Molecular cloning of cDNA encoding the alpha unit of CNGC gene from human fetal heart.

Cyclic nucleotide-gated ion channels (CNGCs) play crucial roles in visual and olfactory signal transduction. As a first step to explore the presence of a CNGC gene in human heart, we cloned a human heart CNGC gene. The sequence consists of 111 bp 5' non-coding region and a 2064 bp open reading frame which is followed by a 459 bp 3' non-coding region. The predicted protein consists of 688 amino acids with a short highly charged segment rich in lysine and glutamate. Sequence comparison indicates that the human heart cDNA is almost identical to the retinal rod photo receptor CNGC cDNA. However, the human cardiac cDNA is lacking a 205 bp Alu fragment in the 5'-uncoding region, has a glutamic acid residue at amino acid position 129, and has a replacement of glutamic acid with a lysine residue at amino acid position 99. Data obtained with northern blot analysis confirm the presence of RNA for the CNGC alpha chain. This channel might play a role in cyclic nucleotide-mediated cellular processes, such as the inotropic effect in the heart.

Ethnic differences in red blood cell sodium/lithium countertransport and metabolic correlates of hypertension: an international collaborative study.

Arterial hypertension is frequently associated with metabolic abnormalities. An abnormal activity of the erythrocyte sodium/lithium countertransport (Na/Li CT), an ion transport system under strong genetic control, is also found in people with hypertension and concomitant metabolic abnormalities. However, little information exists with regard to these clinical associations in different racial groups. The aim of this international collaborative study was to investigate Na/Li CT and the metabolic correlates of hypertension in two comparable samples of normotensive and hypertensive populations in the cities of Naples, Italy, and Shanghai, China, using identical, carefully standardized techniques. Blood pressure, anthropometric and metabolic variables, Na/Li CT, and 24-h urinary Na and K excretion were measured in untreated essential hypertensive (HPT) and normotensive (NT) individuals selected by age (35-60 years), body mass index (BMI; < 30 kg/m2), and blood pressure (BP; HPT, DBP > or = 95 mm Hg; NT, DBP < 90 mm Hg). The analysis of variance with adjustment for age was used to compare the groups. In the Neapolitan population, hypertensive individuals had higher serum triglyceride (P < .05) and uric acid levels (P < .001) than the normotensive group and also had a reduced glucose tolerance (P < .01) and an enhanced insulin response to the oral glucose tolerance test (OGTT) (P < .05). No such differences were seen between normotensive and hypertensive Chinese participants. The Neapolitan population (both NT and HPT) had a higher BMI (P < .01) than their Chinese peers. In the comparison of hypertensive patients in Shanghai and in Naples, the Neapolitans were heavier (P < .001), had a lower HDL/total cholesterol ratio (P < .01), an elevated fasting blood glucose (P < .05), and also a higher glucose (P < .001) and insulin response (P < .001) to OGTT. By contrast, they showed a significantly lower urinary Na/K ratio (P < .001). Na/Li CT was significantly increased in HPT both in Naples (286 +/- 24 v 224 +/- 13 micromol/L RBC x h; P < .05, M +/- SE) and in Shanghai (388 +/- 45 v 265 +/- 30 micromol/L RBC x h; P < .05). Furthermore, Na/Li CT was significantly and inversely associated with HDL cholesterol both in the Neapolitan (P < .01) and in the Chinese (P < .05) population, whereas it was directly correlated with serum triglyceride (P < .001) and serum uric acid (P = .001) only in the Neapolitan population. These results indicate that essential hypertension is associated with a higher prevalence of obesity, impaired glucose tolerance, and hyperinsulinemia in Naples than in Shanghai; and Na/Li CT is linked to both high blood pressure and metabolic abnormalities in the Italian sample, whereas it is an isolated marker of hypertension in the Chinese sample.

Comparison of active treatment and placebo in older Chinese patients with isolated systolic hypertension. Systolic Hypertension in China (Syst-China) Collaborative Group.

