Lymphocyte immunophenotyping by flow cytometry in normal adults. Comparison of fresh whole blood lysis technique, Ficoll-Paque separation and cryopreservation.

In the present report we have assessed the extent to which Ficoll-Paque separation and cryopreservation of mononuclear cells alter the measurement of lymphocyte subsets by flow cytometry. Standard Ficoll-Paque separation increased the percentage of CD4+, CD19+ and CD4+CD45RA+ cells, as well as decreasing that of CD8+, and CD4+CD29+ cells, compared to the fresh whole blood lysis technique. Moreover, cryopreservation caused a depletion of CD4+ p80+ cells, but normal whole blood values were restored following a short incubation.

[Normative parameter characterization for the auditory brainstem response of the normal newborn].

The auditory brainstem response generated by clicks was studied in 119 normal newborns. One ear was studied in detail at 20, 40 and 60 dBnHL, the other at 20 and 30 dBnHL, increasing the intensity if no response was found. The latency for waves I, III and V in the detailed studied ear was 2.7 +/- .39, 5.4 +/- .44 and 7.7 +/- .48, respectively. 85% of newborns had thresholds for auditory brainstem response less than 40 dBnHL, 54% less than 20 dBnHL and 15% between 40 and 60 dBnHL. These findings are compatible with conductive loss in the higher threshold group and support data from Stockard, regarding high risk groups. Additionally, our data suggests that an adequate level to screen for hearing loss in newborns is between 30 and 40 dBnHL.

Estimation of doses to patients from "complex" conventional X-ray examinations.

A numerical method has been developed to estimate the organ doses and effective dose-equivalent for patients undergoing three "complex" examinations (barium meal, barium enema and intravenous urography). The separation of radiological procedures into a set of standard numerical views is based on the use of Monte Carlo conversion factors and measurements within a Remab phantom. The radiation doses measured in a phantom for such examinations have been compared with the predictions of the "numerical" method. Dosimetric measurements with thermoluminescent dosemeters attached to the patient's skin along with measurements of the dose-area product during the examination have enabled us to derive organ doses and to estimate the effective dose-equivalent. Mean frequency weighted values of dose-area product, energy imparted to the patient, doses to a set of organs and effective dose-equivalent in the area of Madrid are reported. Comparisons of the results with those from similar surveys in other countries have been made.

Measurement of radiation doses in the most frequent simple examinations in paediatric radiology and its dependence on patient age.

Radiation doses to patients were measured in four X-ray rooms specifically devoted to paediatric radiology, from two hospitals. The study was performed for the most frequent simple examinations, namely abdomen, hip and pelvis, skull, spine and chest. Patients were classed into four different age groups: 0.1-1 year, greater than 1-5 years, greater than 5-10 years and greater than 10-14 years. Operating X-ray generator parameters and entrance surface doses were recorded for all groups. Representative values were obtained for standard working conditions prior to any correcting action being taken. Dose values are reported, and some of the differences between the results found in the rooms for each examination are discussed. Without attempting to relate adult and paediatric radiology, the entrance surface doses measured and the provisionally recommended CEC values for similar examinations in adult patients are compared.

Effect of indoor residual spraying of DDT and bendiocarb on the feeding patterns of Anopheles pseudopunctipennis in Mexico.

Intense and persistent use of DDT for malaria control has increased resistance and induced exophilic behavior of Anopheles pseudopunctipennis. An evaluation of bendiocarb and DDT to control this species in Sinaloa, Mexico, showed that, in spite of DDT-resistance, both insecticides produced similar effects. Feeding patterns were analyzed to explain these results. Resting mosquitoes were collected over the dry and wet seasons. Anophelines were tested in an ELISA to determine the source of the meals. The human blood index (HBI) ranged from 3.3 to 6.8% in DDT- and from 12.7 to 26.9% in bendiocarb-sprayed houses. Irritability and repellency in DDT-sprayed houses could explain the reduced HBI. In contrast, bendiocarb produced higher mortality. These effects could have affected different components of the vectorial capacity and similarly reduced malaria.

Effect of penicillin and spectinomycin given for urethritis and cervicitis with Neisseria gonorrhoeae: high prevalence of penicillin-resistant isolates.

Efficacy of single-dose spectinomycin (TRO: 2 g intramuscularly) was compared with that of aqueous procaine penicillin G (APPG:4.8 x 10(6) units) plus 1 g of probenecid for treatment of gonococcal urethritis and cervicitis. Cure rates of the 210 patients who received TRO and 190 patients who received APPG were 97.6% and 91.1%, respectively. MICs of antibiotics were determined using the agar dilution method. Those isolates with MICs of APPG of less than 1.0 microgram/ml had low failure rates (2.9%), while strains with increased resistance to APPG (MICs greater than or equal to 1.0 microgram/ml) had higher failure rates (24%). Treatment failures seen with TRO were not correlated to isolates with the higher MICs. Clinical results suggest TRO could be given for treatment of genital gonorrhoea in Puerto Rico due to the high prevalence of both chromosomally-mediated penicillin-resistant Neisseria gonorrhoeae (20%) and penicillinase-producing Neisseria gonorrhoeae (7.5%) strains and the high rate of failure seen with the use of APPG.

