A prospective study of myocardial damage in electrical injuries.

A prospective study was undertaken to determine the incidence of possible myocardial damage following electrical injury. Sixteen patients with non-flash electrical injuries were assessed utilizing serial electrocardiograms (ECG), creatine kinase (CK) and MB creatinine kinase (MB-CK) determinations, technetium 99m stannous pyrophosphate scans, and 24-hour Holter monitors. Results showed that five patients (31%) had abnormal ECG, nine patients (56%) had elevations of the MB-CK isoenzyme, and one patient had a transiently abnormal Holter monitoring. No patient had an abnormal technetium pyrophosphate cardiac scan. Of the nine patients with elevated MB-CK levels, only two had abnormal ECG. None of the patients had clinical evidence of cardiac dysfunction. These results indicate a poor correlation of elevated MB-CK levels with ECG abnormalities, and demonstrate a relatively low incidence of myocardial damage in association with electrical injuries.

Prolonged elimination of piperacillin in a patient with renal and liver failure.

A 22-year-old female with acute leukemia was admitted to the oncology center for evaluation and treatment. Piperacillin was used during her admission for treatment of Pseudomonal sepsis. The patient's renal and liver function deteriorated during the hospital course, requiring large dose adjustments of piperacillin. The elimination half-life of piperacillin was estimated to be 32 hours. Patients with both liver and renal disease require only small doses of piperacillin to achieve therapeutic plasma concentration.

Temporal lobe meningioma masked by polymyalgia rheumatica.

An elderly woman who had a typical polymyalgia rheumatica (PMR) syndrome, associated with severe headache, was considered to have giant cell arteritis despite negative temporal artery biopsies and was treated with corticosteroids. At autopsy, no arteritis, but a large temporal lobe meningioma, was found as the cause of her headache. Both PMR and meningiomas are frequent in elderly patients, and overlapping symptoms may occur. Patients with PMR and headache should be fully investigated to exclude an occult meningioma or other disease, especially when no positive histologic evidence of temporal arteritis can be obtained.

A major human histone gene cluster on the long arm of chromosome 1.

A human histone gene cluster was assigned to chromosome 1 by Southern blot analysis of DNA's from a series of mouse-human somatic cell hybrids with 32P-labeled cloned human H4 and H3 histone DNA as probes. Localization of this histone gene cluster on the long arm of chromosome 1 was confirmed by in situ hybridization of this DNA probe to metaphase chromosomes.

Histone gene expression in human diploid fibroblasts: analysis of histone mRNA levels using cloned human histone genes.

The cellular abundance of H2A, H2B, H3 and H4 histone mRNA sequences was determined prior to and at various times after stimulation of non-dividing human diploid fibroblasts to proliferate. The representation of histone mRNAs was quantitated by electrophoretic fractionation of total cellular RNAs, diffusion transfer to nitrocellulose and hybridization with a series of cloned genomic human histone sequences. The levels of mRNAs for the four core histones were observed to be temporally and quantitatively coupled with both DNA replication and histone protein synthesis. Therefore, a contribution to the regulation of histone gene expression at a transcriptional level is suggested.

On the directionality of classically-conditioned glycemic responses.

Rats were administered either exogenous insulin or glucose as the unconditioned stimulus paired with a distinctive stimulus complex. Following several such pairings, the administration of a saline (test) solution to the animal in that environment produced, in both cases, a reduction in blood-glucose levels (hypoglycemia) as compared to control animals who received saline injections throughout. We suggest that, contrary to previous researchers' assumptions, direct changes in blood glucose levels immediately following the administration of either glucose or insulin should not be treated as the unconditioned response. The response of the CNS to the glucose or insulin, and not the immediate effect of increases or decreases in blood glucose levels, must be considered. Our reanalysis suggests, then, that in the case of glucose conditioning the conditioned response produces hypoglycemia which is in the same direction as that of the unconditioned response. In the case of insulin conditioning, the unconditioned response has not been well identified and consequently the conditioned response may be in the same or opposite direction to that of the UR.

Distribution of DNA insertion element IS5 in natural isolates of Escherichia coli.

DNA from Escherichia coli strains in a reference collection of 72 recent natural isolates (ECOR strains) and 25 natural isolates from the "pre-antibiotic" period 1930-1940 (Murray strains) were studied to determine the genomic abundance of insertion element IS5 and the size of genomic restriction fragments carrying sequences homologous to IS5. Among the ECOR strains, nearly two-thirds lack DNA sequences that hybridize with IS5, and one-half of the remainder have only one copy. Among strains in which IS5 is present, extensive variation in the size of IS5-bearing restriction fragments occurs, in many cases allowing distinction among strains that are judged to be nearly identical in genotype because of the identical electrophoretic mobility of the enzyme coded by each of 11 chromosomal loci. Among the Murray strains in which IS5 is present, the average number of elements per strain is larger, but not markedly so, than among recent isolates. Comparison between duplicate strains in the Murray collection suggests that the rate of accumulation of IS5 elements in prolonged storage in stab tubes corresponds to an apparent probability of transposition of approximately 0.008 +/- 0.002 per IS5 element per year. Because of the extensive genetic variation among strains, insertion elements such as IS5 would seem to be convenient genetic markers with which to detect recent common ancestry among strains.

