[Heterotopy of the mucous membrane in the fundic part of stomach into an adenomatous polyp of the common bile duct with an ulcerous affection formed in it].

The surgical material of 1,340 cholecystectomies as a result of cholelithiasis was analyzed. Heterotopy of the stomach mucous membrane into gallbladder was discovered in two cases; its heterotopy into the common bile duct with an ulcer formed in the heterotopic section of the mucous membrane in the fundic part of stomach was discovered in one case.

[Results of organ-preserving operations in gastric ulcer].

The authors analysed the immediate and late results of selective proximal or truncal vagotomy with excision of the ulcer from the gastric lumen or wedge resection of the stomach in patients with various localization of the gastric ulcers. The best results were produced (Visick I-II) in patients after selective proximal vagotomy with excision of the ulcer from the gastric lumen (in 93%), the results were poorer in selective proximal vagotomy with wedge resection of the stomach and in truncal vagotomy with excision of the ulcer or wedge resection of the stomach (only in 20%, 38%, and 30% of patients, respectively). The results were unsatisfactory (Visick IV) mainly after vagotomy with wedge resection of the stomach due to recurrences and postvagotomy functional disorders. The authors claim only limited use of organ-preserving operations to be admissible.

[Clinical morphology of enteropathies].

The author describes the morphological substrate and pathogenesis of the most common enteropathies (gluten enteropathy, collagen sprue, Whipple's disease, common variable immunodeficiency), discusses the potentials and limits of morphological methods for differential diagnosis of enteropathies, emphasizes the high informative value of duodenal biopsies that are as good as jejuno-biopsies. He considers it incompetent to use the term "chronic enteritis" which is widely spread in our country. The term stands for nosological diagnosis, making the treatment of patients worse.

[The dynamics of the morphological liver changes in primary biliary cirrhosis (based on the data from repeat biopsies)].

Repeat biopsy examinations (38 specimens) from 18 patients with primary biliary cirrhosis (PBC) showed association of the most active PBC with pronounced ductule proliferation, especially with ductule necrosis. The semiquantitative R. Knodell technique allows defining most characteristic histological parameters and correlations. In 3 patients the biopsies helped detect a combination of PBC with viral hepatitis resultant in cirrhosis. Repeat biopsies contribute to specification of the initial diagnosis, to evaluation of PBC activity and progression.

[Mucous membrane of the gastric antrum in late periods after selective proximal vagotomy with surgical drainage of the stomach and duodenum].

A clinicomorphological analysis was performed of the gastric antrum mucous membrane of 60 patients after selective proximal vagotomy because of stenosing duodenal ulcer. The patients were subdivided into two randomized groups depending on the type of the draining operation. The first group had a gastroduodenoanastomosis for the gastric drainage and the second group underwent duodenum plasty (drainage of the duodenum). Discontinuation of the pylorus function in the gastric drainage operation resulted in higher incidence and intensity of duodenum-gastric reflux and made more severe antrum gastritis; reflux-gastritis developed frequently. Morphological signs of reflux-gastritis in 95% of cases coincided with the results of the radionuclide method. Duodeno-gastric reflux decreased as a rule Helicobacter pylori (HP) contamination of the mucous membrane but did not eliminate it completely in all cases. The combination of these two factors resulted in more severe gastritis than that provoked by each of them separately. The etiology of gastritis and its intensity may be determined only by morphological examination of gastric mucous membrane. The results of endoscopic and histologic methods coincided in 70% of cases. Duodenal-gastric reflux, HP and high acid production play a certain role in ulcer recurrence.

[Mucous membrane of the jejunum in allergy to cereal protein].

Biopsies of the jejunum from 61 children, 6 months-4 years of age, including 35 with allergy to corn protein, 18 with celiac disease and 8 children without stomach and intestine pathology were studied. Biopsies were fixed in 10% formalin or in Carnois fixative, the sections were stained with hematoxylin and eosin, 0.5% toluidine blue (pH 0.5), and immunoperoxidase method was used for revealing IgA. The structure of the mucosa was not changed in the allergic patients, the content of the granulated and ungranulated labrocytes was increased, their number being decreased in the elimination diet. The number of plasma cells was diminished, and in only few of them IgA was found in children with severe alimentary malabsorption. A selective deficiency of IgA was diagnosed which determines the therapy of such children. Hyperregenerative atrophy of the mucous membrane and predominance of degranulated cells over undergranulated were noted in celiac disease.

[The intestinal microflora and acid-forming function of the stomach in peptic ulcer patients with Helicobacter pylori bacteriosis].

Seventeen duodenal ulcer patients were examined. H. pylori were detected in all the patients by histological and bacteriological methods. Changes in the enteric microflora, manifested by a decrease in the number of lacto- and bifidobacteria and by an increased level of opportunistic microflora, were established. Cimetidine treatment and the suppression of the acid-producing function of the stomach augmented enteric dysbacteriosis and H. pylori contamination of the gastric and duodenal mucosa.

[A new international classification of gastritis].

New, published in 1989-90, international classifications of gastritis take into consideration its topography, the degree of morphological changes and etiology. This approximates the morphological diagnosis of gastritis to the nosological one. The most frequent cause of the gastritis is pyloric Helicobacter. Their presence should be indicated in the morphological diagnosis as this predetermines the therapeutic policy.
