Neonatal hyperexcitability in relation to plasma ionized calcium, magnesium, phosphate and glucose.

Twenty-two full-term babies were examined with a quantitative neurological assessment on days 1, 2 and 5 in order to confirm the clinical impression of hyperexcitability observed at the routine examination on day 1. Ultrasound examination of the head was normal in all cases. Plasma ionized calcium, phosphate and glucose in the hyperexcitability group fell within the neonatal reference ranges, as described previously. However, serum magnesium was significantly lower on day 1, but spontaneously normalized on day 5. Routine check-ups at the well-baby-clinics did not reveal any major illnesses or developmental abnormalities.

Effects of age on contractile and enzyme-histochemical properties of fast- and slow-twitch single motor units in the rat.

1. Contractile, enzyme-histochemical and morphometrical properties of muscle fibres were studied in single motor units of tibialis anterior (t.a.) and soleus muscles in young (3-6 months) and old (20-24 months) male albino rats. The technique of measuring glycogen depletion as a marker of previous muscle contraction was used for direct correlation of enzyme-histochemical and contractile parameters within single motor units of the fast- and slow-twitch type. 2. In t.a., the fast-twitch motor units covered 18 +/- 9 and 22 +/- 16% (P = not significant, n.s.) of t.a. cross-sections, included 148 +/- 65 and 162 +/- 63 muscle fibres per unit (P = n.s.) and had a cross-sectional area of 0.50 +/- 0.32 and 0.44 +/- 0.22 mm2 (P = n.s.) in the young and old animals, respectively (means +/- S.D.). 3. In soleus, the slow-twitch motor units covered 53 +/- 11 and 71 +/- 12% (P = n.s.), included 55 +/- 10 and 83 +/- 13 muscle fibres per unit (P less than 0.01) and had a total cross-sectional area of 0.14 +/- 0.02 and 0.22 +/- 0.06 mm2 (P less than 0.01) in the young and the old animals, respectively. The calculated number of motor units in soleus accordingly decreased (P less than 0.01) from 49 +/- 10 in the young to 29 +/- 10 in the old animals resulting in a loss of muscle fibres and an increased innervation ratio in old age (mean +/- S.D.). 4. Clusters of more than three muscle fibres were rarely seen in any of the glycogen-depleted motor units in either the young or the old animals. However, in the slow-twitch motor units of old animals the muscle fibres were less randomly distributed within the motor unit territory (P less than 0.05), indicating a denervation-reinnervation process. 5. The contraction and half-relaxation times of the isometric twitch were significantly prolonged in old age. In 274 randomly isolated single motor units of t.a. the contraction time increased from 13 +/- 1 in young animals to 17 +/- 3 ms in old ones and the half-relaxation time from 12 +/- 2 to 16 +/- 5 ms (P less than 0.01 in both cases). In 236 randomly isolated soleus single motor units, the contraction and half-relaxation times increased (P less than 0.001) from 24 +/- 5 to 31 +/- 7 ms and from 26 +/- 8 to 35 +/- 9 ms, respectively (mean +/- S.D.).(ABSTRACT TRUNCATED AT 400 WORDS)

Diagnosis of pulmonary tuberculosis by selected-ion monitoring: improved analysis of tuberculostearate in sputum using negative-ion mass spectrometry.

Coded sputum specimens from patients with tuberculous (8 specimens) and nontuberculous (15 specimens) pneumonia were analyzed for tuberculostearic acid by selected-ion monitoring of the pentafluorobenzyl ester derivative by using negative-ion mass spectrometry. The analytical results agreed with those from culturing of the specimens, except in three cases in which the chromatograms were difficult to interpret owing to insufficient amounts of sputa available. Negative-ion mass spectrometry provided superior sensitivity and selectivity for detection of tuberculostearic acid compared with the previously used gas chromatographic-mass spectrometric methods. The technique reported here should have considerable potential for the rapid diagnosis of pulmonary tuberculosis by direct analysis of sputum.

Ultrastructural localization of acidic compartments in cells of isolated rabbit PCT.