Cell cycle regulation of human histone H1 mRNA.

A cloned genomic DNA fragment containing a human histone H1 gene has been used to analyze histone H1 gene expression in two human cell lines (HeLa S3 and WI-38). The cellular abundance of histone H1 mRNA was compared with that of core (H2A, H2B, H3, and H4) histone mRNAs as a function of the cell cycle: core and H1 histone mRNA levels are related both to each other and to the apparent rate of DNA synthesis and are rapidly destabilized after DNA synthesis inhibition. The use of three synchronization protocols, and of transformed and normal diploid cells in culture, suggests that the detected core and H1 histone mRNA levels are regulated by similar mechanisms in continuously dividing human cell lines and nondividing cells stimulated to proliferate.

Specific deletion occurring in the directed evolution of 6-phosphogluconate dehydrogenase in Escherichia coli.

A novel genetic change leading to increased activity of 6-phosphogluconate dehydrogenase (6PGD) in E. coli has been observed. The mutation is a deletion of approximately 0.4 kilobase pairs occurring between the structural gene of 6PGD (gnd) and one copy of an insertion element (IS5) found normally in E. coli K12 a few hundred base pairs upstream (counterclockwise) from gnd at 44 minutes on the conventional genetic map. The deletion is associated with a threefold higher activity of 6PGD and a 57% increase in the maximum growth rate when cells are grown in gluconate.

Hepatic toxicity of low doses of mithramycin in hypercalcemia.

The medical records of patients receiving mithramycin for the treatment of hypercalcemia at The Johns Hopkins Oncology Center were reviewed. Of 67 evaluable patients, 11 (16%) developed hepatic toxicity when graded using standard criteria. The pattern of laboratory abnormalities noted suggested mild hepatocellular damage that was reversible within 2 weeks. Review of these patients did not identify factors which predispose to hepatic toxicity, although failure to do so may reflect the limited number of toxic patients evaluated. Mild, reversible hepatic dysfunction appears to be a more common sequela of mithramycin administration than has previously been recognized.

Progressive cutaneous herpes simplex infection in acute myeloblastic leukemia. Successful treatment with interferon and cytarabine.

A patient had acute myeloblastic leukemia and extensive progressive cutaneous herpes simplex virus infection. Complete and rapid healing of skin lesions with remission of the leukemia occurred during 15 days of therapy with human leukocyte interferon and minimal doses of cytarabine hydrochloride. Pharmacokinetic studies showed that the patient had a defective antiviral interferon response, which was effectively corrected by treatment with interferon alpha. This may help to explain the dramatic response of both conditions to therapy.

Antiracism and the level of health services: a sociomedical hypothesis.

Little attention has been paid to the validity of the "reverse discrimination" position that antiracist initiatives in the health sector would be associated with reduced services for whites. This paper advances the sociomedical hypothesis that antiracism leads to an increase in the level of health services and opportunities available to both minority and white populations.FOUR TYPES OF US HEALTH CARE UTILIZATION AND TRAINING DATA ARE EXPLORED: annual rates of discharge from short-stay hospitals, percent of population seeing a physician during the year, rates of hypertension treatment and control, and admissions to the first-year class of US medical schools. These data are examined according to race for years prior and subsequent to the upsurge of antiracist activity that characterized the Civil Rights Movement era.From the early or mid-1960s to the mid- or late 1970s, hospital discharges, physician visits, and hypertension treatment and control for minorities and whites increased substantially. Generally these increases were proportionally greater for minorities. Although the percentage of increase in minority medical school admissions was necessarily accompanied by a decline in percentage of admissions of whites, the absolute number of whites admitted rose substantially as overall class size grew.These data do not support the "reverse discrimination" notion of one racial group benefiting at the expense of another. The data are consistent with the hypothesis that antiracist efforts in the health sector lead to an expansion of services and opportunities for minority and majority populations.

The (Ca + Mg)-ATPase of reticulocytes.

Rabbits were made reticulocytotic by repeated bleeding or by injection of phenylhydrazine. Up to reticulocyte levels of 70%, the baseline activity of magnesium-dependent calcium-stimulated adenosine triphosphatase ( ( Ca + Mg)-ATPase, EC 3.6.1.3.) of the red cell plasma membrane was not significantly changed. Maximal activity in the presence of calmodulin was significantly reduced, the stimulation by the activator falling from about 350% to about 50%.

Continuous five-day infusion of PALA and 5FU: a pilot phase II trial.

In order to determine whether a simultaneous infusion of N-(phospho-n-acetyl)-L-aspartic acid (PALA) was able to increase the rate of antitumor response to 5-fluorouracil (5-FU), a pilot study was conducted. Of 10 evaluable patients with previously drug-untreated colorectal carcinoma, there were only two partial responses, lasting 2.3 and 1.6 months. No partial responses were observed in three evaluable patients with soft tissue sarcomas. The dose-limiting toxicity was dermatitis. The simultaneous infusion of PALA and 5-FU is not likely to produce a high number of antitumor responses of long duration in patients with colorectal carcinoma.
