Vitamin E and fatty acid composition of human milk.

The vitamin E and fatty acid composition of human milk was determined in 40 milk samples (six colostral, 10 transitional, and 24 mature) obtained at different stages of lactation. Vitamin E was determined by high performance liquid chromatography with fluorescence detection of the various tocopherols. The total tocopherol level was significantly higher in early milk than in mature milk. The difference was due to a high content of alpha-tocopherol, as the content of beta- and gamma-tocopherol was similar in the three milk types. The total tocopherol content in mature milk correlated significantly with both the total lipid and the linoleic acid content. Significantly higher tocopherol/linoleic acid ratios were found in both colostrum and transitional milk than in mature milk. The colostral milk differed from the other milk types in fatty acid composition, as it had a lower content of lauric acid and a higher content of arachidonic acid and docosahexaenoic acid. The linoleic acid levels reported here are considerably higher than those reported previously in Sweden. Still, the ratio of alpha-tocopherol equivalent/linoleic acid exceeded 0.5 mg/g in all but three milk samples.

The effect of dietary gamma-tocopherol on serum tocopherols in formulated infants.

The levels of alpha-, beta- and gamma-tocopherol were determined in two formulas (Nutramigen and Soja-semp) and in 20 infants fed either of these formulas. The gamma-tocopherol concentration in Nutramigen was 22.8 +/- 1.9 mumol/l; and that in Soja-semp 3.7 +/- 0.5 mumol/l. This difference was reflected in the serum tocopherols of the infants, since in the 10 infants fed Nutramigen, gamma-tocopherol accounted for 16.7 +/- 7.9% of the total tocopherols compared to 4.2 +/- 1.8% in the 10 infants fed Soja-semp. The study shows that gamma-tocopherol accumulates in the serum of infants fed large amounts of it. The serum gamma-tocopherol level should therefore be taken into account, when estimating the vitamin E status in humans, especially when the intake of gamma-tocopherol is high.

A rapid method of visualizing the pancreatic islets for studies of islet capillary blood flow using non-radioactive microspheres.

A technique is described for visualization of the pancreatic islets in the rat by simple dark field illumination of pieces of the frozen and thawed pancreas. By using this method, microspheres, injected for studies of the capillary blood flow, can be readily located and counted in both the islets and the exocrine parenchyma. The procedure could be useful both for rapid estimations of the total number of islets and for studies of their microcirculation.

beta-Hexosaminidase in serum from patients with cirrhosis and cholestasis.

THe enzyme activity level of beta-hexosaminidase (beta-N-acetylglucosaminidase, EC 3.2.1.30; 2-acetamido-2-deoxy-beta-D-glucoside acetamidodeoxyglucohydrolase) in serum from patients with different forms of cirrhosis and cholestasis was found to be elevated. A comparison is also made with routine liver parameters. In the light of recent findings it seems probable that the increased level depends upon depressed clearance of this enzyme by the non-parenchymal cells in the liver.

beta-Hexosaminidase level in serum from patients with viral hepatitis as a measure of reticuloendothelial function.

The lysosomal enzyme beta-hexosaminidase was elevated in different forms of hepatitis. The enzyme level was correlated to the aminotransferases in the acute stage of hepatitis A and B. In hepatitis B there was also a correlation to ALP and bilirubin. In chronic hepatitis B a significant correlation was found only to ALAT. The elevated beta-hexosaminidase levels are attributed to defective non-parenchymal liver cell function in hepatitis.

TLC determination of biotin in a lyophilized multivitamin preparation.

A TLC method for the determination of biotin was developed. The method is selective and sensitive and permits the determination of biotin in the presence of all water-soluble vitamins as well as glycine, edetate sodium, and methylparaben. The main steps are: dissolution of the lyophilized multivitamin preparation in water, addition of the internal standard, application on a thin-layer plate, separation from interfering components, visualization by spraying with 4-dimethylaminocinnamaldehyde, and in situ determination by reflectance measurements. Spraying the plate with paraffin after the coloring procedure increases the sensitivity of the method. The coefficient of variation is 2.3% with 0.15 microgram of biotin/spot. Quantities as low as 10 ng/spot can be determined.

Medicinal iron to low birth weight infants.

Serum ferritin concentrations were measured during the first 6 months of life in 28 low birth weight infants (mean birth weight 1820 g range 900-2460; mean gestational age 34 weeks range 29-37) fed a standard formula fortified with ferrous sulphate. Fifteen of the infants received supplementary medicinal iron (ferrous succinate) from 3 weeks of age, and 13 only from 2 months of age. All were given vitamin E from 10 days of age. The serum ferritin values did not differ between the groups at 1-2 days, 8-10 weeks or at 6 months. Furthermore, there were no signs of hyperhaemolysis at 8-10 weeks in the group receiving medicinal iron early. The data indicate that the iron content in the formula is sufficient until 2 months of age, but also that thereis no disadvantage in starting medicinal iron at 3 weeks of age, if the diet is sufficient in vitamin E.

Release of insulin in vitro from normal and duct-ligated rat pancreas.

