[Fractures of the calcaneus. The indications for surgical treatment].

The authors report the results obtained from a study of 41 cases of calcaneal fractures. Surgical treatment, when possible, is to be applied because it allows a satisfactory reconstruction of the subtalar and calcaneocuboid joint. The choice of various surgical methods is facilitated by computerised tomography which allows a precise assessment of the calcaneum. The quality and the stability of the reduction determine in a significant way the functional result.

Operatory indications and licence of craniostenosis.

On average 0.04% of the population is affected by craniostenosis. It is believed there are anatomo-clinical forms at moderate risk and others at high risk. The neurosurgical indications are very different according to various Authors. The forms at high risk which are caused by an isolated synostosis of the coronal system with brachicephalia are either associated to other synostosis (oxicephalia) or to facial synostosis should always be operated, with various techniques, within the first 6 months of life. Basing on the experience of 3 cases of craniostenosis, deliberately not operated on, followed and controlled for over 20 years from the decision of not to operate, all in excellent general condition both neurological and psychiatric, the Authors consider and discuss the criteria of surgical indications with particular regard to clinical objectivity (implicity disregarding a precise value of the craniometric data).

Angiopathic parkinsonism: does it exist?

The Authors report on 150 cases of patients suffering from Parkinson's disease. The patients represent 8.3% of total parkinsonian patients and are selected on the basis of clinical and laboratory criteria that allow the diagnosis of "angiopathic parkinsonism" as an autonomous entity. Cerebral blood flow alterations in Parkinson's disease and associated dementing symptoms are also discussed. In conclusion, the Authors emphasize the usefulness of recognizing the "angiopathic parkinsonism" in view of therapeutical implications.

[The distal pedicle fasciocutaneous flaps of the leg. Analysis of 21 cases of lower limb reconstruction].

The fascio-cutaneous flaps with distal pedicle, with the new discoveries on fascial and septal circulation of the lower limbs, are a valid reconstructing method for the loss of substance localized on the third inferior of the leg and in the heel's region. In 21 patients, 18 men and 3 women, fascio-cutaneous flaps were employed for the traumatic loss of substance, just in two cases consequentially to removal of maligns lesions, all of them were omolaterals except in three cases in which cross-leg were used. The results are satisfying if we remember that just in two cases we had partial necrosis and in other two cases we had to reinstate the flap which had survived just in his deep position. The advantages of using this technique are supported by his good results and simple execution, realising anyhow that complicated or contaminated trumas need more complex plastic reconstruction.

Hydroxyapatite-coated cylindrical implants in the posterior mandible: 10-year observations.

A total of 443 hydroxyapatite-coated cylindrical implants were placed into the posterior mandibles of consenting patients from July 1985 through December 1991. Implants were placed into ridges that had sufficient bone volume. Implants were also placed in ridges with thin bone, grafted bone, or immediate extraction sites, which occasionally resulted in increased morbidity. All were followed yearly with clinical and radiographic examinations. Life table analyses were performed using SAS software. Ten-year cumulative survival and nonmorbid (based on bone loss criteria) rates were determined for the four posterior tooth locations and for all posterior locations combined. Cumulative survival for implants was 0.858 +/- 0.056 in the first premolar region, 0.833 +/- 0.041 for the second premolar region, 0.785 +/- 0.041 for the first molar region, and 0.718 +/- 0.064 for the second molar region. When all posterior mandibular locations were taken together, the cumulative survival was 0.793 +/- 0.025. Cumulative nonmorbid rates were 0.816 +/- 0.056 for the first premolar, 0.729 +/- 0.049 for the second premolar, 0.600 +/- 0.046 for the first molar, and 0.507 +/- 0.066 for the second molar. The cumulative nonmorbid rate was 0.646 +/- 0.028 for all sites combined. Comparisons between sites indicated that the second molar site had statistically more morbidity than did the other three sites. Most of the problems were identified with 8-mm-long implants, especially in the second molar location. Clinical parameters, including the presence of keratinized gingiva and a hygienic restoration, were associated with survival. Both mechanical and inflammatory compromise were found to be reasons for implant failure.

[TSH anti-receptor antibodies in Graves' disease].

The purpose of this study was to evaluate the sensitivity, specificity and predictive value of thyrotropin receptor antibody (TRAb) in the diagnosis of Graves disease. TRAb was tested by an isotopic receptor assay-TRAK Henning-in 80 newly diagnosed, untreated Graves disease patients (group I), 63 with other thyroid diseases (group II) and 60 controls (group III). In group I, 11 patients were TRAb negative and 7 were considered in the gray area (TRAb between 9 and 14 U/L). In group II, only 2 patients had TRAb 9 U/l and all controls were TRAb negative. For statistical analysis patients with TRAb in gray area were excluded. Sensitivity and specificity for this assay were 84.5 and 100% respectively. Predictive value of 100% affords certainty that a hyperthyroid patient with a positive TRAb has Graves disease, not sequining a scintigram.
