Renal tubular acidosis: its types and role in renal calculosis.

In the light of published observations attention is called to the various forms of renal tubular acidosis (RTA), its etiology, pathologic significance, excretion of an "alkaline-persistent" urine in the presence of hyperchloremic acidosis, and to the possible consequences: nephrocalcinosis and renal calculosis. The frequency of incomplete RTA of various causes in calculosis is pointed out. The patient material of the authors and the results of phosphate therapy are presented, stressing the importance of alkalizing therapy and its prophylactic value.

Study on the role of tyrosine side-chains at the active centre of emulsin beta-D-glucosidase.

The role of exposed tyrosine side-chains in enzyme-catalyzed reaction by sweet almond emulsin beta-D-glucosidase (beta-D-glucoside glucohydrolase, EC 3.2.1.21) has been studied using N-acetylimidazole as the specific reagent. The changes in activity, binding affinity and kinetic parameters (Km, V) as a result of acetylation of the phenolic hydroxyl groups have been determined. The acetylation increased the Km values of both beta-glucosidase and beta-galactosidase activities, whereas V remained unchanged. Similarly, the binding affinity for immobilized phenyl beta-D-glucopyranoside decreased appreciably. After the removal of the acetyl groups the enzyme regained 96% of the original activity. It is concluded that the tyrosine moieties, located in the active centre of the enzyme, have both glucoside and galactoside binding functions.

Dumping syndrome and diarrhoea after vagotomy.

The frequency and severity of the post-vagotomic dumping syndrome and diarrhoea were studied in 470 cases. After vagotomy the incidence of the dumping syndrome is lower and that of diarrhoea higher than after conventional gastric resection. The dumping syndrome occurred in 8.30 percent [39 cases], but only three patients belonged into the severe group [0.80 percent]. The frequency of post-vagotomy diarrhoea was 11.49 percent [54 cases] of which six belonged into the severe group [1.34 and 1.04 percent, respectively]. The possibility respectively of preventing or reducing the frequency of these side-effects is discussed.

Incidence of cholelithiasis after truncal and selective vagotomy.

In order to establish the incidence of cholelithiasis after vagotomy, the patients operated upon in the period January 1st 1966, to December 31st 1971, were reexamined. The incidence of cholelithiasis was 12% after truncal and 2.4% after selective vagotomy, thus complications involving the gall bladder are more frequent after truncal than after selective vagotomy.

Participation of epsilonADP and epsilonATp in the reactions of oxidative phosphorylation in rat liver mitochondria.

The 1,N6-ethenoadenine nucleotide analogs epsilonADP and epsilonATP, contrary to recent findings (1), are shown to be unable to penetrate the inner mitochondrial membrane of intact rat liver mitochondria and can not be used as substrates by the respiratory chain enzymes in oxidative phosphorylation. On the other hand, these analogs are able to participate in transphosphorylation reactions, being good substrates for mitochondrial phosphotransferases located in the intermembrane space, such as nucleosidediphosphate kinase and adenylate kinase.

Nucleotide specificity of pyruvate kinase and phosphoenolpyruvate carboxykinase.

Various analogues of adenosine 5'-diphosphate with modifications in the heterocyclic base residue were tested as substrates of rabbit muscle pyruvate kinase (ATP:pyruvate 2-O-phosphotransferase, EC. 2.7.1.40) and guinea pig liver mitochondrial phosphoenolpyruvate carboxykinase (GTP:oxaloacetate carboxy-lyase (transphosphorylating), EC 4.1.1.32). The significance of different structural elements for the enzyme-substrate interaction is discussed. While pyruvate kinase shows a rather broad specificity for these analogues, phosphoenolpyruvate carboxykinase has a more stringent requirement for nucleotides, the intact keto and NH groups at C6 and N1 of the pyrimidine ring representing essential sites for the phosphoenolpyruvate carboxykinase substrate interaction. The biological significance of the different substrate specificities of pyruvate kinase and phosphoenolpyruvate carboxykinase is discussed as a possible metabolic control factor.

[Complications of the vagotomy during the operation and in the early postoperative period (author's transl)].

426 subdiaphragmatic vagotomy operations were performed by the authors. On basis of the results obtained they give full details of the complications arising in the course and in the early postoperative period after vagotomy, and also of the possibilities preventing these complications. They call the attention to the more frequent injuries of the spleen and the oesophagus, and to the increasing number of the subphrenic abscess. Finally, they emphasize the low mortality rate of the vagotomy in comparison to the traditional gastric resections.