Some indicative parameters on diagnostic radiology in Spain: first dose estimations.

The Medical Physics Group at the Complutense University of Madrid has been co-ordinating, for approximately 1 year, a project on optimization of radiation protection in diagnostic radiology, in co-operation with the other states of the European Community. Exhaustive data on the subject, which offer accurate results on patient dosimetry for the different types of examination, are the final aim of the project. So far, it has been possible to analyse in detail the data from the National Institute of Health (NIH), which manages the care of about 96% of the Spanish population, plus the findings from several hospitals, outpatient centres and private clinics of the community of Madrid, which allowed us to perform the first dose estimations and to extrapolate them to the rest of Spain. The following estimations are presented: annual frequency of different examinations, their variation from 1985 to 1986, number of diagnostic rooms used for a given minimum of annual examinations, organ doses for different examinations, effective dose-equivalent, genetically significant dose for some examinations, as well as the collective dose.

[Biliary lithiasis].

We have reviewed our experience with 2,430 cases of lithiasis of the biliary tract, 500 of them located in the common bile duct and 104 corresponding to residual lithiasis. We have made a general reflexion on the problems aroused by this very common al condition, which has an imprevisible, some times severe, outcome. The recent acquisition of new conservative methods, as disolvents and lithotrix, and the diagnostic and therapeutic role of endoscopy have revolutionated the state of the art and have introduced controversial points still unsolved. As it is common in clinical practice, biliary lithiasis should be approached in a multidisciplinary fashion, employing the most convenient diagnostic procedure for the clinical situation of the patient; a priori no accredited procedure should be disregarded.

The maternal-infant center in the control of hepatitis B.

A seroepidemiologic survey of HBV infection was carried out in four population groups served by the Maternity Hospital of Majorca, an island in the Mediterranean. The prevalence of HBV markers (HBsAg, anti-HBs, anti-HBc), determined by enzyme-immunoassay, reached 17.2% among 864 parturients, 17.2% in 783 hospital personnel, 2.7% and 19.2% among the 186 children and 73 husbands of parturients with no HBV markers, 22.8% and 43.8%, respectively, among the 44 children and 48 husbands of HBsAg chronic carrier parturients. The prevalences of HBsAg in the same groups reached 0.9%, 1.5%, 0% and 0%, 11.4% and 2.1%, respectively. The carrier state represents 50% of all infection cases among children and 4.8% among husbands of carrier women. The prevalence of markers, which was especially high among personnel working in maintenance services (39.1%), on surgical wards (35.3%), among midwives (27.6%) and in cleaning and laundry services (27.1%), and laboratories (21.4%), is used as an indicator in defining priorities for the vaccination of people at risk. Sixty-seven newborn babies of HBsAg carrier mothers were administered one dose of HBIG and three doses of hepatitis B vaccine; at 12 months of age, the seroconversion rate was 98.5%, whereas 1 child (1.5%) became a chronic carrier. Maternity hospitals represent the ideal centers in which to carry out the detection of HBsAg in pregnant women prior to parturition, to ensure that all newborns of chronic carrier women receive passive-active immunization, and to screen their family contacts and vaccinate those who are found susceptible to the infection.

The effect of wheat bran on the excretion of cadmium in rats.

The influence of wheat bran as a source of dietary fibre on the excretion of ingested cadmium from diet was studied. The cadmium contents in the diets, faeces, urine, liver, kidney and spleen were determined, as well as the weight gains, the relative weights of the cited organs, and the levels of alkaline phosphatase in serum. No evidence is provided that an increasing portion of wheat bran in the diets induced an increase in cadmium excretion. Changes in the studied toxicity parameters were not found.

Cellular immunity and IgE levels in atopic patients.

Twenty young adult atopic patients and their matched controls were studied. Spontaneously generated and Con-A-induced suppressor T cell functions as well as Natural Killer (NK) activity against K-562 target cells, measured in a short-time 3H-thymidine uptake, were evaluated. Suppressor T cell activity in the patients was more than 2 SD lower than that found in the controls and there was, contrary to expectation, a direct correlation between suppressor function and serum IgE levels. Atopic patients showed a statistically significant lower NK activity than normal controls when related to a low IL-2 production. Both facts inversely correlated with the concentration of IgE in serum. We concluded that atopic patients' vulnerability to viral infections may be due to defective NK activity. Suppressor T cell function is abnormal in these patients. Both defects could be due to a faulty immunoregulatory helper function.