BACKGROUND: Isolated systolic hypertension occurs in around 8% of Chinese people aged 60 years or older. In 1988, the Systolic Hypertension in China (Syst-China) Collaborative Group started to investigate whether active treatment could reduce the incidence of stroke and other cardiovascular complications in older patients with isolated systolic hypertension. METHODS: All patients were initially started on masked placebo. After stratification for centre, sex and previous cardiovascular complications, alternate patients (n=1253) were assigned nitrendipine at 10-40 mg daily, with the addition of captopril at 12.5-50.0 mg daily or hydrochlorothiazide at 12.5-50.0 mg daily, or both, if a sufficient blood pressure fall was not obtained. In the remaining 1141 control patients, matching placebos were administered similarly. RESULTS: At entry, sitting blood pressure averaged 170.5 mmHg systolic and 86.0 mmHg diastolic, age averaged 66.5 years and total serum cholesterol was 5.1 mmol/l. After 2 years of follow-up, sitting systolic and diastolic blood pressures had fallen by 10.9 mmHg and 1.9 mmHg in the placebo group and by 20.0 mmHg and 5.0 mmHg in the active treatment group. The intergroup differences were 9.1 mmHg systolic (95% confidence interval 7.6-10.7 mmHg ) and 3.2 mmHg diastolic (95% confidence interval 2.4-4.0). Active treatment reduced total strokes by 38% (from 20.8 to 13.0 endpoints per 1000 patient-years, P=0.01), all-cause mortality by 39% (from 28.4 to 17.4 endpoints per 1000 patient-years, P=0.003), cardiovascular mortality by 39% (from 15.2 to 9.4 endpoints per 1000 patient-years, P=0.03), stroke mortality by 58% (from 6.9 to 2.9 endpoints per 1000 patient-years, P=0.02), and ail fatal and nonfatal cardiovascular endpoints by 37% (from 33.3 to 21.4 endpoints per 1000 patient-years, P=0.004). CONCLUSIONS: Antihypertensive treatment prevents stroke and other cardiovascular complications in older Chinese patients with isolated systolic hypertension. Treatment of 1000 Chinese patients for 5 years could prevent 55 deaths, 39 strokes or 59 major cardiovascular endpoints.

Cardiac protection by long-term treatment with captopril in patients with acute myocardial infarction.

OBJECTIVES: To assess the effects of long-term angiotensin-converting enzyme (ACE) inhibitor treatment with captopril on cardiac function in acute myocardial infarction (AMI). METHODS: One hundred and one patients with AMI who were admitted to hospital within 72 hours of the onset of symptoms with no cardiogenic shock were randomly allocated to captopril (n = 52; group I) and conventional treatment (n = 49; group II). Left ventricular (LV) systolic performance and diastolic transmitral flow velocity profiles were assessed by Doppler echocardiography at admission (1.2 +/- 1.1 days), before discharge (27 +/- 10 days) and during follow-up (363 +/- 31 days). RESULTS: At one year follow-up, in group I LV end-diastolic volume decreased, and ejection fraction increased due to a disproportionate decrease in end-systolic volume. The incidence of cardiac dilatation was reduced. LV early diastolic filling velocity (E) increased and late atrial filling velocity (A) decreased, resulting in an elevation of E/A ratio. However, the mean values of LV systolic and diastolic functional parameters were unchanged in group II. CONCLUSIONS: Long-term treatment with captopril exerts a beneficial effect on cardiac protection for patients with AMI.

[The use of pediculated super-long randomized flap in repairing soft tissue defects of the head and neck].

A double-pediculated randomized flap of the shoulder and thorax was designed. The flap was transferred to cover the wound on the recipient area. The wound on the donor site was closed directly or covered by split skin graft. From January 1990 to January 1997, this method was used in 10 patients. After operation, a six month to three year following up indicated that the flaps were completely survived and the soft tissue defects of the head and neck were repaired. As a consequence, the pediculated super-long randomized flap was effective in repairing soft tissue defect.

Relative abundance of organochlorine pesticides and polychlorinated biphenyls in adipose tissue and serum of women in Long Island, New York.

