Cystosarcoma phyllodes is an unusual breast neoplasm that rarely metastasizes. Most series report chemotherapy, radiation, and hormonal therapy to be ineffective. Three patients were treated with cisplatin and etoposide combination chemotherapy with effective palliation in two patients. Radiation therapy was effective in controlling symptomatic metastasis in all three patients. Hormonal therapy was ineffective in two patients despite the presence of positive hormone receptors. Chemotherapy and radiotherapy may be more effective in the treatment of this tumor than has been reported, although there is no apparent role for hormonal therapy. Functional hormone receptors are probably not present.
In this quality assurance audit, 197 drug information requests received over a 10-day period were evaluated for completeness, legibility, and appropriate documentation according to standards set by the ASHP's Special Interest Group on Drug and Poison Information Practice. Deficiencies in documentation of the caller's telephone number (lacking in 21% of forms), time a response was needed (58%), background information (18%), references searched (16%), and staff review (31%) were noted. Corrective action included emphasis on quality control via improved documentation and daily review of completed consultations by senior staff. Three months later, a reaudit of 240 drug information requests found marked improvement in documentation (information regarding the above-mentioned variables was lacking in 1.7%, 2.5%, 2%, 1.3%, and 3.3%, respectively, of request forms). However, increased documentation resulted in increased response times--30% of questions were not answered the same day as asked, and 12% were not answered in the time frame requested.

A case of a hemangiopericytoma tumor metastatic to the lung from a central nervous system primary tumor in the posterior fossa is reported. The tumor was not responsive to treatment with cyclophosphamide, Adriamycin (doxorubicin), vincristine, DTIC (CYVADIC), high-dose methotrexate with citrovorum factor rescue, aziridinobenzoquinone (AZQ), or dibromodulcitol. However, the patient has had a prolonged survival, lasting for more than 26 years from original diagnosis, despite extensive recurrent primary disease and massive pulmonary metastases. Clinical and pathologic characteristics of these tumors are reviewed, with emphasis on the potential for long-term survival.
Twenty-two patients with inflammatory breast carcinoma received preoperative chemotherapy consisting of weekly administration of cyclophosphamide, doxorubicin hydrochloride, fluorouracil, and vincristine sulfate for six weeks. Postoperative therapy consisted of 22 weeks of biweekly administration of these drugs. Regional radiotherapeutic consolidation followed chemotherapy. Nineteen patients completed therapy. Twelve of these patients remain disease free (median, 15 months; range, four to 32 months). Median disease-free survival for all 22 patients is 13 months or more (range, zero to 32 months). Median overall survival is 18 months or more (range, one to 33 months). This regimen compares favorably with prolonged adjuvant and maintenance chemotherapy for inflammatory breast carcinoma.
