Physiological effects of plus lens application.

The prescribing of low-power spherical convex lenses for nearpoint has been advocated on the basis of associated visual and physiological responses. A review of research related to this topic reveals that the physiological effects of plus lenses are highly individualized. The purpose of this paper is to reevaluate research on this subject in the context of clinical prescribing.

STYCAR ball acuity in relation to contrast and blur.

The graded balls test of Sheridan is used to screen for vision problems in infants and poorly communicative patients. To simulate what the infant with a refractive error may see, we photographed black and white spheres of identical size through various plus lenses. The results show that the white ball becomes progressively easier to identify with increasing blur. The size of the blur circle is shown to be directly related to background contrast. Because the white balls are viewed against a black cloth at 10 ft to enhance contrast, this may significantly bias the test results. Consequently, a neutral background may be a more appropriate surface upon which to assess the visual acuity by this technique.

Management of Adie syndrome.

In most instances the patient with pupillotonia presents with under accommodation in the affected eye. An unusual case of Adie's pupil with ciliary spasm is discussed. As a result of the unpredictable accommodative response, the necessity of evaluating and prescribing for binocular balance is underscored. Periodic reevaluation of accommodation in relation to symptomatology is emphasized.
