Effect of fasting on the lung glutathione redox cycle in air- and oxygen-exposed mice: beneficial effects of sugar.

Fasting increases susceptibility to hyperoxic lung damage in mice, at least in part, by decreasing lung glutathione level. To determine whether fasting alters other components of the glutathione redox cycle, and whether a diet of sugar alone reverses fasting's effects, normally fed, sugar-fed, and fasted mice were exposed to room air or 100% oxygen for up to 4 days. In air-exposed mice, fasting decreased glutathione peroxidase (GP) and glutathione reductase (GR) activities 15% to 20% on days 3 and 4 (p less than 0.01) and glutathione level 25% to 30% on days 2 to 4 (p less than 0.05). When corrected for protein concentration, GP and GR values were similar to those in the fed mice, but glutathione levels remained lower on days 2 and 3 (p less than 0.05). Oxidized glutathione (GSSG) was unchanged, but the ratio of GSSG to total glutathione (reduced glutathione plus GSSG) increased on day 2 (p less than 0.05). In oxygen-exposed fed mice, GP increased 62% and GR increased 39% on day 4 (p less than 0.05), the time when the lung injury was most severe; glutathione increased 30% on days 3 and 4 (p less than 0.05); and GSSG increased threefold and eightfold on days 3 and 4 (p less than 0.01). Oxygen-exposed fasted mice were all dead by day 3 (versus no deaths in the fed mice), failed to increase GR and total glutathione in response to the oxidant stress, and increased GP and GSSG on day 3 to the same extent as the fed mice did on day 4.(ABSTRACT TRUNCATED AT 250 WORDS)

Signal fluctuations due to individual droplets in inductively coupled plasma atomic emission spectrometry.

Large emission intensity fluctuations are observed from analyte species in inductively coupled plasmas. Relative standard deviations are as large as 71% when emission is viewed with time resolution of 10 microseconds. Low in the plasma, peaks in atom emission intensity are accompanied by depressions in ion emission. This behavior appears to be due to local cooling by aerosol droplets. High in the plasma, peaks in atom emission are followed by peaks in ion emission. These emission spikes result from atomization and ionization of analyte from vaporizing particles. Laser light scattering experiments show that droplets or particles exist in a conventional 1.0-kW plasma up to 20 mm above the load coil. Emission signals detected high in the plasma correlate with laser light scattering signals below.

Immunophenotypic characterization of an unusual T-cell lymphoma presenting as anterior uveitis. A clinicopathologic case report.

A 54-year-old woman presented with a unilateral, anterior uveitis that progressed to hypopyon over 4 months despite treatment with steroids. One hundred percent of the cells collected from aspirates of the anterior chamber of the affected eye were morphologically large granular lymphocytes. The aspirated cells were demonstrated by flow cytometry to be a uniform population of T lymphocytes with a diploid genome and an S fraction of 2.3%. On further investigation, the patient was found to have an extensive abdominal malignant lymphoma with the same immunophenotype but different morphologic features than the anterior chamber lymphoid infiltrate. In contrast to the cells in the anterior chamber, the abdominal tumor was highly aggressive as indicated by the cellular morphologic features and the S fraction of 43%. DNA hybridization studies of the abdominal lymphoma demonstrated a T beta 2 T-cell receptor gene rearrangement. The use of these modern diagnostic methods should facilitate the diagnosis of intraocular lymphomas and may have important therapeutic and prognostic implications in the future.

Induction of proteins in interferon-alpha- and interferon-gamma-treated polymorphonuclear leukocytes.

Interferon-gamma (IFN-gamma) treatment of polymorphonuclear leukocytes (PMNs) results in an activation of their functions. Studying the IFN responsiveness of PMNs and using antibodies to the IFN-induced proteins, we have observed the ability of IFN-alpha to stimulate the production of the IFN-induced 67,000 and 56,000 dalton proteins and the ability of IFN-gamma to induce the synthesis of the 67,000, 56,000, and 42,000 dalton proteins. The induction of these proteins is dependent on de novo RNA synthesis, as its induction is inhibited if the IFNs and actinomycin D are added to the cells simultaneously. The results of this study confirm the ability of PMNs to carry out gene activation and demonstrate the ability of PMNs to respond to both IFN-alpha and IFN-gamma.

