[Results of resection of gastroduodenal ulcers].

The principles of a standardized technique of partial gastrectomy are presented. Number and success of resections were reviewed between 1948 and 1981. Follow-up studies were performed twice (1969 and 1982) which showed a success in 93% according to Visick Grade I and II. Taking the incidence of cancer in the remnant stomach into consideration, we found the number of cancers after partial resection to be very low as far as the patients of our surgical unit were concerned. A close connection seems to exist between the technique of gastric resection and the appearance of cancer in the remnant stomach.

[Diagnosis and therapy of cancer of the large intestine (author's transl)].

A review is presented of the cases treated in this department between 1944 and 1979. Radical surgery was carried out in 388 patients out of 746, whereby the majority of patients were in the older age brackets. Paralytic ileus was present in about 25% of cases and three-stage procedure was, thus, required in the management of these patients. The mortality decreased from 16.6% to 2.1%. The 5-year survival rate was on the lower limits of the range quoted in the literature. Aetiology diagnosis and preoperative management is discussed.

[Treatment of carcinoma of the rectum (author's transl)].

620 patients with carcinoma of the rectum treated in the years 1944 to 1966 are compared with 301 patients treated in the years 1967 to 1978. There was an increase of the operations alltogether and of the curative operations in particular. In the same space of time the mortality was reduced remarkably. The 5-year-survival time after resections was 38%, after abdomino perineal excision 48%.

Tumor host relations. VI. Is alpha-ketoglutarate a tumor marker? Association with tumor extent in humans--correlation with tumor size in rats.

A 4-year survey is presented on the alpha-ketoglutarate (KG) values in whole blood of 200 patients with malignant neoplasms mainly of the gastrointestinal tract and the female breast. A group of patients with benign surgical diseases served as control. KG showed an association with the extent of the primary tumor classified according to the TNM-scheme. The percentages of single values above the 2s-range of the control were as follows: T2: 18%,T3: 41%, and T4: 64%. The mean value of KG had highly significantly increased already in stage T2 as compared to the control group. Rats bearing the Yoshida sarcoma showed a significant correlation between the tumor size and the daily excretion of KG into urine during the early stage of tumor growth. The results suggested that KG cannot be regarded as an early tumor marker in humans, but may be of some value as an aid for the differential diagnosis in advanced tumor stages.

Tumor host relations. I. Increased plasma cortisol in tumor-bearing humans compared with patients with benign surgical diseases.

Plasma cortisol was measured by a protein binding technique in 81 patients with malignant tumors of different extent and various sites and in 82 patients with benign surgical diseases. The mean value of the tumor patients (x +/- s = 165 +/- 69 micrograms cortisol/l plasma) was increased significantly compared with the benign surgical disorders (100 +/- 45 micrograms/l). Within the group of patients with benign surgical disorders there was little variation by the type of disease (cortisol mean values given in brackets): benign breast tumors (95), gall stones (107), ulcer of the stomach and duodenum (96), hernia (78), appendicitis acuta (112), and struma (90). The results are in accordance with the hypothesis that glucocorticoids are involved in the increased protein catabolism of skeletal muscles and other signs of cachectic tumor patients.

Tumor host relations. II. Influence of tumor extent and tumor site on plasma cortisol of patients with malignant diseases.

The increase of plasma cortisol in patients with tumors of five different sites compared with a control group of patients with benign surgical diseases amounted to: +39% (breast), +34% (stomach), +86% (intestine), +60% (skin) and +194% (gall bladder). The first detectable increase of cortisol occurred in patients with tumors classified T 2 according to the TNM scheme (+27% above the control). Highly significant increases were observed for T 3 (+82%) and T 4 (+77%) patients. Patients with palpable lymph nodes showed a most significantly increased cortisol mean value compared with patients without palpable lymph nodes. Similarly, the cortisol mean value of patients with distant metastases was significantly higher than the corresponding value of tumor patients without distant metastases. The question remains open, whether the primary site, the extent of the tumor or the occurrence of metastases is the main determinant for the cortisol increase.

[Surgical therapy of peptic ulcer of the stomach (author's transl)].

Initially recent opinions about the etiology of the peptic ulceration of the stomach, diagnostical possibilities and the indication for surgical treatment are discussed. In view of the malignant degeneration and the increasing incidence of "early cancer", the 2/3 resection-method is prefered now again.

[Progress in the surgical treatment of biliary disease (author's transl)].

Surgical techniques in the treatment of diseases of the gallbladder and the common bile duct have hardly changed over the past 30 years, except in the case of operations on the papilla. On the other hand, the intraoperative diagnosis of stones in the common bile duct according to Mirizzi, combined with the introduction of pressure measurements, has achieved quite a measure of success. Moreover, preoperative diagnosis in patients with jaundice can be achieved simply and with greater accuracy nowadays by means of ERCP, with a consequent positive influence on the operative technique.

[Gastroscopy after gastric operations (author's transl)].

Gastroscopy after surgery of the stomach gives exact information about functional and morphological alterations. The aim of gastroscopy is to detect complications of the early postoperative period as well as of the later phase. Early diagnosis of gastric carcinoma after partial gastrectomy for benign lesions is only possible by routine gastroscopy.

[Experiences with cephradine (Sefril) in septical-surgery (author's transl)].

100 patients with septic-surgical diseases were treated with the cephalosporine-derivate Cephradine (Sefril). 79 patients were treated by surgical intervention in advance of the antibiotic therapy, 21 by Sefril in combination with conservative therapy only. The continuance of medication amounted to two weeks in 94%. A complete healing of the pyogenic infection was obtained in 86 patients. Complications occured in 6%.

[Citrate values in the whole blood of patients with malignant tumors of various locations].

Based on reports on elevated concentrations of 2-oxoglutarate in the blood of human subjects with tumors of various locations, data are presented on the concentration of citrate and pyruvate in the whole blood of 41 patients with malignant tumors (mainly of the female breast and of the digestive tract), 30 patients with benign surgical diseases and 12 healthy subjects. Adopting a level of significance of 2P=0.10 differences of concentration were found between the following collectives: (1) The mean value of citrate for the patients in tumor stage T4 was decreased by 23%, compared with healthy persons, and by 19%, compared with patients with benign surgical diseases. (2) The mean value of pyruvate for the patients with malignant tumors was increased by 40%, compared with healthy subjects and by 17%, compared with patients with benign surgical diseases. (3) The maximum of the pyruvate concentration was reached in tumor stage T3, where an increase of 52% was noted, compared with healthy subjects. (4) The pyruvate value of the patients with tumors of the digestive tract was increased by 34%, compared with healthy subjects. With regard to the total group of patients with malignant tumors, even with a low level of significance, no differences in citrate values were found as compared with the control groups. Thus the results do not indicate a disturbance of the citric acid cycle in the organism of tumor bearing hosts in general.
