Right ventricular infarction: relationships between ST segment elevation in V4R and hemodynamic, scintigraphic, and echocardiographic findings in patients with acute inferior myocardial infarction.

The relationship between ST segment elevation on the right precordial lead V4R and the hemodynamic, echocardiographic, and myocardial scintigrapic signs suggestive of right ventricular (RV) infarction was studied in 42 patients with acute inferior myocardial infarction. Twenty-two patients had ST segment elevation in V4R. Among these patients, a significant correlation was demonstrated between V4R ST segment elevation and the hemodynamic (p less than 0.001), scintigraphic (p less than 0.001), and echographic (p less than 0.02) criteria for acute RV infarction. These results support the validity of this new electrocardiographic sign as a practical means in aiding the clinical detection of RV involvement with acute transmural inferior myocardial infarction.

Reciprocating tachycardia using bilateral anomalous pathways: electrophysiologic and clinical implications.

A patient who had disabling supraventricular tachycardia showed electrophysiologic evidence of coexistent right and left Kent bundles. The supraventricular tachycardia was accelerated by sequential antegrade and retrograde conduction over the right and left Kent bundles, respectively. Spontaneous blocking of the conduction through the right Kent bundle was associated with a slowing of the tachycardia and with the appearance of right bundle branch block (RBBB). Antegrade conduction was through the left bundle branch and was followed by an early retrograde atrial activation through the left Kent bundle, with consequent shortening of the ventriculoatrial (VA) interval. Thus, the coexistence of bilateral Kent bundles can be suspected whenever shortening of the VA interval in external electrocardiographic recordings occurs in the presence of a tachycardia with a RBBB pattern in a patient with right-sided preexcitation. Our data also showed that bilateral accessory pathways may have different electrophysiologic properties and that unequal response to antiarrhythmic drugs may be expected.

[Ultrastructure of the liver in patients with cholesterol stones receiving chenodesoxycholic acid (author's transl)].

The ultrastructure of the liver was investigated in 5 normal persons and 10 patients with cholesterol stones. In the patients numerous intramitochondrial crystals could be found; precipitated cholesterol-digitonin complexes were found as well in the canaliculi, the sinus and the pericanalicular areas. 5 patients received chenodesoxycholic acid (10 mg/kg/day); liver biopsies taken 3 and 6 months after start of treatment did not show any more the changes described above.

Action of neomycin on the intraluminal phase of lipid absorption.

Administration of a single 1 g dose of neomycin sulfate to five healthy subjects simultaneously with a test meal caused a marked increase in the proportion of fatty acid and bile acid in the ultracentrifuged deposit of aspirated intestinal contents. Labeled cholesterol was precipitated in a similar manner in two hypercholesterolemic patients. Neomycin had no effect on the pancreatic lipase concentration or on the pH of intestinal contents. These results confirm that the ability of neomycin to precipitate micellar lipids is due to interaction between the polybasic neomycin molecule and ionized fatty acids and bile acids. This mechanism provides an explanation for both the steatorrhea and hypocholesterolemia induced by this compound.

Corticotropin-releasing factor (CRF) and neurotransmitters modulate melanotropic peptide release from rat neurointermediate pituitary in vitro.

Isolated pituitary neurointermediate lobes from adult male Sprague-Dawley rats were incubated for up to 90 minutes in media containing stimulatory or inhibitory agents, to study the potential effects on secretion of alpha-melanocyte stimulating hormone (alpha-MSH) from the pars intermedia. Dopamine (DA), as expected, inhibited the release of alpha-MSH, as measured by radioimmunoassay (RIA), while corticotropin-releasing factor (CRF) and serotonin (5-HT) stimulated peptide release to varying degrees over the incubation period. A combination of DA with CRF produced an inhibition of peptide release, suggesting that DA suppresses the stimulatory effects of the hypothalamic peptide. Immune staining of pituitary tissue for beta endorphin or alpha-MSH, and quantified for staining intensity, supported the specific effects of stimulation or inhibition of the respective treatments. In addition, electron microscopy of incubated tissues demonstrated alterations in secretory vesicles and membranous organelles which were consistent with peptide secretion patterns. Our in vitro findings suggest that neurotransmitters and hypothalamic peptide, which are present together in vivo in the pituitary intermediate lobe, interact at the level of the endocrine cells to modulate pro-opiomelanocortin peptide secretion.

Countercomplementarity and strong ferromagnetic coupling in a linear mixed mu-acetato, mu-hydroxo trinuclear copper(II) complex. Synthesis, structure, magnetic properties, EPR, and theoretical studies.

The structural and magnetic data of the trinuclear compound [Cu3(L)2(CH3COO)2(OH)2(dmf)2] (HL = N-(2-methylpyridyl)toluensulfonylamide) are reported. The compound crystallizes in the monoclinic system, space group P2(1)/n (no. 14), with a = 11.6482(6) A, b = 13.5772(6) A, c = 13.5306(7) A, alpha = 90 degrees, beta = 92.859(5) degrees, gamma = 90 degrees, and Z = 2. The three copper atoms form an exact linear arrangement. Neighboring coppers are connected by a hydroxo bridge and a bidentate syn-syn carboxylato group. The coordination spheres of the terminal copper atoms are square pyramidal with a dmf molecule as the apical ligand. The central copper has a regular square planar geometry. The mixed bridging by a hydroxide oxygen atom and a bidentate carboxylato group leads to a noncoplanarity of the adjacent basal coordination planes with a dihedral angle of 51.96(9) degrees. Susceptibility measurements (2-300 K) reveal a strong ferromagnetic coupling, J = 93(6) cm-1, in the mixed-bridged moiety leading to a quartet ground state that is confirmed by the EPR spectra. The ferromagnetic exchange coupling is discussed using DFT calculations on model compounds that have shown a countercomplementary effect of the hydroxo and acetate bridges.