Loss of oxygen tolerance in newborn rabbits: relationship to changes in eicosanoid and antioxidant levels.

Relative tolerance of newborn animals to hyperoxia has been reported. This study investigated the age limitation of oxygen tolerance and mechanisms for its loss. Developmental changes in lungs of normoxic New Zealand rabbits were studied on days 1, 3, 4, 5, and 10 of life. These were contrasted with newborn and 7-day-old rabbits exposed to greater than 95% O2 for 65 hours. Normoxic rabbits demonstrated a decrement in bronchoalveolar lavage (BAL) 6keto-PGF1a, thromboxane B2, and lower lung catalase, total glutathione, and superoxide dismutase with maturation. Newborns were more tolerant to oxygen than 7-day-old rabbits. Oxygen exposure beginning on day 1 did not result in identifiable lung damage. Exposure beginning on day 7 resulted in microscopic evidence of injury and significant increases in BAL white cells, neutrophils and protein, and a trend toward higher BAL LTB4 compared to normoxic age-matched controls. Antioxidants were higher in the hyperoxic 7 day-olds, but remained lower than values in hyperoxic newborns. These results suggest that loss of oxygen tolerance in maturing rabbits is related to a developmental decrement in antioxidants and prostacyclin.

Compensatory ovarian hypertrophy occurs by a mechanism distinct from compensatory growth in the regenerating liver.

The mechanism by which compensatory ovarian growth occurs is complex and not completely understood. To compare the molecular events in compensatory ovarian growth with those known to occur in other compensatory growth processes such as the regenerating liver, the temporal pattern of proto-oncogene expression and dexoyribonucleic acid synthesis was investigated in rat ovarian tissue after unilateral castration. One hundred fifty female rats were subjected to either a left hemioophorectomy or a sham oophorectomy. Twenty-four rats from each group were put to death at 3 and 14 days after the initial procedure and the ovaries were weighed. There was a mean compensatory weight increase in the right ovaries of the hemioophorectomy group of 7.9% at 3 days and 22.5% at 14 days. The temporal pattern of proto-oncogene expression was determined by removing the right ovary from six rats in each group at 4, 8, 12, 24, 36, and 48 hours after the initial procedure. The ovaries were paired into three samples in each group for each time point and the ribonucleic acid was extracted. Dot blot hybridization was performed on each ribonucleic acid sample with radiolabeled complementary deoxyribonucleic acid probes for the proto-oncogenes c-myc, c-HA-ras, and c-fos. There was no significant increase in proto-oncogene expression in the right ovaries of the hemioophorectomy group when compared with the right ovaries of the sham oophorectomy group. The temporal pattern of dexoyribonucleic acid synthesis was determined by removing the right ovary from three rats in each group at 8, 12, 24, 36, and 48 hours after the initial procedure. Each rat had been injected intraperitoneally with [3H]thymidine 2 hours before the right oophorectomy. The specific activity of dexoyribonucleic acid extracted from each ovarian sample did not demonstrate a significant increase in ovarian dexoyribonucleic acid synthesis after hemioophorectomy or any significant difference in dexoyribonucleic acid synthesis between the hemioophorectomy and the sham oophorectomy groups. This report concludes that compensatory ovarian growth occurs by a mechanism distinct from compensatory growth in the regenerating liver.

Role of eicosanoids in relative oxygen tolerance of newborn rabbits.

