[An immunohistochemical study on ciliary sulcus tissue in cases of intraocular lens implantation with ciliary sulcus suture fixation].

OBJECTIVE: To observe abnormal cell multiplication in rabbits ciliary sulcus with posterior chamber intraocular lens (PC-IOL) implantation and ciliary sulcus fixation technique. METHODS: 96 New Zealand rabbits were implanted with PC-IOL, and they were divided into two groups: one with ciliary sulcus suture fixation and the other without the suture fixation. The eyeballs were extracted at the following postoperative times: 1/2, 1, 2, 3 months. The reactions of the proliferating cell nuclear antigen (PCNA) in ciliary sulcus of the rabbits were studied with immunohistochemistry technique in the above two groups and the normal control group. RESULTS: There was no abnormal proliferating cell reaction in ciliary sulcus of the rabbits with the two methods of PC-IOL implantation techniques. CONCLUSIONS: IOL ciliary sulcus suture fixation is a convenient method for PC-IOL implantation under the circumstance when rupture of the posterior capsule occurs intraoperatively or in the second stage implantation.

[Virological and clinical features of patients with sporadic hepatitis C].

In this study, the transmission route in 16 sporadic hepatitis C (SHC) patients was investigated. Three of them were surgeons who had often had occupational needlestick accidents, another 3 had close household contact with their spouses who had been diagnosed as chronic posttransfusion viral hepatitis C (PTHC), and the remaining 5 had potential parenteral exposure such as tooth extraction, injection or inoculation and so on. Five patients with SHC didn't have such history, their transmission route was not determined. Our result showed a lower viremia level in patients with SHC when compared to PTHC patients (the serum dilutions for HCV RNA detection was 10-100 times in the former and 100-10000 times in the latter. P<0.01). Only 1 patient with SHC was anti-HCV positive. Comparing to PTHC, the patients with SHC in our study had milder liver demage and lower ALT levels, and most of them (10/16) were symptomless.

[Detection and significance of HCV RNA in saliva, seminal fluid and vaginal discharge in patients with hepatitis C].

To investigate the transmission of HCV infection through family contact, we detected HCV RNS in body fluids (saliva, seminal and vaginal discharge) of 16 serum HCV RNA positive patients (including 7 men and 9 women) and in sera of their family members. The positive rates of HCV RNA in the body fluids were 31.25% (5/16) in saliva, 57.14% (4/17) in seminal fluid and 22.22% (2/9) in vaginal discharge, respectively. Among the family members in our series, all were negative for both anti-HCV and HCV RNA, despite two spouses positive for HCV RNA. This result strongly suggested the potential possibility of the transmission of HCV infection through the family contact.

Carcinoid tumor of the common bile duct producing gastrin and serotonin.

Carcinoid tumors of the biliary tract are rare. We report a 47 year-old man who was unexpectedly found to have a nonobstructing carcinoid tumor of the common bile duct during orthotopic liver transplantation for decompensated cirrhosis. No metastases were noted. Five months after resection of the common bile duct and liver transplantation, the patient had no evidence of residual tumor. The carcinoid was a sclerotic tumor of insular type and was immunoreactive for gastrin and serotonin, but nonfunctional. We review the literature on carcinoids of the extrahepatic bile duct.

Immunohistochemical detection and significance of hepatitis C virus antigen.

OBJECTIVE: To study the expression and clinical significance of hepatitis C virus (HCV) antigens in liver tissues of patients with liver disease. PATIENTS AND METHODS: Hepatitis C virus antigen in paraffin-embedded liver tissues was detected by immunohistochemical method using polyclonal anti-HCV and monoclonal anti-HCV NS3. RESULTS: Among the 252 liver tissues, 25 (9.92%) were positive for HCAg (detected with polyclonal anti-HCV), and 19 (7.74%) were positive for HCV-NS3 (detected with monoclonal anti-HCV-NS3). All of the 19 HCV NS3 positive tissues were also positive for HCAg. Their distribution in liver tissue was similar, locating in the cytoplasm of liver cells. In most tissues, the positive cells scattered in liver lobulus, without obvious approach to the necrosis and infiltration aggregation; while a few samples showed positive cells clustered or diffused, with some necrosis or/and inflammatory infiltration. Of the 252 sera samples, 71 had HCV antibody or/and HCV RNA, showing a good correlation between HCV antigen in liver tissues and HCV marker in sera, but the detection rate of HCV antigen in liver tissues was much lower than that of HCV markers in sera. Two cases had HCV antigen in their liver with no HCV markers in their sera. CONCLUSION: Stored paraffin-embedded liver tissue can be used in the study of the immunopathology of HCV: no definite relationship between the expression of HCV antigens and liver damage has been found. Further study of the pathogenesis of HCV is needed.