Some organochlorine pesticides (OCPs) and PCBs are under investigation as possible risk factors for breast cancer because of their estrogenic properties and widespread presence in the environment. It is important to know whether adipose tissue used by some investigators and serum assays used by others can provide comparable information on body burden. Concentrations of seven OCPs or their breakdown products as well as 14 PCB congeners were measured in the adipose tissue and serum of 293 women enrolled as controls in a case-control study of environmental factors for breast cancer in Long Island, New York, a high-risk region. Adipose OCP/PCB levels were measured using a supercritical fluid extraction method developed by the authors. 1,1-Dichloro-2,2-di(4-chlorophenyl)ethylene (p,p'-DDE) was detected in all adipose and serum samples; two chlordane derivatives, beta-hexachlorocyclohexane (a lindane isomer) and hexachlorobenzene, were detected in at least 92% of adipose samples. The di-ortho hexachlorinated PCB congeners 2,4,5,2',4',5'-hexachlorobiphenyl and 2,3,4,2',4',5'-hexachlorobiphenyl were detected in all adipose and over 98% of serum samples. 1,1-Dichloro-2,2-di(4-chlorophenyl)ethylene comprised 77% of total pesticide residues in adipose and 71% in serum. 2,4,5,2',4',5'-Hexachlorobiphenyl comprised 24% of adipose and 21% of serum PCBs. The relative concentration patterns of the 14 PCB congeners were similar to those reported in other human studies and were also typical of patterns reported in environmental samples from various biota, including mammals and birds, but differed substantially from patterns reported in occupationally exposed workers. All adipose-serum correlations for pesticides and most PCBs were statistically significant. Either serum or adipose OCP/PCB levels of a variety of environmental organochlorine compounds may serve as useful biomarkers of body burden.

Parallel comparative trial of amlodipine and nitrendipine monotherapy in patients with essential hypertension.

OBJECTIVES: To compare the blood pressure effects of two dihydropyridine calcium channel blockers, amlodipine and nitrendipine, in 488 patients with essential hypertension. METHODS: The study used a randomized, single-blind design of 4 weeks' duration conducted at four medical centres in China. Patients were randomized to receive either amlodipine monotherapy (5-10 mg once daily; n = 334) or nitrendipine (10 mg twice or three times daily; n = 1 54). Blood pressure was evaluated by standard blood pressure measurements before and after treatment, and by 24 h ambulatory blood pressure monitoring in a subgroup of patients (n = 18). RESULTS: Both systolic and diastolic blood pressures were reduced from baseline after 4 weeks of amlodipine and nitrendipine monotherapy. Diastolic blood pressure was reduced by 14.4% in the amlodipine group, which was significantly better than the 13.0% reduction in the nitrendipine group (P< 0.05). In addition, blood pressure response rates were significantly better with amlodipine monotherapy than with nitrendipine monotherapy. In the subgroup of patients undergoing 24 h ambulatory blood pressure monitoring, both systolic and diastolic blood pressure were reduced from baseline in the amlodipine and nitrendipine groups. Adverse effects were generally mild, with dizziness, flushing, palpitation, headache, drowsiness and ankle oedema being the most common. Rushing and headache were more frequent in the nitrendipine group than in the amlodipine group (P< 0.05 for flushing and P<0.01 for headache). CONCLUSIONS: Amlodipine monotherapy reduced blood pressure more effectively than nitrendipine monotherapy in patients with essential hypertension and was associated with fewer adverse events.

Preferential interaction of sentrin with a ubiquitin-conjugating enzyme, Ubc9.

Sentrin is a ubiquitin-like molecule that has been shown to interact with the death domains of Fas and tumor necrosis factor receptor 1 (TNFR1), PML, Rad51, Rad52, and RanGAP1. We have reported previously that sentrin can be conjugated to other proteins in a manner analogous to protein ubiquitination (Kamitani, T., Nguyen, H. P., and Yeh, E. T. H. (1997) J. Biol. Chem. 272, 14001-14004). Furthermore, the conserved C-terminal Gly-Gly residues are required for sentrinization to occur. To identify enzymes which play a role in sentrinization, the yeast two-hybrid system was used to screen a human placenta cDNA library using sentrin as bait. A strong positive interacting clone was found to contain a cDNA insert encoding the ubiquitin-conjugating enzyme, Ubc9. The interaction between sentrin and Ubc9 required the ubiquitin domain and the C-terminal Gly-Gly residues of sentrin. This interaction appears to be specific because sentrin could only interact weakly with UbcH5B, but could not interact with HHR6B, UbcH6 nor E2-EPF. In vitro translated sentrin could be precipitated by a GST-Ubc9 fusion protein, but not by glutathione S-transferase. A beta-mercaptoethanol-sensitive Ubc9-sentrin conjugate could also be identified in the in vitro binding assay. Substitution of the conserved cysteine residue of Ubc9 by serine abolished the formation of the Ubc9-sentrin conjugate. Taken together, Ubc9 is a strong candidate to be the key conjugating enzyme in the sentrinization pathway.

Neuropathology of degenerative cell death in Caenorhabditis elegans.