Prolonged exposure to hyperoxia can result in significant lung injury, although newborn animals are more oxygen-tolerant than adults. Mechanisms affording tolerance to the newborn are incompletely understood. This study examined the hypothesis that eicosanoids play a significant role in newborn oxygen tolerance. One litter of term newborn albino rabbits and 15 adult rabbits were exposed to 65 hours of greater than 95% O2. An additional litter of newborns served as a normoxic control. Normoxic newborn rabbits had very high quantities of 6-keto-PGF1a and low TXB2 in bronchoalveolar lavage (BAL) fluid. Sixty-five hours of oxygen exposure in newborn rabbits produced no evidence of lung injury on light microscopy, 97% of BAL white cells were alveolar macrophages and BAL protein was low. An equal period of oxygen exposure produced significant lung injury in adult rabbits. BAL fluid from oxygen-injured adults contained a 17-fold greater percentage of PMN and 16-fold higher protein than oxygen-exposed newborns. Hyperoxic adults had significantly lower 6-keto-PGF1a, and significantly higher LTB4 and LTC4 in BAL compared to hyperoxic newborns. This study confirms the hypothesis of relative oxygen tolerance in newborn rabbits compared to adults, and suggests that this tolerance may have been afforded by higher pulmonary levels of the protective prostacyclin metabolite.

Prostaglandin E1 infusion fails to prevent hyperoxic lung injury in adult rabbits.

Arachidonic acid (AA) metabolites may be important mediators in the hyperoxic lung injury process. We have previously demonstrated, in an adult model of hyperoxic lung injury, that bronchoalveolar lavage (BAL) fluid levels of AA metabolites of the cyclooxygenase pathway increase before evidence of overt injury. Nonselective inhibition with indomethacin or dexamethasone failed to ameliorate the injury process, possibly because production of prostaglandin I2 (PGI2) was suppressed. In this study, we attempted to ameliorate hyperoxic lung injury using an infusion of prostaglandin E1 (PGE1), since PGE1 has some of the potentially protective effects of PGI2. Thirty-two adult rabbits were exposed to greater than 95% oxygen; eight served as controls and 24 received PGE1 infusion (five, nine, and ten received 0.1, 0.06, and 0.03 micrograms/kg.min, respectively). At the end of the 65-h exposure period, BAL of the left lung was performed; the right was saved for light microscopy. PGE1 infusion at the 0.06 and 0.03-micrograms/kg.min doses resulted in significantly fewer polymorphonuclear leukocytes (PMN) in BAL fluid (p less than .05). However, PGE1 infusion did not significantly ameliorate the lung injury process. In summary, although PGE1 infusion inhibited the influx of PMN into the lung, treatment did not result in any significant amelioration of the hyperoxic lung injury process.

Product line management and continuum of care.

"Product line management" is a concept that is receiving much interest and attention in the health care industry and one that can be applied to managing the continuum of care. Success with the concept is highly varied, however, largely because it is neither well understood nor even clearly defined. Many managers and academicians believe the concept holds great promise for health care organizations. Others argue it is but a passing fad. Once it is understood and properly applied, the concept can assist at several levels in managing the continuum of care. This article seeks to provide a framework to assist managers in (1) understanding the various perspectives on product line management and other organizational innovations, and (2) choosing the most appropriate approach for their organizations. The article is based on both research of the relevant literature, and data collected formally from seven hospitals and multi-institutional systems at various stages of developing, implementing, and managing product lines, in addition to anecdotal data obtained through teaching and consulting.

Fragmentation of cellular DNA is a nonspecific indicator of responsiveness to tumor necrosis factor.

Tumor necrosis factor (TNF) or lymphotoxin (LT) treatment of cells sensitive to the anticellular action of TNF results in the degradation of their cellular DNA into fragments that are multiples of about 200 base pairs. The specificity of this DNA fragmenting effect was examined. The DNA of cells dying either as a result of exposure to interferon-gamma (IFN-gamma) or as a result of having exhausted their culture media was observed to be fragmented into multiples of 200 base pairs. Antibody to TNF or LT failed to block the IFN-gamma-mediated DNA fragmentation and antibodies to IFN-gamma, TNF, and LT failed to block the DNA fragmentation observed in the cells dying as a result of having exhausted their culture media. Thus the fragmentation of cellular DNA appears to be nonspecific effect of cell death that can be induced by a variety of treatments.