[A survey on distribution of red cell blood group systems in naxi and primi ethnic groups].

A survey of distribution of red cell blood group systems, including ABO, MNSs, Rh and P, was carried out on the Naxi and Primi ethnic groups in Yunnan province. The results based on 104 cases in each of the two ethnic groups showed that both Naxi and Primi possessed a high gene frequency r of 0.6082 and 0.6882, respectively, with gene frequency p = q. The gene frequency m of Naxi (0.8509) was found to be very high among the populations studied in China until now, only next to that of Lizu (0.8709). The most common phenotype of Rh system was CcDE- in both Naxi and Primi, with a quite high cDE frequency. No case of Rh negative was observed in the two ethnic groups. The P1 in Naxi approximated to that in Primi. The red cell blood group systems and their genetic distances suggested that the Naxi and Primi was genetically close to ethnic groups of North China, but different from those of South China. This fact suggests that these two ethnics groups originated from the North China.

[The development of skin external expander and clinical applications].

The skin external expander that was designed and developed by the authors is a new device to produce extra skin tissue for the repair of defects by continuous traction over the skin. Clinical application in 38 cases since June 1993 have shown that the method has following advantages: easy in manipulation, unnecessary to bury the expander under the skin, safe and less complications with reliable effects. Accurate measurement of the expanded skin can be obtained with the method. Six months' follow-up of 18 cases demonstrated satisfying results.

A pK change of acidic residues contributes to cation countertransport in the Ca-ATPase of sarcoplasmic reticulum. Role of H+ in Ca(2+)-ATPase countertransport.

Proteoliposomal vesicles reconstituted with sarcoplasmic reticulum ATPase and exogenous lipids sustain ATP-dependent Ca2+ uptake and H+ ejection, as well as net charge displacement by Ca2+. We have studied the effect of lumenal (inner) and medium (extravesicular) pH variations on the countertransport ratios of H+ and Ca2+. We find that the Ca2+/H+ molar ratio is approximately 1 when the lumenal and medium pH is near neutrality, but changes with a specific pattern when the medium pH is varied in the presence of a constant lumenal pH and when the lumenal pH is varied in the presence of a constant medium pH. Empirical analysis of the experimental data shows that the apparent pK of the residue(s) releasing H+ into the medium is approximately 6.1, whereas the apparent pK of the residue(s) binding lumenal H+ is approximately 7.7. Assuming that the same acidic residues are involved in H+ and Ca2+ countertransport, our findings suggest a lower affinity for H+ in their outward orientation (prevalent in the ground state of the enzyme) and a higher affinity for H+ in lumenal orientation (prevalent in the phosphorylated state of the enzyme). Cyclic pK changes, coupled to ATP utilization, promote cation exchange, Ca2+ uptake, and H+ ejection by the vesicles. The stoichiometry of countertransport and net charge displacement is matched by a corresponding electrogenic behavior. A calculation of voltage development related to initial rates of charge transfer (dV/dt = (dQ/dt)/Cm) is given as a corrective replacement of a previous steady state calculation.

Ca2+/H+ countertransport and electrogenicity in proteoliposomes containing erythrocyte plasma membrane Ca-ATPase and exogenous lipids.

