Postoperative radiotherapy in rectosigmoid cancer Dukes' B and C: interim report from a randomized multicentre study.

The design, and complications seen during the first 2 years, of a randomized trial of postoperative radiotherapy for rectosigmoid cancer Dukes' B and C are presented and discussed. It is concluded that the present complication rate-below 10% in 221 patients-permits continuation of the intake, which is planned to include 550 patients, to demonstrate a possible increase in crude 5-year survival by 15% (60-75% in Dukes' B and 25-40% in Dukes' C), on the basis of a 0·01 significance level and a probability that the experiment will be successful of 0·90.

Urinary incontinence in old age. A controlled clinical trial of emepronium bromide.

The effect of the anticholinergic agent, emepronium bromide (Cetiprin), was studied in a double blind crossover study of a group of elderly patients with urinary incontinence and uninhibited bladder contractions during cystometry. There was no statistically significant difference between the subjective effect of emepronium bromide and that of placebo, and no change in the cystometric parameters. The overall subjective cure or improvement rate was 79%. The effect of anticholinergic drugs in the treatment of urinary incontinence in elderly patients with uninhibited bladder contractions might to some extent be due to an improvement in the patients' understanding and acceptance of the bladder disorders.

Partial deletion of the short arm of chromosome 3.

A case of deletion of the short arm of chromosome 3 (46,XY,del(3)(p253) is described. The patient is a youth of 18 years in an institution for the mentally retarded. Phenotypically, he presents congenital heart disease, hypertelorism, ptosis, epicanthus, blepharophimosis, strabismus, nystagmus, synophrys, low-set ears, frequent infections, epilepsy (abnormal EEG and grand mal seizures), "rocker bottom" feet, flat occiput and muscular hypotonia. The parents are healthy and with normal karyotypes. A silent allele in the GPT system was found in the mother, the propositus and 4 of the 5 siblings.

Intracranial Saccular Aneurysms. Results of treatment in 851 patients.

The series studied comprises all 851 patients with symptom-producing intracranial saccular aneurysms admitted to the departments of neurosurgery in Denmark in the five-year period of 1970-1974. The series was divided into seven clinical stages according to Hunt's classification. More than half of the patients were grouped in stages IV and V (Hunt grade 2-3). Of the patients, 76% were found primarily suitable for operation. The mortality within the individual stages was to a great extent independent of the time of operation. A total of 94% of the aneurysms were localized within the region of the carotid artery, with a roughly equal distribution among the internal carotid, anterior communicating, and middle cerebral arteries. Multiple aneurysms were found in 19% of the patients subjected to panangiography. There was a distinct correlation between the severity of the spasms and the clinical condition. The course of operation was complicated in 49% of the patients, the complications being equally distributed within the various clinical stages. The mortality for the patients who underwent operation (total 567) was 32%, and 80% of the deaths were due to direct or indirect consequences of aneurysmal bleeding. At the follow-up performed two to seven years after operation, 52% of the survivors were fully capacitated, 20% were partly capacitated, and 28% were incapacitated. Based on a retrospective analysis, we have started a comprehensive prospective study with registration of available parameters in patients with saccular aneurysms admitted to all departments of neurosurgery in Denmark.

Importance of flow and haematocrit for metabolic function of perfused rat liver.

The influence of flow rate and haematocrit on the metabolic function of once-through perfused rat liver, as assessed by galactose elimination and oxygen uptake, was studied. Rats weighting from 195 to 220 g were used. Each of eleven experiments consisted of four periods; the blood flow rate was 10 ml/min in the first and the last period, and it was varied at random from 0.6 to 18 ml/min in the second and the third period. Haematocrit values of either 0.40, 0.28 or 0.16 1/1 were constant in each experiment. The input concentration of galactose was 1.7 mmol/l. Both galactose elimination and oxygen uptake were independent of the flow rate between 6 and 18 ml/min. At flow rates below 6 ml/min (i.e. below 0.9 ml/min/g liver) both functions were reversibly decreased in parallel with the flow. The values were highly correlated. There was a trend towards a lower galactose elimination rate and oxygen uptake at lower haematocrit values. It is emphasized that flow rate and haematocrit must be kept above certain limits in metabolic studies with perfused rat liver.

Comparison of the chloride electrode and gravimetric chloride titration sweat tests.

The chloride electrode sweat test and gravimetric chloride titration technic are compared. The two technics gave comparable mean values, ogives, equivalent points, and overlap for patients with cystic fibrosis and healthy subjects. As a single test with either technic has an unacceptable risk for false-positive or false-negative results, tests should always be done in pairs. A statistical interpretation technic based on prevalence and technical variability of the two tests will decrease the number of undiagnosed subjects and will define probability of cystic fibrosis in the undiagnosed group.

Reliability of histo-pathological diagnosis of squamous epithelial changes of the uterine cervix.

The reliability of histological diagnosis of squamous epithelial changes was tested by letting 13 pathologists read 1,001 consecutive cervical biopsies twice. Intra-observer and inter-observer agreement, variance, and deviation of diagnosis were determined. The diagnostic ability showed great individual variation and no significant correlation to experience in pathology. The diagnosis of invasive cancer had a high diagnostic specificity, and the diagnostic sensitivity of the diagnosis of no significant epithelial changes was high too. The reliability of the diagnosis of dysplasia and carcinoma in situ proved unsatisfactory.