In Caenorhabditis elegans necrosis-like neuronal death is induced by gain-of-function (gf) mutations in two genes, mec-4 and deg-1, that encode proteins similar to subunits of the vertebrate amiloride-sensitive epithelial Na+ channel. We have determined the progress of cellular pathology in dying neurons via light and electron microscopy. The first detectable abnormality is an infolding of the plasma membrane and the production of small electron-dense whorls. Later, cytoplasmic vacuoles and larger membranous whorls form, and the cell swells. More slowly, chromatin aggregates and the nucleus invaginates. Mitochondria and Golgi are not dramatically affected until the final stages of cell death when organelles, and sometimes the cells themselves, lyse. Certain cells, including some muscle cells in deg-1 animals, express the abnormal gene products and display a few membrane abnormalities but do not die. These cells either express the mutant genes at lower levels, lack other proteins needed to form inappropriately functioning channels, or are better able to compensate for the toxic effects of the channels. Overall, the ultrastructural changes in these deaths suggest that enhanced membrane cycling precedes vacuolation and cell swelling. The pathology of mec-4(gf) and deg-1(gf) cells shares features with that of genetic disorders with alterations in channel subunits, such as hypokalemic periodic paralysis in humans and the weaver mouse, and with degenerative conditions, e.g., acute excitotoxic death. The initial pathology in all of these conditions may reflect attempts by affected cells to compensate for abnormal membrane proteins or functions.

mago nashi mediates the posterior follicle cell-to-oocyte signal to organize axis formation in Drosophila.

Establishment of the anteroposterior and dorsoventral axes in the Drosophila egg chamber requires reciprocal signaling between the germ line and soma. Upon activation of the Drosophila EGF receptor in the posterior follicle cells, these cells signal back to the oocyte, resulting in a reorganization of the oocyte cytoplasm and anterodorsal migration of the oocyte nucleus. We demonstrate that the gene mago nashi (mago) encodes an evolutionarily conserved protein that must be localized within the posterior pole plasm for germ-plasm assembly and Caenorhabditis elegans mago is a functional homologue of Drosophila mago. In the absence of mago+ function during oogenesis, the anteroposterior and dorsoventral coordinates of the oocyte are not specified and the germ plasm fails to assemble.

Predictive value of clinic blood pressure variability for pressure changes during placebo in elderly hypertensives.

OBJECTIVES: Syst-China is the ongoing placebo-controlled double-blind outcome trial in older (aged 60 years or more) Chinese patients with isolated systolic hypertension (systolic blood pressure 160-219 mmHg and diastolic blood pressure < 95 mmHg). This article is based on the data accumulated until 31 August 1992. Its purpose is to investigate the extent to which the variability in the clinic blood pressure readings at baseline could predict the blood pressure changes observed in the placebo arm of the trial. METHODS: From 2379 patients recruited into the trial, 728 [455 men and 273 women, aged 66.7+/-5.5 years (mean +/- SD)] were selected, because their blood pressure readings for the three run-in visits as well as 3, 6 and 12 months after random allocation were available. Overall and between-visit blood pressure variabilities at baseline were estimated from the two readings obtained with the subject seated during the first and second run-in visits. The baseline blood pressure used to calculate the blood pressure changes during follow-up was the average of the two readings during the third run-in visit. RESULTS: The blood pressure variability at baseline was larger for women than it was for men. For all of the subjects combined, the blood pressure had decreased by 4.1+/-14.4 mmHg (P < 0.001) systolic and 0.5+/-6.7 mmHg (P < 0.06) diastolic by the 3-month follow-up visit, by 8.5+/- 15.2 and 1.4+/-7.5 mmHg, respectively, after 6 months and by 10.3+/-15.7 and 1.9+/-7.9 mmHg, respectively, after 1 year (p < 0.001 for all). Stepwise multiple regression analysis showed that sex, age, alcohol intake and the blood pressure at baseline were significant determinants of the long-term (1 year) blood pressure changes. Aftger adjustment for the aforementioned covariates, the between-visit variability was a significant predictor of the changes in the diastolic blood pressure after 1 year of placebo treatment for the men (partial r+/-SEM -0.36+/-0.12, P < 0.01) and for all of the subjects (-0.19+/-0.09, P < 0.05). For men, the partial regression coefficient between the overall variability and the changes in the diastolic blood pressure also attained statistical significance (-0.39+/- 0.14, P < 0.01). CONCLUSION: For older Chinese patients with isolated systolic hypertension, in particular for men, a higher blood pressure variability at baseline was associated with a larger decrease in diastolic blood pressure during 1-year follow-up on placebo, explaining up to 2% of the variance of the observed changes. Similar associations were not observed for systolic blood pressure.