Incidence of anaphylactic self-treatment in an outpatient population.

Insect-allergic and food-allergic patients with a history of previous anaphylactic reactions were retrospectively and prospectively evaluated for incidence of self-administration of epinephrine or antihistamines. Participants were prescribed either Ana-KitR or Epi-PenR dosage forms. In 33 patients, 82 incidents were noted during 1,252 patient-months, an average of 0.78 incidents per patient-year (PPY). A mean of 0.38 epinephrine uses PPY was recorded, ranging from 0 to 2.82 doses PPY. Antihistamine uses averaged 0.69 PPY, ranging from 0 to 4.47 doses PPY. Antihistamines alone quelled reactions in 13 of 17 incidents involving Ana-Kits. Food-allergic patients were more likely to self-medicate, with 1.93 anti-allergic drug uses PPY, compared with 0.49 uses PPY for insect-allergic patients. Six of 22 insect-related epinephrine uses occurred during winter months. No statistically significant difference was noted between drug administration rates determined retrospectively and prospectively.

Unmet needs and unused skills: physicians' reflections on their liberal arts education.

Physicians who graduated from 1955 to 1982 from three liberal arts colleges in southeastern Pennsylvania were asked about the ways that their undergraduate education had prepared or failed to prepare them for careers in medicine and about changes that they would, in retrospect, have made in their courses of undergraduate study. For many, college had failed to meet their perceived need, as physicians, for skill in dealing with people, but had provided skills in the form of basic science knowledge and willingness to be different that exceeded the demands of their careers. They wished that in college they had taken more courses in the humanities--especially art, history, music, and English literature--and less chemistry, mathematics, physics, and biology. Would-be physicians should be encouraged to take full advantage of the humanizing opportunities of a liberal arts education with confidence that it will contribute to their future professional and personal lives.

Correlation between the anticellular and DNA fragmenting activities of tumor necrosis factor.

The treatment of cells sensitive to the anticellular effect of tumor necrosis factor (TNF) with TNF results in a degradation of their cellular DNA into DNA fragments that are multiples of 200 base pairs. TNF treatment of cells resistant to the anticellular effect of TNF, but bearing receptors for TNF, fails to result in any DNA fragmentation. Incubation conditions, such as temperature, the presence of metabolic inhibitors or amino acid deprivation, that modulate the effectiveness of TNF or affect the rate at which TNF exerts its anticellular effect have a similar effect on the ability of the TNF to generate DNA fragments. Thus the TNF-mediated DNA fragmentation and the rate at which it occurs correlates with the rate at which cells respond to the anticellular effect of TNF and, as such, might serve as a marker for the responsiveness of cells to TNF.

The secondary structure of peptides derived from caseins: a circular dichroism study.

Three peptides have been formed by proteolytic digestion of individual casein proteins and their secondary structures characterised by far-UV circular dichroism (CD). Peptide alpha s1(1-23), residues 1-23 of alpha s1-casein, was generated by treatment of the parent protein with chymosin. Peptides beta(1-28) and beta(1-52), residues 1-28 and 1-52 of beta-casein, were plasmin- and chymotrypsin-generated fragments, respectively. Analysis of the CD spectra revealed that in aqueous solution all three peptides have secondary structures composed exclusively of beta-sheet and random coil. A limited amount of alpha-helix was formed in two of the three peptides upon treatment with high concentrations (greater than 40% (v/v] of 2,2,2-trifluoroethanol. Partial dephosphorylation (60%) of beta(1-28) and beta(1-52) by treatment with alkaline phosphatase resulted in homogeneous preparations, as judged by polyacrylamide gel electrophoresis, which exhibited increased hydrophobicity. This reduction in the level of phosphorylation of serine residues 15, 17, 18 and 19 led to increased propensity for helix formation in the peptides in the presence of 2,2,2-trifluoroethanol, but no alpha-helical structures were detected in the dephosphorylated peptides in the absence of 2,2,2-trifluoroethanol.