Acidic intracellular compartments were identified in cells of isolated perfused rabbit proximal convoluted tubules (PCT) with the weak base, N-(3-[(2,4-dinitrophenyl)amino]propyl)-N-(3-aminopropyl)methylamine, dihydrochloride (DAMP), a congener of dinitrophenol (DNP) and immunogold staining at the ultrastructural level in Lowicryl-embedded tissue. DAMP is protonated upon entrance into an acidic compartment and binds to protein. Glutaraldehyde fixation cross-links the DAMP-protein complex to maintain the complex in its original environment. Monoclonal antibodies directed against DNP cross-react with DAMP to identify its location in tissue sections. Accumulation of colloidal gold-conjugated antibodies to the monoclonal anti-DNP antibodies, indicating the presence of an acidic environment, was found in large endocytic vacuoles (diam greater than 0.5 micron) and lysosomes, and to a lesser extent in endocytic vesicles. Qualitatively, activity greater than background was not found in other organelles, including the Golgi apparatus and the cell nuclei. The specificity of the technique was tested by treating tubules with the ionophore, monensin, to collapse the intracellular pH gradient. This treatment resulted in the complete disappearance of the specific localization of colloidal gold particles. Quantitative analysis of the number of colloidal gold particles per area of cross-sectioned cell organelle demonstrated that monensin especially affected those counts obtained over large endocytic vacuoles and lysosomes. The present ultrastructural observations, therefore, identify endocytic vesicles, endocytic vacuoles, and lysosomes as acidic intracellular compartments in isolated rabbit PCT. This technique affords the opportunity to identify the location of acidic compartments along the entire nephron at the ultrastructural level.

Endocrine changes and urinary citrate excretion.

Urinary citrate affects the ion-activity products of calcium oxalate and calcium phosphate and inhibits the growth of these crystals. Women are less prone to develop calcium stones and because they also excrete more citrate than men, an effect of sex steroids on citrate excretion might be important. We therefore analysed urinary citrate and creatinine before and during treatment with estrogen alone or together with medroxyprogesterone acetate in 29 postmenopausal women and at different gestational ages in 19 pregnant women. Urinary citrate and creatinine was also determined before and after orchidectomy in ten men with carcinoma of the prostate. The excretion of citrate and the ratios between citrate and creatinine were not significantly altered by pregnancy or orchidectomy. Neither did treatment with estrogen or estrogen/medroxyprogesterone acetate affect these variables. We were unable to explain the difference in citrate excretion between men and women by effects of sex steroids.

Hyperoxaluria.

Urinary oxalate is considered to play a crucial role in the formation of renal stones. In this respect hyperoxaluria constitutes a special problem, mainly because of the specific physicochemical properties of oxalate. The appropriate management of patients with this disorder must be based on a thorough understanding of the absorption, metabolism and excretion of oxalate. Different reasons for high oxalate excretion as well as analytical problems and our current therapeutic possibilities are covered in this review.

Calcium and magnesium status in pregnant women. A comparison between treatment with calcium and vitamin C in pregnant women with leg cramps.

60 pregnant women underwent a double blind trial with calcium or ascorbic acid (1 g twice daily) as treatment for leg cramps. There was no significant difference between the two treatment groups with respect to clinical improvement. In 14 out of 60 patients the symptoms were totally abolished and in another 27 patients the symptoms were significantly decreased by the treatment (irrespective of drug used). In 17 patients the symptoms were unaffected while only two patients experienced an increase in frequency of their leg cramps during therapy. Serum total and ionized calcium concentrations, serum total magnesium and albumin concentrations were determined and were not significantly changed throughout therapy in any of the groups. No biochemical differences were found between the different treatment regimens or between those patients relieved or not relieved of their symptoms. Serum magnesium concentrations were at or just below the lower normal limit (for non pregnant women) in treated women and pregnant controls.

High-energy phosphates and surface oxygen pressure fields in skeletal muscle after high-energy trauma.

The effect of a high energy missile trauma on energy metabolism and tissue oxygenation in uninjured parts of skeletal muscle was studied in anaesthetized pigs up to 72 hours after the trauma. High energy phosphates (HEP) were measured in muscle biopsies, and muscle tissue oxygenation was measured as muscle surface oxygen pressure fields by an oxygen electrode. In the traumatized group significantly decreased levels of HEP were found in spite of normal tissue oxygenation 72 hours after the trauma. In the control group both the HEP and the muscle tissue oxygenation were unchanged compared to the pretrauma situation. Decreased synthesis of HEP due to tissue hypoxia or inadequate nutrition could not account for the difference between the control and the trauma group. It was suggested that hypermetabolism induced by the high energy trauma caused increased utilization and thereby decreased concentrations of the HEP 72 hours after the trauma. It was also concluded that the general posttraumatic metabolic changes found in uninjured parts of the muscle tissue differed from the local changes of traumatized muscle described by other investigators, and therefore probably were caused by other mechanisms.

Motor unit fibre density in extremely hypertrophied skeletal muscles in man. Electrophysiological signs of muscle fibre hyperplasia.