[Analysis of high frequency X-ray images in 43 cases of breast cancer].

OBJECTIVE: To analyse a series of breast cancer images in high frequency X-ray for more accurate diagnosis of breast cancer. METHODS: Forty three high frequency X-ray images of breast cancer surgically confirmed were comprehensively analyzed. RESULTS: Of the 43 breast cancer cases, 42 were primary breast cancers and one secondary. Tumor mass was present in 38 cases, with burrs in 26. Malignant calcification was found in 13 cases, and structural derangement in 21 cases. There were 30 cases with abnormal blood vessels, and 7 cases with thick skins. Ductal and lymph drainage features were seen in 9 cases. There were 5 cases showing ox horn sign. The high frequency X-ray diagnosis of breast cancer conformed with pathologic diagnosis in 95.6% of the cases. CONCLUSION: The advantages of high frequency X-ray are its extremely high clarity and good contrast. Pictures can be taken form various axises which help detect occult and tiny cancer.

[Nucleotide sequence of the Rhizobium huakuii common nodution genes nodA and nodBC].

An another clone pRaN109 which contains nod genes was isolated and identified from the clone bank of R. huakuii 159 DNA using the 32P-labeled 2.3 kb R. meliloti nod DNA fragment as a hybridization probe. Analysis of homologous DNA-DNA hybridization and nucleotide sequence indicated that the 9 kb EcoRI fragment of pRaN109 DNA carried the nodD1BC genes and the 18 kb EcoRI fragment of pRaN109 DNA contained nodD2A genes. The common nodA gene was separated by 6.7 kb from the nodBC genes. Both the nodA and nodB-nodC cistrons showed nod box upstream of these genes. Nucleotide sequence analysis of the genes describes a obviously different organization of common nod genes in R. huakuii 159 compared with that reported for most of the strains from different genera described up to now.

[mRNA expression of metastasis-suppressor gene nm23 in carcinoma of buccal mucosa. II. Quantitative reverse transcription PCR amplification].

The nm23 gene is a conspicuous metastasis-suppressor gene. The authors detected both nm23-H1 and nm23-H2 mRNA levels in 47 tissues samples of patients with carcinoma of buccal mucosa (CBM) by quantitative reverse transcription PCR amplification. The results showed that expression levels of both nm23-H1 and nm23-H2 mRNA varied in normal buccal mucosa, leukoplakia, adjacent nontumorous mucosa, submandibular gland, CBM and lymph nodes with or without metastasis. The nm23-H1 mRNA expression levels in CBM with lymph nodes metastases were lower than those in CBM without metastases (P < 0.05), while no significance of nm23-H2 mRNA expression levels was found between CBM with and CBM without metastasis (P > 0.05). The results were comparative to those of Northern blotting of the same cases. The authors concluded that, as also in the study of Northern blotting, the expression of nm23-H1 mRNA significantly correlated inversely with lymph node metastasis in CBM, while the expression of nm23-H2 mRNA not. Q-RT-PCR was a useful method to detect the mRNA levels of nm23 gene in buccal carcinoma.

[Repair of the splicing defect of thalassemic allele beta IVS-2-654 C-->T in cultured human erythroid cells by antisense RNA].

OBJECTIVE: To investigate the repair of the splicing defect of thalassemic allele beta IVS-2-654 C-->T (beta 654) in cultured human adult erythroid cells (hAE) by antisense RNA A2. METHODS: Vectors expressing antisense RNA fragments A2 which targeted against the aberrant splice sites of beta 654 pre-mRNA were introduced into hAE by lipid-mediated DNA-transfection method. Quantitation of beta-globin mRNA by RT-PCR and micro-biosynthesis of globin chain assay were used to measure the expression of globin genes in hAE from day 0 to day 8 after transfection. Meanwhile, hAE transfected with random antisense sequence was used as control. RESULTS: Antisense RNA A2 treatment restored the correct splicing of beta 654 pre-mRNA. The proportions of correctly spliced beta-globin mRNA (beta/beta + beta*) increased from 0.024 (before transfection) to 0.380 (8th day after transfection) in hAE obtained from individual with beta 654/ beta 41-42 mutations and, from 0.396 to 0.883 in hAE obtained from individual with beta 654/beta A genotype. Correspondently, microglobin biosynthesis of chain revealed that the ratio of beta/alpha increased from 0.052 to 0.359 in the former and, from 0.624 to 0.820 in the later case, respectively. There was no difference with or without antisense RNA A2 treatment on hAE obtained from normal controls. CONCLUSIONS: Antisense RNA A2 treatment could restore efficiently the correct splicing of beta 654 pre-mRNA and, consequently improve the imbalance of globin chains in thalassemic cells. This method is of potential clinical interest in gene therapy of the disease.