A reconstituted proteoliposomal system was obtained with Ca-ATPase purified from human erythrocyte membrane (plasma membrane, PM ATPase), and liposomes prepared by reverse-phase evaporation. The reconstituted PM ATPase behaved as an electrogenic Ca2+/H+ exchanger and, under optimal conditions, utilization of 1 mol of ATP was accompanied by uptake of one Ca2+ by the vesicles, and ejection of one H+ from the lumen of the vesicles. Ca2+ uptake was greatly (5-fold) stimulated by the addition of calmodulin, and by collapsing the H+ gradient with the ionophore carbonyl cyanide p-trifluoromethoxyphenylhydrazone. In the presence of calmodulin and p-trifluoromethoxyphenylhydrazone, the reconstituted system sustained transport rates of 1.00 +/- 0.12 mumol of Ca2+/mg of protein min-1 (30 degrees C), reaching asymptotic levels of 8.05 +/- 0.41 mumol of Ca2+/mg of protein (i.e. 20 mM lumenal Ca2+). The corresponding net charge transfer produced a maximal electrical gradient of 40.5 +/- 1.8 mV at steady state. Demonstration of the electrogenic behavior of the PM ATPase, obtained for the first time with these experiments, was critically dependent on the detergent used in the reconstitution procedure. The lumenal pH rise had a much greater rate-limiting effect on the pump, than the electrical potential developed by the pump.

Distribution of red cell blood groups in Han subpopulations of Inner Mongolia, Gansu and Henan Provinces, China.

Three Han subpopulations, Gansu, Henan, and Inner Mongolia were examined for red cell blood group systems, including ABO, Rh, MNSs, Kidd, Duffy, Diego, and Xg. There was no MS type in Gansu, no SS type in Henan and no MNS and NS types in Inner Mongolia, no CCDE- and CcDEE types in Henan, and no ccDee type in Inner Mongolia. The gene frequencies of S (0.0975) and P1 (0.2584) as well as the MS haplotype frequency (0.0629) in Gansu were the highest, while the Di(a) gene frequency (0.0151) in Henan was the lowest among the subpopulations of Han nationality studied so far. The Xg(a) gene frequency in Henan was 0.6055, the highest value observed in China. The genetic distances and phylogenetic tree of eight Han subpopulations showed that the Han in Henan were closest to the Han in Inner Mongolia, and that the Han in Fujian were closest to the Han in Guizhou among others.

Distribution of red cell blood group systems in Achang and De'ang ethnic groups in China.

A survey on the distribution of red cell group systems, including ABO, MNSs, Rhesus and P, was carried out in the Achang and De'ang ethnic groups in Yunnan Province, South-West China. The Achangs are characterized by the highest frequency of IA in China, while the De'angs show a high frequency of IO and CDe. The distribution of these blood group systems in Achang and De'ang exhibits the same characteristics observed in other ethnic groups of South China.

Detection of cell-mediated immunity in type I diabetes mellitus.

Type I insulin dependent diabetes mellitus (IDDM) is thought to result from chronic, cell-mediated, 'autoimmune' islet damage. Antibody testing is extensively used to define and follow the pre-diabetic population. However, the assay for cell mediated immunity (CMI) should be more predictive of impending disease. This report shows that it is possible to detect human islet reactive CMI in the IDDM patient. Groups of athymic CD-1 nu/nu mice were injected intraperitoneally with either mononuclear blood cells (MBCs) or plasma from 10 newly diagnosed Type I diabetic patients and 10 normal control subjects. Both glycemic control and histopathology were used to assay islet specific CMI in diabetic individuals. None of the injected mice showed any impairment of glycemic control. However, MBCs from six of 10 diabetic patients, but from none of the 10 normal subjects, induced significant mononuclear cell infiltrate in the pancreas of the recipient mice (P = 0.005). The infiltrate was focused on the islet tissue and no damage was seen in control tissues. No histological abnormalities were observed when plasma was transferred. We conclude that cellular reactivity seen in this model is tissue specific and disease associated. Our findings provide evidence that CMI to human islet tissue can be detected in IDDM patients.

Facilitation of specific tolerance induction in adult mice by RS-61443.

RS-61443 is an immunosuppressive agent that facilitates pancreatic islet allograft acceptance in two mouse strain combinations (BALB/c----CBA and C57Bl/6J----BALB/c). A remarkable feature of this agent is its ability to facilitate long-term graft acceptance after a short (30 days) period of treatment; following withdrawal of the agent 40-70% of islet allografts are maintained for an indefinite period. This long-term graft acceptance has been shown to result from specific tolerance induction in the recipient animal. The state of specific tolerance is an active rather than a passive form of tolerance, such as clonal deletion or anergy. Active tolerance induction is cyclosporine-sensitive, although cyclosporine enhances graft acceptance when used in combination therapy with RS-61443, this agent inhibits tolerance development under the influence of RS-61443.
