The midline: diagnosis and treatment.

The importance of coordinated midlines often is unappreciated as it relates to the treatment plan for the orthodontic case. All three of the patient's midlines--facial, maxillary, and mandibular--must be considered if ideal correction is to be achieved. Proper differential diagnosis of the cause will allow the practitioner to appropriately use either inter- or intraarch mechanics for the resolution of midline discrepancies. Midline correction should be undertaken from the initiation of treatment and once all midlines are coordinated they should be maintained as a guide for any further force systems used in completing the case. Functional, dental, and iatrogenic midline discrepancies are discussed pertaining to their diagnosis and treatment.

Informed consent and orthodontics.

The legal doctrine of informed consent has struck a chord of fear in most dental practitioners today. This, in turn, has caused many orthodontists to assume a defensive attitude in their practices. A better posture might be to use informed consent to their own and their patients' best advantage. This is accomplished by using the doctrine to enhance patients' education and understanding of their orthodontic problems, the benefits of corrective therapy, any risks associated therewith, and viable treatment alternatives. The end result is an open and honest relationship with the patients, which promotes doctor/patient autonomy.

Electrocautery in orthodontics.

Electrocautery has all but disappeared from the clinical dental setting. Through technical advances, there have been developed new instruments which must now challenge the profession to re-examine the use of electrocautery as a valuable adjunct, particularly in the field of orthodontics. This article will describe a simple yet effective battery-operated electrocautery unit. Indications and examples of its use will also be presented.
