Salivary immunoglobulins in cancer patients with chemotherapy-related oral mucosa damage.

In 40 patients with neoplastic disorders treated with chemotherapy containing anthracyclines or 5-fluorouracil, objective changes of the oral mucosa were registered in 22 patients (55%). Serum IgG and IgA levels and the mean serum IgG/IgA ratio were normal. On the contrary, the mean IgG/IgA salivary ratio was 1.27 (normally below 1.0) due to an increased salivary concentration of IgG (mean 0.095 g/l), but also due to a decreased IgA concentration (mean 0.075 g/l); the IgG/IgA ratio in saliva was higher in patients with objective changes of the oral mucosa (1.53). Values of the periodontal indices were compatible with the diagnosis of a manifest periodontal disease, which tended to be more severe than in control groups. A positive correlation between the gingival index and concentration of IgG in saliva, a non-linear correlation between the gingival index and salivary IgA and a positive linear correlation between serum IgA concentration and intensity of periodontal attachment level recession, indicate local and systemic immune responses to periodontal tissue alterations and dental plaque components. The IgA related local humoral immune response is, however, operating at a lower concentration level than in healthy individuals.

Mutant oligonucleotide extension amplification: a nonlabeling polymerase-chain-reaction-based assay for the detection of point mutations.

We have developed a new nonradioactive method for the identification of mutations causing beta-thalassemia and cystic fibrosis. The method is based on the priming of polymerase chain reaction (PCR) by an oligonucleotide complementary to the DNA sequence containing the mutation of interest, which anneals only to the perfectly matched sequence under high stringency conditions. A more upstream oligonucleotide primer is also included in the reaction to discriminate between homozygosity and heterozygosity for a particular mutation and serves as a control for the efficiency of the amplification.

Hb Yokohama [beta 31 (B13)Leu----Pro] detected as a de novo mutation in a Yugoslavian boy.

Hb Yokohama [beta 31 (B13)Leu----Pro] was observed in a young Yugoslavian boy as a de novo mutation. The child exhibited severe transfusion-dependent hemolytic anemia. The variant was detected and quantitiated at 10.5% by reversed phase high performance liquid chromatography. In vitro globin chain synthesis showed a slight imbalance with an alpha/beta ratio of 1.38. Structural characterization of the abnormal beta chain was done by high performance liquid chromatographic analysis, on material obtained by high salt precipitation. The mutation was confirmed by sequencing of the amplified DNA.

Long-term results of combined operative reduction of the hip in older children.

Long-term results of open reduction, combined with femoral shortening, iliac osteotomy, reorientation of the upper femur, and medial iliopsoas transfer, are analyzed in 144 hips, according to patients' answers to follow-up questionnaires. Age at surgery ranged from 7 to 15 years with a follow-up of 9-24 years (mean 13 years). Two-thirds of patients had good results, one-fourth had fair results, and one-tenth had poor results. This was in accordance with radiologic rating in 225 hips, with a mean follow-up of 5 years. Quality of results decreased with age at surgery. The combined procedure is not advised after the appearance of pubertal signs. Since radiologic results favored Chiari's iliac osteotomy and subjective results favored the Pemberton type, individual choice is advised, according to the severity of acetabular dysplasia and the age of the patient. Redislocations, when left alone, had better results than did hips that were operated on again.

[Absenteeism and pathological conditions during pregnancy. Experiences at the Pregnancy Consultation Health Center "Boris Kdrić"].

Looking at numbers we come to the conclusion that out of 1656 pregnant women the majority of 1003 or 60.56% was followed by different pathological conditions, where imminent miscarriage was present in 752 cases or 74.9%. With employed women the percentage of illnesses is 69.58% whereas with the unemployed it is only the half 35.1%. The number of pregnant women who were absent from work was 654 or 76.85% with a total of 66.739 days sick leave, averaging 102 days of sick leave per pregnant woman. At the same time, there were 241 or 24.02% of hospitalised patients with a total of 5.200 hospital days, the average being 26,6 hospital days per hospitalised pregnant woman. The absence from work of pregnant woman is not only of medical nature but is a much complex problem, says the author.

[Detection and quantification of immunoglobulins in gingival cervical fluid in periodontal disease].

Gingival fluid components are capable of functioning at the tooth surface in an analogous way as that of blood in tissues. Our investigation showed strong host antibody responses in gingival fluid in periodontal disease. The level of IgG in gingival fluid in comparison with serum antibody level showed local host responses in gingival tissue through the periodontal disease.

[Hemoglobin O Arab in interaction with beta 0-thalassemia].

Clinical, hematologic and hemoglobin composition data on the first case of Hb 0-Arab in association with beta 0-thalassemia in Yugoslavia are reported here. The propositus was a 26-years-old female from Strumica who was admitted to the hospital for several times because of anemia, hepatosplenomegaly, occasional abdominal pains, malaise and fatigue. Laboratory results presented: Hb 10.0 g/dl, RBC 3.84.10(12)/L, PCV 0.260 l/l, MCV 68 fl, MCH 26 pg, reticulocyte count 1.8%, anisopoikilocytosis, polychromasis, numerous target cells, total bilirubin 2.1 mg/dl, (indirect 1.7 mg/dl), serum-Fe 32.3 microM/L. A starch gel electrophoresis of hemolysate provided evidence for the presence of abnormal hemoglobin (approximately 85%) and Hb F (approximately 15%); the Hb A was absent. Familial screening showed her father was heterozygous for the abnormal hemoglobin, whereas the mother was heterozygous for beta-thalassemia. In vitro biosynthesis disclosed a total absence of beta globin and reduced synthesis of beta x x and gamma globin. The alpha/beta x + gamma-globin ratio was 1.77 (normal, 1.0 + 0.1). Amino acid analysis revealed that lysine substituted for glutamic acid at the position one hundred twenty-one of the beta chain (= Hb 0-Arab or beta 121 Glu----Lys).

[The epithelium of the periodontal pocket--an electron microscopy analysis].

The aim of this study was to examine intra- and intercellular changes that characterize the periodontal pocket epithelian. Biopsy specimens of human gingiva were prepared for an electron microscope study by a routine histological procedure. Results of the histological preparations analyses revealed changes significant for pathogenesis of gingivitis and periodontal disease because they cause destruction of epithelial barrier. Such a damaged periodontal pocket epithelium enables further invasion of harmful noxae and spreading of destructive processes into deeper parts of the periodontium.