Protection against Fas/APO-1- and tumor necrosis factor-mediated cell death by a novel protein, sentrin.

Fas/APO-1 and TNF receptor 1 share a common signaling motif in their cytoplasmic tail called the "death domain." Using the death domain as bait in the yeast two-hybrid system, several death domain-containing proteins that participate in cell death signaling have been identified. Here we report the isolation of a novel protein, sentrin, which interacts with Fas/APO-1 and TNF receptor 1 but not with FADD/MORT1 or CD40. Two-hybrid interaction assays reveal that sentrin associates only with the signal-competent forms of Fas/APO-1 or TNF receptor 1 death domains. Sentrin is a novel protein of 101 amino acids with homology to ubiquitin, Nedd8, and a Saccharomyces cerevisiae protein, Smt3. When overexpressed, sentrin provides protection against both anti-Fas/APO-1 and TNF-induced cell death.

A bcl-2/IgH antisense transcript deregulates bcl-2 gene expression in human follicular lymphoma t(14;18) cell lines.

The 14;18 chromosome translocation, characteristic of most human follicular B-cell lymphomas, juxtaposes the bcl-2 gene with the IgH locus, creating a bcl-2/IgH hybrid gene. By mechanisms that are still under investigation, this event increases the cellular levels of the bcl-2 mRNA and thereby induces an overproduction of the antiapoptotic BCL-2 protein which is likely responsible for neoplastic transformation. In an effort to identify potential upregulators of bcl-2 activity in t(14;18) cells, we found, by strand-specific RT-PCR, a bcl-2 antisense transcript that is present in the t(14;18) DOHH2 and SU-DHL-4 but not in the t(14;18)-negative Raji and Jurkat lymphoid cell lines, and thus appears to be dependent on the bcl-2/IgH fusion. This antisense transcript is a hybrid bc1-2/IgH RNA, that originates in the IgH locus, encompasses the t(14;18) fusion site and spans at least the complete 3' UTR region of the bcl-2 mRNA. To achieve some insight into its biological function, we treated the t(14;18) DOHH2 cell line with oligonucleotides (ODNs) by specifically targeting the bc1-2/IgH antisense strand. These ODNs lowered bcl-2 gene expression, inhibited neoplastic cell growth by inducing apoptosis. We would like to propose the hypothesis that the bc1-2/IgH antisense transcript may contribute, by an unknown mechanism, to upregulation of bcl-2 gene expression in t(14;18) cells. The possibility has been considered that the hybrid antisense transcript mask AU-rich motifs present in the 3' UTR of the bcl-2 mRNA characterized in other genes as mRNA destabilizing elements.

Antihypertensive therapy debate: contribution from the Shanghai Trial Of Nifedipine in the Elderly (STONE).

BACKGROUND: In 1993, a number of influential bodies reported the results of consensus conferences on the management of hypertension, including the World Health Organization/International Society of Hypertension, the fifth United States Joint National Committee (JNC-V), the British Hypertension Society, the Canadian Hypertension Society (CHS) and the New Zealand National Advisory Committee. Most of these guidelines were in general agreement. The most significant difference was in the choice of first-line agents, whether to use only beta-blockers or thiazides (JNC-V/CHS) or to use any of the five major classes of antihypertensive agents. EVIDENCE FROM THE SHANGHAL TRIAL OF NIFEDIPINE IN THE ELDERLY (STONE): At present, this is the only placebo-controlled trial on newer classes of medications for which results are available. The trial has both strengths and weaknesses. Among the strengths of this trial is the comparability of the subjects in both arms and the robustness of the results. STONE does at least suggest that outcomes with newer agents like long-acting nifedipine are comparable to outcomes in major clinical trials in the elderly using diuretics and beta-blockers.