Single fibre electromyography (SFEMG) was performed on the left quadriceps, extensor digitorum communis, and biceps brachii muscles of four highly trained bodybuilders (BB). Muscle fibre areas, as assessed from biopsy samples, were similar in the BB as in age-matched habitually active male controls despite large differences in limb circumferences. This indicates a greater muscle fibre number in BB. An abnormally high muscle fibre density (FD) was recorded in two subjects exposed to extremely severe exercise for 14 years or longer, while those with normal FDs had been training extensively for a considerably shorter time (4-6 years). The abnormal FD was due either to an ephaptic transmission of action potentials between adjacent muscle fibres, or a muscle fibre hyperplasia, or most probably a combination of both, in response to a long-term functional overload. From the present results one cannot say whether the suggested muscle fibre hyperplasia had any major influence on the muscle hypertrophy per se, since all BB possessed large muscle volumes.

Effects of age on enzyme-histochemical fibre spectra and contractile properties of fast- and slow-twitch skeletal muscles in the rat.

Enzyme-histochemical fibre spectra and contractile properties were studied in fast-twitch (extensor digitorum longus (EDL) or tibialis anterior (TA)) and slow-twitch (soleus (S)) muscles of young adult (6 months) and old (20-24 months) male albino rats. It was found that ageing affected fibre size, fibre type proportions, and contractile properties of muscle tissue in both qualitative and quantitative terms and that these age-related alterations differed between fast- and slow-twitch muscles. In the fast-twitch TA and EDL, no differences were observed in either the total number of fibres, the cross-sectional area or the absolute and relative numbers of different muscle fibre types and subtypes between young adult and old animals. In the slow-twitch S, on the other hand, both the total number of muscle fibres and the average cross-sectional fibre area were smaller in the old animals. The fibre loss and fibre atrophy were most pronounced in type II fibres, especially type IIA. In TA, twitch force was higher and tetanus force was unaltered in the old as compared with the young adult animals, resulting in an increased twitch:tetanus ratio in old age. In S, on the other hand, both these forces were lower in the old animals and the twitch:tetanus ratio was accordingly unchanged with age. When the tetanus force was related to age-related differences in total muscle fibre cross-sectional area, no differences were found in the maximum force-generating capacity of maintained contractile material in either fast- or slow-twitch muscles between the two age groups. Probable mechanisms underlying the above alterations are discussed.

Biochemical risk factors in patients with renal staghorn stone disease.

Thirty-three patients operated on for renal staghorn calculous disease were studied retrospectively with respect to urine and stone composition, bacteriuria, and abnormalities of the urinary tract. Calcium phosphate was the most common stone constituent, present in 30 of 31 concrements. Twenty-one of these stones also contained magnesium ammonium phosphate, despite the fact that only 10 patients had presented evidence of urinary tract infection during the initial period of the disease. Twenty-four-hour urine composition was normal in only 3 patients. In 59 per cent an increased CaOx risk index was observed suggesting that CaOx risk factors might contribute to the development of staghorn concrements. A metabolic evaluation of staghorn stone formers appears to be of importance for design of the postoperative treatment.

Clinical results of allopurinol treatment in prevention of calcium oxalate stone formation.

Allopurinol in a daily dose of 300 mg. was administered to 99 patients with calcium oxalate stone disease. Treatment was started irrespective of urine composition and was continued for up to 8 years. Only 43 per cent of the patients treated for 5 or more years remained free of further stone formation, a result not better than observations in untreated stone patients. When patients were subgrouped with respect to recurrent or nonrecurrent stone formation during treatment, the former group, besides being followed for longer intervals than the latter group, had a urine composition suggesting a higher crystallization risk. We concluded that with the possible exception of hyperuricosuria or hyperuricemia the indication for allopurinol treatment of recurrent calcium oxalate stone disease is weak. The results also demonstrate clearly the problems combined with evaluation of prophylactic medical therapy in patients with calcium stones. The necessity of long-term followup and analysis of the biochemical risk situation is emphasized.

Excitability in free muscle transfers: an optimal method of monitoring tissue circulation?

Recent methods of postoperative monitoring of free muscle transfers were used in 4 patients operated on with free revascularized gracilis muscle. The contractility/electromyographic (EMG) activity in response to electrical stimulation was used as an index of viability, i.e., intact circulation. However, during operations it was observed that contractions as well as EMG potentials could be evoked 1 to 2 hours after blood flow had been arrested. Muscle excitability was further studied in the gracilis muscle from a patient who had undergone hemipelvectomy; it was found that contractility and EMG potentials were evoked at least 4 hours after circulatory arrest. It thus seems that loss of muscle excitability is not as early a sign of impaired blood supply as recently has been suggested. Spontaneous EMG activity, which can be recorded 2 to 3 weeks after denervation, is reported to be affected earlier than muscle excitability by circulatory arrest. Thus, the recording of spontaneous EMG activity in previously denervated muscles is proposed as an alternative and simple electrophysiological method for post-operative monitoring of vascularization in free muscle transfers.