Tumor necrosis factor and IFN induce a common set of proteins.

The treatment of cells with TNF or IFN results in the development of an antiviral state and in the induction of a common set of proteins with m.w. of 80,000, 67,000, and 56,000. The induction of the 80,000- and 56,000-Da proteins after TNF treatment is dependent on the synthesis of an intermediary protein, whereas the induction of the 67,000-Da protein appears to occur as a direct result of the TNF treatment. The effects of antibodies to IFN on the TNF-mediated effects have been evaluated and reveal that the incubation of TNF-treated cells with antibody to rIFN-beta 1 greatly reduces the antiviral effectiveness of the TNF treatment and blocks the ability of TNF to induce the 80,000-Da protein. Incubation with antibodies to either IFN-alpha or IFN-gamma failed to affect the TNF-mediated responses. Thus, the induction by TNF of each of the proteins is regulated differently and is mediated through both IFN-dependent and IFN-independent mechanisms.

Site specific effects of acute exercise on muscle and adipose tissue metabolism in sedentary female rats.

The effects of endurance exercise on muscle, and adipose tissue metabolism were investigated. Female lean Zucker rats swam for two hours at high intensity. Three groups were examined: pre-exercise control (C), exercised (E) and 24 hours post-exercise (E-24). Exercise increased fat cell lipolysis in the inguinal depot (p less than 0.05) while no effect was detected in the parametrial depot. In contrast, parametrial pad lipoprotein lipase (LPL) activity decreased after exercise with 24 hours post-exercise values being reduced below E and C rats (p less than 0.05). Gastrocnemius LPL activity remained unchanged during exercise while heart LPL increased, E having higher values than C and E-24 (p less than 0.05). Gastrocnemius, but not heart, citrate synthase activity increased with exercise, with E-24 values increased compared to E and C (p less than 0.05). These results demonstrate that adipose tissue's response to exercise is site specific, and suggests a distinct physiological role for different adipose depots. Muscle LPL and citrate synthase activities were modified differently for gastrocnemius and heart, confirming the distinct metabolic response to exercise of these two muscles.

Analysis of bronchoalveolar lavage in allergic bronchopulmonary aspergillosis: divergent responses of antigen-specific antibodies and total IgE.

Bronchoalveolar lavage (BAL) was performed in eight patients with allergic bronchopulmonary aspergillosis (ABPA) at a time when chest roentgeongraphy did not reveal an infiltrate, and respiratory status was stable. BAL was tolerated well by all patients with only one patient experiencing mild wheezing. BAL fluid recovery averaged 40%, and total cells/lavage were 22.3 x 10(6) (range 3.5 to 49.5 x 10(6)). Cell viability, as determined by trypan blue exclusion, averaged 48% (range 34% to 60%). Mean values for cellular elements were macrophages, 62%; epithelial cells, 12%; lymphocytes, 16%; neutrophils (PMN), 4%; and eosinophils, 6%. Isotypic antibodies to Aspergillus fumigatus (Af) in BAL and serum were detected by an amplified indirect ELISA. Antibodies to Af in BAL expressed as optical density/albumin (milligrams per milliliter) were compared to BAL from six nonatopic patients. IgE-Af and IgA-Af in BAL were elevated in patients with ABPA compared with six nonatopic patients. The ratios of Ig-Af in BAL to peripheral blood in patients with ABPA were 48 (range 18 to 75) for IgE-Af, 96 (range 37 to 159) for IgA-Af, and 0.94 (range 0.24 to 1.40) for IgG-Af, suggesting local production of IgE-Af and IgA-Af in the bronchoalveolar compartment. Total serum IgE correlated directly with IgE-Af in BAL (rs = 0.67; p less than 0.02). However, the ratio of total BAL IgE/albumin divided by total serum IgE/albumin was 0.93 +/- 0.94, suggesting that the bronchoalveolar compartment is not the source of the significant elevations in total serum IgE in ABPA.
