Observations on the injury mechanism of anterior cruciate ligament tears in skiers.

The purpose of this study was to demonstrate the uniqueness of the injury mechanism for acute complete tears of the anterior cruciate ligament in recreational alpine skiers by using patterns of osseous injury as detected by magnetic resonance imaging. We treated 42 patients (average age, 32 years) who met the following criteria: 1) acute complete anterior cruciate ligament tear incurred during skiing, 2) no history of knee injury, 3) magnetic resonance imaging within 30 days of injury, and 4) arthroscopy and anterior cruciate ligament reconstruction or both for documentation of all intraarticular injury. An osseous injury detected by magnetic resonance imaging was seen in the lateral femoral condyle in 17 (40%) of the patients and was located over the terminal sulcus in 15 of these patients; only 1 had arthroscopic evidence of chondral injury. Four (10%) of the 42 patients had a bone bruise on the medial femoral condyle; all were located at the extreme posterior margin. Thirty-four patients (81%) had a bone bruise on the lateral tibial plateau; 33 were located at the extreme posterior margin and 27 of these were at the posterolateral tibial rim. Twelve patients (29%) had a bone bruise on the medial tibial plateau; all were located on the posterior margin.

The effect of the cytochrome P-450 suicide inactivator, 1-aminobenzotriazole, on the in vivo metabolism and pharmacologic activity of flurazepam.

Flurazepam (F) is an extensively prescribed hypnotic (Dalmane) whose in vivo activity has been suggested to be due to its primary metabolites, hydroxyethyl flurazepam (HEF) and N-desalkylflurazepam (DAF). In order to determine the intrinsic pharmacologic activity of F, mice were administered various doses of the cytochrome P-450 suicide inactivator, 1-aminobenzotriazole (ABT), 1 hr before the ip administration of 1 mg/kg 14C-F. One hr after 14C-F, 70 mg/kg pentylenetetrazole was administered iv and the mice were observed for convulsions. F alone offered no protection from convulsion (mean brain concentrations were 3.9, 32, and 11 ng/g for F, DAF, and HEF, respectively). F with 25 mg/kg ABT also offered no protection despite a 6-fold increase in brain concentrations of F. F with 100 mg/kg ABT offered a 57% protection from convulsions (mean brain concentrations were 99, 62, and 41 ng/g for F, DAF, and HEF, respectively). One mg/kg F with 400 mg/kg ABT offered 100% protection from convulsions (brain concentrations were 190, 47, and 18 ng/g for F, DAF, and HEF, respectively). These data indicate that F has intrinsic pharmacologic activity which must be considered when evaluating the pharmacodynamics of F.

Effectiveness of a universal adhesive on dentine--an in-vitro study.

The purpose of this study was to test the effectiveness of a universal bonding agent (Prisma Universal Bond) when used in dentine cavities. The measurement of marginal contraction gaps was the basis for determining the effectiveness of this bonding agent. The use of the bonding agent led to a significant reduction in restoration marginal gaps. Thermocycling of restorations with the bonding agent resulted in a widening of the marginal gaps though this was not significant. It was interesting to note that some thermocycled restorations showed no marginal gaps.