Ligation of the pancreatic duct causes atrophy of the acinar cells but leaves the endocrine tissue intact. In the present study, a partial ligation of the pancreatic duct was performed in the rat, and the in vitro insulin response to glucose was compared from both the atrophic and non-atrophic portions of the pancreas. Subsequent morphological studies of the duct-tied portion of the pancreas indicated a complete lack of acinar cells and a possible neoformation of ducts, fat cells and connective tissue. However, islets were present in normal amounts and appeared well perserved. Measurements of the insulin release in vitro from this ligated pancreatic tissue showed that an increase of the glucose concentrations from 3.3 to 16.7 mmol/1 resulted in a six-fold stimulation. A further two-fold stimulation was seen after addition of theophylline to the high-glucose incubation medium. In addition, only under the latter conditions was there a significantly increased insulin release from both non-atrophic portion of the pancreas and from the pancreas of sham-operated animals. It is concluded that duct ligation does not diminish the glucose sensitivty of rat islet B-cells. Thus, the present study does not support the view proposed previously that islet tissue is functionally of a foetal nature following duct ligation.

Variation of serum ferritin in low birth weight infants with maternal ferritin, birth weight and gestational age.

Serum ferritin measured at birth in 69 low birth weight infants proved to vary with gestational age as well as with weight. The increase with gestational age was even more striking when the infants small for gestational age were excluded. The relation between maternal and infant serum ferritin concentration was investigated for 2 groups of infants and their mothers (*preterm and term infants, respectively). Neither in preterm nor in term infants was the serum ferritin found to vary with that in the respective mothers.

Type I congenital dyserythropoietic anaemia with myelopoietic abnormalities and hand malformations.

Type I dyserythropoietic anaemia was diagnosed in an infant, who presented with respiratory distress and hepatosplenomegaly soon after birth. Anaemia became manifest during the neonatal period. The case clearly proves the congenital nature of the disease. Abnormalities of the myelopoietic series indicate that it might be a stem cell disease and the presence of skeletal anomalies of the hands suggests a genetic relationship to some cases of Fanconi and Diamond anaemia. No serum lipid or vitamin E deficiency was present as in type II congenital dyserythropoietic anaemia. Serial serum ferritin determinations indicated that iron stores are increased early in type I congenital dyserythropoietic anaemia despite no transfusion load.

Vitamin E requirements of preterm infants.

Differences between feeding practices in earlier investigations prompted the present study of iron and vitamin E supplementation in breast milk fed preterm infants. A new and highly sensitive technique for quantitation of alpha-tocopherol in serum was used. Studies on 34 infants with a birth weight below 2000 g or gestational age less than or equal to 35 weeks showed that supplementation with 16.5 mg tocopheryl acetate/day from 10 days of age resulted in a significantly higher haemoglobin concentration and lower reticulocyte count at 8-10 weeks than supplementation with 1.5 mg/day (p is less than 0.05). Studies on 23 infants with a birth weight of 2000-2499 g revealed subnormal alpha-tocopherol levels in 2 of the infants given 1.5 mg tocopheryl acetate/day but there was no effect on the haemoglobin concentration at 8-10 weeks. There were no untoward effects of an early iron supplementation with 2-3 mg Fe++ (as ferrous succinate)/kg/day. It is concluded that extra supplementation with vitamin E is advisable also in breast milk fed preterm infants. A low dosage iron supplementation from 3 weeks of age is safe.

Kinetic studies of phagocytosis of IgG-coated latex particles with a thrombocyte counter.

An assay was developed making possible the measurement of the initial rate of phagocytosis of IgG-coated polyvinyl toluene latex particles by human leukocytes. With the use of the volume-discrimating ability of a Trombo-Counter, the small latex particles could be selectively counted, but cells and internalized particles were not counted. The phagocytic uptake was greatley enhanced by an IgG coating of the particles as compared to the uptake of uncoated or albumin-coated particles. An almost complete inhibition of particle uptake was obtained at ice-bath temperature and in the presence of N-ethyl maleimide. Heat-inactivated serum and monomeric IgG inhibited the phagocytic reaction. In the presence of fresh serum a stimulatory effect on the final phagocytic capacity was noticed, but the initial rate of particle uptake varied depending on the preincubation time of particles with serum.

Reduction of isoniazid bioavailability in normal men by concomitant intake of food.

The influence of food intake on the bioavailability of isoniazid (INH) has been examined in nine healthy male volunteers. INH was administered as a single oral dose, both in fasting state and together with a standardized breakfast. Numerous venous blood samples were obtained 5 min-6 hours after the INH ingestion, and the concentrations of unmetabolized INH in serum were assessed by spectrophotometry. The observations indicate that both the peak concentration and the total amount of INH absorbed are greatly reduced when the drug is ingested together with food. Hence it is recommended that, in the treatment of tuberculosis with INH, the drug should be given on an empty stomach. The data may also have some bearing on the use of INH for assessing acetylation rates and estimating dosages of hydralazine and related drugs.

A system for computer-assisted ECG recording at rest and exercise.

Great investments have been made in computerized ECG interpretation but little attention has been paid to rationalization of such costly activities as recording, labelling and mounting ECGs, and still less to the presentation of ECGs. A system for such tasks is described. It includes a PDP8/E computer and modified ink jet ECG recorders, which write alphanumeric text and function as analogue input- and output-devices for the computer. On-line computer service is given independently to two laboratories for ECG at rest, VCG or exercise ECG. After averaging the ECG signal, the computer writes out a properly labelled ECG in an easily surveyable form. The visual interpretation of the ECG is entered and is then written on a new sheet together with the averaged 12-lead ECG and a rhythm strip. In exercise testing, the computer records and processes the ECG at predetermined intervals. The processing includes an automatic ST classification. A summarized presentationof the ECG before, during and after exercise allows easy visual identification of ECG changes. Experience of the system has mainly been obtained from 2 years of routine exercise ECG recording and to a limited extent from ECG and VCG recording at rest.
