives of the 2-methyl ellipticinium (NME) series, i.e. 2,6-dimethylellipticinium (6-Me-NME) and 2-methyl-6-n-propylellipticinium (6-Pr-NME), have been studied as cytotoxic compounds. Their uptake by NIH-3T3 cells and efflux have been measured by a sensitive and specific high performance liquid chromatographic assay. These compounds are equitoxic if we compare their cytotoxicity by two methods: growth inhibition and cloning efficiency. However, they accumulate in NIH-3T3 cells at different steady state levels and the efflux rates are not similar. This raises the question of the mode of action of these drugs.
