Spontaneous pneumothorax from bronchogenic carcinoma.

Spontaneous pneumothorax secondary to bronchogenic carcinoma is an unusual occurrence. Reported here is a patient with adenocarcinoma of the lung whose presenting symptom was spontaneous pneumothorax. A brief review of the literature is presented.

Accidental overdose intoxication: a hazard of drug smuggling.

Three patients involved in illicit drug smuggling via the swallowing of high dose, high purity drugs packed in multiple condoms are reported. Two of these patients experienced drug overdose symptoms due to leakage or rupture of the condoms in the GI tract. They were treated successfully.

Hydatidiform mole with a live fetus.

The incidence of a live fetus associated with a hydatidiform mole is extremely rare.(1) While hydatidiform mole without a fetus in the United States has a reported incidence of 1 in 2000 pregnancies, the incidence of hydatidiform mole with a coexisting fetus varies from 1:10,000 to 1:100,000 pregnancies.(2)

Osteosarcoma of the maxilla treated with radiation therapy and surgery.

Osteosarcoma (osteogenic sarcoma) presumably arises from mesenchyme and is characterized by the production of malignant osteoid by the sarcomatous stroma. When all cases are considered, osteosarcoma occurs more frequently in the younger age group (peak range of 10 to 25 years of age) and commonly involves extremity bones, particularly the lower. Osteogenic sarcoma rarely arises in the maxilla. The usual treatment is radical maxillectomy, postoperative radiation therapy, and chemotherapy. Results usually are very poor.A case is presented of a 33-year-old man in whom a diagnosis of osteogenic sarcoma of the maxilla was made eight years previously. The treatment was unusual. Preoperative radiation therapy was accomplished by drilling holes in the tumor and adjacent maxilla into which were implanted high-dose radium needles. This was followed by a left radical maxillectomy. The patient is cured and is without functional deficiencies up to the present time.

Why quality assurance?

Quality assurance programs in radiology are essential and each radiologist must be committed to exert comprehensive efforts toward excellent quality control. Identification and evaluation of a radiological problem, corrective action, and good record keeping are essential features of a well-managed quality assurance program. This paper discusses the background and impact of these programs on providing safe radiologic services to patients.

Acupuncture in the People's Republic of China.

Recent worldwide interest in acupuncture has been manifested. Although the rationale for its efficacy as an anesthetic agent is unknown, 25 to 35 percent of surgery in the People's Republic of China is performed under acupuncture anesthesia. The authors discuss the indications, advantages and disadvantages, and complications of acupuncture anesthesia and comment on current theories of the mechanism of its action.

Familial achalasia of the esophagus in mother and son: a possible pathogenetic relationship.

Familial achalasia most often has been recorded in siblings, and only rarely (twice previously) in a parent and offspring. This report concerns a third case of achalasia in a parent and offspring (mother and son). Clinical and radiographic features of the esophageal achalasia are presented. The possibility that genetic factors may play a role in the etiology of this disorder is discussed.

Spinal cord compression from metastatic breast carcinoma: treatment by radiation therapy alone.

There are few reports in the medical literature of treatment of compression of the spinal cord by the use of radiation therapy alone. Laminectomy has been the standard procedure to relieve compression of the spinal cord and should be performed within 48 hours of initial symptoms. In this patient, radiation therapy was the only modality used. It was supplemented by diethylstilbestrol in the course of follow-up. The result of radiotherapy so far has been excellent. The authors emphasize that radiotherapy was begun three months after development of paraplegia, which makes this an interesting case for presentation.

[Breast cancer in men].

Carcinoma of the breast is relatively uncommon in men. Represents somewhere between 0.9 to 1.5 of all tumors of the breast. Most patients are within the 7th to 8th decade of life. The most common symptoms at the time of presentation are a tumor mass in either breast, ulceration of the nipple or nipple retraction or fixation of the skin. Extension to the axiliar nodes takes place early in the course of the disease. The presence of nipple discharge should be considered expression of carcinoma of the breast unless otherwise proven. The final diagnosis is established by biopsy. The most common form of carcinoma of the breast is the ductal carcinoma. The surgical management takes basically that followed in carcinoma of the breast in the female patient. Also orquiectomy has been used which is equivalent to ovariectomy in woman. In the presence of metastasis both estrogens and androgens have been used. Hypophysectomy and post-op radiation therapy has been used. Most recently chemotherapy has been widely used in those cases with widespread metastasis. However, in spite of all these efforts carcinoma of the breast in the male still carries a very serious prognosis. We present our experience in 16 cases.

Particle radiation therapy: requiem or reveille.

The 1960s and 1970s witnessed a surge of many institutions devoted to electron therapy. Currently, many facilities are adding or have added particle types of radiation to their armamentarium against cancer. The authors review the concepts, problems, and potentials of this form of therapy.
