Atypical (dedifferentiated) chondrosarcoma or osteosarcoma with preponderant chondroblastic differentiation?

A bone tumor was detected in the distal right femur of a 15-year-old girl. The age of the patient, the localization, and the X-ray picture were in favor of an osteosarcoma. However, histology was consistent with a "highly differentiated chondrosarcoma with dedifferentiation and focal osteosarcomatous metaplasia". The tumor was resected with preservation of the limb. One year later the tumor recurred, and the leg was exarticulated. Histology was again of the same type, as was the case in a single lung metastasis taken out another half year later. Two series of chemotherapy according to an osteosarcoma protocol had no recognizable effect. An unambiguous diagnosis cannot yet be made.

[The treatment of Scheuermann's kyphosis with the Milwaukee brace (author's transl)].

The application of the Milwaukee brace during adolescence is indicated not only in scoliosis, but also in severe forms of Scheuermann's kyphosis. In Basel between 1970 and 1979, 55 patients with severe thoracic kyphosis were treated with the Milwaukee brace. 45 of these patients were followed for a longer period of time. The results of the measurements of the kyphotic angles and of the angles of the wedges vertebral bodies are presented in detail. In those children who were followed to adulthood an average curvature correction from 40 degrees to 32 degrees was found. This represents a correction exceeding 50%, if a physiological angle of 25 degrees is presumed. Also, measurements showed erection of the wedged vertebral bodies after completion of growth. Consequently, the use of the Milwaukee brace for rigid thoracic kyphosis of more than 35 degrees in adolescents is advised.

Chromosome deletion and multiple cartilaginous exostoses.

We report a 13 year-old girl with manifestations strikingly reminiscent of the tricho-rhino-phalangeal (TRP) II or Langer-Giedion syndrome. A terminal deletion of 8q must be assumed to be the cause of her condition till proven otherwise. A similar chromosome abnormality should be searched for (blindly) in other cases of the TRP II previously thought to have had normal chromosomes.

[Indications for osteosynthesis in fractures in children].

Surgical treatment for fractures in childhood should be rare. Indications for open reduction and internal fixation is given especially in fractures within the joint region, which includes namely epiphyseal fractures, dislocated joint fractures (patella, olecranon, femoral neck, radial head, flake fractures) and dislocated ligament injuries. The same is true in children's fractures with accompanied vascular- or nerve-injuries and in multiple fractures, multiple injuries and open fractures grade III. A very relative indication to osteosynthesis is given in severe axial deviations which can not be corrected conservatively. In the last group the indication depends on the age of the child, localization of the fracture and the mainly displaced plane, but also depends on whether the department in question is experienced in and can fully exploid the conservative possibilities of closed reduction and stabilization.