Serum phosphate and 2,3-diphosphoglycerate in severely burned patients after phosphate supplementation.

Serum phosphate (S-P) and 2,3-diphosphoglycerate (2,3-DPG) were monitored for 10 days after severe burn trauma in 16 patients treated with total parenteral nutrition, including supplementation with 25-75 mmol phosphate daily. Phosphate metabolism was previously shown to be disturbed in patients with severe burns, and hypophosphatemia has been reported to disturb oxygen transport by reducing synthesis of 2,3-DPG in the red cells. In this study, S-P was decreased in the first post-trauma week because of a negative phosphate balance the first 4 post-trauma days, but 2,3-DPG remained at normal concentrations. These findings indicate that post-traumatic disturbance in red-cell phosphate metabolism may be prevented by administration of phosphate and that such supplementation is important in the metabolic management of severely injured patients.

The diagnostic and predictive value of cerebrospinal fluid lactate in children with meningitis. Its relation to current diagnostic methods.

One hundred and thirty-three children with suspected meningitis aged from 11 days to 16 years were investigated with routine cerebrospinal fluid (CSF) laboratory methods: microscopy of a Gram-stained smear, bacterial culture, determination of leukocytes, lactate, and the CSF/blood glucose ratio. On the basis of bacterial cultures and clinical course, the children were classified into three groups: bacterial meningitis (n = 18), aseptic meningitis (n = 28), and a control group (n = 87). The main intention was to study the relation between current diagnostic methods and lactate. CSF lactate levels and cell counts, related significantly (p less than 0.01) better to the presence of bacterial meningitis than CSF/blood glucose ratios. Lactate levels exceed 2.4 mmol/l in all children with bacterial meningitis, but in none of the control group. Of 28 children with aseptic meningitis 3 had lactate in the range 2.5-2.7 mmol/l, while the others had values of 2.4 mmol/l or less. We consider CSF lactate to be the best predictor in the clinical decision to institute antibiotic treatment of children with suspected bacterial meningitis.

A thin-layer chromatographic method for separating methyl esters of mycobacterial mycolic acids.

A two-dimensional thin-layer chromatographic method was developed which allowed separation of the different mycobacterial mycolic acids as methyl esters. Dichloromethane (once) and petroleum ether:acetone (95:5, v/v twice or three times) were used as solvents. Alkaline saponification of freeze-dried cells followed by methylation of the mycolic acids using iodomethane gave satisfactory results, whereas methylation using boron trichloride-methanol complex or trans-esterification through direct acid methanolysis was found to degrade epoxy-mycolates. The chromatographic method developed here is rapid and informative, and should prove valuable in routine mycobacterial differentiation.

Ionic transport in individual renal epithelial cells from adult and young rats.

Renal epithelial cells were isolated from the outermost superficial cortex of adult and young rats. The cells, likely of proximal origin, were plated on silicon pieces, and cultured during 1-3 days. Intracellular content and concentrations of K, Na, Cl, and P, and the kinetics of change in intracellular content, after inhibition of Na-K ATPase by incubation with ouabain or in K-free medium, were measured in individual cells in small populations using electron probe analysis. In control medium, concentrations in mM were approximately: K, 130; Na, 15; Cl, 28; P, 140. After 6 h inhibition of Na-K ATPase, cells exchanged all K for Na, and the intracellular Na concentration increased to 139 mM in K-free medium. The Cl concentration increased at most to 46 mM. The sum of intracellular K + Na + Cl did not increase more than 25% after 24 h incubation in K-free medium. There were no differences in intracellular K, Na, and Cl for adult and young rat cells in similar conditions. The half-times of K efflux and Na influx after inhibition of Na-K ATPase measured in adult rat were approximately 16-20 min. In the absence of serum, in K-free medium, the half-times of K efflux and Na influx in young rat cells were approximately 30 min, significantly higher than the half-time in the presence of serum, and with ouabain, being approximately 13 min. Histograms of distributions of K and Na content showed that the cells behaved as a single functional population. Ouabain Ki was estimated to be 10(-4) M. After 24 h preincubation in K-free medium, when returned in 5 mM K-containing medium, adult rat cells recovered rapidly normal intracellular K and Na concentrations. Using this approach, expression of the kinetics ionic transport properties of renal epithelial cells during development, and the hormonal influences on terminal differentiation may be studied.
